On 09/01/2023, Atascosa County deputies and other law enforcement officers located Edward Garcia on Interstate 37 near Mathis Road following a homicide investigation out of Corpus Christi. Authorities identified Garcia as a murder suspect and reported that he previously worked as a law enforcement officer with the Nueces County Precinct 4 Constable’s Office. Officers stopped his vehicle on I-37. Police stated Garcia exited wearing a ballistic vest and armed with a rifle. During the confrontation on the highway, officers opened fire and struck him. He died from gunshot wounds. Authorities reported one rifle was involved. Law enforcement shut down portions of I-37 during the incident. Multiple agencies responded, and the Texas Rangers are investigating the shooting.

Detroit police were called to the Garden Bowl on Woodward on a report of an intoxicated man causing a disturbance. When police arrived, 71-year-old Daryl Vance had already left the bar and was no longer causing any problems, witnesses said. Brown aggressively approached Vance and threatened to punch him. The interaction quickly escalated, and Brown punched Vance in the jaw, causing him to fall and strike his head. Brown was left lying and died less than a week after the incident. Vance was on life support for 20 days before he died on Sept. 21 from blunt force trauma to his head. “There was no de-escalation,” Crawford says. “The officer escalated it. He wasn’t defending himself. That’s why Mr. Vance died. This is about seeking justice for Mr. Daryl Vance.”

On 08/30/2023, at approximately 5:35 p.m., Vallejo Police Department officers located a white Toyota Avalon near Tennessee Street and Tuolumne Street. Police alleged that the Avalon matched the description of a vehicle used in multiple recent armed robberies in Vallejo. Officers attempted a felony stop specifically because of the vehicle's alleged connection to those robberies. The driver continued driving away at high speed, and Vallejo police pursued.
According to police and later reporting based on dispatch traffic and witness accounts, the Avalon ignored traffic lights and stop signs while the pursuit reached speeds of approximately 90 mph in areas with 30 mph speed limits. The vehicle also traveled into opposing traffic while officers pursued.
Vallejo police reported losing sight of the Avalon approximately 1.5 miles east on Tennessee Street, around the 3300 block, and ending that portion of the pursuit. Approximately 30 seconds later, another police unit saw the Avalon traveling westbound at high speed on Springs Road near Mosswood Avenue and weaving through traffic. Officers turned around to follow it. Police said the Avalon was several blocks ahead when they saw debris and dust from a collision near Springs Road and Tregaskis Avenue.
The Avalon crashed into two uninvolved vehicles. One was a white Toyota Tundra driven by Hudson Joseph Standley, 59, who went by Joe. Standley had just dropped off his business partner and was traveling on Springs Road when the pursued Avalon crashed into the rear of his truck. Standley was killed at the scene.
The Avalon also struck another vehicle. A female passenger in the Avalon was seriously injured and transported to a hospital. Police reported recovering a loaded firearm from the Avalon. The driver got out following the collision, ran from the scene and initially escaped police.
The fatal crash was not investigated under Solano County's Officer Involved Fatal Incident Protocol. The protocol excluded deaths caused by a vehicle fleeing police, meaning the incident did not trigger the outside-agency investigation led by the Solano County District Attorney that would apply to some other law-enforcement-related fatalities.
Later reporting identified significant broader concerns about Vallejo Police Department pursuits. The Vallejo Sun found that Vallejo officers and the pursued Avalon reached approximately 90 mph in 30 mph zones during the pursuit that killed Standley. The outlet subsequently reported that Vallejo police engaged in 197 vehicle pursuits during 2023, a 33% increase over the previous year, and that the department was seeking ways to reduce pursuit-related crashes.
On 03/20/2024, more than six months after Standley's death, Vallejo police located and arrested the person they alleged had driven the Avalon. Police disclosed that the driver was a juvenile and therefore did not publicly release his identity. The arrest was made during a felony traffic stop near Ohio Street and Sutter Street. Police described him as wanted in connection with the August 30 vehicular homicide.

On 08/31/2023, at approximately 6:15 p.m., Miami-Dade Police Department officers responded to a Chevron gas station near Northwest 30th Avenue and Northwest 58th Street after a reported theft involving a woman. Police spokesperson Detective Angel Rodriguez said a man allegedly attempted to take the woman's purse or another item from her vehicle. Local 10 reported that police alleged a struggle occurred between the woman and the man.
The man left the gas station in a car. A witness flagged down arriving Miami-Dade officers and pointed out the vehicle and the direction it had traveled. Officers located the car and attempted a traffic stop specifically because police believed its driver was the person involved in the reported gas-station theft. When the driver continued driving, Miami-Dade officers pursued.
Miami-Dade police subsequently said officers terminated the pursuit because of safety concerns. Police alleged that after officers discontinued the chase, the driver continued at speed and struck two other vehicles before crashing into a tree near Northwest 58th Street and Northwest 15th Avenue.
The impact severely damaged the car. Miami firefighters had to cut away the vehicle's roof to extricate the driver. Paramedics transported him to Jackson Memorial Hospital's Ryder Trauma Center, where he died from his injuries.
The deceased man's family publicly challenged the police account. Relatives told Local 10 that they believed officers had pursued the wrong person and disputed allegations that he had committed the theft. A relative who went to the crash scene said the family found the man's wallet lying there. She questioned the official assertion that police had discontinued the pursuit and specifically asked why the rear of his vehicle was damaged if officers had stopped chasing him.
Local 10 followed the case again on 09/03/2023 after speaking with the man's family. The family continued to deny the police allegations and sought answers about the circumstances of the pursuit and fatal crash. Police had still not publicly identified the deceased driver at that time.

