On September 23, 2021, a deputy from the Tate County Sheriff's Office attempted to stop Keith Cole, who was wanted on a domestic violence warrant, on Highway 4. Cole pulled over, exited his vehicle, and fled into the woods. The deputy pursued and found Cole hiding behind a tree. Police allege that Cole emerged with his “hand on his waistband” and the deputy shot and killed him. No weapon was found on Cole after the incident. Police did not provide video or other evidence to document the narrative.

On September 22, 2021, 19-year-old Daisha Reynolds was killed during a high-speed police pursuit that began in Wasatch County and ended in Summit County, Utah. A Wasatch County deputy located a stolen vehicle on State Route 113 and attempted a traffic stop. The driver did not stop. Police escalated a chase through Midway. The chase was temporarily halted twice when deputies lost visual contact. The pursuit resumed on River Road and continued onto U.S. Route 40, with speeds exceeding 95 mph.
Utah Highway Patrol troopers deployed a tire-deflating device on U.S. 40 to slow the fleeing vehicle. A Summit County Sheriff's deputy attempted a second deployment of tire spikes near mile marker 2. The driver swerved to avoid the spikes, lost control, and the vehicle rolled multiple times.
Reynolds, the passenger, was ejected from the vehicle and pronounced dead at the scene.
A multi-agency critical incident investigation was initiated, involving the Utah Attorney General’s Office, West Valley Police Department, and Park City Police Department.

On 09/22/2021, the Ramsey County Sheriff's Office responded to a call for a felony domestic assault. Troy Allen Engstrom, who was the man identified, shot at pursuing officers, hitting one officer's vehicle just above the driver's side windshield and a Ramsey County deputy's vehicle just below the driver's side windshield. Ramsey County Deputy Donald Rindal rammed Engstrom with his car, killing him.

On September 21, 2021, at approximately 11:29 a.m., the Cayuga County 911 Center received a report of shots fired in Auburn, New York. Officers from the Auburn Police Department responded to the scene at 12 Wheeler Street. Upon arrival, officers were met with gunfire coming from an upstairs window of the residence. In response, officers opened fire, fatally shooting 30-year-old Brandi Rene Baida.

Upon police arrival, two individuals fled the scene. The female driver initially stopped but allowed the male passenger, identified as 39-year-old Noah Daniel Boswell, to drive off. Police escalated a chase and placed stop sticks. Boswell drove through the stop sticks. When he stopped, police escalated a 15-hour standoff on the Natcher Bridge near Maceo, Kentucky. Police alleged that he eventually pointed a gun at them. Boswell allegedly died from a self-inflicted gunshot wound. On September 21, 2021, officers from the Las Vegas Metropolitan Police Department were dispatched to South 11th Street and Lewis Avenue due to reports of a man with a gun. The man, Trent Xavier Leach, ran into an abandoned building. Per police, Leach allegedly began shooting indiscriminately into the surrounding area. Officer William "Bill" Jeffrey and Sgt. Michael Vance arrived at an apartment complex to arrest 30-year-old Deon Ledet just before 7:30 a.m. The officers were serving a warrant for Ledet's arrest for skipping bond on two felony drug charges from November 2020. After the officers announced their presence, Ledet opened fire from inside the apartment. Ledet reportedly was armed with a handgun and an assault rifle. Ledet was killed. Jeffrey, 54, died of multiple gunshots. Vance, 53, was wounded and expected to survive.

On 09/20/2021, at approximately 12:55 p.m., Signal Hill Police Department officers responded to a report of a stabbing at a grocery store in the 1600 block of East Willow Street in Signal Hill, California. Upon arrival, officers found a man who had been stabbed in the upper torso. He was transported to a hospital in fair condition. Witnesses provided a description of the attacker, later identified as 42-year-old Phillip Lopez, who was reportedly fleeing westbound on Willow Street. An officer located Lopez and attempted to detain him.

The officer exited his vehicle to pursue on foot. Lopez produced a large knife and moved toward the officer. The officer opened fire, striking Lopez. He was transported to a hospital, where he was pronounced dead.

Adrian Lamont Cameron Sr., a 47-year-old Black male, was wanted for a state parole violation and was being questioned in a homicide case. Police located him at a Days Inn motel in Nashville, Tennessee, on September 18, 2021. After a woman exited the motel room and was escorted away, Cameron emerged with an AR-15 and fired at officers, who shot and killed him.

