NARRATIVE:
On 04/28/2021 at approximately 8:30 p.m., Lyndon Fitzgerald Williams, 53, was crossing the four-lane Dr. Martin Luther King Jr. Boulevard near Caswell Street when Kinston Police Officer Treyston Sean Johnson struck him with a marked police cruiser. Williams, who was homeless, was well known to people in the neighborhood and was frequently seen walking in the area. He died at the scene.

Kinston police initially said Johnson may have been responding to a call but could not confirm whether he was actively responding when he struck Williams. Johnson suffered non-life-threatening injuries and was placed on administrative leave while the North Carolina State Highway Patrol investigated.

Witness Darius Howell disputed the later official account of Johnson's speed. Howell said the cruiser passed him immediately before striking Williams and estimated it was traveling 55 to 60 mph. The Highway Patrol's completed crash report instead estimated Johnson's speed at approximately 35 mph. Investigators reported that Williams was crossing mid-block, was wearing dark clothing and that a streetlight in the area was not working.

Williams' family publicly questioned the circumstances of his death, while community members remembered him as a familiar presence in the area. The Highway Patrol ultimately did not charge Johnson. Kinston police returned him to full duty after the investigation, with the city's interim police chief later stating that Williams was considered at fault because he was not using a crosswalk.

Treyston Sean Johnson remained a Kinston police officer after the fatal collision and was still employed by the city in 2022. In July 2024, he was hired and sworn in as a police officer in Muncie, Indiana. His killing of Williams resurfaced publicly after the hiring; Muncie Police Chief Nathan Sloan said Johnson disclosed the fatal collision during his hiring interview and defended his qualifications to serve.

NARRATIVE:
On 04/28/2021 at approximately 8:20 p.m., 5-year-old Sariah Marie Sheppards was riding in a 2003 Honda Accord driven by her mother, La'Shae Northington, when a fleeing 2007 Scion ran a red light at East 9th and Sutro streets and crashed into their car. Sariah suffered fatal injuries and died several hours later at Renown Regional Medical Center. Her mother survived.

Police had attempted to stop 33-year-old Jesse Allen Bennett of Eugene, Oregon, because he had felony warrants from Oregon involving domestic violence and attempted kidnapping. Both Sparks and Reno police were involved in efforts to stop Bennett before the crash. Police later maintained that officers were no longer actively pursuing the Scion at the moment it struck Sariah's mother's car.

Bennett ran the red light and struck the Honda. He and his passenger abandoned the Scion and fled on foot. Bennett was captured and arrested; authorities reported charges including DUI and possession of drugs for sale in addition to charges arising from the fatal crash.

Two days later, Sariah's mother joined dozens of family members, friends and community members at a vigil at the crash site. Balloons, flowers and candles covered the intersection. A motorcyclist who witnessed the crash followed Bennett after he fled and provided police information that helped locate him; Sariah's family publicly thanked him. Family remembered Sariah as an exceptionally bright, affectionate child whose smile drew people to her.

Sariah's mother subsequently sued Bennett and the cities of Reno and Sparks. The lawsuit alleged that police negligence contributed to Sariah's death and challenged the agencies' handling of the attempt to apprehend Bennett. The case reached the Nevada Supreme Court in 2022 after the district court dismissed the claim against Sparks. The Supreme Court dismissed Northington's appeal on procedural grounds because claims against Bennett and Reno were still pending, meaning there was not yet a final appealable judgment resolving the entire case.

At approximately 9:09 p.m., a Shawnee Township police officer attempted to stop Hunnaman as he traveled north on State Route 501 on a 2007 Honda CB900 motorcycle. On 04/27/2021 shortly before 1:00 a.m., Carlos Lopez-Melendez, 49, allegedly began a series of apparently random shootings from a white Jeep Cherokee in Los Angeles. A driver near Figueroa Street and Exposition Boulevard was shot and survived. About 10 minutes later, Lopez-Melendez allegedly shot and killed 24-year-old Alexis Carbajal as Carbajal sat with his wife in a Starbucks drive-through near Figueroa and 28th streets. Minutes later, 42-year-old Uber driver Mingzhi Zhu was shot and killed in his Cadillac Escalade near Figueroa and 7th streets while on his way to pick up a passenger.

