On 04/10/2021, at approximately 11:00 p.m., San Fernando Police Department officers responded to a 911 call reporting gunfire in the alley behind the 600 block of S. Brand Boulevard. Witnesses described the suspect as a heavyset male adult leaving the area in an older model minivan.

At around 11:01 p.m., officers located a vehicle matching this description on Harps Street near Fourth Street, with Guillermo "Memo" Amezcua, 48, seated in the driver’s seat. Officers issued multiple commands over approximately five minutes for Amezcua to show his hands. Police alleged that Amezcua was uncooperative and observed manipulating an unknown object.

To gain compliance, an officer deployed three less-lethal stun bags aimed at the rear window area of the vehicle, but these attempts were unsuccessful. At approximately 11:06 p.m., police reported observing a muzzle flash consistent with a gunshot from inside the vehicle. Officers opened fire, fatally striking Amezcua, who was pronounced dead at the scene.

The Los Angeles County Sheriff's Department investigated the officer-involved shooting. According to an autopsy report dated 04/14/2021, Amezcua sustained five gunshot wounds, with three being fatal. The report also documented tattoos on Amezcua, including "Memo" and "Jennifer Marie" on his neck and "Guillermo Amezcua Jr." on his left shoulder.

San Marcos Police Department officers responded to a report of a man walking in traffic on southbound I-35 around 12:19 a.m. Upon arrival, officers saw Rescue Eram walking along the retaining wall and crossing traffic. Officers attempted to arrest him and reportedly observed him holding a knife or similar object. Eram allegedly threatened officers by moving aggressively toward them, prompting at least one officer to shoot and kill him.

On April 9, 2021, at approximately 6:57 a.m., Georgetown Police Department officers responded to reports of a man attempting to break into cars in a shopping center parking lot near Connector Road in Georgetown, Kentucky. Upon arrival, officers encountered 31-year-old Deshund Andrea Tanner, who fled on foot and attempted two carjackings during his escape. When officers confronted Tanner during the second carjacking attempt, he produced a firearm. Officers opened fire, killing him. The officers involved—Sgt. Joseph Payton, Officer Cole Centner, Officer Gregg Muravchick, and Officer Christian Squires—were later cleared of any wrongdoing and were honored for their actions during the incident.

On April 9, 2021, U.S. Deputy Marshals responded to a residence on the 300 block of North Hampton Road in Springfield, Ohio, at sunrise to arrest 50-year-old Douglas C. Barton. Barton was wanted for allegedly threatening a judge.

Two men, a woman, and a child were parked at the entrance of an apartment complex when officers stopped to investigate. On April 7, 2021, around 6:45 p.m., Philadelphia Police officers conducted a traffic stop on a blue Kia Optima in the 1500 block of West Somerville Avenue after it failed to stop at a stop sign. The vehicle had four occupants. Upon running their information, officers found that two individuals, including 24-year-old James "Poppie" Alexander, had outstanding warrants. As officers attempted to remove Alexander from the vehicle, he allegedly drew a firearm and fired at them, striking Officer Matthew Ponente in the foot. Alexander then exited the vehicle and reportedly fired additional rounds at the officers. In response, Officers Christopher Burton, Charles McCairns, Michael Braun, David Tamamoto, and Matthew Ponente discharged their weapons, fatally wounding Alexander.

At about 4 a.m. on April 7, 2021, a Dodge truck, driven by off-duty Officer Christopher Carson, was traveling southbound on U.S. 75 in a portion of the roadway under construction. The vehicle struck Noah Joseph Ford, who was in the main lanes of the highway. Ford was killed in the incident.