On 08/31/2023, at approximately 6:11 a.m., El Centro Police Department dispatch received a 911 call reporting a petty theft at the Chevron gas station at 1850 South Imperial Avenue. The caller reported that Elizandro Vargas, 31, had taken two alcoholic beverages without paying and walked away. A witness followed Vargas by car while remaining in contact with police dispatch.
According to the witness, Vargas subsequently picked up a brick from the front yard of a residence near Ocotillo Drive and Waterman Avenue and allegedly threatened her with it. She backed away and called 911 again. Officers Daniel Valencia and Victor Ruiz located Vargas at approximately 6:18 a.m., about three-tenths of a mile from the gas station. Valencia reported that Vargas was carrying a brick and requested backup.
Valencia followed Vargas on foot while repeatedly ordering him to stop and drop the brick. Ruiz followed in a patrol vehicle. Body-camera footage documented Vargas continuing to walk away while holding the brick. According to the California Department of Justice's later investigation, Vargas did not threaten either Valencia or Ruiz during this portion of the encounter.
At approximately 6:21 a.m., El Centro Police Sergeant Anthony Hernandez reminded the responding officers over the radio that the underlying offense was Penal Code §488, petty theft. Ruiz repeated the message to Valencia. Valencia and Ruiz then stopped following closely and discussed alternatives, including identifying Vargas and pursuing the petty-theft charge later.
Officers Adrian Hirales and Subah Eid arrived in separate patrol vehicles at approximately 6:22:29 a.m. The California DOJ later determined that Hirales and Eid drove past Valencia and Ruiz and did not coordinate with the officers already on scene before engaging Vargas. Hirales parked approximately 37 feet from Vargas and got out. He initially drew his Taser but almost immediately reholstered it and drew his handgun.
Hirales moved from the protection of his driver's-side door to the front of his patrol vehicle and approached to within approximately 17.5 feet of Vargas while repeatedly ordering him to get on the ground and drop the brick. Vargas stopped walking and turned toward Hirales. According to body-camera and in-car video reviewed by the DOJ, Vargas then threw the brick toward Hirales. Hirales moved to his left and fired twice. The brick missed Hirales and struck his patrol vehicle. One bullet missed Vargas and the other struck him in the upper left chest. The first shot was fired at approximately 6:22:53 a.m., about 20 seconds after Hirales exited his patrol vehicle.
Vargas remained standing after being shot. Approximately one minute and 22 seconds later he sat on the curb. Officers handcuffed him at approximately 6:24:26 a.m. and began providing medical assistance. Paramedics arrived at approximately 6:31 a.m. and transported Vargas to El Centro Regional Medical Center, where he was pronounced dead at 7:28 a.m.
The subsequent autopsy found a single gunshot wound entering Vargas's upper left chest, traveling upward and exiting through his upper left back. The forensic pathologist determined that the gunshot damaged vital organs and caused his death. Toxicology detected alcohol, methamphetamine and amphetamine.
The shooting was recorded by multiple cameras. Hirales, Valencia and Ruiz had activated body-worn cameras, and the shooting was also captured by in-car cameras belonging to Hirales and Eid. Eid had left his body camera at the police station and therefore did not have body-camera footage of the encounter. Investigators also obtained Chevron surveillance video, 911 and dispatch recordings, and civilian cellphone video.
The physical evidence established that the object Vargas threw was a retaining-wall brick measuring approximately 3.15 by 5.51 by 7.87 inches and weighing 8.36 pounds. Investigators recovered two cartridge casings from Hirales' Sig Sauer P320 9mm handgun.
Hirales voluntarily spoke with California DOJ investigators. He said he initially drew his Taser intending to obtain Vargas's surrender but switched to his handgun because he was concerned the Taser might fail. Hirales acknowledged that he could have used his patrol vehicle for cover but chose to position himself in front of it because he wanted to apprehend Vargas. Hirales told investigators that he underestimated Vargas and did not expect him to throw the brick.
The California Department of Justice investigated the killing under Assembly Bill 1506 because Vargas was considered an unarmed civilian for purposes of the statute. The DOJ completed its investigation and released its final report on 03/24/2026. It concluded that prosecutors could not prove beyond a reasonable doubt that Hirales did not reasonably believe deadly force was necessary to defend himself from imminent death or serious bodily injury. The DOJ therefore declined criminal prosecution.
The DOJ nevertheless identified significant problems with the police tactics that led to the shooting. Its Police Practices Section found that Valencia and Ruiz had appropriately maintained distance, used verbal de-escalation and discussed a tactical approach. In contrast, the DOJ found that when Hirales and Eid arrived, they failed to coordinate with the officers already handling the encounter. Hirales moved closer to Vargas, abandoned cover and switched from his Taser to his handgun.
The DOJ specifically concluded that by closing the distance and positioning himself directly in front of Vargas, Hirales diminished his available options, leaving himself without sufficient time or distance to continue de-escalation or use alternatives to deadly force. The DOJ further found that before Hirales' intervention, there was no imminent threat of harm based merely on Vargas holding a brick and walking away, and that the later-arriving officers had time to assess the situation and formulate a tactical plan rather than immediately engaging him.

