On May 20, 2021, at around 6:00 a.m., Buffalo Police responded to a report of two people shot on Layer Avenue. The victims, 27-year-old Megan Wisniewski and 63-year-old Margie Ramos, were killed in the shooting.
Later, at approximately 11:00 a.m., police located the suspect, 36-year-old Rafael Perez, and engaged in a standoff with him. Around noon, police reported that Perez was found dead from a self-inflicted gunshot wound, ending the standoff.

Police responded to a 911 call reporting a person lying in the back seat of a maroon sedan parked on Bauer Avenue and Frontier Trail. Suffolk County Police Officer James Skidmore arrived on the location and approached the vehicle. Upon engaging with Jesse Bonsignore, the officer claimed that Bonsignore exited the car holding a knife and made threats against him. Officer Skidmore physically grabbed Bonsignore, during which Bonsignore reportedly tried to take the officer’s firearm. Officer Skidmore then shot and killed Bonsignore.

The New York Attorney General’s Office investigated the incident, concluding that Officer Skidmore’s use of deadly force was legally justified under New York law, citing the perceived threat during the confrontation. Bonsignore’s family, however, has filed an $85 million wrongful death lawsuit against the Suffolk County Police Department and Officer Skidmore, alleging excessive force and wrongful death.

On May 19, 2021, at approximately 11:53 p.m., a Virginia State Police trooper conducting stationary radar on eastbound Interstate 64 noticed a motorcycle and a Hyundai Genesis speeding at 102 mph in a 65 mph zone. The trooper attempted to stop the vehicles, but both drivers fled the scene. A second trooper joined the pursuit, with speeds exceeding 125 mph. The driver of the Genesis lost control of the vehicle as he approached the Bowers Hill Interchange, ran off the road, and crashed into a tree line. The motorcycle continued and was lost by the troopers. The driver of the Genesis, Javaud Aubrey Ahangari, was killed in the crash.

On 05/19/2021, an Ionia County Sheriff's deputy was dispatched to investigate a report of a stolen vehicle. The deputy located the allegedly stolen pickup truck and initiated a pursuit that spanned through Clinton County and ended in Eaton County. The chase concluded in Mulliken where Robert NazmiRouhman Jr. allegedly reversed his truck into the deputy's SUV, an action which police classified as felonious assault on a police officer. In response, the deputy shot and killed Rouhman Jr.

Deputies from the Collier County Sheriff's Office responded to a call for assistance from the Hendry County Sheriff's Office in the pursuit of a stolen dump truck driven by Rudolfo Villanueva into Immokalee. The truck did not stop, crashing into a vehicle on New Market Road, causing injuries to that driver. Villanueva continued driving recklessly, crashing into another vehicle and driving away from deputies into Farm Worker Village. Despite attempts to stop the truck, Villanueva continued to drive erratically. Deputies shot at the truck, which eventually crashed into a canal. Villanueva was killed.

On 05/19/2021, deputies from the Grand Forks County Sheriff's Office escalated a southbound chase on I-29, reaching speeds exceeding 100 mph. They deployed stop sticks south of Hillsboro, puncturing both front tires of the vehicle. The driver, Randy Cole, continued driving on the front rims, entered a construction zone, crossed the centerline into oncoming traffic, and collided with a northbound semi-truck. Cole was pronounced dead at the crash site. The incident is under investigation by the North Dakota Highway Patrol.

They discovered 22-year-old Arik Reed suffering from a gunshot wound after attempting to prevent the kidnapping of 17-year-old Oliver "Ollie" Taylor. Kenneth William Peden III fled the scene with Taylor in a white Ford F-150 truck.

Multiple law enforcement agencies, including the Marion County Sheriff's Office, the Silverton Police Department, and the Jackson County Sheriff's Office, escalated a chase during which police engaged in a shootout. Taylor was found with a gunshot wound and transported to a hospital, where he died from his injuries on May 19, 2021. Police have not disclosed who shot and killed Taylor during the shootout. Law enforcement has not released ballistics reports to confirm which bullet caused Taylor's death. A grand jury cleared the officers involved, determining that either way, the use of force was justified.

On May 18, 2021, Salisbury Police Officer B.W. Jones struck and killed pedestrian Howard Miller, 59, while responding to a mental health call. Officer Jones was traveling eastbound on Old Concord Road near Gold Hill Drive; the exact speed at the time of the incident has not been disclosed. The area has limited street lighting. Miller was in the roadway when Officer Jones hit him. The Salisbury Police Department conducted an internal investigation of the incident. As of the latest available information, the department is awaiting the results of the full investigation from the State Highway Patrol.

