On October 14, 2021, at approximately 2 p.m., Edgecombe County Sheriff’s Office deputies responded to a residence on McNair Road in Tarboro, North Carolina, after receiving a report that 32-year-old Jermaine Harris was chasing a family member with a knife. Upon arrival, Harris lunged at one deputy and stabbed him twice in the back. A second deputy, who arrived shortly afterward, discharged their firearm, fatally striking Harris. The injured deputy was transported to Vidant Medical Center in Greenville, where they were treated and later released.

At approximately 12:00 p.m. on October 14, 2021, the Trigg County Sheriff's Department responded to a disturbance on South Road in Cadiz, Kentucky, where 48-year-old Harold Floyd Jett allegedly fired a shot at a vehicle during an altercation. Jett drove away. Officers escalated a chase at high speeds that continued into the city of Cadiz. The chase ended at Hilltop Street, where Jett entered the residence of his aunt, 71-year-old Mary Dullenty.

On October 13, 2021, officers from the Augusta Police Department responded to a report of a man with a knife at the Bread of Life Shelter, located at 155 Hospital Street in Augusta, Maine. Police arrived and confronted 34-year-old Dustin J. Paradis, who was reportedly holding a chef's knife. Officers alleged that he walked toward them. Two officers opened fire and killed him. The Maine Attorney General's Office deemed the shooting justified.

On October 13, 2021, a Geary Police officer saw a 46-year-old man holding a knife near the I-40 and US-281 interchange in Hinton, Oklahoma. An off-duty officer driving past the scene stopped to assist, and a third officer arrived soon after. The third officer attempted to use a taser. Police allege that the man lunged at the off-duty officer with the knife when he saw the Taser. One of the officers opened fire, striking the man and fatally wounding him.

On October 13, 2021, off-duty NYPD officer Yvonne Wu was waiting at the Brooklyn apartment of her ex-girlfriend, Jenny Li, located near 19th Avenue and 79th Street. When Li returned to the apartment accompanied by 24-year-old Jamie Liang, Wu allegedly shot both women.

Jamie Liang was pronounced dead at the scene. Jenny Li sustained injuries but survived the shooting. Wu, who remained at the scene, was arrested by responding officers.

At approximately noon, officers were dispatched to an apartment complex for a family disturbance involving a gun. A caller reported seeing a man with a gun, shooting into the air and pointing the gun at a woman. The woman involved also called police and reported that Christopher Ryan Vasquez was trying to kill her. Officers arrived and Vasquez barricaded himself in his apartment. He occasionally approached the window and cursed at officers while holding one and sometimes two handguns. He also pointed the guns at his own head and at one point placed one in his mouth. He reportedly said he was not going to come out peacefully, threatened suicide, and stated he was going to kill himself. At approximately 6:41 p.m., officers sent gas into the apartment, and Vasquez fired his handgun toward an armored vehicle, and an officer fired one round at him. He fell away from the window and out of sight. Shortly after, additional gunfire could be heard from inside the apartment. A robot camera revealed Vasquez lying dead on the floor of the apartment with a gun nearby.

On 10/13/2021, the Pittsburgh Police Department, including Officer Keith Edmonds, responded to a report of an alleged bicycle theft in the Bloomfield neighborhood. Officers encountered Jim Rogers holding the bicycle in question. When Rogers did not comply with verbal commands, Officer Edmonds fired his Taser multiple times. Rogers was handcuffed and placed in a patrol vehicle, then transported to Mercy Hospital, where he was pronounced deceased on 10/14/2021. The Allegheny County Medical Examiner ruled his death an accident. Officer Edmonds was terminated following an internal investigation. The City of Pittsburgh settled a federal wrongful death lawsuit with Rogers' family for $8 million in April 2023.

On 10/13/2021, Michigan State Police were seeking Steven David Schumann, who was wanted for absconding parole related to a charge of felonious assault. When troopers approached Schumann's vehicle in the 9200 block of Lindsey Road in Plainwell, a woman riding with him became involved in the situation. Schumann allegedly produced a weapon, escalating the encounter into a hostage situation. Two troopers then shot and killed Schumann. The hostage sustained injuries during the encounter.

On October 12, 2021, at approximately 11:40 a.m., members of the Knoxville Police Department's Special Operations Squad were assisting agents from the Tennessee Bureau of Investigation's Drug Investigation Division and the 5th Judicial Drug Task Force in executing a search warrant at a residence in the 2900 block of Sevier Avenue. Upon arrival, officers encountered 43-year-old Bryan Calvin with a gun. Preliminary reports indicate that Lee pointed the firearm at officers. They engaged him in a shootout and killed him. Lee was struck and pronounced dead at the scene.