At approximately 1:24 a.m., LAPD Transit Services Division officers near Washington Boulevard and Maple Avenue heard gunfire and saw the Jeep leaving at high speed. Officers followed and reported seeing Lopez-Melendez fire into a white Tesla. Police broadcast an emergency call and initiated a pursuit as additional units and an LAPD helicopter joined.

The pursuit continued for nearly three hours over several Southern California freeways. Police alleged Lopez-Melendez fired from the Jeep during the pursuit. California Highway Patrol officers eventually deployed spike strips, flattening three tires. Lopez-Melendez continued driving until the Jeep became disabled on the westbound 91 Freeway at the Raymond Avenue overpass in Fullerton at approximately 3:20 a.m.

LAPD SWAT responded and armored vehicles boxed in the Jeep. Police attempted to negotiate with Lopez-Melendez, including an unsuccessful attempt to have a family member speak to him. Officers also deployed flashbangs and tear gas. Police said Lopez-Melendez eventually fired through the Jeep's windshield at SWAT officers.

LAPD SWAT Officer James Brown returned fire. Police said Lopez-Melendez fired again and Brown fired additional rounds, striking him multiple times. Lopez-Melendez was pronounced dead at the scene. Police recovered a handgun from the Jeep.

Investigators later searched Lopez-Melendez's home and found additional firearms, including an AR-15-style semiautomatic rifle. Police investigated whether racial hatred played a role because two of the people targeted were Asian, but officials initially described the shootings as apparently random and did not establish a motive.

LAPD's Force Investigation Division investigated the fatal shooting, with the Office of the Inspector General monitoring the investigation. The case was also subject to review by the police chief, Board of Police Commissioners and Los Angeles County District Attorney's Office.

On 04/27/2021 at approximately 3:20 p.m., Harris County sheriff’s deputies pursued a male driver in a stolen vehicle in northeast Houston. The pursuit traveled onto North McCarty Street. Contemporary reports did not disclose where deputies first encountered the vehicle or the circumstances through which police determined it was stolen.

In the 4500 block of North McCarty, the driver lost control and crashed into a utility pole. The vehicle erupted in flames and the driver died at the scene. He was the only person in the vehicle, and no additional deaths or injuries were reported.

On 04/27/2021, Bakersfield police shot and killed 20 year old Dalton James Gerrit Kooiman following an attempted traffic stop in southeast Bakersfield. Police attempted to stop a vehicle they said was stolen. The driver did not stop and continued driving before crashing the vehicle near Feliz Drive.
After the crash, officers attempted to take Kooiman into custody. During the encounter that followed, police stated that Kooiman fired a weapon toward officers. Officers opened fire and killed him at the scene.
The Kern County District Attorney reviewed the shooting and ruled the use of deadly force justified. No officers were charged.

On 04/26/2021, 42-year-old Robyn Naftel Herring of Homewood was killed when a Nissan Juke fleeing a Vestavia Hills Police Department officer crashed into the Jeep Wrangler in which she was riding on Lakeshore Parkway near Interstate 65 in Homewood, Alabama.

Herring was riding in the front passenger seat of the Jeep. Her boyfriend was driving, and her two young daughters were riding in the back seat. They were traveling near Herring's home when the police pursuit reached Lakeshore Parkway.

The fatal sequence began at approximately 6:35 p.m. when a Vestavia Hills police officer stopped a Nissan Juke for speeding on northbound Interstate 65 near Alford Avenue. According to police, the officer obtained the driver's license, registration and insurance information and walked back toward the patrol vehicle to conduct a wanted-person check. The driver then drove away and the officer pursued.

The pursuit continued north on Interstate 65 toward the Lakeshore Parkway exit. After approximately a quarter-mile, the fleeing vehicle exited toward Lakeshore Parkway, crossed a median, became airborne and crashed into the Jeep carrying Herring and her family.

Herring sustained fatal injuries and was pronounced dead at the crash scene. Her boyfriend and two daughters survived. The Vestavia Hills officer stopped at the wreck, rendered first aid and requested emergency medical assistance.

