On 04/09/2021, Stephenson County sheriff’s deputies shot and killed 52 year old David William Determan during a standoff at Citizens State Bank in Lena. Deputies responded around 9:14 a.m. after Determan entered the bank holding a firearm. After a deputy arrived on scene, Determan fired a round into the air outside the bank and remained in conversation with officers while holding the weapon. During the encounter, Determan lifted the firearm in the direction of deputies. Deputies opened fire and killed him at the scene. Authorities closed the downtown area during the response.

On April 7, 2021, around 6:45 p.m., Philadelphia Police officers conducted a traffic stop on a blue Kia Optima in the 1500 block of West Somerville Avenue after it failed to stop at a stop sign. The vehicle had four occupants. Upon running their information, officers found that two individuals, including 24-year-old James "Poppie" Alexander, had outstanding warrants. As officers attempted to remove Alexander from the vehicle, he allegedly drew a firearm and fired at them, striking Officer Matthew Ponente in the foot. Alexandria PD escalated a chase of a vehicle that they felt was involved in nearby gunfire. The vehicle did not stop and drove northbound on Interstate 295. Police continued to chase the driver into Southeast Washington, D.C., where he exited the vehicle and jumped over a jersey barrier, falling approximately 23 feet onto Malcolm X Avenue. While still being chased, a U.S. Park Police officer deployed a stun gun, and Alexandria Police officers took him into custody. He was pronounced dead at the crash site.

On April 6, 2021, Oneonta Police Department Sergeant Ralph Pajerski and an unidentified partner responded to a domestic incident at 48 River Street in Oneonta, New York. Upon arrival, they encountered Tyler R. Green, 23, who was allegedly threatening his girlfriend with a knife while holding their two-year-old son. On April 3, 2021, just after 6:00 a.m., officers with the Vallejo Police Department responded to a call of a man slumped behind the wheel of an idling car. When officers approached, the 32-year-old drove away in a stolen vehicle, reached speeds around 85 mph in a 40 mph zone, and covered just over one mile before losing control and crashing into a tree. First responders pronounced him dead at the scene. The vehicle was stolen from Walnut Creek. The Solano County Major Crimes Task Force and the California Highway Patrol’s Multidisciplinary Accident Investigation Team are leading investigations.

On 04/03/2021, Riverside County sheriff’s deputies went to a residence on La Fortuna Lane in Moreno Valley to locate Juan Carlos Estrada. Deputies used patrol vehicle speakers to order Estrada to come outside. While deputies were outside the home, a dispatcher received a call from inside the residence indicating Estrada was armed. Estrada later exited the home and moved toward deputies and their patrol vehicles. Deputies opened fire, and Estrada was shot and killed at the scene. The shooting was investigated by outside authorities.

On 04/03/2021, Phoenix police shot and killed 26 year old Jose Arenas during an evening encounter in west Phoenix. Police responded to an Arco gas station near 75th Avenue and Indian School Road around 8 p.m. after receiving reports of a disturbance. According to police, officers chased Arenas from the gas station into a nearby restaurant. Officers confronted Arenas inside the business and issued commands. During the confrontation, an officer opened fire, killing Arenas at the scene. Police stated that Arenas had a firearm during the encounter. No officers were charged. The Maricopa County Attorney’s Office later cleared the shooting. Available reporting does not indicate public protests, lawsuits, or family statements challenging the police account.

Around 3:30 a.m., Corvallis Police responded to a call from the Days Inn motel staff reporting a man acting aggressively with a knife. Officers encountered Jeffrey W. Appelt, 32, who reportedly had a knife. An officer fired a Taser, but Appelt retreated toward the motel entrance. A second officer attempted to take him into custody, but Appelt evaded the officer. As a third officer arrived, Appelt continued approaching while holding the knife. Officer Daniel Granillo then shot and killed Appelt.

On 04/03/2021, a standoff occurred at a residence located at 601 6th Street in Carlstadt, New Jersey, resulting in the lockdown of two city blocks. On April 2, 2021, at approximately 5:15 a.m., Long Beach Police Department officers responded to a 911 call reporting a man who appeared to have had a heart attack and a woman threatening suicide at a residence in the 2800 block of Golden Avenue. Upon arrival, officers observed a large amount of blood inside the residence and were immediately confronted by 38-year-old Lauren Nicole Archibeque, who was armed with a rifle. Officers quickly exited the residence, set up a perimeter, and attempted to de-escalate the situation by making repeated attempts to communicate with Archibeque, urging her to surrender. She did not respond.

After some time, Archibeque emerged at the doorway of the residence, and officers observed her pointing the rifle at them. At that point, three officers discharged their firearms, fatally wounding her. She was pronounced dead at the scene. Upon re-entering the residence, officers discovered a 49-year-old man on a bedroom floor with a serious laceration to his neck. He was also pronounced dead at the scene. The Los Angeles County Coroner’s Office later determined that the man's death was a suicide.

The Long Beach Police Department is conducting a full and thorough multi-level review of the incident, and the Los Angeles County District Attorney’s Office is conducting an independent investigation, as is standard protocol for officer-involved shootings resulting in injury or death.
