Officer(s) : Matthew Sidders; David Burtch
Cause of Death : Gunshot
Event : Killed by police during maritime SWAT operation
OPEN RECORDS SUMMARY :
On 07/09/2026, Alaska State Troopers searched for 58-year-old Philip Thomas Phillips of Wasilla after he failed to return to Palmer Superior Court during his criminal trial. Phillips fired his attorney on the first day of trial and chose to represent himself. After a jury was selected, he stopped appearing in court. The judge determined Phillips had intentionally absconded, waived his right to attend, and allowed the trial to continue in his absence. A jury convicted Phillips that day of first-degree sexual abuse of a minor, second-degree sexual abuse of a minor and harassment involving a boy who was 12 years old when the offenses occurred in August 2020. The court issued a no-bail warrant for Phillips.
Investigators determined Phillips had made preparations to leave Alaska before the trial. He sold his house earlier in June, sold his truck during the week before trial, purchased a satellite telephone with an international service plan, sold or disposed of other belongings and told people he intended to leave the state. Phillips traveled to Whittier, boarded a sailboat and left the harbor. His electronic monitoring device stopped transmitting early on 07/07/2026.
At approximately 8:07 p.m. on 07/09/2026, Alaska State Troopers, Alaska Wildlife Troopers and members of the Alaska State Troopers Southcentral SWAT Team located Phillips aboard a boat anchored in the Whittier area. Law enforcement personnel approached the vessel and attempted to negotiate his surrender. Phillips was armed with a handgun and remained aboard the boat.
Troopers deployed less-lethal weapons and chemical agents during the operation. Phillips responded by putting on ballistic body armor and a gas mask. According to the Alaska Department of Public Safety, Phillips then directed his handgun toward the officers. Alaska State Trooper Sergeant Matthew Sidders and Wasilla Police Department Sergeant David Burtch fired at Phillips, striking him. Sidders was a 14-year Alaska State Troopers veteran, and Burtch was an 11-year Wasilla Police Department veteran serving with the multi-agency SWAT team.
Tactical personnel boarded and secured the vessel after the shooting. Phillips died aboard the boat. His body was transported to the Alaska State Medical Examiner’s Office for examination. The Alaska Bureau of Investigation assumed responsibility for investigating the shooting, and the completed investigation will be reviewed by the Alaska Office of Special Prosecutions.

Location : Rammel Way
City : Decatur
County : DeKalb
State : Georgia
Agency : DeKalb County Police Department
Officer(s) : Derrick Harris Jr.
Cause of Death : Gunshot
Event : Killed by police during armed robbery investigation
OPEN RECORDS SUMMARY :
On 07/08/2026, DeKalb County Police Officer Derrick Harris Jr. shot and killed 19-year-old Seth Jayden Eccles in the backyard of a residence on Rammel Way while attempting to arrest him on outstanding warrants. According to the Georgia Bureau of Investigation, Eccles complied with the officer's commands after being located at gunpoint. As Harris attempted to handcuff him, the officer's firearm discharged, striking Eccles. Eccles died at the scene. Following its investigation, the GBI arrested Harris and charged him with involuntary manslaughter and reckless conduct, concluding the shooting occurred while Eccles was being taken into custody rather than during an active confrontation.
According to the GBI's court filings:
-Seth Jayden Eccles, 19, had been located in the backyard of a home after officers searched for him in connection with an armed robbery investigation.
-Officer Derrick Harris Jr. confronted him at gunpoint.
-Eccles complied with every command.
-He got on the ground.
-He was lying face down (prone position) with his hands behind his back while Harris attempted to handcuff him.
The affidavits allege Harris made a critical mistake:
He kept his pistol in one hand while trying to apply handcuffs with the other.
During that process, the gun allegedly discharged by itself.
Prosecutors characterize that conduct as a "gross deviation from the standard of care," which forms the basis for the reckless conduct charge. The involuntary manslaughter charge is based on causing a death while committing that misdemeanor offense.
Harris was fired by the DeKalb County Police Department. He had originally been hired in 2014 and for unknown reasons voluntariy resigned in 2020, was then rehired in 2026. The investigation remains ongoing, and the case will be presented to the DeKalb County District Attorney for prosecution.