On May 18, 2021, at approximately 11:00 a.m., deputies from the Jackson County Sheriff's Office responded to a reported domestic assault at a residence in the 1500 block of Queens Branch Road near Wimer, Oregon. Before their arrival, 32-year-old Rodney Purslow left the home in a vehicle. The injured victim, identified as hisfather, informed deputies that Purslow might be armed and was potentially a danger to himself.

Purslow returned to the residence in his vehicle, holding a loaded .22 caliber rifle. Purslow allegedly walked toward the deputies who open fire, killing him.

On May 17, 2021, at approximately 5:00 p.m., a Monterey Park Police Department officer was nearing the end of his shift and went to the Monterey Park Fire Department's headquarters, located in the 300 block of West Newmark Avenue, to refuel his vehicle. A man, later identified as 57-year-old Yong Liu, followed the officer in a Nissan SUV into a secured parking lot. After the officer parked and exited his vehicle, Liu, armed with a knife, allegedly charged at the officer. The officer opened fire, striking Liu. Liu was transported to a nearby hospital, where he was pronounced dead.

The Los Angeles County Sheriff's Department is assisting the Monterey Park Police Department in the investigation of this officer-involved shooting. The names of the involved officer(s) have not been publicly disclosed.

On 05/17/2021, the Dickinson Police Department responded to a report of a domestic incident involving a weapon at the 3300 block of Fourth Avenue East around 1:14 a.m. Upon arrival, officers reported that Xavier A. Kilby fired from inside the apartment toward officers or another individual. Police opened fire, striking Kilby. He was pronounced deceased at the location. On 05/17/2021, Charles County Sheriff's Office deputies responded after a family member reported that Denzell Nathan Clarke, who was wanted by police, was experiencing a mental health crisis. Upon arrival at the 6300 block of Josephine Road in Waldorf, two deputies were wounded while attempting to serve a warrant. Clarke allegedly shot at the deputies, who then exchanged gunfire. Clarke subsequently locked himself inside the home. When officers entered, they found Clarke dead from gunshot wounds.

On May 16, 2021, at approximately 2:14 a.m., officers observed a vehicle driven by Marcus "Marc the Barber" Jeffries driving erratically. Suspecting he was under the influence of drugs or alcohol, officers attempted to stop the vehicle. Jeffries fled the scene, and a pursuit followed through surrounding neighborhoods. The chase ended when Jeffries crossed railroad tracks at a high rate of speed and crashed into a pole, killing him.

Lee Esther Anderson, a 42-year-old Black woman, died from a fentanyl overdose on May 16, 2021, while in custody at Santa Rita Jail in Dublin, California. The fentanyl was smuggled into the jail by another woman who hid the drugs in her body cavities. Before her death, Anderson wrote a blog describing poor health and safety conditions at the jail, including exposure to animal and insect feces in food, unsanitary living conditions, and rodent infestations. The Alameda County Sheriff’s Office is investigating the circumstances of her death.

On 05/16/2021, Baltimore Police Department officers responded to a call at 1800 block E. Lafayette Avenue in Baltimore at 10: 39 a.m. regarding a family disturbance. Upon arrival, they were informed by someone outside that a man, later identified as Timothy Fleming, was inside with a knifeand holding a woman at knifepoint. Officers entered the home and proceeded to the second floor, where they encountered Fleming, the woman, and another family member. Fleming allegedly grabbed the woman by her hair and made a motion as if to stab her. Two officers then fired their weapons, killing Fleming.

On May 14, 2021, deputies with the Augusta County Sheriff's Office responded to a 911 call reporting a break-in at a home on Blue Fish Lane. Upon arrival, deputies encountered a man who fled the scene and ran into the woods behind the house. The deputies followed the man, but as they reached the wood line, Jeffrey Bruce allegedly turned around, had a knife and charged toward a deputy. The deputy responded by shooting and killing Bruce.

On May 14, 2021, around 6:30 p.m., officers from the Indianapolis Metropolitan Police Department responded to an apartment complex in the 7800 block of Red Mill Drive following a report of an armed, suicidal person. Monolito Ford had allegedly fired at least one shot into the air before police arrived. Three officers encountered Ford, who was reportedly armed and walking toward them. After officers ordered him to stop, he allegedly pointed his firearm at the officers, prompting them to shoot and kill him.