The fleeing driver got out of the wrecked Nissan and ran into nearby woods. Police searched the surrounding area, and the U.S. Marshals Task Force subsequently located him at an apartment complex in Tuscaloosa. He surrendered on 04/29/2021 after approximately 30 minutes of negotiations.

Herring was a nurse practitioner at UAB and had worked in nursing for approximately 20 years. She earned bachelor's and master's degrees in nursing from UAB and worked in gastrointestinal surgery. She was the mother of two daughters, Anna and Mary.

Friends described Herring as compassionate, energetic and deeply involved with her family and community. She enjoyed running, hiking, biking, camping and spending time outdoors. Friends told WBRC that Herring routinely made time for people who were struggling and remembered her as a source of support and encouragement.

Herring's obituary described her as a devoted mother whose daughters were her greatest joy. She was also remembered for her nursing career, friendships, love of the outdoors and commitment to helping others.
DISPOSITION:
Jordan Markice Ricks was indicted on reckless-murder and leaving-the-scene charges arising from the crash that killed Herring. He subsequently pleaded guilty to reckless manslaughter and was sentenced to 17 years in prison.

On 04/26/2021, 25-year-old Precious Lean Ramizan Nievas and her 22-year-old brother, Philip Angelo Ramizan Nievas, were killed when a Honda Accord fleeing Santa Clara County Sheriff's Deputy Ryan Vesey crashed into their Toyota Camry at Lawrence Expressway and Mitty Way in San Jose. The siblings were returning home from work and were only minutes from their family's home.

At approximately 9:53 p.m., Vesey was driving behind the Honda on Lawrence Expressway when he activated his emergency lights to initiate a traffic stop. Vesey did not activate his siren. The driver accelerated away and Vesey pursued north on Lawrence Expressway.

Vesey initially told San Jose police investigators that he attempted the stop because the driver changed lanes and made a U-turn without signaling. Subsequent examination of Vesey's dash-camera recording showed that the driver had used his turn signal. Vesey later changed his account and said he attempted the stop because the Honda was speeding.

The fleeing vehicle continued north on Lawrence Expressway at extremely high speed. Precious and Philip were traveling south and had a green arrow to turn left onto Mitty Way. As they entered the intersection, the vehicle under pursuit ran the red light at approximately 97 to 98 mph and broadsided their Camry.

The force of the collision propelled the Camry approximately 200 to 250 feet into a concrete sound wall, where it came to rest on its side. Precious and Philip were pronounced dead at the scene.

Precious was born in Manila, Philippines, on 06/26/1995. She was the oldest of five children. After her mother died from leukemia when the children were young, Precious assumed a maternal role in the family and helped care for her younger siblings. Approximately five years before her death, her father, Al Nievas, moved the family from the Philippines to the United States seeking a better life.

Precious and Philip worked together at Cepheid in Sunnyvale and were driving home from work when they were killed. Their brother Patrick said they were approximately three minutes from home.

Precious's boyfriend, John Clemente, described her as the love of his life and said he had planned to propose after they finished school. Family and friends remembered Precious as selfless, hardworking and caring. The family's GoFundMe described her as a mother figure to her younger siblings and remembered her for a smile people would not forget.

Precious and Philip were buried following a joint service on 05/06/2021. Their funeral home published their full names, dates of birth, birthplace in Manila, photographs and a joint tribute video containing additional family images.
LAWSUIT:
Precious and Philip's father, Al Nievas, sued Santa Clara County over Deputy Vesey's pursuit. The lawsuit alleged that Vesey initiated an unlawful and dangerous pursuit for a minor traffic matter, failed to activate his siren and violated Sheriff's Office pursuit policy.

In March 2025, Santa Clara County approved a $5 million settlement with Al Nievas over the deaths of Precious and Philip.

Deputy Ryan Vesey was not criminally charged for his role in the pursuit. Whether the Sheriff's Office disciplined Vesey has not been publicly established; the department declined a records request seeking related training or disciplinary information.

At approximately 9:53 p.m., Vesey was driving behind the Honda on Lawrence Expressway when he activated his emergency lights to initiate a traffic stop. Vesey did not activate his siren. The driver accelerated away and Vesey pursued north on Lawrence Expressway.