On 08/30/2023, at approximately 2:15 p.m., San Antonio Police Department Street Crimes Unit officers were conducting surveillance in the 6800 block of Cross Spring for Michael Kirkland, 40. SAPD said Kirkland was wanted on an active warrant for aggravated assault against a public servant. Police later said he also had other outstanding felony and misdemeanor warrants, including aggravated assault with a deadly weapon.
Officers located Kirkland in a vehicle and attempted a traffic stop. Kirkland continued driving away. During the pursuit, he stopped long enough to allow a woman who was riding with him to get out of the vehicle, then continued driving. According to SAPD's subsequently released critical-incident video, Kirkland struck a marked police patrol vehicle, drove the wrong way on the Interstate 10 East access road and crashed into a civilian vehicle.
Kirkland got out of the crashed vehicle and ran while officers pursued him on foot. SAPD alleged that Kirkland fired multiple rounds toward the pursuing officers. One officer, subsequently identified as Officer J. Owen, an eight-year SAPD veteran assigned to the Street Crimes Unit, was struck in the arm. Two officers returned fire as Kirkland continued running down a grassy embankment toward the main lanes of Interstate 10.
According to police, Kirkland reached the interstate and attempted to obtain another vehicle while continuing to fire his gun. SAPD alleged that he pointed and fired at passing motorists while attempting to carjack them. Police said one woman refused to surrender her vehicle and Kirkland fired at her; she was not struck.
Two SAPD officers exchanged additional gunfire with Kirkland on Interstate 10. Police gunfire struck Kirkland, and he died at the scene. Officer Owen was transported to a hospital in stable condition. The shooting and investigation resulted in an hours-long closure of Interstate 10 around South New Braunfels Avenue.
On 09/23/2023, SAPD released its critical-incident video of the encounter. The release included body-camera footage showing the vehicle pursuit, Kirkland running from police, exchanges of gunfire and the final shooting on Interstate 10. SAPD stated that the Bexar County District Attorney's Office was reviewing the police shooting.
Kirkland's death became part of a broader public dispute between SAPD Chief William McManus, the San Antonio Police Officers Association and Bexar County District Attorney Joe Gonzales over people accused of violent crimes who remained in the community. Gonzales publicly reviewed seven cases involving Kirkland dating from 2019 through 2023. Several had been dismissed because prosecutors could not locate or obtain cooperation from complainants or witnesses, while another evading case was dismissed because prosecutors reported that law enforcement had not provided video needed to establish the pursuit.

OFFICER BACKGROUND
Officer Owen has a significant disciplinary history:
2019 — 27-day suspension. SAPD disciplined Jason Owen after he used a homophobic slur and profanity toward a man while clearing a crowd on Southeast Military Drive. The investigation also found that Owen failed to activate his body camera for the first seven minutes of the encounter.
2022 — 15-day suspension. Owen was riding with Officer Anthony Brown when their marked patrol car, traveling about 71 mph, collided with another vehicle. The officers continued driving without stopping to check on the people in the other car. Investigators also found that Owen turned off the patrol vehicle's in-car recording system multiple times, turned off his body camera while speaking with another officer, and failed to report Brown's policy violations to a supervisor.
2024 — another 5-day suspension. SAPD found Owen had been working an unreported off-duty job as a courtesy officer at his apartment complex while on limited-duty status. Records say he improperly accessed department calls-for-service information in connection with that work, failed to obtain the required permit for the outside employment, and failed to report his change of address.

On 08/28/2023, at approximately 10:30 p.m., a Texas Department of Public Safety trooper conducted a traffic stop on John Luther Stanford, 39, on U.S. Highway 259 in Gregg County. Stanford continued driving away and the trooper pursued him for approximately 10 miles into Harrison County. The vehicle pursuit ended on FM 450 in the Harleton area.
Stanford stopped the vehicle, got out and ran into a wooded area while the DPS trooper pursued. DPS alleged that Stanford fired a handgun at the trooper as he ran. The trooper returned fire, resulting in an exchange of gunfire during the foot pursuit. Stanford continued farther into the wooded area.
Stanford was subsequently located dead in the woods. Initial DPS information described his death as occurring following the exchange of gunfire with the trooper, but a later DPS investigative report provided the crucial clarification that the trooper's gunfire did not kill Stanford. Investigators determined that Stanford sustained a self-inflicted gunshot wound to the head.