Event : Killed by federal task for serving arrest warrant
OPEN RECORDS SUMMARY :
On 07/08/2026 at approximately 10:30 a.m., members of the Drug Enforcement Administration assigned to the Memphis Safe Task Force responded to the Extended Stay America hotel near Poplar Avenue and Aaron Brenner Drive to serve a drug-related arrest warrant on Alfonso Ivy, 47. The Memphis Safe Task Force is a multi-agency initiative that includes federal, state, and local law enforcement personnel.
Task force members surrounded Ivy's hotel room and attempted to take him into custody. According to the Tennessee Bureau of Investigation and the U.S. Marshals Service, the situation escalated as officers made entry into the room. Authorities stated Ivy pointed a handgun at the agents during the attempted arrest. A DEA agent fired, striking Ivy. He died at the scene. Investigators recovered a handgun inside the hotel room.
The Tennessee Bureau of Investigation was requested by Shelby County District Attorney General Steve Mulroy to conduct an independent investigation into the shooting. TBI crime scene investigators processed the hotel room, collected physical evidence, interviewed participating officers and witnesses, and reviewed available body-worn camera and other evidence. TBI later identified the decedent as Alfonso Ivy, 47, on July 9 after notification of next of kin.
The shooting occurred four days after another fatal Memphis Safe Task Force shooting involving Tennessee National Guard personnel, making it the second fatal use of force involving the task force within a week and the fourth fatal shooting connected to the task force since its creation. The incident drew renewed public scrutiny of the federally supported task force and prompted additional calls for transparency regarding its operations and use of force.
The TBI will submit its completed investigative findings to the Shelby County District Attorney General's Office for review upon completion of the investigation.

OPEN RECORDS SUMMARY :
On 07/08/2026 at approximately 4:55 a.m., the Boerne Police Department Communications Center received multiple 911 calls reporting a man standing in the roadway in the Bent Tree subdivision wearing only his underwear and firing a handgun. One caller reported the man had fired a shot in the caller's direction. Four Kendall County Sheriff's Office deputies responded to the 100 block of Bent Tree Drive and located Austin Hodo, 18, still armed with a handgun.
Dash camera video released by the Kendall County Sheriff's Office shows deputies repeatedly ordering Hodo to drop the handgun. During the approximately 46-second encounter, deputies gave more than two dozen verbal commands while using patrol vehicles for cover. Hodo continued walking toward the deputies while holding the handgun in his left hand and a cloth in his right hand.
According to the Sheriff's Office, Hodo raised the handgun and fired toward the deputies. The deputies returned fire, striking Hodo. Deputies immediately approached, secured the handgun, and rendered emergency medical aid, including the application of tourniquets, until EMS personnel arrived. Hodo was pronounced dead at the scene.
The Kendall County Sheriff's Office later released dash camera video, 911 recordings, and dispatch radio traffic documenting the encounter. The department identified one of the involved officers as Corporal Cheyenne Weber, an 11-year veteran of the Boerne Police Department who was assigned to the operation. The identities of the remaining three deputies who fired or were present during the encounter have not been publicly released.
Austin Hodo had recently graduated from the Geneva School of Boerne and planned to attend Texas Tech University. Investigators have not publicly identified a motive for his actions. Event : Killed by police following homicide investigation
OPEN RECORDS SUMMARY :
On 07/08/2026, investigators in Pierce County, Washington, identified Hayes Miles McCloud, 24, of Eatonville, as the suspect in two fatal shootings that occurred earlier that morning. The first victim, a 35-year-old man, was found dead inside a residence in Puyallup at approximately 2:40 a.m. Roughly 30 minutes later, a second man was fatally shot in Tacoma. Investigators determined the two victims were related and believed both killings were targeted. McCloud fled Washington after the shootings.
Automatic license plate reader cameras later detected McCloud's silver 1996 Toyota 4Runner traveling south through Lewis County toward Oregon. At approximately 6:19 p.m., officers located the vehicle parked on Sunset Boulevard near Greenway Drive in Seaside. Officers from the Seaside Police Department, Oregon State Police, Gearhart Police Department, Cannon Beach Police Department, and the Clatsop County Sheriff's Office converged on the location to arrest McCloud.
According to the Oregon State Police, officers approached the vehicle and attempted to take McCloud into custody. McCloud opened fire on the officers from inside the vehicle. Officers returned fire during the exchange. McCloud then drove away from the scene while still under gunfire. A short distance later, officers found the 4Runner stopped with McCloud inside suffering from gunshot wounds. He was pronounced dead at the scene.
The Oregon State Police Major Crimes Section assumed the investigation into the officer-involved shooting. Detectives processed both shooting scenes, collected firearms evidence, documented the vehicle, and interviewed the involved officers and civilian witnesses. The identities of the officers involved have not been publicly released.
The Pierce County Sheriff's Office continued investigating the two Washington homicides while coordinating with Oregon investigators. The deaths of the two Washington victims and the fatal officer-involved shooting in Seaside remain part of a multi-state investigation.

