On May 9, 2021, deputies from the Franklin County Sheriff's Office pursued a car driven by Ricky G. Cruz, 28, in the northbound lanes of U.S. 395 near Pasco, Washington. As Cruz approached the intersection with Foster Wells Road, he crossed into the southbound lanes of the highway and collided head-on with an oncoming semi-truck. Both Cruz and his passenger, Karlie Moore, 23, were killed in the crash.

On 05/09/2021, police in Greenville, South Carolina responded to a 911 call in which David “Whit” Oliver, 44, reported he was being harassed and was on the phone with the operator when gunshots were heard. Officers located Jeffrey Mark Murray a short time later and pulled him over about a mile from the initial incident. According to police statements, Murray exited his vehicle holding a firearm and fired at an officer and a trainee. Officers returned fire and struck Murray. Murray was taken to a hospital, where he died from his injuries. Oliver died at the scene of the original shooting.

On 05/09/2021, Glendale Police officers responded to a reported armed robbery near 51st Avenue and Bethany Home Road in Glendale, Arizona. Officers located Maxwell Jerome Davis in the area and attempted to take him into custody. According to police, Davis made verbal statements toward officers and moved into a nearby mobile home community. Officers later found him between parked vehicles. Police reported that Davis held an object and moved his hand toward his waistband. An officer fired, striking Davis. He was transported to a hospital, where he was pronounced dead.

On 05/08/2021 around 6:40 a.m., Baltimore County police responded to multiple 911 calls reporting gunfire and a house fire in the 7500 block of Maury Road in the Woodlawn and Windsor Mill area. Police identified Everton Garfield Brown, 56, as the shooter. Police said Brown forced entry into a neighbor’s home, stabbed and shot Ismael Quintanilla, 41, and shot Sara Alacote, 37, as she ran from the home. Police said Brown shot two additional neighbors outside, killing Sagar Ghimire, 24, and wounding another man. Police said Brown set his own residence on fire and officers encountered him outside near a parking lot while he had a handgun. Police said officers repeatedly ordered him to drop the gun. Police said four officers opened fire and shot Brown. Police took him into custody and transported him toward a hospital, where he was pronounced dead. Later reporting said Baltimore County police released body-worn camera video from the shooting.

On 05/09/2021 at about 6:13 a.m., a dispatcher watching a security camera feed at the Leicester Police Department saw an SUV repeatedly strike the station’s front entrance. Leicester police responded to the station. Reporting and the Worcester County District Attorney said Zachary Richardson got out of the SUV and aimed what appeared to be a rifle at officers. Officers opened fire and shot Richardson. Officers provided medical aid until EMS arrived. EMS transported Richardson to Saint Vincent Hospital in Worcester, where he was pronounced dead. Massachusetts State Police investigated under the Worcester County District Attorney’s Office.

On 05/08/2021, police responded to a 911 call from a homeowner in the 4000 block of Northwest 33rd Street, Oklahoma City. The caller reported that Daniel Hobbs, who was under a victim protection order, was present in their yard. Upon arrival, officers engaged in a struggle with Hobbs. During the altercation, an officer fired their weapon, killing Hobbs.

On 05/08/2021, Deputy R.W. Marcelle of the Frederick County Sheriff’s Office attempted to stop a 2003 Cadillac traveling recklessly on Martinsburg Pike (U.S. 11) near Welltown Road. The driver, identified as Kondwani Rogers of Frederick County, continued driving at speeds exceeding 100 miles per hour. The deputy activated his emergency equipment, but Rogers continued south on North Frederick Pike (U.S. 522) toward Winchester. During the pursuit, the Cadillac crossed the center line, sideswiped a Honda Civic, then collided head-on with a Chevrolet Trailblazer. The Cadillac overturned and caught fire. Rogers was pronounced dead at the scene. One driver was hospitalized with serious but non-life-threatening injuries, and another declined treatment.

On 05/08/2021, members of the Riverside Police Department’s Public Safety Engagement Team, along with code enforcement and social service personnel, encountered Felix Jerry Marquez in the 3700 block of Elizabeth Street while conducting outreach in an area frequented by people experiencing homelessness. When officers attempted to detain Marquez, he produced a handgun and aimed it toward officers. A deputy then shot Marquez. Marquez was taken to a local hospital where he died from his injuries. A handgun was recovered next to his body at the scene.

On 05/08/2021, Clarksville Police Department officers responded to a domestic disturbance call at a residence on Hedge Apple Drive. Police arrived after reports that Adonis Traughber had shot his ex-girlfriend inside the home. When officers arrived at the scene, Traughber came out of the residence and fired shots at police. Officers returned fire, striking Traughber. He was transported to a hospital, where he died from his injuries. The Tennessee Bureau of Investigation took over the investigation following the shooting.

On May 7, 2021, at approximately 12:30 a.m., Providence Police officers responded to reports of a man screaming in the street near 345 Angell Street, possibly under the influence of narcotics. Upon arrival, they encountered 34-year-old Joseph L. Ventre Jr., who was rolling on the ground and groaning unintelligibly. Officers attempted to calm Ventre and requested assistance from the Providence Fire Department. Due to his erratic behavior and for the safety of all involved, officers decided to restrain Ventre. Body camera footage shows officers holding Ventredown on his stomach for approximately 90 seconds while handcuffing him. Shortly thereafter, Ventre became unresponsive. Emergency responders initiated CPR and transported him to Rhode Island Hospital, where he was pronounced dead at 1:41 a.m. The Rhode Island Medical Examiner later determined that Ventre's death was due to the combined toxic effects of methamphetamine and methadone during police restraint.