On 08/31/2023, at approximately 7:00 a.m., Delaware State Police were conducting an operation to locate and arrest Tremaine D. Jackson, 28, of Wilmington. A later Delaware Department of Justice investigation established that Jackson had 21 active arrest warrants, primarily involving alleged retail thefts, four pending probation violations and two out-of-state warrants. Police were also aware of two vehicle pursuits involving Jackson earlier in August 2023.
DSP’s Electronic Surveillance Unit located Jackson at the Lowe’s Home Improvement store at 2225 Hessler Boulevard. Plainclothes officers located him inside the store and, because police considered him a flight risk, requested additional personnel. Members of DSP’s Special Operations Response Team and Drug Unit responded and established a perimeter around the store while officers inside monitored Jackson.
Police alleged that Jackson filled a shopping cart with merchandise and left through the rear of the Lowe’s without paying. SORT Troopers Joshua Cary, Mitchell Moyer and Michael Efelis were waiting behind the business in an undercover pickup truck. When they approached, Jackson pushed the shopping cart in front of the police truck and ran toward a car parked along a guardrail. Cary injured his quadriceps while beginning the foot pursuit and fell to the ground. Moyer continued after Jackson and reached the driver's side of Jackson's vehicle as Jackson got inside and started it.
Moyer attempted to pull Jackson through the driver's-side opening while Jackson held onto the steering wheel. Efelis drove the undercover police pickup in front of Jackson's car, deliberately positioning the truck so Jackson's vehicle was confined between the police truck and a guardrail. The Delaware DOJ described this as a police “pinch” maneuver.
Efelis then opened the police truck's driver's-side door, further obstructing Jackson's path, and got out. According to the DOJ reconstruction, Jackson accelerated and his car pushed against the police truck, causing Efelis to move onto the hood of Jackson's car to avoid being struck. Moyer remained at the driver's side attempting to remove Jackson.
A second undercover police truck arrived carrying Lieutenant Matt Fuski, Trooper Dan Eby and Trooper Thomas Macauley. Macauley saw Cary injured on the ground and initially believed Jackson's vehicle might have struck him. Macauley then saw Moyer at the driver's side of Jackson's car and Efelis on its hood. Macauley climbed onto the hood beside Efelis.
According to the DOJ investigation, Jackson continued accelerating while officers ordered him to stop. Efelis warned Jackson that he would shoot if Jackson continued. Efelis then fired one round into Jackson. Police reported that Jackson briefly appeared as though he might stop but then accelerated again. Macauley subsequently fired one round. Both bullets struck Jackson in the chest. Moyer was still attempting to remove Jackson from the vehicle when the shots were fired, while Efelis and Macauley were positioned on its hood.
Additional troopers arrived immediately afterward and removed Jackson from the car. Police provided medical treatment, including assistance from a SORT medic. Jackson was alive and responsive when transported by ambulance but died at the hospital. An autopsy determined that he sustained two gunshot wounds to the chest and that the gunshots caused his death.
DSP initially withheld the identities of both Jackson and the officers. On 09/03/2023, police publicly identified Jackson and disclosed only that the shooters were four-year and 12-year DSP veterans. Both shooters were placed on administrative leave while DSP and the Delaware Department of Justice investigated.
Jackson's family and the Delaware NAACP subsequently called for an independent investigation and release of video from the killing. The Delaware NAACP also called for a federal pattern-or-practice investigation of Delaware State Police.
The Delaware Department of Justice Division of Civil Rights and Public Trust later identified the shooters as Trooper Michael Efelis and Trooper Thomas Macauley. Investigators reviewed video, dispatch records, civilian and police witness interviews, scene photographs and medical records. Both Efelis and Macauley gave recorded interviews explaining their decisions to fire.
On 02/17/2025, approximately 18 months after Jackson was killed, the Delaware DOJ released its final use-of-force determination. Prosecutors concluded that Efelis and Macauley reasonably believed that Jackson's continued acceleration threatened Moyer, who was positioned between Jackson's vehicle and the guardrail, as well as Efelis and Macauley. The DOJ concluded that each officer's single gunshot was legally justified in defense of themselves and another officer and that neither officer committed a criminal offense.

On 08/30/2022, Cedar Rapids Police Department Investigator Christopher Christy and Sergeant Bryson Garringer shot and killed 22-year-old William Isaac Rich outside the Inn Circle apartment complex at 5560 6th Street SW after police responded to a domestic disturbance. Rich's family initially disputed the police account and organized protests demanding release of the officers' body-camera recordings. Police released the footage approximately nine days after the killing.
Police were dispatched at approximately 12:55 a.m. following a report of a violent domestic incident involving Rich and a woman at the apartment complex. Police were told before arriving that Rich might have a knife. Christy and Garringer encountered Rich outside the building.
The later-released body-camera footage provides substantially more information about the encounter than the original police statement. Rich can be seen outside while officers repeatedly order him to show his hands and get on the ground. He initially complies with some commands and kneels with his hands raised.
This became the central issue raised by Rich's family and supporters immediately after his death. Family members maintained that Rich had been surrendering and questioned why police shot him. Advocates for Social Justice worked with Rich's family and helped organize demonstrations demanding that Cedar Rapids release the body-camera footage rather than requiring the public to rely solely on the officers' account.
The footage subsequently showed that the encounter did not end while Rich remained kneeling with his hands raised. As officers moved closer to arrest him, Rich got back onto his feet. A knife is visible in his hand. Police repeatedly ordered him to drop it.
The confrontation then unfolded extremely quickly. Rich moved toward the officers while still holding the knife. Christy and Garringer fired their handguns, striking him multiple times. Rich collapsed on the pavement.
The video therefore resolved an important part of the initial factual dispute. Rich had been kneeling with his hands raised, as his family and supporters said, but the footage also showed that he subsequently stood, had a knife and moved toward police immediately before Christy and Garringer fired.
Rich died at the scene. The Iowa Division of Criminal Investigation took over the investigation. Christy and Garringer voluntarily submitted to interviews with DCI investigators and were placed on paid administrative leave while the shooting was reviewed. Christy had 14 years of law-enforcement experience and Garringer had 12.
Rich's family continued pressing for transparency after his death. Advocates for Social Justice worked with them during the protests and publicly emphasized the importance of releasing police video quickly after fatal shootings. The body-camera footage was shown to Rich's family before Cedar Rapids police released it publicly.
William was only 22. He left behind his parents, Sandra Rich and Ronald Rich, nine siblings, grandparents, nieces, a nephew and a large extended family. His funeral was held on 09/07/2022 in Cedar Rapids, followed by burial at Oak Shade Cemetery in Marion.
The Iowa Attorney General's Office subsequently reviewed the DCI investigation, including the body-camera evidence and statements from the officers. On 10/05/2022, the Attorney General concluded that Christy and Garringer's gunfire was legally justified.
Linn County Attorney Nick Maybanks independently reviewed the Attorney General's findings and announced that his office accepted that conclusion. Maybanks said his office would conduct no further investigation and would not pursue criminal charges against either Christy or Garringer.
Significantly, Advocates for Social Justice did not continue disputing the legal justification after reviewing the released evidence. The organization said it concurred with the conclusion that the officers' actions were legally justified in the final confrontation. However, it emphasized that the case still demonstrated a need for greater police training in de-escalation and cultural competency.
That distinction is important to the later record: Rich's family and supporters initially challenged the police account and forced greater transparency through protests and demands for the body-camera footage. Once the footage was available, it confirmed both that Rich had initially knelt with his hands raised and that he subsequently stood with a knife and moved toward officers immediately before they fired.
No later civil wrongful-death lawsuit, settlement, criminal prosecution of the officers or subsequent official finding overturning the Attorney General's justification decision was located during this update.

