A resident of an apartment complex on North Shore Drive in Jeffersonville called the police for a welfare check on another resident, 65-year-old Richard D. Glass. Glass had allegedly made concerning statements about going back to prison before walking into his apartment. Then, 911 received another call, this time from Glass. He claimed that someone was trying to hack his phone and wanted officers to help. Jeffersonville police and EMS, but Glass would not open the door. When dispatchers reached Glass on the phone, he said he did not think the people in the hallway were police and repeated statements about having a gun. Police then saw Glass through a window with at least two firearms. Shots were heard in the apartment. Glass was shooting at police amassing outside of his window. SWAT shot and killed him through the window.

On August 14, 2023, Philadelphia police officers Mark Dial and his partner initiated a traffic stop on Eddie Irizarry in the Kensington neighborhood after observing him driving erratically and the wrong way down East Willard Street. Upon stopping, Irizarry remained seated in his vehicle. Within approximately six seconds of exiting his patrol car, Dial fired six shots through the closed driver's side window, fatally wounding Irizarry. Initially, police reported that Irizarry had lunged at officers with a knife outside the vehicle. However, body-worn camera footage later contradicted this account, showing Irizarry inside the car at the time of the shooting.Dial was charged with multiple offenses, including first-degree murder, third-degree murder, voluntary manslaughter, possession of an instrument of crime, reckless endangerment, and official oppression. In September 2023, a judge dismissed all charges due to insufficient evidence, but they were reinstated a month later upon appeal. In August 2024, the first-degree murder charge was dropped, and Dial was released on bail pending trial.The trial commenced on May 19, 2025. During the proceedings, Dial testified that he feared for his life, believing Irizarry was armed with a gun, based on his partner's shout of "gun" and the appearance of a knife handle that resembled a firearm. The prosecution argued that Irizarry posed no immediate threat, as he was seated in his car with the windows rolled up. On May 22, 2025, the jury delivered a mixed verdict: Dial was found guilty of voluntary manslaughter, possession of an instrument of crime, and reckless endangerment, but not guilty of third-degree murder and official oppression. Police claim he “started reaching into the vehicle” and the officer “thought” he was reaching for a weapon and shot and killed the boy.