Event : Driver killed by police chase crash
OPEN RECORDS SUMMARY :
On 07/04/2026 at approximately 1:20 a.m., a Lakewood Police Department officer observed a red Dodge Challenger traveling westbound on South 74th Street at a very high rate of speed. The officer activated the patrol vehicle's emergency lights and siren and attempted to stop the vehicle. Before the officer could catch up, the Challenger continued driving away at a high rate of speed.
The Challenger crossed multiple lanes of traffic near South Tacoma Way and crashed into a power pole and railroad crossing equipment. The impact ejected both the driver and passenger from the vehicle. The vehicle immediately caught fire, and fireworks inside the vehicle began exploding, preventing first responders from immediately determining whether anyone remained inside the burning car. Officers later determined that both occupants had been ejected and had died at the scene.
Because the collision occurred within Tacoma city limits, the Tacoma Police Department assumed responsibility for the crash investigation. Lakewood Police Department Sgt. Charles Porche stated that investigators had not determined whether drugs or alcohol contributed to the crash. Authorities had not publicly identified either occupant.

Lorenzo Salgado Araujo
Age :52
Gender :Male
Race : Latino/a/e
Date : 07/07/2026
Location : 6800 block of Canal Street
City : Houston
County : Harris
State : Texas
Agency : U.S. Immigration and Customs Enforcement (ICE); Homeland Security Investigations (HSI)
Officer(s) : Not disclosed
Cause of Death : Gunshot
Event : Bystander killed by federal immigration agents during enforcement operation
OPEN RECORDS SUMMARY :
On 07/07/2026, 52-year-old Lorenzo Salgado Araujo was shot and killed by a U.S. Immigration and Customs Enforcement officer during an immigration enforcement operation in Houston’s Magnolia Park area. Salgado Araujo was not the person federal agents were originally seeking.
.
Salgado Araujo was a Mexican construction contractor who had lived in the United States for approximately 35 years and had no reported criminal record. He was driving a white work van and was on his way to pick up workers when federal immigration officers encountered him.
.
Federal authorities were conducting an operation seeking two Guatemalan men who were believed to be associated with a similar white van. DHS later acknowledged that Salgado Araujo was not one of the intended targets of the enforcement action. None of the people in his van were the people agents had originally set out to arrest.
.
ICE officers followed and attempted to stop Salgado Araujo’s van near Canal Street. Federal authorities alleged that Salgado Araujo did not comply with commands and maneuvered the vehicle in an attempt to escape.
.
DHS alleged that Salgado Araujo rammed an ICE vehicle and then attempted to run over an immigration officer. According to the federal account, the officer fired in self-defense after perceiving the moving van as a deadly threat.
.
Witnesses who were inside the van disputed that account. They said federal officers were positioned alongside the van rather than directly in front of it and denied that Salgado Araujo was attempting to run an officer over. Their statements therefore directly challenge the central justification federal authorities gave for the shooting.
.
Available reporting indicates that the fatal shot was fired from the side of the van rather than from directly in front of it. The absence of body-camera footage has made the competing accounts especially significant.
.
The ICE officer fired into the van and struck Salgado Araujo in the abdomen. Emergency medical personnel responded at approximately 6:51 a.m., and Salgado Araujo was transported to Ben Taub Hospital, where he died.
.
The officer who killed Salgado Araujo has not been publicly identified. Federal authorities also have not released body-camera or dash-camera footage documenting the shooting.
.
