On 10/10/2021, Harris County sheriff’s deputies encountered Kenneth Anderson at the scene of a vehicle crash in Houston, Texas. Deputies believed Anderson was experiencing a drug related medical crisis and placed him into custody. While detained, deputies reported that Anderson became uncooperative and deployed a Taser multiple times. Deputies restrained him and placed him in a patrol vehicle. Emergency medical personnel responded and found Anderson unresponsive but did not transport him to a hospital. Deputies then drove Anderson to the county booking center, where he remained unresponsive. He was transported to a hospital and later died. The incident prompted internal and civil review. A federal civil rights lawsuit was later dismissed after a court granted qualified immunity to the involved deputy.

Deputies reported seeing Perry Lamar Stringfellow Jr. loading an air conditioning unit into a vehicle. Deputies ordered him to stop. Police stated that Stringfellow instead drove the vehicle toward deputies. Deputies opened fire, striking Stringfellow. He was pronounced dead at the scene. On October 9, 2021, at approximately 2:46 a.m., Maryland Transportation Authority Police Officer Theodore Jeremenko observed a silver Chevrolet Monte Carlo commit a traffic violation near the intersection of I-395 and Conway Street in Baltimore, Maryland. Suspecting the driver might be impaired, Officer Jeremenko initiated a pursuit.

The driver, identified as 26-year-old Jawuan James Ginyard, initially pulled over on I-95 South but then fled as Officer Jeremenko approached his vehicle. The officer chased him onto I-695, with Ginyard reaching speeds exceeding 100 mph and weaving through traffic. Upon taking the Wilkens Avenue exit ramp, Ginyard lost control of his vehicle, which struck the median, causing him to be ejected from the car. Officer Jeremenko rendered first aid until paramedics arrived, but Ginyard was pronounced dead at the scene.

The Maryland Attorney General's Independent Investigations Division conducted a review of the incident. In April 2022, the Baltimore County State’s Attorney’s Office declined to file charges against Officer Jeremenko, determining that his actions were not grossly negligent and did not cause Ginyard's death.

Officers were responding to a DWI call a block away when they heard gunshots in the entertainment district at around 1 a.m. The officers arrived to find two groups of men shooting at one another. An officer began firing at the men. Michael "Mikey" Carothers was killed. It was not immediately known whether the teen was killed by officer-fired bullets or from the other group involved in the shooting.

At approximately 4:30 a.m. on October 9, 2021, a Middlesboro police officer shot and killed 35-year-old Timothy Houston. Houston was reportedly wanted in Claiborne County, Tennessee, on charges stemming from a previous shooting. The incident occurred in Middlesboro, Kentucky. On 10/09/2021, Angel Acevedo Jr. was driving a Toyota Corolla near the Garden State Parkway North at Asbury Ave in the Asbury Park area of New Jersey when his vehicle was struck by a pickup truck driven by an off-duty police officer who was en route to work. Investigators determined that the off-duty officer was operating his vehicle under the influence of intoxicating substances and lost control, causing the truck to leave the Parkway, go airborne over an embankment, and collide with Acevedo’s car. Acevedo and his wife, Daniela Correia Salles, were both pronounced dead at the scene. The Monmouth County Prosecutor’s Office charged the off-duty officer with two counts of second-degree vehicular homicide in connection with the crash. Authorities also cited the driver for reckless driving, failure to maintain lanes, failure to wear a seatbelt, and having an open container of alcohol in the vehicle.

Daniela Correia Salles and the driver of the vehicle, Angel Acevedo Jr., were both pronounced dead at the scene. Officers located Travis Daniel Carlon inside a residence and established a perimeter after he remained inside for an extended period. On October 8, 2021, Officer Dustin Broughton of the Mexico Police Department responded to an unknown incident at 1167 Main Street in Dixfield, Maine, involving 29-year-old Matthew Allen Marston. On 10/08/2021, around midnight, Tulsa Police officers saw Tommy Carmichael, 46, driving a white truck in West Tulsa. Carmichael was a person of interest in the death of his mother, Carol Longacre, who had been found dead in her bathtub earlier that day. Officers followed Carmichael as he drove toward the home where Longacre’s body was discovered.
Carmichael arrived at the house near South Utica Avenue and East 61st Street and tried to enter the property. Before officers could apprehend him, Carmichael shot himself and was pronounced dead at the home.

