Cory M. Orsenico, 32, was reported for dangerous driving. An Oregon State Police trooper initiated a pursuit to stop him, resulting in a minor collision between their vehicles. On April 22, 2021, at approximately 7:15 a.m., the Coos County Sheriff's Office received reports of a reckless driver in a red Mitsubishi Eclipse near Port Orford, Oregon. Witnesses reported the driver engaging in dangerous behaviors, including speeding, swerving between lanes, making sudden U-turns, and speeding through town at 60 mph.
The Oregon State Police and Bandon Police Department jointly escalated a pursuit. An Oregon State Police trooper located the vehicle near mile marker 247 on U.S. Highway 101, near Belloni Boys Ranch. A minor collision occurred between the trooper’s vehicle and the red Mitsubishi, prompting the driver to flee southbound on the highway.
The driver then crossed over the double yellow line and collided head-on with a vehicle in the opposite lane.

On 04/21/2021, an officer from the Wyoming Department of Public Safety noticed a vehicle without a license plate near 44th Street SW and S. Division Avenue in Grand Rapids, which matched the description of a stolen car reported earlier that evening. The Wyoming Department of Public Safety is under the city of Wyoming, Michigan, not the state of Wyoming. The officer attempted to pursue Shynia Jones, who was driving the vehicle, but she did not stop. Shortly after, another officer spotted Jones' car and escalated a chase to between 80 and 90 mph. The officer began a pursuit but allegedly “called off the chase” moments before she crashed into another vehicle, then a boulevard median at high speed, became airborne, lost control, and collided with a light pole on the east side of the Street, killing her.

The driver did not stop and drove northbound on State Route 9. Police continued to chase him into Toquerville, where the driver navigated through side roads and returned to the highway. Around 3:32 a.m., while still being chased, the vehicle rolled over, ejecting the driver. He was pronounced dead at the crash site. A female passenger was hospitalized. The vehicle had been reported stolen out of Salt Lake City.

On 04/21/2021, Escondido police shot and killed 59 year old Steven John Olson during a street encounter near East Second Avenue and South Broadway. Police responded to reports of a man striking vehicles with a metal pole. One officer located Olson and reported that he was not posing a threat to the public at that time. Olson then left the area on foot.
A second officer later encountered Olson on the street. During that encounter, the officer issued commands for Olson to drop the pole. As Olson continued moving toward the officer while holding the pole, the officer opened fire and killed him at the scene.
The San Diego County District Attorney later reviewed the shooting and ruled the use of deadly force justified. No officers were charged.

On 04/21/2021, around 12: 05 a.m., Worcester Police Department officers encountered Phet Gouvonvong at 265 Grafton St in Worcester. Gouvonvong was wearing body armor, appeared to be armed with what looked like an assault rifle, and had an explosive device with wires attached, along with carrying a backpack. During the encounter, Gouvonvong reportedly became agitated and made a move toward the police. Police shot and killed him.

On 04/20/2021, officers from the Detroit Police Department responded to an incident at E 8 Mile Road and HannaStreet in Detroit involving a man in a mental health crisis. On 04/20/2021, a 22-year-old male driving a Dodge Charger was allegedly chased at high speed by officers of the Grosse Pointe Park Police Department when he lost control of his vehicle at the intersection of ConnerStreet and E Jefferson Avenue in Detroit, landed under the trailer of a semi-truck, killing him. Police did not disclose the reasons why they escalated the fatal chase.

On April 20, 2021, at approximately 2:17 a.m., a California Highway Patrol officer attempted to stop a white Chevrolet pickup at Fairfax Road and Niles Street for a registration violation. The driver, 25-year-old Branden Lewis LeMay Sr., failed to stop and fled northbound on Fairfax Road before turning westbound onto Panorama Drive. West of Mt. Vernon Avenue, the pickup lost control, veered off the roadway, and rolled down an embankment, landing on its roof in the westbound lane of China Grade Loop. LeMay was ejected from the vehicle and pronounced dead at the scene. A passenger sustained moderate injuries and was taken to a local hospital. The Kern County Coroner’s Office confirmed LeMay’s identity and ruled the death accidental. No family statements or employment details were released.

On 04/20/2021, Lakewood police shot and killed a man during an armed search near West 3rd Place and Depew Street. Police responded to reports of shots fired and were told that one man had shot another before leaving the area. Officers later located the wounded person at a hospital and established a perimeter to search for the shooter.
During the search, officers encountered the armed man. Police reported that the man threatened to shoot officers. Officers opened fire and killed him at the scene.

On April 20, 2021, Columbus Police Officer Nicholas Reardon responded to a report of a fight outside a residence on the 3100 block of Legion Lane in Columbus, Ohio. Upon arriving at the scene, Reardon observed 16-year-old Ma'Khia Bryant holding a knife and charging toward another individual.

On 04/19/2021, Dothan police shot and killed 39 year old Doward Sylleen Baker following a traffic stop near Trim Street and Third Avenue. An officer stopped Baker’s vehicle for not displaying a license plate. During the stop, police stated the officer observed a firearm inside the vehicle. Baker drove a short distance and stopped again nearby.
Police reported that Baker exited the vehicle while holding a gun. An officer deployed a Taser. During the encounter, police stated that Baker fired a shot that struck an officer in the leg. A second officer arrived, and officers opened fire, killing Baker at the scene.
The Houston County District Attorney reviewed the shooting and ruled the use of deadly force justified. Baker’s family later filed a civil lawsuit against the City of Dothan and the police department seeking damages. The civil case was pending at the time of later reporting.