James Andrew Black, aged 27, died on September 18, 2021, following a high-speed police chase in Phippsburg, Maine. During the pursuit, Black's vehicle crashed into a utility pole near 610 Main Road during a high-speed chase for undisclosed reasons conducted by Sagadahoc County Sheriff's Office. He was pronounced dead at the scene.

On September 18, 2021, at approximately 3:20 a.m., an off-duty U.S. Park Police officer and a female companion were confronted by a group of four individuals, allegedly led by John Shortt, as they left a nightclub near 1440 Utica Avenue in Brooklyn, New York. Shortt and another man wearing a balaclava reportedly approached the officer and his companion and pointed a handgun at them.
The officer drew his own weapon and fired, striking Shortt. Two other individuals, identified as Tyshawn Whidbee and Joshua Cooper, then approached from another direction. Whidbee allegedly pulled out a gun and began firing at the officer and the woman, hitting both of them. The officer fatally shooting Cooper.

On 09/18/2021, the Union Township Police Department responded to a situation involving Christopher E. Rush on the 3800 block of West Main Street in Hillsville, Pennsylvania. Around 8:45 p.m., officers and troopers from several local police departments arrived to find Rush armed with a knife. Rush reportedly stabbed several victims and assaulted a Mahoning Township police officer. An officer from the Union Township Police Department opened fire, fatally shooting Rush. Andrew "Drew" Brandon Christian, a 34-year-old White male, was killed by police during an arrest attempt on September 16, 2021, at a residence on Ptarmigan Trail in Memphis, Tennessee. At approximately 12:40 p.m., officers from the Memphis Police Department and Shelby County Sheriff's Office attempted to serve arrest warrants at the home. Upon entering the residence, officers were fired upon. Officers opened fire in response. Officers exited the house, with two sustaining injuries after jumping through a window. Deputies later entered the home and found Christian dead from a gunshot wound.

Deputies were dispatched after receiving a call about a disturbance and a person with a weapon. When deputies arrived, they spoke with the 911 caller who confirmed that the man was a family member and still inside the home. On September 15, 2021, members of a U.S. Marshals' task force attempted to serve an arrest warrant at 6 Vinewood Place in Rochester, New York, for 24-year-old Dedrick Jamon James. As officers attempted to apprehend him, James allegedly returned back into the residence and struggled with officers.

On September 14, 2021, the Waukesha Fire Department responded to a medical call about Paul J. Bruner, 58, who was having difficulty breathing. Police were attempting to arrest Bruner for domestic abuse as he was under a domestic abuse injunction order. Police said Bruner was uncooperative and refused medical attention. The woman involved in the incident had visible minor injuries. After determining that domestic violence had occurred, officers attempted to arrest Bruner. He allegedly became combative, and officers used a stun gun (electronic control device) and physical force to take him into custody. Bruner later died during the arrest.

On 09/14/2021, at approximately 1:35 a.m., two Mantua Township police officers responded to a 911 call at a residence on Elm Avenue. During an encounter outside the home, one of the officers shot and killed Charles Frederick Sharp III, 49. Police reported that a .45-caliber handgun was recovered near Sharp. An FBI agent deputized as a U.S. Marshal was attempting to serve federal and state arrest warrants on Michael Jonathan Cortez for unknown reasons. The agent arrived at a store, exited his vehicle with his weapon drawn, and entered the store. Surveillance footage does not capture events inside the store. According to reports, the agent gave Cortez unknown commands. Cortez attempted to flee, bumped into the agent, and allegedly pulled a gun, resulting in the agent shooting him dead. Inconsistencies were noted between the FBI agent’s report and physician accounts of the entry wound. The FBI has not disclosed the agent's identity or provided the store’s security footage.

Deputies arrived after 1:30 a.m. at a game room near Channelview. One man was killed near the entrance. It was uncertain if he was wounded during an exchange of gunfire with a deputy or earlier during a possible encounter with a security guard, but since he was alive during the exchange, police were present during his death.