Collins' longtime neighbor Lewis Browder told NBC12 that shortly before the shooting Collins called him and asked whether Browder would take care of his cat if something happened to him. Browder, who said he had known Collins for approximately 40 years, reported seeing Collins firing into the air before police arrived. Another neighbor said Collins' behavior was highly unusual for him.

Hopewell police alleged that arriving officers encountered Collins holding two firearms. Police repeatedly ordered him to put the guns down. Browder witnessed part of the confrontation and said an officer positioned near his house repeatedly told Collins to put the guns down.

On 08/30/2023, San Antonio Police Department property-crime detectives were conducting surveillance of a stolen vehicle that police believed was connected to multiple vehicle burglaries. Detectives followed the vehicle during the day and were waiting for marked patrol officers and an SAPD helicopter to assist when the vehicle entered the Home Depot parking lot in the 600 block of Southwest Loop 410 on San Antonio's West Side at approximately 9:40 p.m. Victor Andres Fernandes, 21, was inside the vehicle with another man.

Officers positioned police vehicles around the car in an attempt to prevent it from leaving. Fernandes got out and ran through the parking lot while officers pursued him on foot. The other man got out and surrendered to police.

SAPD alleged that while Fernandes was running, he reached into his clothing and retrieved a handgun. Plainclothes Detective John Helle and uniformed Officer Christopher Gaige Wesch were involved in the foot pursuit. Helle drew his handgun while running behind Fernandes.
.
During the pursuit, a single gunshot was heard and Helle fell to the ground with a gunshot wound to his torso. Initial reporting created the impression that Helle had been shot by Fernandes. SAPD subsequently disclosed that Helle had accidentally shot himself. Fernandes had not fired at Helle or any other officer during the pursuit.

After Helle shot himself, Wesch saw Helle fall and continued pursuing Fernandes. SAPD alleged that Fernandes was still running with a handgun in his hand. Wesch then fired multiple rounds at Fernandes, striking him multiple times. Critically, SAPD's later critical-incident reconstruction acknowledged that Fernandes never fired his gun at police during the encounter.

Fernandes fell in the Home Depot parking lot. Officers provided medical assistance, but he died at the scene. The Bexar County Medical Examiner subsequently identified him as Victor Andres Fernandes, 21.

The police shooting occurred during an extraordinary period in which numerous SAPD officers had recently been shot or injured. Fernandes' killing was the second fatal SAPD shooting on August 30 alone. Earlier that day, police killed Michael Kirkland following a separate pursuit and gunfire on Interstate 10.

SAPD released dash-camera and body-camera footage on 09/23/2023. The footage clarified a major point that was obscured in the initial reporting: the officer wounded during the encounter had not been shot by Fernandes. On 08/28/2023, at approximately 11:27 p.m., King County Sheriff's Office received a report of an armed carjacking near Southeast 288th Street and 228th Avenue Southeast in Maple Valley. The reporting driver said another vehicle had intentionally rear-ended his silver Hyundai. Police alleged that someone from the striking vehicle then got out with a gun and took the Hyundai. No gunfire was reported during the carjacking.

King County deputies began searching for the Hyundai, and two Black Diamond Police Department officers responded to assist. A Black Diamond officer traveling west in the 22000 block of Southeast 296th Street saw a silver Hyundai traveling rapidly in the opposite direction. The officer slowed and turned around, but by the time the maneuver was completed, the Hyundai was no longer visible.

The officer continued looking for the Hyundai. A King County deputy radioed that the stolen vehicle might be at Lake Sawyer Grocery, prompting officers to respond there, but the vehicle at that location was not the Hyundai they were seeking.

Officers found it off the roadway near Southeast 297th Street and 224th Avenue Southeast, where it had crashed into a tree. On 08/29/2023, members of the U.S. Marshals New York/New Jersey Regional Fugitive Task Force were in the area of West Side Park in Newark attempting to locate and arrest Don Jones, 50, of Newark. Jones had an active arrest warrant stemming from a homicide investigation in Union County. The task force requested assistance from Newark Police Department officers.

Officers encountered Jones inside West Side Park. According to the New Jersey Attorney General's subsequent investigation, officers saw Jones remove a handgun from his waistband. Jones then ran through the park while task-force members and Newark police pursued him on foot and ordered him to drop the firearm.

Jones ran into an open field in the park, where pursuing officers surrounded him. The Attorney General's investigation determined that Jones dropped to a kneeling position and pointed the handgun at his own head. Newark Police Officer Jasmine Jimenez fired her service weapon toward Jones at approximately the same time Jones fired his own gun.