On 05/14/2021, at approximately 3:25 p.m., deputies from the Pennington County Sheriff's Office responded to a call about an intoxicated and out-of-control family member at a residence on Radar Hill Road. Upon arrival, deputies confronted a man, later identified as Ty Quinton Stilwell, who was carrying a long gun. Deputies ordered him to drop the weapon, but Stilwell reportedly leveled the gun in their direction and ignored their commands. In response to the perceived threat, the deputies used deadly force, shooting and killing Stilwell.

Adrian and Hector Murillo were being followed by undercover officers, who attempted to stop them in Ellis County. The brothers fled, leading to a police pursuit. During the chase, one or both men allegedly fired at the pursuing officers. A constable from Hill County became involved in the pursuit and was shot and wounded. The vehicle driven by the Murillos crashed after being disabled by an officer. Adrian and Hector continued exchanging gunfire with the officers. Eventually, the two men surrendered. Adrian Murillo was killed in the exchange.

On May 12, 2021, around 5 p.m., a 52-year-old man was reportedly barricaded inside a vehicle near 173rd Street South and A Street South in Spanaway, Washington. During the standoff, the man allegedly fired a shot at a deputy, though no injuries were reported. Shortly after, the man reportedly shot and killed himself.

Officers located him and his vehicle at Mexico Community Park. Due to the nature of the threats and concerns for public safety, officers established a perimeter around the area. After a 12-hour standoff, Watros reportedly shot and killed himself with a rifle.

On May 10, 2021, at approximately 2:00 a.m., a Louisiana State Police trooper observed a Ford Explorer traveling northbound on U.S. Highway 167 near LA Highway 749 that was driving too fast. The trooper escalated pursuit; the driver did not stop. The trooper allegedly "called off the chase" due to inclement weather and high speeds, just before the Explorer veered off the roadway, struck a parked vehicle, overturned, and hit a utility pole. The Explorer was occupied by four juveniles, including 15-year-old passenger Trashawn Blake Harrison, who was ejected from the vehicle and died. Three other juveniles were injured.

Officers responded to a welfare check at the Spanish Trails Inn around 9:45 p.m. on May 10, 2021, after a man was reportedly refusing to let a woman leave. Upon arrival, officers determined Kalon J. Horton might have been under the influence of an unknown substance and needed medical attention. He was taken into custody without a warrant and transferred to a medical center for a mental evaluation. Horton was released by the hospital overnight. Early the next morning, officers were called to a shooting in progress at an apartment where a woman and two children were present. Officers arrived and announced themselves, requesting the man to come outside. After receiving no response, they entered the apartment, where they shot and killed Horton.

On May 9, 2021, Albuquerque Police Department officers were working near Iliff and Coors in Albuquerque, New Mexico, when they were flagged down regarding an individual reportedly throwing rocks at vehicles in a hotel parking lot. On May 7, 2021, at approximately 12:30 a.m., Providence Police officers responded to reports of a man screaming in the street near 345 Angell Street, possibly under the influence of narcotics. Upon arrival, they encountered 34-year-old Joseph L. Ventre Jr., who was rolling on the ground and groaning unintelligibly. Officers attempted to calm Ventre and requested assistance from the Providence Fire Department. Due to his erratic behavior and for the safety of all involved, officers decided to restrain Ventre. Body camera footage shows officers holding Ventredown on his stomach for approximately 90 seconds while handcuffing him. Shortly thereafter, Ventre became unresponsive. Emergency responders initiated CPR and transported him to Rhode Island Hospital, where he was pronounced dead at 1:41 a.m. The Rhode Island Medical Examiner later determined that Ventre's death was due to the combined toxic effects of methamphetamine and methadone during police restraint.

On May 6, 2021, at approximately 3:54 a.m., officers from the Casper Police Department attempted to conduct a traffic stop on a vehicle. As officers approached, the driver exited the vehicle, and the passenger moved into the driver's seat and allegedly attempted to flee. One officer contacted the driver, while the other officer tried to stop the fleeing vehicle. The vehicle entered I-25, heading into oncoming traffic lanes. The officer, still inside the vehicle with the driver, continued to order the driver to stop. At this point, the officer shot and killed the driver, reportedly stopping the vehicle along the side of the Interstate.

On May 5, 2021, Winston-Salem Police officers responded to reports of a violent altercation at a residence in the 500 block of Lockland Avenue. Edwin "Joey" Joseph Castillo Jr., 34, was involved in chaotic interactions with housemates and police.
Castillo allegedly struck his roommate in the head with a hammer and fired shots at officers. Police opened fire, killing Castillo during the encounter. The Winston-Salem Police Department is investigating the incident and the circumstances leading to the use of deadly force.