Vesey initially told San Jose police investigators that he attempted the stop because the driver changed lanes and made a U-turn without signaling. Subsequent examination of Vesey's dash-camera recording showed that the driver had used his turn signal. Vesey later changed his account and said he attempted the stop because the Honda was speeding.

The fleeing vehicle continued north on Lawrence Expressway at extremely high speed. Precious and Philip were traveling south and had a green arrow to turn left onto Mitty Way. As they entered the intersection, the vehicle under pursuit ran the red light at approximately 97 to 98 mph and broadsided their Camry.

The force of the collision propelled the Camry approximately 200 to 250 feet into a concrete sound wall, where it came to rest on its side. Precious and Philip were pronounced dead at the scene.

Precious was born in Manila, Philippines, on 06/26/1995. She was the oldest of five children. After her mother died from leukemia when the children were young, Precious assumed a maternal role in the family and helped care for her younger siblings. Approximately five years before her death, her father, Al Nievas, moved the family from the Philippines to the United States seeking a better life.

Precious and Philip worked together at Cepheid in Sunnyvale and were driving home from work when they were killed. Their brother Patrick said they were approximately three minutes from home.

Precious's boyfriend, John Clemente, described her as the love of his life and said he had planned to propose after they finished school. Family and friends remembered Precious as selfless, hardworking and caring. The family's GoFundMe described her as a mother figure to her younger siblings and remembered her for a smile people would not forget.

Precious and Philip were buried following a joint service on 05/06/2021. Their funeral home published their full names, dates of birth, birthplace in Manila, photographs and a joint tribute video containing additional family images.
LAWSUIT:
Precious and Philip's father, Al Nievas, sued Santa Clara County over Deputy Vesey's pursuit. The lawsuit alleged that Vesey initiated an unlawful and dangerous pursuit for a minor traffic matter, failed to activate his siren and violated Sheriff's Office pursuit policy.

On 04/25/2021, 43-year-old Raymond Jackson of Rockford was killed when a vehicle fleeing Rockford Police Department officers crashed into the vehicle Jackson was driving at 9th Street and Harrison Avenue in Rockford. Jackson was an uninvolved motorist and had two of his children riding with him.

The fatal sequence began at approximately 4:50 p.m. as Rockford police attempted to apprehend a man wanted on three outstanding arrest warrants. The Winnebago County State's Attorney subsequently said the fleeing driver had been released from jail on furlough before the fatal pursuit.

Police initially attempted to stop the vehicle near Whitman and Church streets. Rockford Police Department SCOPE Team officers subsequently deployed stop sticks near Brooke Road and 9th Street. The vehicle drove over the stop sticks but continued away at high speed as police pursued.

The fleeing vehicle reached speeds reported at approximately 90 mph. Roughly one mile after encountering the stop sticks, the driver approached the intersection of 9th Street and Harrison Avenue and entered against a red traffic signal.

Raymond Jackson was driving through the intersection with the right-of-way when the vehicle under pursuit crashed into his vehicle. The impact critically injured Jackson and injured the two children riding with him. Contemporary reporting identified the children as a 12-year-old boy and a 3-year-old girl.

On 04/25/2021 at approximately 1:38 a.m., Pedro Carbajal, 22, was riding in a Hyundai Elantra with his cousin Jennifer Carvajal, 24, his girlfriend Lexcia “Lexi” Gonzalez, 20, and Grady Ramirez, 19, after the group attended a family birthday party. Pedro and Gonzalez had a young son, Julian.

Florida Highway Patrol Sgt. Jason Moore was driving east on Interstate 4 when his radar clocked the westbound Hyundai repeatedly at 111 mph. Moore made a U-turn, activated his emergency lights and siren and accelerated after the Hyundai. About a minute later, near Mango Road, he caught up with the car.
.
As Moore approached, Carvajal abruptly turned right onto the shoulder, traveled down a grassy slope and up an embankment, and became airborne. The Hyundai clipped a wire fence, flew into the Gator Ford dealership lot, struck a parked Ford F-250, overturned, and crashed into a concrete light pole and palm tree.

