On November 4, 2021, U.S. deputy marshals attempted to serve a warrant on a man who had two outstanding warrants for armed burglaries and felony firearm charges. As deputies approached the man, he allegedly pulled out a firearm and shot and killed himself.

At approximately 12:52 p.m. on November 3, 2021, LaPorte County deputies initiated a pursuit of a reportedly stolen 2020 Infiniti QX60. The vehicle was traveling at high speeds and avoided stop sticks placed on the roadway. The chase continued into Lake County, where the Infiniti reportedly collided with a Porter County Sheriff's vehicle. The impact caused the driver, 23-year-old John Kennedy Fitzgerald Bolden, to lose control, resulting in the vehicle hitting a median wall and rolling over multiple times. The vehicle came to a stop on the opposite side of the median wall. Bolden and a passenger were ejected from the vehicle. Bolden was killed, and the passenger sustained injuries.

At approximately 1:45 a.m. on November 3, 2021, Kentucky State Police troopers responded to a 911 call at the 800 block of Chipman Ridge Road in Williamstown, Kentucky. Upon arrival, troopers encountered 48-year-old Gary Wayne Bressler Sr., who was reportedly in a mental health crisis. Bressler was holding a decorative samurai sword. Despite commands to drop the weapon, Bressler did not comply, leading to the troopers firing their weapons, fatally wounding him. The KSP Critical Incident Response Team is handling the investigation into the incident, and the identities of the involved officers have not been released.

On November 3, 2021, Eau Claire Police responded to reports of a break-in at a residence where the man was allegedly armed with a knife. LeKenneth Q. Miller reportedly stabbed a woman inside the home. Upon arrival, officers confronted Miller and shot and killed him during the encounter.

On 11/03/2021, John Kennedy Fitzgerald Bolden was driving a stolen vehicle on I-94 when law enforcement attempted a traffic stop and initiated a chase; the pursuit continued into Lake County where the fleeing vehicle struck a Porter County Sheriff’s cruiser, then the median wall and rolled over, ejecting Bolden, who was pronounced dead at the scene.

On November 3, 2021, a Wyoming Highway Patrol trooper stopped a Chrysler van on I-80 near Green River. A second trooper arrived, and after speaking with the driver, the individual jumped back into the van and fled. Troopers pursued the vehicle at speeds exceeding 120 mph. The pursuit ended when the van collided with a commercial belly dump truck, killing the driver.

Police said that Kody Marshall, who had been involved in a prior shooting, was then involved in a major accident on the Crosstown Expressway. Police said he immediately fled the accident. Police later found him near a high school, and he reportedly fired at officers. Police returned fire, killing Marshall.

On November 1, 2021, at approximately 4:11 p.m., the Houma Police Department responded to a disturbance call at Downtown Court and East Main Street in Houma, Louisiana. Upon arrival, the first officer encountered 36-year-old Johnny McGee, who reportedly charged at the officer while holding a screwdriver. The officer issued commands to stop and attempted to create distance; however, McGee continued to advance. A second officer arrived on the scene as McGee allegedly raised his arm with the screwdriver toward one of the officers, prompting an officer to discharge his service weapon. McGee was struck by gunfire and transported to a nearby hospital, where he succumbed to his injuries. Family members later revealed that McGee had been diagnosed with bipolar disorder and schizophrenia and was experiencing a mental health crisis at the time of the encounter.

On 11/2021, a 12-year-old boy was killed in South Gate after a high-speed crash involving an off-duty Los Angeles County Sheriff’s deputy. Authorities said Deputy Ricardo Castro was driving at a high rate of speed when his vehicle collided with another car, killing the child. Investigators determined Castro was not responding to a call at the time of the crash. He was later charged with murder and reckless driving in connection with the child’s death.
Prosecutors said Castro had been involved in several previous vehicle collisions before the South Gate crash. He had also received multiple traffic citations — including for speeding — during his time before the fatal crash. The prosecutor’s narrative and the fact that it was included in official comments suggests a pattern of risky driving and citations before the deadly collision. In 2024, Castro pleaded no contest to vehicular manslaughter with gross negligence. The murder charge was dropped as part of the plea. He was sentenced to six years in state prison.

On 10/31/2021, Los Angeles police responded to calls about a man slashing car tires and acting erratically near West Victory Boulevard. Officers located Melkon Michaelidis armed with two knives. Police repeatedly ordered him to drop the knives as he walked toward them. He refused and continued toward officers while holding the blades. Officers then opened fire, using both less-lethal rounds and live ammunition, and shot Michaelidis, killing him at the scene. The Los Angeles County District Attorney reviewed the shooting and concluded the lead officer acted in lawful self-defense of another person based on the totality of the circumstances, including Michaelidis’ actions toward others prior to police arrival.