On October 12, 2021, at approximately 9:33 a.m., Las Vegas Metropolitan Police Department officers responded to an apartment on E Lake Mead Blvd for an assault and battery call. The victim's current boyfriend reported that her ex-boyfriend, Mario Lawrence Martinez, had been harassing them, fired shots, and forcibly entered the apartment to kidnap the woman at gunpoint. Martinez attempted to escape in her car, which wouldn't start, leading him to flee with her on foot. They were located near a shopping center at about 10:50 a.m. Martinez, still armed, did not comply with police orders to surrender, moving through the shopping center, and eventually breaking into a box truck in an alley, where he also shot out its window. During this standoff, Officer Ramon Hiestand and other officers engaged Martinez. After Martinez pointed his gun at officers, the woman escaped, and Hiestand shot him. Martinez continued to fire while on the ground. He fired until he shot and killed Martinez.

Officers pursued a man reportedly wanted for several felony offenses through multiple counties before he fatally shot himself in the head. Police said the man in his 20s was found in his vehicle with a self-inflicted gunshot wound at the end of the chase around 10 p.m.

A trooper pulled his marked cruiser into the path of a motorcyclist on I-4 around 7:30 p.m. The motorcyclist crashed into the front right side of the cruiser and was ejected from his bike. After falling into the road, the motorcyclist was struck by a semi-truck and killed.

On October 10, 2021, at approximately 9 a.m., Indiana State Police responded to a reported domestic disturbance at a residence on East County Road 900 North in Sunman, Indiana. The disturbance involved Joshua "Josh" Edward Ebinger, 37, who was reportedly violating a protection order issued against him by a woman.
Two Ripley County deputies, an Indiana State Police trooper, and a Versailles Police officer arrived at the scene. The woman exited the residence and was moved to a safe location.

On October 10, 2021, at approximately 6 p.m., Brunswick County Sheriff's Office deputies responded to a domestic disturbance at 8627 Wanets Landing Road in Leland, North Carolina. Linda Beck, the caller, reported that her husband, 57-year-old Michael Robert Beck, had fired a firearm in her direction twice and expressed concerns about his volatility. Upon arrival, deputies located Beck on the back deck of the residence. He reportedly produced a firearm and pointed it toward the deputies. Despite repeated commands to drop the weapon, Beck allegedly failed to comply. Two deputies discharged their firearms, fatally striking him.

On October 9, 2021, at approximately 2:46 a.m., Maryland Transportation Authority Police Officer Theodore Jeremenko observed a silver Chevrolet Monte Carlo commit a traffic violation near the intersection of I-395 and Conway Street in Baltimore, Maryland. Suspecting the driver might be impaired, Officer Jeremenko initiated a pursuit.

The driver, identified as 26-year-old Jawuan James Ginyard, initially pulled over on I-95 South but then fled as Officer Jeremenko approached his vehicle. The officer chased him onto I-695, with Ginyard reaching speeds exceeding 100 mph and weaving through traffic. Upon taking the Wilkens Avenue exit ramp, Ginyard lost control of his vehicle, which struck the median, causing him to be ejected from the car. Officer Jeremenko rendered first aid until paramedics arrived, but Ginyard was pronounced dead at the scene.

The Maryland Attorney General's Independent Investigations Division conducted a review of the incident. In April 2022, the Baltimore County State’s Attorney’s Office declined to file charges against Officer Jeremenko, determining that his actions were not grossly negligent and did not cause Ginyard's death.

Officers were responding to a DWI call a block away when they heard gunshots in the entertainment district at around 1 a.m. The officers arrived to find two groups of men shooting at one another. An officer began firing at the men. Michael "Mikey" Carothers was killed. It was not immediately known whether the teen was killed by officer-fired bullets or from the other group involved in the shooting.