Pedro and Gonzalez were ejected. Pedro suffered critical injuries and later died. Gonzalez suffered two broken legs. Ramirez suffered incapacitating injuries after being thrown from the front seat into the rear of the vehicle. Carvajal, the only person wearing a seat belt, survived.
.
At Tampa General Hospital, Trooper Joshua Lugo reported that Carvajal had slurred speech, glassy eyes, difficulty remaining awake and smelled of alcohol. A hospital blood test measured her blood-alcohol concentration at approximately .10. Investigators also used seat-belt bruising and witness information in identifying her as the driver after she denied driving.

Carvajal was permanently barred from driving and was still on probation for a 2014 DUI manslaughter conviction in which, at age 16, she drove drunk through a red light and killed 52-year-old Keith Allen Davis. She had served approximately four years of a five-year prison sentence and was released in 2019.

Pedro's mother, Jazmin Dominguez, attended Carvajal's detention proceedings. Prosecutors told the court that Carvajal had attempted to contact Pedro's family after the crash, and the judge ordered her to have no further contact with the victims or their families. Carvajal was held without bond.

The criminal case continued for nearly four years. In February 2025, Carvajal pleaded guilty to DUI manslaughter, vehicular homicide, three counts of DUI causing serious bodily injury, reckless driving causing serious bodily injury, driving with a suspended or revoked license, a second DUI offense and two counts of DUI causing property damage or personal injury.

On 05/30/2025, Carvajal was sentenced to 67 years in prison. Pedro's relatives gave victim-impact statements describing the lasting effect of his death. The sentence substantially exceeded the prosecution's 33-year recommendation.

Over the following hour, callers reported Orsenico speeding, swerving through traffic, passing on curves, making sudden U-turns, cutting off vehicles and semi-trucks, running red lights without slowing and traveling more than 60 mph through a Bandon school zone. Multiple law-enforcement agencies attempted to locate him.

Near mile marker 247 by Belloni Boys Ranch, an Oregon State Police trooper traveling south encountered Orsenico traveling north. The trooper turned around to follow him, and the patrol vehicle and Mitsubishi made minor contact during the maneuver. Orsenico then accelerated away southbound.

Investigators determined that Orsenico moved from the slow lane across the three-lane highway, crossed the double-yellow centerline and intentionally drove head-on into an oncoming vehicle driven by Dawn Adams, 54, with Jeffrey Adams, 78, as her passenger. Orsenico, Dawn and Jeffrey all died at the scene.

The Coos County Sheriff's Office sought witnesses and video to reconstruct Orsenico's movements and ultimately stated that investigators had reason to believe the fatal collision was deliberate.

On 04/21/2021, Shynia Jones, 18, of Grand Rapids was driving south on Division Avenue when a Wyoming Department of Public Safety officer saw her vehicle near 44th Street. The car had no license plate and police believed it matched the description of a vehicle reported stolen earlier that night. The officer attempted to stop Jones, but she continued south and ran a red light at Division Avenue and 36th Street at high speed. Police subsequently determined that Jones's vehicle was not the stolen vehicle they were looking for.

Jones temporarily eluded police. Another Wyoming officer subsequently encountered the vehicle farther north on Division Avenue and followed it as it traveled at approximately 80 to 90 mph. Police reported that the officer discontinued the pursuit shortly before the fatal crash.

Near Division Avenue and Alger Street, Jones's vehicle collided with another vehicle, struck the boulevard median at high speed, became airborne and went out of control before crashing into a light pole. On April 20, 2021, at approximately 2:17 a.m., a California Highway Patrol officer attempted to stop a white Chevrolet pickup at Fairfax Road and Niles Street for a registration violation. The driver, 25-year-old Branden Lewis LeMay Sr., failed to stop and fled northbound on Fairfax Road before turning westbound onto Panorama Drive. West of Mt. Vernon Avenue, the pickup lost control, veered off the roadway, and rolled down an embankment, landing on its roof in the westbound lane of China Grade Loop. LeMay was ejected from the vehicle and pronounced dead at the scene. A passenger sustained moderate injuries and was taken to a local hospital. The Kern County Coroner’s Office confirmed LeMay’s identity and ruled the death accidental. No family statements or employment details were released.