On 04/19/2021, Fountain police officers and Colorado State Patrol troopers shot and killed 38 year old Robert Paul Garcia following a high-speed pursuit on Interstate 25 in Colorado Springs. Law enforcement responded earlier to a reported attempted robbery at a Dairy Queen in Pueblo West. A short time later, officers spotted a pickup truck linked to the incident traveling northbound on I-25 at speeds exceeding 100 miles per hour.
The truck briefly pulled over, and a passenger exited. The driver then accelerated and continued northbound on I-25. South of North Academy Boulevard, a trooper used a patrol vehicle to force the truck off the roadway. During the stop that followed, officers opened fire. Garcia, who was driving the truck, was struck by gunfire and killed at the scene.
The El Paso County District Attorney reviewed the shooting and ruled the use of deadly force justified. No officers were charged.

On 04/19/2021, Alameda police officers restrained Mario Gonzalez after responding to a report of a man acting erratically at a local business. On 04/18/2021, an incident involving five people being shot and wounded occurred in Detroit, Michigan. A person who was subsequently struck by a car nearby and died was believed to have been involved in this shooting. Following the initial incident, police initiated a pursuit of a black GMC driven by a 27-year-old male man traveling north on Gratiot Avenue. The chase continued until the driver reached 8 Mile Road, where he exited his vehicle at a strip mall. While fleeing on foot, the man allegedly fired shots at pursuing officers. On 04/18/2021, officers from the Burnsville Police Department found Bradley Olsen driving a vehicle with stolen plates when he subsequently crashed. Officers soon received a report that he was involved in a carjacking. Police pursued the carjacked vehicle, and Olsen allegedly fired at pursuing officers before crashing the vehicle and continuing to shoot at police, who then shot and killed him. Officers Dan Anderson, Brent Murray, Paul Oelrich, and Chris Walswick fired their guns.

On April 17, 2021, around 3:30 a.m., Spokane Police responded to a report of a suspected car prowling in the 500 block of South Cannon Street. A victim reported that Vadim A. Grishchuk had broken into his vehicle and pointed a gun at him when confronted. When officers arrived, Grishchuk pointed the gun at police after being given commands and allegedly fired a shot. Officer Brandon Roy shot and killed Grishchuk during the encounter.

On 04/17/2021, Weld County sheriff’s deputies shot and killed 60 year old Smoky Lynn Crockett outside a residence on Casler Avenue in Fort Lupton. Deputies responded to a call reporting that a man was armed with a gun, preventing another individual from leaving the home, and making threats to kill them.
When deputies arrived, they located Crockett outside the residence while he was armed. Deputies engaged with him for several minutes. During the encounter that followed, deputies opened fire and killed Crockett at the scene.

On April 16, 2021, Albuquerque Police Department officers responded to a domestic dispute at a residence on Coyote Canyon Place NW in Albuquerque, New Mexico. Juan James Cordova, 51, was reportedly armed and had fired a shot inside the house and was in mental health crisis per his girlfriend. Officers spent approximately an hour attempting to negotiate with Cordova, urging him to surrender peacefully.

Despite negotiations, Cordova remained uncooperative, holding a firearm and making statements suggesting he intended to provoke a lethal response from the police. At one point, he fired a round, which passed through a window and struck a neighbor's house. When Cordova allegedly pointed his firearm at the officers, Officer Bryce Willsey opened fire and killed him.

Officer Willsey, a member of the Albuquerque Police Department since 2015, had been involved in three prior officer-involved shootings before this incident. In October 2023, Cordova's family filed a lawsuit against the city of Albuquerque and Officer Willsey, alleging excessive force and claiming that Cordova was experiencing a mental health crisis and his death had been avoidable.

On April 15, 2021, at approximately 11:30 p.m., Roswell Police Department officers assisted the Chaves County Sheriff's Office during a pursuit involving a white Ford pickup truck that had fled from a deputy during an attempted traffic stop. Roswell officers deployed two sets of tire deflation devices and missed.
The vehicle then entered the parking lot of a bank at 1901 N Main St in Roswell, New Mexico. The driver, identified as 22-year-old Victor Ivan Barron, exited the truck holding a firearm and attempted to run away on foot. Officers Danny Flott and Abrem Atencio, both with the Roswell Police Department, along with Chaves County Sheriff’s Deputy Samuel Lueras, each discharged their firearms, striking and killing Barron.

On April 15, 2021, 56-year-old Stephen B. Swears of Boynton Beach, Florida, died after evading law enforcement. Swears was being pursued by the St. George Police Department and the South Carolina Highway Patrol on US 15 near Fox Run Road, Dorchester County. While fleeing, his vehicle crossed the center line and collided head-on with another motorist’s vehicle, resulting in his death at 1:30 p.m. No other fatalities were reported, and the crash remains under investigation by the Dorchester County Coroner’s Office and the South Carolina Highway Patrol.

On April 15, 2021, a high-speed pursuit by the California Highway Patrol ended when the driver of a white 2004 Toyota Camry fled southbound on Highway 1 from Dimeo Lane toward Morrissey Boulevard. The vehicle lost control on the off-ramp, struck a concrete curb, collided with a metal guardrail and caught fire. Nineteen-year-old Alyssa Roman of San Jose, a passenger in the vehicle, was pronounced dead at the scene. Three other passengers were injured and the driver was arrested for multiple felony offenses including DUI and evading a peace officer.