At approximately 4:30 a.m. on October 9, 2021, a Middlesboro police officer shot and killed 35-year-old Timothy Houston. Houston was reportedly wanted in Claiborne County, Tennessee, on charges stemming from a previous shooting. The incident occurred in Middlesboro, Kentucky. On October 9, 2021, at approximately 8:00 p.m., officers from the Las Cruces Police Department initiated a traffic stop near the intersection of 5th Street and Wilson Avenue. During the stop, the driver, identified as 35-year-old Gustavo Angel Esparza, opened fire on the officers before fleeing the scene. A brief pursuit ensued, ending at 4th Street and Davis Avenue, where Esparza barricaded himself inside his vehicle. After a standoff lasting over two hours, Esparza allegedly shot at officers, prompting them to return fire, fatally wounding him. A civilian bystander was also injured during the incident and was hospitalized in stable condition. The incident is under investigation by the Dona Ana County Officer-Involved Incident Task Force.

On October 8, 2021, Officer Dustin Broughton of the Mexico Police Department responded to an unknown incident at 1167 Main Street in Dixfield, Maine, involving 29-year-old Matthew Allen Marston. During the unspecified confrontation, Officer Broughton opened fire and killed Marston. The State Medical Examiner determined that Marston's death resulted from a self-inflicted gunshot wound. On 10/08/2021, around midnight, Tulsa Police officers saw Tommy Carmichael, 46, driving a white truck in West Tulsa. Carmichael was a person of interest in the death of his mother, Carol Longacre, who had been found dead in her bathtub earlier that day. Officers followed Carmichael as he drove toward the home where Longacre’s body was discovered.
Carmichael arrived at the house near South Utica Avenue and East 61st Street and tried to enter the property. Before officers could apprehend him, Carmichael shot himself and was pronounced dead at the home.

On 10/08/2021, the Pennsylvania State Police and Coaldale Police Department were involved in a standoff with 47-year-old Richard Lee Sweet in Coaldale, Pennsylvania. Sweet was wanted in connection with the fatal shooting of his girlfriend, Joan Cressman, earlier that day in East Allentown. After the shooting, Sweet fled to his home on the 100 block of Fisher Avenue in Coaldale. Police escalated a large standoff that lasted over four hours. They used loudspeakers while Sweet locked himself inside. Around 4 p.m., neighbors heard a single gunshot from inside the residence. When police entered, Sweet was found with a self-inflicted gunshot wound and was pronounced dead.

On October 8, 2021, at approximately 8:51 p.m., Winston-Salem Police officers responded to a 911 call reporting that 46-year-old Gilbert Lee Collison Jr. had broken into a residence on South Main Street. The caller reported that Collison was armed with a shotgun and had threatened to shoot any law enforcement officers who attempted to intervene. Officers learned that Collison had previously pointed the shotgun at his girlfriend during an argument.

The Winston-Salem Police Department, along with SWAT and crisis negotiators, surrounded the residence and made repeated attempts to communicate with Collison. Despite these efforts, Collison fired shots at the officers. At approximately 2:30 a.m., Collison appeared in the front doorway with the shotgun. Officers used a less-lethal impact baton, striking him in the leg, but Collison responded by firing on the officers. Three officers returned fire, fatally striking Collison.

The North Carolina State Bureau of Investigation is conducting an independent investigation into the incident. The three officers involved have been placed on administrative leave, as is standard protocol for officer-involved shootings.

On October 8, 2021, deputies from the Chippewa County Sheriff's Office conducted a traffic stop on Lance Richard Stelzer, who was riding a moped in Goetz, Wisconsin. During the stop, a physical altercation broke out between Stelzer and Deputy Martin Folczyk. Stelzer allegedly sprayed Folczyk with wasp/hornet killer during the struggle. In response, Deputy Folczyk shot and killed Stelzer.

On October 7, 2021, at approximately 9:06 a.m., Cleveland Police Department officers responded to a mental disturbance call at a residence on the 700 block of Springhill Drive NE. Upon arrival, three officers encountered 71-year-old Lloyd Grant McClung Jr., who was armed with a shotgun. Despite repeated verbal commands to drop the weapon, McClung advanced toward the officers and fired a round from his shotgun. Two officers opened fire, striking McClung. McClung was transported to a local hospital, where he was pronounced dead.

On 10/07/2021, around 5: 45 p.m., Brockton Police Department officers responded to reports of a man with a gun at 62 Taber Avenue. in Brockton. Upon arrival, gunshots were exchanged, and a Brockton police officer was reportedly struck several times. The gunman, Kevin M. Serpa, then retreated into the home, where he locked himself for several hours while police attempted negotiations. At approximately 9: 30 p.m., Serpa was outside the home when he shot and killed himself.