On 05/07/2021, deputies responded to a call reporting an assault involving a crowbar in the 22000 block of Margarth Street. Deputies located a man in a nearby field holding the tool after reports he struck another person during an argument. Deputies attempted to gain compliance and deployed a Taser without effect. When the man advanced toward a deputy holding the crowbar, deputies opened fire and killed him. Public reporting did not release his name. On May 7, 2021, law enforcement officers from the Carteret County Sheriff's Office, Morehead City Police Department, and North Carolina State Patrol pursued Richard Kelly Murphrey, 35, who was wanted on several outstanding charges such as carrying a concealed weapon, and numerous drug-related offenses.

On 05/07/2021, Calaveras County sheriff’s deputies responded to reports of an argument between a man and woman at the Sierra Inn Motel in San Andreas. The suspect left the motel before deputies arrived. While deputies were interviewing the woman near an apartment complex on Lewis Avenue, the suspect ambushed and shot a deputy. The deputy and the woman were able to move to safety. Additional responding officers located the suspect inside an apartment holding an elderly female hostage. Officers attempted de-escalation and to secure the release of the hostage. The suspect did not comply with commands and remained armed. Shots were then fired, killing the suspect and injuring the hostage. The hostage and the deputy who had been shot were taken to hospitals and expected to recover.

Upon arrival, officers made contact with Robbie Leigh Hodge. During the encounter, two officers discharged their weapons, striking Hodge. He was pronounced dead at the scene. On 05/06/2021, deputies from the Oakland County Sheriff's Office were dispatched to a residence at the intersection of Mead Road and Wimberly Drive in Rochester Hills following a complaint that a man was threatening suicide. Initial contact was made with the man via phone, during which a single gunshot was heard, and communication was subsequently lost. As officers approached, the driver exited the vehicle, and the passenger moved into the driver's seat and allegedly attempted to flee. One officer contacted the driver, while the other officer tried to stop the fleeing vehicle. The vehicle entered I-25, heading into oncoming traffic lanes. The officer, still inside the vehicle with the driver, continued to order the driver to stop. At this point, the officer shot and killed the driver, reportedly stopping the vehicle along the side of the Interstate.

On 05/05/2021, Las Vegas Metropolitan Police Department officers responded to reports of gunfire at an apartment complex on Fremont Street. On May 3, 2021, 32-year-old Brandon Lee Shockley Sr. was pursued by law enforcement officers from Indiana into Illinois. Shockley was wanted for attempted homicide.

The pursuit began on I-70 near the Indiana-Illinois state line, where Indiana State Police deployed stop sticks, causing Shockley's vehicle to suffer a flat tire. Despite this, he continued driving into Clark County, Illinois, eventually veering off the road and coming to a stop in a ditch near milepost 153.

Officers reported hearing a gunshot from inside the vehicle. In response, a Vigo County Sheriff's Deputy and a West Terre Haute Police Officer fired their weapons at the car. At around 2:55 a.m., Ford County dispatch sent a be-on-the-lookout for a person wanted in Garden City who allegedly was involved in a stabbing. Gray County deputies located Luis Salgado's vehicle at around 3:17 a.m. The deputies attempted to stop the vehicle, but Luis Salgado continued driving. Deputies escalated a chase into Ford County, where Ford County deputies joined the chase at 3:24 a.m. Spike strips were deployed west of Dodge City, deflating one of the tires. Salgado continued driving east into Dodge City before veering off the roadway, striking a power pole, and rolling several times. He was ejected and died at the crash site.

On 05/02/2021, David Earle Hartenstein, 39, of Fitchburg, Massachusetts, was traveling northbound in the southbound travel lanes on NH-101. Police attempts to stop the wrong-way vehicle, including the use of tire deflation devices, were unsuccessful. Officers from multiple agencies set up rolling roadblocks to prevent other drivers from encountering the vehicle. Near the Route 101 split in Manchester, Hartenstein veered off the roadway and into the woods. The crash resulted in his death.

On May 2, 2021, at 6:18 p.m., Hamilton Township Police officers responded to a residence on the 1200 block of Adams Road in Loveland, Ohio, following a 911 call from a neighbor. The call reported that Anthony Williams, a 58-year-old male, was intoxicated, threatening suicide, and discharging a firearm in his home. The caller indicated that Williams’ partner had fled to the neighbor's house seeking help.

Officers Chris Wall and Josh Clift located Williams in the backyard of the residence at 6:55 p.m. Williams was allegedly holding a firearm. Officers opened fire, killing him.

On 05/02/2021, around 2 a.m., Berlynn Matthews stopped to assist Joshua Day, who had been struck by a car while crossing North Point BoulevaRd in Dundalk. While Matthews was helping Day, off-duty Officer William Collazo-Brown on a motorcycle struck both of them, leading to their deaths. Collazo-Brown was subsequently arrested and charged with DUI-related offenses.

Barber's vehicle collided with a black GMC Yukon, killing two occupants. Barber was charged with three counts of vehicular homicide, aggravated DWI, and possession of an open container.

On May 1, 2021, around 6 p.m., the Porter County Sheriff's Office responded to a 911 call regarding a disturbance at a residence located at 716 Long Run Road in South Haven, Indiana. After several hours of attempting to communicate with a man inside the home without success, officers entered the residence. Inside a bedroom, they encountered Alexander T. Tuzinski, who was reportedly armed with a handgun. Officers shot and killed Tuzinski during the confrontation.

Around 10:50 p.m. on May 1, 2021, officers in an unmarked patrol car attempted to stop Jose Astrain Jr. for an unspecified traffic violation. Astrain refused to stop and sped away at high speed. The officers turned off their emergency lights and did not pursue him. Astrain lost control of his Ford Fusion, fishtailed into oncoming traffic, and crashed into a pickup truck, resulting in his death.