Jimenez's gunfire did not strike Jones or anyone else. Jones sustained a self-inflicted gunshot wound to the head. This later forensic determination corrected early breaking-news reports that initially described the incident as a police shooting and reported that an officer had shot the man.

On 08/29/2023, shortly before 8:00 a.m., Charlotte-Mecklenburg Police Department detectives went to the home of Jeremy Keith Asbury, 31, in the 3000 block of Columbus Circle in west Charlotte while investigating a reported violent sexual assault from the previous day. CMPD Sexual Assault Unit detectives obtained arrest warrants charging Asbury with multiple felonies arising from that investigation.

According to police, the underlying investigation involved a woman's report that she had been abducted at gunpoint, taken to another location and sexually assaulted at gunpoint on 08/28/2023. The allegation was under investigation when detectives went to Asbury's residence, and the warrants were obtained in connection with that allegation.

When detectives arrived at Asbury's home, police reported that he was armed and barricaded himself inside. Detectives and Metro Division patrol officers attempted to communicate with him for an extended period before requesting assistance from CMPD's Special Weapons and Tactics Team.

CMPD SWAT officers responded and surrounded the residence. Police also coordinated with members of Asbury's family and an outside community organization in an effort to communicate with him and obtain a peaceful surrender. The standoff continued for approximately six hours.

During the standoff, Asbury used Facebook Live to broadcast from inside the residence. Video from the incident subsequently circulated publicly and documented portions of the police operation outside his home. Asbury was a father.

Shortly after 2:00 p.m., officers outside the residence heard a gunshot. SWAT officers and MEDIC personnel entered the home and found Asbury suffering from a self-inflicted gunshot wound with a firearm present. CMPD reported that nobody else was inside the residence when officers entered.

On 08/28/2023, shortly after noon, Grays Harbor County Sheriff’s Office deputies were dispatched to the 1300 block of Ocean Beach Road near Copalis Crossing after a 911 caller reported that a man had shot and killed himself. When deputies were dispatched, however, the man was still alive. A second caller, a woman at the location, contacted 911 shortly afterward and reported that the man was experiencing a mental health crisis, was heavily armed and needed assistance.

The residence was in an isolated area without nearby neighbors. Deputies arrived and attempted to contact the man, who was inside a trailer. While deputies were outside, they heard two gunshots coming from the trailer. Deputies evacuated two other people from the property.

According to the Grays Harbor County Sheriff’s Office, the man then fired a .45-caliber handgun toward responding deputies. One bullet struck a sheriff’s patrol vehicle. No deputy was struck by the gunfire.

The Sheriff’s Office activated its Crisis Response Unit and deployed an armored vehicle to the property. The specialized unit arrived at approximately 1:30 p.m. More than 20 law-enforcement officers ultimately responded, including eight Grays Harbor County sheriff’s deputies and personnel from the Hoquiam, Aberdeen and Montesano police departments and Washington State Patrol. Washington Department of Fish and Wildlife officers closed the roadway around the property.

Law-enforcement personnel attempted to negotiate with the man by telephone during the standoff. The telephone conversation eventually ended abruptly. The Crisis Response Unit subsequently approached the trailer and found the man dead inside from a self-inflicted gunshot wound.

Police determined that the Kia had been reported stolen from a Canton resident. Officers activated their emergency lights and sirens and attempted to stop the vehicle specifically because it was a reported stolen vehicle that officers observed speeding. The driver continued driving away and Akron police pursued.

The driver was later identified as Miguel Gipson, 18. During the pursuit, the Kia reached speeds of approximately 100 mph. Akron Deputy Police Chief Brian Harding said officers continually evaluated the danger of the pursuit and eventually decided that the excessive speed made continuing the chase unsafe.

Police reported terminating the pursuit near Kelly Avenue and 6th Avenue after approximately one mile. The Kia continued northbound at high speed after officers discontinued the chase.

Approximately one mile farther north, near Kelly Avenue and 3rd Avenue, Gipson rear-ended an uninvolved blue Kia that was stopped in the northbound lane. The stolen Kia then struck a utility pole and caught fire. The crash occurred only moments after police reported terminating the pursuit.

Three 18-year-old men were inside the stolen Kia. Stephonne Jackson, 18, was a passenger. Jackson and the two other men were removed from the wreckage and transported to area hospitals. Body-camera footage subsequently obtained by the Akron Beacon Journal documented officers arriving at the burning Kia and attempting to remove the three young men. The vehicle was pinned against a fence with its front end smoking and burning. One person remained trapped in the front passenger seat, and officers pulled him from the vehicle through a rear door.

At approximately the same time as the pursuit and crash, Akron police were investigating the shooting of a 28-year-old man on nearby Eller Avenue. Early in the investigation, officers considered whether the three men in the stolen Kia might have been connected to that shooting. Body-camera footage recorded officers discussing that possibility at the crash scene. Subsequent investigation, however, determined that the shooting and fatal crash were not connected.

Police initially did not know which of the three 18-year-olds had been driving the Kia. The subsequent investigation identified Gipson as the driver. The events leading to Holland’s death began on 08/25/2023 in the Cary community. Bell County Sheriff’s deputies Austin Poindexter and Samson Churchwell were attempting to serve Holland with felony warrants near Kentucky Highway 66. Police alleged that when the deputies contacted Holland, he drew a firearm and fired at them. The deputies returned fire and took cover, and Holland ran into a wooded area.

Following that encounter, authorities obtained additional warrants accusing Holland of two counts of criminal attempt to commit murder of a police officer, first-degree wanton endangerment and possession of a firearm by a convicted felon. Law enforcement searched for Holland into the early morning of August 26 before suspending that phase of the search at approximately 4:00 a.m.