On 10/08/2021, the Pennsylvania State Police and Coaldale Police Department were involved in a standoff with 47-year-old Richard Lee Sweet in Coaldale, Pennsylvania. Sweet was wanted in connection with the fatal shooting of his girlfriend, Joan Cressman, earlier that day in East Allentown. After the shooting, Sweet fled to his home on the 100 block of Fisher Avenue in Coaldale. Police escalated a large standoff that lasted over four hours. They used loudspeakers while Sweet locked himself inside. Around 4 p.m., neighbors heard a single gunshot from inside the residence. When police entered, Sweet was found with a self-inflicted gunshot wound and was pronounced dead.

On October 8, 2021, at approximately 8:51 p.m., Winston-Salem Police officers responded to a 911 call reporting that 46-year-old Gilbert Lee Collison Jr. had broken into a residence on South Main Street. The caller reported that Collison was armed with a shotgun and had threatened to shoot any law enforcement officers who attempted to intervene. Officers learned that Collison had previously pointed the shotgun at his girlfriend during an argument.

The Winston-Salem Police Department, along with SWAT and crisis negotiators, surrounded the residence and made repeated attempts to communicate with Collison. Despite these efforts, Collison fired shots at the officers. At approximately 2:30 a.m., Collison appeared in the front doorway with the shotgun. Officers used a less-lethal impact baton, striking him in the leg, but Collison responded by firing on the officers. Three officers returned fire, fatally striking Collison.

The North Carolina State Bureau of Investigation is conducting an independent investigation into the incident. The three officers involved have been placed on administrative leave, as is standard protocol for officer-involved shootings.

On October 8, 2021, deputies from the Chippewa County Sheriff's Office conducted a traffic stop on Lance Richard Stelzer, who was riding a moped in Goetz, Wisconsin. During the stop, a physical altercation broke out between Stelzer and Deputy Martin Folczyk. Stelzer allegedly sprayed Folczyk with wasp/hornet killer during the struggle. In response, Deputy Folczyk shot and killed Stelzer.

On 10/08/2021, police responded to a report of a man acting strange and standing on a vehicle in the 900 block of Harlem Street in Altoona, Wisconsin. Officers from the Altoona Police Department, the Eau Claire County Sheriff’s Office, and the Eau Claire Police Department encountered Demetrio Jackson, who was found to be resisting arrest and running from police. Officers deployed a Taser multiple times and restrained Jackson in a prone position. Jackson complained that he could not breathe during the restraint. Emergency medical personnel were summoned, provided care, and injected Jackson with a sedative before transporting him to a hospital. Jackson remained minimally responsive and died at a hospital approximately two weeks later.

On 10/08/2021, law enforcement officers from the Evans County Sheriff’s Office and the Hagan Police Department were operating a license checkpoint in Evans County, Georgia, when Ramone Javaris Dwight drove through the area and continued driving instead of stopping. Police chased the vehicle through the area at high speeds. During the chase, police reported gunfire involving the vehicle. The pursuit ended near Ponderosa Road and Firetower Road, where Dwight exited the vehicle and additional gunfire occurred between Dwight and officers. On 10/08/2021, deputies from the Lancaster County Sheriff’s Office were conducting a welfare check at an apartment on 3000 Fast Lane in Indian Land, South Carolina, after family members reported Derrick Clinton missing. Officers obtained a search warrant and entered the residence to locate Clinton. Deputies said Clinton was inside the apartment and fired a weapon at officers during the encounter. A Lancaster County deputy returned fire, striking Clinton. Clinton was pronounced dead at the scene. Upon arrival, three officers encountered 71-year-old Lloyd Grant McClung Jr., who was armed with a shotgun. Despite repeated verbal commands to drop the weapon, McClung advanced toward the officers and fired a round from his shotgun. Two officers opened fire, striking McClung. McClung was transported to a local hospital, where he was pronounced dead.