On April 6, 2021, Oneonta Police Department Sergeant Ralph Pajerski and an unidentified partner responded to a domestic incident at 48 River Street in Oneonta, New York. Upon arrival, they encountered Tyler R. Green, 23, who was allegedly threatening his girlfriend with a knife while holding their two-year-old son. On 04/04/2021, Michigan State Police troopers were conducting a welfare check at around 8 p.m. when they discovered a 62-year-old woman dead from an apparent homicide in her home in Lake Township. The house was empty except for the victim at the time of discovery. Through investigation, troopers identified a man, Adam Briggs, a 30-year-old relative of the victim. A broadcast alert was issued for Briggs. Shortly after 1 a.m., the Cadillac Police Department spotted the vehicle associated with Briggs and attempted a traffic stop. Briggs did not stop. Police escalated a with multiple agencies. During the chase, Briggs allegedly fired shots at the pursuing officers. No one was hit. Missaukee County Sheriff's deputies used stop sticks to disable Briggs' vehicle. Briggs then ran away on foot. His body was found shortly after 6 a.m. with a self-inflicted gunshot wound, confirming his death as a suicide.

On April 4, 2021, Michael Ray Wagner, a 47-year-old white male from Geneva, Indiana, was killed during a police chase in Shelbyville, Kentucky. After reports that Wagner had stopped his vehicle on the roadway and pointed a gun at another motorist, officers located him. Around 3:30 a.m., Corvallis Police responded to a call from the Days Inn motel staff reporting a man acting aggressively with a knife. Officers encountered Jeffrey W. On April 2, 2021, at approximately 5:15 a.m., Long Beach Police Department officers responded to a 911 call reporting a man who appeared to have had a heart attack and a woman threatening suicide at a residence in the 2800 block of Golden Avenue. Upon arrival, officers observed a large amount of blood inside the residence and were immediately confronted by 38-year-old Lauren Nicole Archibeque, who was armed with a rifle. Officers quickly exited the residence, set up a perimeter, and attempted to de-escalate the situation by making repeated attempts to communicate with Archibeque, urging her to surrender. She did not respond.

After some time, Archibeque emerged at the doorway of the residence, and officers observed her pointing the rifle at them. At that point, three officers discharged their firearms, fatally wounding her. She was pronounced dead at the scene. Upon re-entering the residence, officers discovered a 49-year-old man on a bedroom floor with a serious laceration to his neck. He was also pronounced dead at the scene. The Los Angeles County Coroner’s Office later determined that the man's death was a suicide.

The Long Beach Police Department is conducting a full and thorough multi-level review of the incident, and the Los Angeles County District Attorney’s Office is conducting an independent investigation, as is standard protocol for officer-involved shootings resulting in injury or death. Vinetta Martin, a Black female, died by suicide on April 2, 2021, while in custody at Santa Rita Jail in Dublin, California. At approximately 6:45 p.m., a 2003 Mack truck tractor towing a pole trailer was traveling east on Highway 94. A 2017 DPS Chevrolet patrol vehicle was traveling westbound. The DPS Trooper, Trooper Brady Germeroth, identified a traffic violation on another eastbound vehicle and made a U-turn. As the patrol vehicle re-entered the eastbound lane, the Mack truck driver swerved into the westbound lane, striking a 2019 Jeep Wrangler head-on. The truck continued to swerve and struck the back right side of the DPS vehicle, which then overturned and caught fire. The driver of the truck and the trooper were not injured. Melanie Painter, the driver of the Jeep, was killed in the crash.

On 03/31/2021, police responded to a home at 247 Sullivan Street in Claremont, New Hampshire, following reports of gunshots being fired. Jeffrey Ely had barricaded himself inside the residence. After an extended standoff, gunfire was exchanged between Ely and New Hampshire State Police. Troopers Nicholas Cyr, Stefan Czyzowski, Gary Ingham, Shane Larkin, William Neilsen, and Noah Sanctuary fired at Ely, who was subsequently killed. Officer Noah Sanctuary had been involved in a previous shooting incident in February of 2022. Sergeant Stefan Czyzowski and again Sergeant Noah Sanctuary were among six troopers who fired their weapons during a standoff in Plymouth, where Kevin Steinfeldt, died from a self-inflicted gunshot wound under police duress.

Shortly after 10 p.m. on March 31, 2021, gunshots were reported to the police. Approximately 25 minutes later, officers located a pickup truck and attempted a traffic stop. The driver did not stop, leading officers on a pursuit that ended in a dead-end parking lot near Southeast 122nd Avenue and Mill Street. Police initiated a standoff, during which a shot was heard from within the vehicle. Officers deployed gas into the truck, after which the man died by suicide.