Two days later, on August 28, police received information placing Holland in the Bradfordtown community. According to the arrest citation subsequently filed against Rebecca Caldwell, 47, of Lexington, Caldwell initially told officers Holland had been at her location on Bradfordtown Spur but had left and she did not know where he was. Police alleged that Holland was actually hiding nearby behind a vehicle and was armed with two handguns.

A Bell County sheriff’s deputy and Pineville police officers confronted Holland at approximately 7:33 p.m. The encounter ended with police shooting Holland. Bell County Deputy Coroner Jason Steele pronounced him dead at the scene.

Kentucky State Police initially released very little information about the circumstances of the fatal gunfire, stating only that the Bell County Sheriff’s Office requested an independent investigation after the shooting and that a man had died. On 08/28/2023, at approximately 8:42 p.m., the San Diego Police Department received multiple 911 calls reporting gunfire at a residence in the 500 block of Iona Drive in San Diego's Encanto neighborhood. Officers from SDPD's Southeastern Division were dispatched and arrived within minutes.

The first responding officers found 74-year-old Maria Rosario Morales and a dog lying in the driveway with multiple gunshot wounds. Morales was later identified as the mother of 43-year-old Jesse Glen Nelson. Police alleged that Nelson had shot his mother and two dogs before officers arrived. Morales and one dog died from their injuries, and investigators subsequently found a second dog dead at the property.

As officers approached Morales and the wounded dog to provide aid, police alleged that Nelson opened fire on them from inside the residence. SDPD Officer Darwin Anderson returned fire toward the house and then helped coordinate an emergency effort to remove Morales from the driveway. SDPD SWAT Officer Matthew Steinbach, serving as a sniper, positioned himself on the roof of a nearby residence. According to the later San Diego County District Attorney review, the on-scene SWAT commander authorized Steinbach to use deadly force unless Nelson was observed unarmed and actively surrendering and was not attempting to break through the police perimeter.

Police alleged that Nelson subsequently emerged or moved toward the front of the burning residence while carrying a rifle and moving in the direction of officers positioned outside. From his sniper position, Steinbach fired at Nelson, striking him in the back.

Officers provided medical aid before Nelson was transported to a hospital. He was pronounced dead at approximately 10:58 p.m. The Medical Examiner determined that Nelson died from gunshot wounds and classified the manner of death as homicide.

Matthew Steinbach had served with SDPD for approximately 11 years at the time Nelson was killed and was assigned to the Western Division and SWAT. Steinbach was the SWAT sniper who fired the fatal shot that struck Nelson in the back.

Steinbach had been involved in an earlier police shooting on 01/03/2019. SDPD officers responded to a Midway District apartment after a 911 call concerning 23-year-old Ryan Bowers, who had reportedly cut his own throat. Bowers survived the shooting despite initially suffering life-threatening injuries.

On 08/27/2023, Jersey City Medical Center personnel contacted police concerning Andrew Jerome Washington, 52, who was experiencing a mental health crisis inside his apartment on Randolph Avenue in Jersey City. Washington's family told authorities that a neighbor had heard loud noises and banging from his apartment and contacted relatives. His family explained to responding officers that Washington was experiencing a psychiatric crisis and needed medical assistance.

Jersey City police arrived at approximately 2:36 p.m. and spoke with Washington's relatives outside the residence. Police attempted to communicate with Washington through his closed apartment door. Washington remained inside, and officers ultimately requested the department's Emergency Services Unit.

Emergency Services Unit officers arrived and continued attempting to communicate with Washington through the closed door for more than 30 minutes. Officer Stephen Gigante was among the ESU officers communicating with Washington. The encounter was recorded by police body-worn cameras, and the New Jersey Attorney General later publicly released the recordings along with the relevant telephone call and police radio transmissions.

At approximately 3:28 p.m., ESU officers forced open Washington's apartment door. According to the Attorney General's investigation, Washington was holding an approximately 13-inch knife and advanced toward the officers as the door opened. Officer Felix DeJesus deployed his Taser at Washington while Officer Stephen Gigante simultaneously fired his service weapon twice, striking Washington twice.

Emergency medical personnel who had remained outside were brought into the apartment and provided medical treatment. Washington was transported to Jersey City Medical Center, where he was pronounced dead at 4:47 p.m.

On 08/25/2023, at approximately 5:30 p.m., Sumter Police Department officers responded to a disturbance at the Red Lobster on Broad Street in Sumter. Investigators said the incident began as an altercation over a social-media post. Police alleged that an unidentified 16-year-old punched an employee and displayed a handgun before leaving in a silver 2014 Cadillac CTS driven by 17-year-old Ravon Lamar Harris, a Sumter High School student.

Police subsequently located the Cadillac near Old Pocalla Road and Manning Road. The Sumter County Sheriff’s Office was asked to assist Sumter police in stopping the vehicle because police believed it was connected to the reported armed altercation at the restaurant.

During the attempted traffic stop, police alleged that the 16-year-old passenger exited the Cadillac carrying a handgun and ran from officers. Harris remained behind the wheel and continued driving away. A Sumter County sheriff’s deputy pursued Harris with emergency lights and siren activated.

The pursuit continued south on Manning Road and then onto U.S. Highway 521 South. According to the Sheriff’s Office, Harris reached speeds exceeding 100 mph.

Near Mulberry Church Road, Harris lost control of the Cadillac. The vehicle struck an embankment, overturned multiple times and caught fire. The pursuing deputy stopped at the crash, requested EMS and fire personnel and used emergency equipment to extinguish the burning vehicle.