On October 6, 2021, officers from the New York City Police Department responded to an apartment on West 147th Street and Adam Clayton Powell Jr. Blvd. in Harlem to arrest 23-year-old Antonio Armstrong for his involvement in a recent domestic incident. When officers attempted to make contact with Armstrong, he allegedly opened fire on them. Armstrong locked himself inside the apartment. Police escalated a standoff that lasted approximately three hours. The standoff ended when Armstrong reportedly shot and killed himself.

On October 6, 2021, at approximately 6:00 a.m., correctional officers with the Hardeman County Sheriff's Office were outside the Hardeman County Criminal Justice Complex when they were allegedly threatened by an armed individual. Three officers from the Bolivar Police Department responded, but the man had already fled. Shortly thereafter, the individual returned, exited his vehicle while still armed, and confronted the officers. The officers shot and killed 43-year-old Andra Davon Murphy.

Additional Information: The family of Andra Davon Murphy has retained attorney Ben Crump, asserting that Murphy was not the individual who initially threatened the correctional officers and is seeking the release of video footage to clarify the events leading to his death.

On October 6, 2021, U.S. Marshals attempted to serve an arrest warrant on Cody James Herman, who was wanted for charges including aggravated assault and weapons violations. As officers approached the home, they were fired upon, resulting in one agent being shot and wounded. Officers fell back, and negotiations began. Several children and adults exited the residence, and after continued negotiations, police entered the home and found Herman dead from a self-inflicted gunshot wound.

On October 6, 2021, around 1:10 a.m., officers from the Milwaukee Police Department attempted to stop a vehicle for reckless driving. The driver of the vehicle did not comply and fled the scene, leading to a police pursuit. During the chase, the fleeing vehicle struck and killed 34-year-old pedestrian Calob House. Officers reportedly called off the pursuit after the incident.

Police stated a woman told deputies her son had assaulted her. Deputies entered the residence and alleged they encountered Guillermo San Miguel Sanchez holding a knife. Deputies opened fire and killed him.

On 10/05/2021, at approximately 4:33 a.m., Pennsylvania State Police responded to reports of shots fired during a robbery at a rest area along I-80 in Loganton, Pennsylvania. Upon arrival, officers encountered a male individual who had barricaded himself inside his rig and refused to comply with police orders. The individual drove his rig into two police vehicles. Officers opened fire and shot and killed him.

Tom Sharkey was wanted for questioning regarding the murder of his wife, 26-year-old Alexis Sharkey. He was found shot in the head inside his daughter's Fort Myers home. At approximately 10 p.m., police initially spoke with Sharkey, but he ran upstairs and shot and killed himself.

On October 5, 2021, at approximately 2:15 p.m., agents from the U.S. Bureau of Alcohol, Tobacco, Firearms and Explosives (ATF) were attempting to arrest 40-year-old Corey Daniel Wellman in connection with a federal drug investigation. The incident occurred in the parking lot near the Country Café at 605 Murfreesboro Pike in Nashville, Tennessee. During the attempted arrest, police engaged him in a shootout, resulting in the death of Wellman and the critical injury of an ATF agent, who was transported to Vanderbilt University Medical Center for treatment. No Metro Nashville Police Department officers were injured or discharged their weapons during the incident.

Additional Information: Wellman had an active federal warrant for his arrest related to a months-long drug investigation initiated in June 2021, which included undercover heroin purchases. Family and friends have questioned the circumstances of the arrest, expressing concerns about the methods used by the ATF agents and whether they properly identified themselves during the operation.

On 10/05/2021, Pennsylvania State Police located Nurgazy Mamyrov after a reported robbery in Clinton County. Police stated troopers tracked him to a rest stop along Interstate 80 in Greene Township.