On September 13, 2021, a Georgia State Patrol trooper attempted to stop a vehicle driven by a neighbor transporting 12-year-old Le’Den Boykins and another child home from church. The driver refused to comply and continued on Interstate 20. Troopers escalated a chase that reached speeds of approximately 90 miles per hour. Despite knowing minors were in the car, one trooper performed a Pursuit Intervention Technique (PIT) maneuver, causing the vehicle to flip several times.
The driver was Charlie Moore, a neighbor who regularly took Le’Den and another child to church. According to the Georgia State Patrol incident report and coverage by 11Alive and WXIA, troopers originally stopped Moore for reckless driving and suspected DUI after he allegedly refused to lower his window and refused to produce ID. Troopers claimed that they reported smelling alcohol and seeing “erratic movements” in the vehicle.
When Moore refused to comply, one trooper said:
“If he drives off, we’ll chase him.”
A few seconds later, Moore accelerated away, initiating the chase.
Le’Den Boykins was killed instantly. The dash-cam video released in January 2023 showed troopers acknowledging that children were in the vehicle moments before the PIT maneuver.

No blood-alcohol test results were ever released.
Court records do not show a confirmed BAC in the case file, and the family’s attorney has said repeatedly that Moore was not intoxicated. The state did not pursue a DUI conviction; instead, he was charged with felony murder, vehicular homicide, and fleeing and eluding.
Notoxicology evidence proving the allegations of intoxication has appeared in any of the filings reviewed or in media coverage through 2024. The state cleared the troopers of criminal wrongdoing.
The footage sparked statewide outrage and renewed scrutiny of Georgia’s pursuit tactics, particularly the use of high-risk maneuvers on vehicles carrying passengers. Boykins’ family publicly demanded criminal accountability and filed notice of intent to sue the state. The Georgia State Patrol stated that its internal investigation found troopers acted within current pursuit policy, though the agency confirmed that policy review discussions are ongoing.

On 09/13/2021, Las Vegas Metropolitan Police responded to a restaurant for a report of a man armed with a knife who was attempting to start a fight with an employee. Around 12:02 a.m., officers were doing a routine drug patrol when they approached a man in his vehicle. The man, Rodolfo Gonzales, noticed officers approaching, and he reportedly tried to avoid arrest. Gonzales allegedly tried to pull a handgun from his waistband, and the officers shot and killed him.

On 09/12/2021, at approximately 10:15 p.m., two Gloucester Township police officers pursued a driver suspected in a hit-and-run incident but reportedly broke off the pursuit. The fleeing driver was killed.

On September 11, 2021, around 3:30 a.m., Lawton Police officers attempted to stop a black Chevrolet Camaro for alleged reckless driving. The driver, 21-year-old D'monte L. Harris, did not stop. Police escalated a high-speed chase that continued until Harris lost control of the vehicle at the intersection of SW Sheridan Road and SW Jefferson Avenue, resulting in a fatal crash.

The pursuit was initiated by officers, but they reportedly lost sight of Mott's vehicle. Shortly after, Mott's vehicle crashed into another vehicle, causing the car to jump the curb and strike 3-month-old ApollineMong-Guillemin, who was being pushed in a stroller by her mother. Tragically, Apolline was killed in the incident.

A deputy pulled over an SUV, and one of the passengers bailed out of the car and took off on foot with a backpack. The deputy called for backup and another deputy began a chase on foot. William Frank Hull allegedly shot at the deputy, who shot and killed him.

On September 11, 2021, deputies from the Lincoln County Sheriff's Office, along with state police, responded to a domestic violence call along Spring Canyon Road near Ruidoso. Upon arrival, the man fired shots at the officers. After hours of negotiations, deputies heard gunfire from within the residence. Upon entering, they discovered the man deceased from a self-inflicted gunshot wound.

On September 10, 2021, at approximately 12:45 p.m., Corporal John Diggs of the Prince George's County Police Department, who resided at an apartment complex in the 1100 block of Owens Road in Oxon Hill, Maryland, was informed by a complex employee that Cedric Ricardo Williams, 29, who had been previously banned from the property, was on the premises. In full uniform, Corporal Diggs approached Williams and instructed him to leave. Williams refused, leading to a verbal confrontation that escalated into a physical struggle outside the building and onto a roadway.

During the altercation, Williams retrieved a handgun from his bookbag. The weapon allegedly malfunctioned which was not specified further. Williams then discarded the handgun into a wooded area. Corporal Diggs fired his Taser twice in an effort to subdue Williams. Williams retrieved what appeared to be a gun from the wooded area and pointed it at Corporal Diggs. Corporal Diggs discharged his service weapon multiple times, fatally striking Williams.

Upon investigation, both weapons possessed by Williams were all determined to be realistic replicas. The Prince George's County Police Department's Internal Affairs Division is leading the investigation, with oversight from the Prince George's County State's Attorney's Office.