Harris suffered critical injuries in the crash and was airlifted to Prisma Health Richland Hospital in Columbia. He remained hospitalized for approximately two days before dying from his injuries on Sunday, 08/27/2023. Sumter County Coroner Robbie Baker identified him following his death.

The coroner reported that Harris was not wearing a seat belt. An autopsy was scheduled at the Medical University of South Carolina in Charleston. The South Carolina Highway Patrol and Sumter County Coroner’s Office investigated the fatal crash.

Police apprehended the 16-year-old passenger after he ran from the attempted traffic stop. Authorities alleged that five handguns were ultimately recovered in connection with the teenagers and that several had been reported stolen during vehicle break-ins in Sumter city and county the previous night. On 08/27/2023, at approximately 11:10 a.m., Los Angeles Police Department Devonshire Division Officers Eric Hernandez and Randy Portillo were patrolling near Chase Street and Canby Avenue in Northridge when they encountered 24-year-old Angel Ledesma near the entrance to an apartment complex in the 18000 block of Malden Street. The officers had not been dispatched to a crime involving Ledesma; they initiated the encounter after observing him near the apartment entrance and believing he might be loitering.

The officers stopped their patrol vehicle and questioned Ledesma about whether he lived at the apartment complex. Ledesma told them that he did. The officers nevertheless exited their patrol vehicle and continued the encounter, including attempting to check his identification. Body-camera footage later released by LAPD documented the encounter.

According to LAPD, the officers believed Ledesma had a knife. Ledesma denied having one. As the encounter continued, Ledesma moved down the apartment entrance ramp and over a fence. LAPD alleged that Ledesma was holding an open folding knife with an approximately 2¾-inch blade.

Police alleged that Ledesma then ran toward Hernandez and Portillo while holding the knife with its blade pointed toward them. The officers ordered him to drop it. Both Hernandez and Portillo fired their service weapons at Ledesma.

Ledesma was struck multiple times and collapsed on the pavement. Los Angeles Fire Department paramedics responded, but Ledesma was pronounced dead at the scene. LAPD reported recovering the open folding knife as evidence.

LAPD's Force Investigation Division investigated the shooting. The investigation included body-worn camera recordings, additional video and photographic evidence, witness interviews, crime-scene evidence, forensic evidence and Ledesma's autopsy.

On 08/27/2023, shortly after noon, a 22-year-old Lake Elsinore man was riding a yellow motorcycle southbound on Interstate 15 near State Route 76 when a California Highway Patrol officer attempted to stop him for speeding. The motorcyclist continued driving away while CHP pursued. The pursuit traveled into Valley Center and then west toward Escondido by way of the Valley Center grade. CHP notified the Escondido Police Department and requested assistance.

Before the Escondido officer was able to get behind the motorcycle, the rider entered the intersection of East Washington Avenue and Harding Street against a red traffic signal and collided with the front bumper of a vehicle traveling north on Harding with a green light.

The collision threw the motorcyclist from the motorcycle. He then struck the front of a second vehicle traveling westbound. Officers provided lifesaving aid until Escondido Fire Department paramedics arrived, but the motorcyclist was pronounced dead at the scene.

On 08/27/2023, Indianapolis Metropolitan Police Department officers responded repeatedly to a residence in the 4500 block of Woodland Drive concerning 40-year-old Kendall Darnell Gilbert, who was experiencing an escalating mental health crisis. The fatal encounter followed several police responses to the same home over the preceding three days, all concerning Gilbert's mental health and reported threats toward family members.

On 08/25/2023, a caller told police that Gilbert had schizophrenia and was sending family members strange and concerning messages. Officers responded but made no report. At approximately 1:30 a.m. the following day, police were called again after Gilbert reportedly destroyed property in his bedroom and would not open the door for relatives. Officers concluded at that time that he did not present a danger to others and again left without taking him into custody.

At approximately 8:30 p.m. on August 26, police returned to the Woodland Drive residence after another reported mental health episode. Family members reported that Gilbert was threatening to decapitate relatives. Officers helped family members leave the residence and provided them with information about mental health resources.

On August 27, police were called back at approximately 5:20 p.m. after a family member reported that Gilbert was hallucinating, making threats and threatening to burn down the house. Officers advised family members to leave the residence.

At approximately 6:45 p.m., police received another call after an adult woman at the residence activated a medical-alert button and reported that someone was trying to kill her. IMPD Northwest District officers returned to the home for a welfare check.

Officers encountered Gilbert outside the residence holding a machete. According to police, Gilbert made threats toward both family members and officers. Video recorded by a neighbor showed Gilbert standing on the sidewalk with the machete while numerous police officers remained positioned behind vehicles.

Police removed other family members from the residence and requested IMPD's SWAT team and Crisis Negotiation Unit. A psychologist with the Crisis Negotiation Unit was also present. Negotiators attempted to communicate with Gilbert, but IMPD later said they were unable to establish meaningful communication with him.

At approximately 9:10 p.m., SWAT officers moved to take Gilbert into custody. Police deployed a distraction device and multiple less-lethal weapons during the attempt. According to IMPD, Gilbert then moved toward the SWAT officers and made contact with an officer's ballistic shield while still armed with the machete.

IMPD Lieutenant John Perkins fired his department-issued weapon at Gilbert, striking him at least once. Officers and emergency medical personnel provided treatment before Gilbert was transported to a hospital in critical condition. He subsequently died from the police gunfire.