On March 30, 2021, Helen Marie Quarles was driving home from her job at the state probation office. As she attempted to make a left-hand turn onto eastbound Route 3, her vehicle was struck by a Culpeper County Sheriff's Office-issued Chevy Tahoe SUV driven by Lt. Ashleigh Baughan. The officer, who was allegedly driving 75 mph in a 55 mph zone, was not responding to an emergency or pursuing another vehicle, and her emergency lights and sirens were not activated at the time of the crash.

On March 30, 2021, at approximately 11:00 a.m., officers from the Bossier City Police Department responded to a report of an allegedly stolen vehicle near a business in the 500 block of Barksdale Blvd, Bossier City, LA. Upon arrival, officers encountered 30-year-old Billy Ray Hill, who reportedly fled on foot but was apprehended by police after a brief chase. Details of the arrest are unknown. After the arrest, Hill became unconscious and died shortly thereafter.

The Louisiana State Police conducted an investigation into the incident. The Bossier Parish Coroner's Office performed an autopsy, which indicated no external or internal injuries or diseases. The coroner suggested that Hill may have suffered a life-threatening cardiac arrhythmia; however, this diagnosis could not be confirmed, and the final cause of death was listed as undetermined.

On March 30, 2021, Officer Johnathan Brown of the Bristol Police Department responded to a call about gunshots at the Rodeway Inn. Upon arrival, Officer Brown and other officers encountered Jonathen Blake Kohler, who was sitting in his vehicle. After a brief interaction, Kohler drove toward the officers, and Officer Brown discharged his weapon, fatally wounding Kohler. The incident resulted in a lawsuit filed by Kohler's family, which was later dismissed in favor of Officer Brown, with the court ruling the officer's use of deadly force was justified.

A Clay County deputy attempted to stop Gage Allen Salsman, who was on a motorcycle, in Henrietta. During the stop, Salsman reportedly threw something from his pockets and began speeding away, yelling for the officers to stop chasing him. Wichita Falls police and the sheriff's office set up ahead to wait for the chase. Salsman eventually collided with an SUV and was killed on impact.

A domestic disturbance occurred at the Quality Inn, starting around 6:30 a.m. A woman called officers there several more times, with the last call shortly after 12:30 p.m. Michael Leon Hughes had forced his way back into the room after having left earlier. When two officers arrived, Hughes refused to leave. As officers attempted to take Hughes into custody, a struggle ensued lasting more than 60 seconds with one of the officers. Hughes reportedly gained control of Officer J. H. Wing's Taser, stunned him once, and was then shot and killed by the officer.

On March 29, 2021, at approximately 9:11 a.m., officers responded to a domestic incident at a home involving Pierce L. Flinchum. When approached by police, Flinchum ran into the house and barricaded himself inside. Police obtained an arrest warrant for him on charges of assault and battery, and possession of a firearm as a felon. Throughout the day, police made multiple attempts to contact him by telephone. At approximately 10:30 p.m., officers entered the home and discovered Flinchum dead from a self-inflicted gunshot wound.

Nathaly Medina, 19, was a passenger in a car that Tulsa police attempted to stop for reckless driving. The driver, Jose Lara-Garcia, did not stop, leading officers to initiate a pursuit. The chase continued until the vehicle crashed into several poles before entering the Arkansas River. Medina died in the crash. The incident is under investigation by the Tulsa Police Department.

On 03/29/2021, at approximately 3:50 a.m., LAPD officers attempted to stop Matthew Blaylock near 7th and Bixel Streets. Blaylock fled on foot, and officers escalated a chase that ended near 8th and Bixel Streets, where Blaylock allegedly produced a handgun. Officers opened fire, killing him. A handgun was recovered at the scene. Body camera footage of the incident has been released and is under review. The LAPD's Force Investigation Division is investigating the shooting, and the Los Angeles County District Attorney's Office will review the case.