On October 6, 2021, Rochester Police Department officers responded to an armed robbery in progress at a Family Dollar store located at 715 W Main Street in Rochester, New York. During the encounter, 24-year-old Simran S. Gordon exchanged gunfire with officers.

On October 6, 2021, officers from the New York City Police Department responded to an apartment on West 147th Street and Adam Clayton Powell Jr. Blvd. in Harlem to arrest 23-year-old Antonio Armstrong for his involvement in a recent domestic incident. When officers attempted to make contact with Armstrong, he allegedly opened fire on them.

Armstrong locked himself inside the apartment. Police escalated a standoff that lasted approximately three hours. The standoff ended when Armstrong reportedly shot and killed himself.

On October 6, 2021, at approximately 6:00 a.m., correctional officers with the Hardeman County Sheriff's Office were outside the Hardeman County Criminal Justice Complex when they were allegedly threatened by an armed individual. Three officers from the Bolivar Police Department responded, but the man had already fled. Shortly thereafter, the individual returned, exited his vehicle while still armed, and confronted the officers. The officers shot and killed 43-year-old Andra Davon Murphy.

Additional Information: The family of Andra Davon Murphy has retained attorney Ben Crump, asserting that Murphy was not the individual who initially threatened the correctional officers and is seeking the release of video footage to clarify the events leading to his death. There is no followup information at this time.

On October 6, 2021, U.S. Marshals attempted to serve an arrest warrant on Cody James Herman, who was wanted for charges including aggravated assault and weapons violations. As officers approached the home, they were fired upon, resulting in one agent being shot and wounded. Officers fell back, and negotiations began. Several children and adults exited the residence, and after continued negotiations, police entered the home and found Herman dead from a self-inflicted gunshot wound.

On October 6, 2021, around 1:10 a.m., officers from the Milwaukee Police Department attempted to stop a vehicle for reckless driving. The driver of the vehicle did not comply and fled the scene, leading to a police pursuit. During the chase, the fleeing vehicle struck and killed 34-year-old pedestrian Calob House. Officers reportedly called off the pursuit after the incident.

On 10/05/2021, at approximately 4:33 a.m., Pennsylvania State Police responded to reports of shots fired during a robbery at a rest area along I-80 in Loganton, Pennsylvania. Upon arrival, officers encountered a male individual who had barricaded himself inside his rig and refused to comply with police orders. The individual drove his rig into two police vehicles. Officers opened fire and shot and killed him.
The Pennsylvania State Police are conducting an internal self-investigation.

Tom Sharkey was wanted for questioning regarding the murder of his wife, 26-year-old Alexis Sharkey. He was found shot in the head inside his daughter's Fort Myers home. At approximately 10 p.m., police initially spoke with Sharkey, but he ran upstairs and shot and killed himself.

On October 5, 2021, at approximately 2:15 p.m., agents from the U.S. Bureau of Alcohol, Tobacco, Firearms and Explosives (ATF) were attempting to arrest 40-year-old Corey Daniel Wellman in connection with a federal drug investigation. The incident occurred in the parking lot near the Country Café at 605 Murfreesboro Pike in Nashville, Tennessee. During the attempted arrest, police engaged him in a shootout, resulting in the death of Wellman and the critical injury of an ATF agent, who was transported to Vanderbilt University Medical Center for treatment. No Metro Nashville Police Department officers were injured or discharged their weapons during the incident.

Additional Information: Wellman had an active federal warrant for his arrest related to a months-long drug investigation initiated in June 2021, which included undercover heroin purchases. Family and friends have questioned the circumstances of the arrest, expressing concerns about the methods used by the ATF agents and whether they properly identified themselves during the operation.

On 10/05/2021, at approximately 2: 20 p.m., a domestic disturbance was reported at 29 Grant Street in Taunton, leading to a response from the Taunton Police Department. Upon arrival, a person had barricaded themselves inside the home, and it was reported that there was a firearm present. Officers attempted to communicate with the individual for several hours. Around 8 p.m., police entered the home and discovered Peter M. Rose deceased, having apparently committed suicide.

On October 4, 2021, at approximately 3:30 a.m., Walton Police officers responded to a mental health call at a residence on Griswold Street in Walton, New York. Upon arrival, the responding officer made contact with 66-year-old Paul J. Weeden inside the home.
During their interaction, Weeden reportedly pointed what was believed to be a firearm at the officer. The officer ordered Weeden to drop the weapon. When he allegedly failed to comply, the officer shot and killed Weeden.