The three men who were riding with Salgado Araujo became important witnesses because their accounts contradicted the federal description of the shooting. Immigration authorities detained the men after Salgado Araujo was killed, creating concern that key eyewitnesses to a fatal federal shooting were simultaneously being held in immigration custody.
.
At least one of those witnesses, Daniel Tirado Pantoja, became the subject of separate federal litigation seeking his release from immigration detention. His testimony was considered particularly significant because he disputed the assertion that Salgado Araujo attempted to run down an ICE officer.
.
The fatal shooting triggered investigations by federal authorities, including the Department of Homeland Security Office of Inspector General and the FBI. Harris County officials and local advocates also demanded an independent investigation because of the conflicting accounts and the lack of publicly available video evidence.
.
The FBI initially investigated the incident in part as a possible assault on a federal officer, reflecting the government's contention that Salgado Araujo used the van as a weapon. Local officials and civil-rights advocates argued that the shooting itself also required independent scrutiny rather than relying exclusively on the federal agency involved.
.
After the killing, federal authorities drew attention to several small plastic bags containing a white crystal-like substance found inside Salgado Araujo’s work van. The FBI sought a search warrant to test the material for possible narcotics, including methamphetamine.
.
The decision to publicly disclose the suspected-drug theory before laboratory results were available drew criticism from Salgado Araujo’s family and civil-rights advocates, who argued that the allegation unfairly suggested criminal conduct unrelated to the circumstances that caused ICE to stop the van.
.
Salgado Araujo’s family said the material was a homemade electrolyte mixture prepared for Salgado Araujo and members of his construction crew to help them remain hydrated while working in Houston’s summer heat.
.
Laboratory testing subsequently undermined the federal drug implication. On 08/13/2026, U.S. Attorney Aaron Reitz confirmed that testing found that the substance in the plastic bags was not methamphetamine. Reitz did not publicly identify the substance or announce that any controlled substance had been found.
.
The negative methamphetamine result is significant because the alleged drugs had no connection to why ICE officers initially encountered Salgado Araujo. Federal officers had approached his vehicle while searching for different people associated with a similar van, and DHS subsequently acknowledged that Salgado Araujo was not an intended target.
.
The central unresolved question therefore remains the justification for the fatal gunfire: federal authorities allege that Salgado Araujo attempted to use his van against an officer, while eyewitnesses inside the vehicle say no officer was in a position to be run over and dispute the government's description of the encounter.
.
Salgado Araujo’s killing prompted large protests, vigils and calls for greater transparency in Houston. His family, LULAC, the ACLU and elected officials demanded release of evidence and an independent review of the shooting.
.
The League of United Latin American Citizens also organized a GoFundMe for Salgado Araujo’s family following his death.
.
As of 08/15/2026, the officer who fired the fatal shot had not been publicly identified, no body-camera footage had been released, and the investigation into whether the use of deadly force was legally justified remained unresolved. The subsequent confirmation that the material found in Salgado Araujo’s van was not methamphetamine removed one of the post-shooting allegations that federal authorities had publicly raised about him.
.
OFFICER BACKGROUND:
The involved ICE officer has not been publicly identified.