On October 31, 2021, Pennsylvania State Police responded to a call at approximately 5:11 p.m. in the 100 block of Gilberg Lane, West Hanover Township, Pennsylvania. Upon arrival, troopers encountered Glenn William Custer, a 59-year-old white male, who was behaving erratically and armed with a knife. Custer reportedly refused commands to drop the weapon and began moving toward occupied residences. He then walked back toward the residence where the incident began and approached a trooper. In response, the trooper opened fire, fatally shooting Custer. The Pennsylvania State Police are handling the case and internal self investigation.

Officers located Riggs' vehicle in the 1000 block of Cottage Stone Lane and initiated a traffic stop. During the encounter, officers allegedly heard a gunshot. Police open fire killing Riggs. It remains unconfirmed, whether he fired any shots or not. The Tennessee Bureau of Investigation (TBI) conducted an independent investigation into the circumstances leading to the shooting. On 10/29/2021, an off-duty Massachusetts State Police trooper drove a vehicle on Interstate 93 in Boston while intoxicated. The trooper struck a highway median and came to a stop in the roadway. Christopher Zike, who was riding a motorcycle, then crashed into the trooper’s disabled vehicle and was killed. Prosecutors charged the trooper with motor vehicle homicide and operating under the influence. He later pleaded guilty and was sentenced to approximately two and a half years in prison.

Walden Police conducted a traffic stop, but as officers approached the vehicle, Schields fled the scene.

At approximately 4 a.m. on October 29, 2021, dispatchers received a 911 call reporting individuals in a Jeep SUV threatening to shoot the caller. Terre Haute Police attempted to stop the Jeep, but the driver, 35-year-old Shawn Michael Passwater, allegedly fled. Officers pursued the vehicle until it crashed at S 13th Street and Lockport Street. Passwater was ejected from the SUV and killed. A passenger in the vehicle sustained injuries during the crash.

At about 11 p.m., Montgomery County deputies chased Justin Frostad, who was traveling eastbound on County Road 3875 in a 2007 Kia Optima. Police never disclosed the reason for the pursuit. Frostad lost control of his vehicle after crossing the railroad tracks, veered off the road, and flipped several times. The vehicle came to rest on its roof in a field, where Frostad died from injuries.

On October 29, 2021, Clinton police attempted to intercept a man accused of stealing a truck from Jackson and driving into Clinton. The man refused to stop, leading to a chase that ended with him returning to Jackson. On 10/28/2021, Colorado Springs police responded to reports of a man firing a gun into a vehicle in the area of South Wahsatch Avenue. Officers later located Ahmad Akeem Abdul Muhammad on foot. Police stated that during the encounter Muhammad raised a firearm in the direction of officers. Officers opened fire, and Muhammad continued moving away from police. Officers located him again a short distance later. Police reported that Muhammad again raised a firearm toward them, and officers opened fire a second time, killing him. The El Paso County District Attorney reviewed the shooting and ruled the officers’ actions justified.

On 10/27/2021, Boise police responded to reports of a man threatening to jump from an overpass near West Myrtle Street and South Capitol Boulevard. Officers located Zachary Snow on the overpass. Police stated that Snow moved away from officers while pointing a dark object in their direction. Officers then opened fire, shooting and killing Snow at the scene. Prosecutors later cleared the officers of criminal wrongdoing. On October 26, 2021, Dulce Maria Castro Perez, 21, of Hudson, Colorado, was riding in a vehicle struck by a stolen car during a Brighton police pursuit. The drive, ran a stop sign at the intersection of Bridge Street and Mt. Bierstadt Street and crashed into the vehicle she was in. Perez was ejected and died from her injuries at the hospital.

On October 26, 2021, Gustavo Mosqueda Ortega, 25, of Montrose, Colorado, was a pedestrian struck and killed during a Brighton police chase. Police had chased a vehicle they believed to be stolen which hit Ortega along Mt. Bierstadt Street. Ortega was taken to a hospital but died from his injuries.