OPEN RECORDS SUMMARY :
On 07/07/2026, deputies with the Seneca County Sheriff's Office and officers from the Bloomville Police Department attempted to arrest Jeffrey R. Sergent, 43, of Bloomville on felony domestic violence charges. Officers located Sergent driving a pickup truck and attempted a traffic stop. Sergent continued driving until officers blocked his vehicle at the intersection of Township Road 8 and State Route 67.
Officers spent approximately ten minutes attempting to persuade Sergent to exit the truck and surrender. During the standoff, Sergent removed several knives from his pockets while remaining inside the vehicle, rolled up the windows, and stopped communicating with officers. Additional deputies arrived and positioned patrol vehicles to contain the truck.
According to the Sheriff's Office, Sergent then shifted the truck into reverse in an unsuccessful attempt to escape because a Bloomville police cruiser blocked his path. He immediately shifted into drive while deputies remained in front of and beside the truck. Multiple deputies fired into the vehicle, striking Sergent before the truck could move through the officers' positions. Sergent died at the scene.
The Seneca County Coroner determined Sergent sustained gunshot wounds to the head and arm. The final autopsy report and toxicology findings remained pending at the time of the latest public update. One deputy sustained a hand injury from shattered vehicle glass during the shooting.
The Ohio Bureau of Criminal Investigation assumed the independent investigation into the shooting. On July 13, 2026, Sheriff Fredrick Stevens released selected body-worn camera footage from the encounter. The deputies who fired their weapons were placed on administrative leave pending completion of the investigation. Their identities had not been publicly released.

Tyshawn Cooper
Age :21
Gender :Male
Race : Black
Date : 07/06/2026
Location : Ocean Boulevard near D Street
City : Hampton
County : Rockingham
State : New Hampshire
Agency : Hampton Police Department
Officer(s) : Not disclosed
Cause of Death : Suicide
Event : Suicide under police duress
OPEN RECORDS SUMMARY :
On 07/05/2026 at approximately 1:20 a.m., Hampton Police Department officers responded to reports of a shooting near Ocean Boulevard during the Fourth of July holiday weekend. Investigators determined that Tyshawn Cooper, 21, shot a 23-year-old man and a 25-year-old woman without provocation while they were walking in the Hampton Beach area. Both victims sustained serious gunshot wounds and were transported to hospitals.
Officers quickly located Cooper near the intersection of P Street and Ashworth Avenue. According to the New Hampshire Attorney General's Office, officers confronted Cooper and ordered him to stop. During the encounter, Cooper produced a handgun, raised it, and shot himself in the head at essentially the same time an officer fired a single round. Cooper died at the scene. The Office of the Chief Medical Examiner later determined his cause of death was a self-inflicted gunshot wound and ruled the manner of death suicide.
The New Hampshire Attorney General's Office, New Hampshire State Police Major Crime Unit, and Hampton Police Department conducted a joint investigation. Investigators recovered the handgun, processed multiple crime scenes, interviewed numerous witnesses, reviewed surveillance video and officer body-worn camera recordings, and completed forensic examinations. The Attorney General's Office concluded that the officer discharged one round but that Cooper's fatal injury resulted from his own gunshot.
Subsequent investigation identified Cooper as an active-duty U.S. Navy sailor stationed in the region. Investigators found no known relationship between Cooper and either victim, and authorities described the shootings as an apparently random act of violence. Both surviving victims were later released from the hospital while continuing their recovery.

City : McClure
County : Alexander
State : Illinois
Agency : Alexander County Sheriff's Office
Officer(s) : Not Disclosed
Cause of Death : Drowned
Event : Drowned during police pursuit
OPEN RECORDS SUMMARY :
REASON FOR CHASE:
Police attempted to arrest the man following a reported domestic battery.
.
NARRATIVE:
On 07/06/2026, an unidentified adult man drowned in a creek near Grapevine Trail and Levee Road in rural Alexander County, Illinois, while fleeing an Alexander County sheriff's deputy who was attempting to take him into custody following a reported domestic battery.
.
At approximately 4:30 p.m., an Alexander County Sheriff's Office deputy responded to a report of a domestic battery in McClure. According to the Illinois State Police, when the deputy attempted to take the man into custody, he ran.
.
Alexander County deputies searched for the man after he fled. Deputies subsequently located him in a creek near Grapevine Trail and Levee Road.
.
Illinois State Police reported that emergency responders were unable to reach the man before he drowned. The available official account does not explain how the man entered the creek, how long he was in the water, whether officers maintained visual contact with him, or what rescue efforts were attempted before responders reached the area.

The Alexander County Sheriff's Office requested that the Illinois State Police investigate the death. ISP publicly announced the investigation on 07/07/2026 and described it as an active investigation, stating that no additional information was available at that time.