Officers were sent to an apartment on River Bend Drive in response to a domestic disturbance call at approximately 12:35 a.m. Foster had a gun. Officers arrived, and during the encounter, Foster became agitated and reached into a pile of clothing on a dresser for a gun. He fired at Officer Tom Seabolt, striking his police radio. Officer Timothy Lanham opened fire and killed Foster. The incident occurred in the presence of Foster's wife and children. The Kentucky State Police Critical Incident Response Team handled the investigation and shielded the names of the officers. The family expressed concern about the lack of transparency and the absence of body camera footage.

On 10/26/2021, Lima Police Department officers escalated a chase to speeds exceeding 100 mph while traveling westbound on Koop Road following a reported shoplifting incident at Walmart. On 10/26/2021, Lake County Sheriff’s Office deputies were dispatched to a residence near Leroy Center Road and Taylor Road after Scott Wright reportedly told dispatchers he was going to kill someone. Upon arrival, deputies encountered Wright allegedly restraining a 60-year-old woman at gunpoint. Wright released the woman and returned inside the home.

Deputies held the home for the next 4 to 5 hours. They stated that Wright appeared intermittently in the windows and the garage armed with a long gun and a handgun. Around 9 a.m., six hours into the standoff, Wright allegedly began setting fires inside the home. He exited the smoke-filled residence into the backyard with a long gun. Deputies fired beanbags and alleged that Wright raised his rifle. Deputies opened fire, killing him.

On 10/25/2021, DeKalb police responded to a reported domestic disturbance at a residence on Tilton Park Drive. Officers reported that Kristopher Kramer had been drinking and was inside the home during the response. Police said Kramer had injured himself prior to their arrival. After several minutes of communication, Kramer opened the door. Police reported that Kramer held a bladed object and moved toward officers. An officer deployed a Taser and a bean bag round, and another officer then opened fire, fatally shooting Kramer. The shooting was reviewed and later cleared.

On 10/25/2021, Jacob Bergquist opened fire inside the Boise Towne Square shopping mall, killing two people and wounding four others. Officers responded to reports of an active shooter and confronted Bergquist outside the mall. Police reported that Bergquist exchanged gunfire with responding officers in the parking lot area. Officers shot and killed him at the scene. The Ada County Coroner later identified Bergquist as the shooter. The incident prompted a large law enforcement response and evacuation of the shopping center.

Erik Black allegedly stole an 18-wheeler with a flatbed trailer. He fled police and jumped out of the truck as it was still in motion. The truck struck a home. Two people who were in bed inside the residence were struck by the truck. A man was injured, and Aylene "Nicole" Heaney was killed. Black was charged with murder and reckless aggravated assault. The company that owns the truck was able to use a GPS to locate the truck.

On October 25, 2021, a 78-year-old man contacted Mitchell County dispatch to report that he had been kidnapped at gunpoint and held for 26 hours by an acquaintance, John Roudybush, 59. When officers attempted to serve a search warrant at Roudybush's home located at 2446 190 Road in Glen Elder, Kansas, he refused to exit the residence. After an hours-long standoff, officers heard a gunshot from inside the home. Upon entry, they found Roudybush deceased from a self-inflicted gunshot wound.

Corey Stanley was wanted in connection with a home invasion and assault, accused of breaking into a couple's home and beating them with a rock. Police found him outside a white sedan parked in a parking lot. As officers approached, Stanley got back into the driver's seat and allegedly drove toward them, prompting police to open fire, killing him.

On 10/25/2021, at approximately 3:31 p.m., Mansfield Police Department officers responded to a 911 call reporting that a woman had fled her residence after being shot multiple times by her husband, Robert Neibarger. The woman, seeking help from a neighbor, sustained serious gunshot injuries.

Police surrounded Neibarger's home and initiated an hours-long standoff, attempting to communicate with him to de-escalate the situation. Despite these efforts, Neibarger did not respond. At approximately 6:45 p.m., officers entered the residence and found Neibarger deceased from a self-inflicted gunshot wound.

The incident, categorized as a suicide under police duress, remains under investigation by the Mansfield Police Department.

On October 25, 2021, Oklahoma City Police attempted to stop a vehicle for a traffic violation near I-44 and SW 104th Street. The driver, identified as 16-year-old Milton Neal, fled. Police escalated a high-speed pursuit. Police allegedly called off the chase immediately before Neal ran a red light at the intersection of SW 104th Street and S Pennsylvania Avenue, colliding with bystander Sean Donahue's vehicle. Donahue, 35, was driving a company vehicle for Diagnostic Laboratory of Oklahoma at the time and was killed instantly. Neal and his two passengers sustained non-life-threatening injuries and were transported to a local hospital.
