Location : County Highway K and Rainbow Drive
City : Village of Maine
County : Marathon
State : Wisconsin
Agency : Lincoln County Sheriff's Office; Marathon County Sheriff's Office
Officer(s) : Unknown
Cause of Death : Suicide
Event : Suicide under police duress
OPEN RECORDS SUMMARY :
On 07/28/2026 at approximately 10:50 a.m., a Lincoln County Sheriff's Office deputy attempted to stop a vehicle driven by John D. Wallace, 48, of Oregon, Wisconsin, after observing multiple traffic violations. Wallace continued driving away while the deputy pursued. The pursuit entered Marathon County, where deputies from the Marathon County Sheriff's Office joined the pursuit.
Wallace continued driving until his vehicle left the roadway near the intersection of County Highway K and Rainbow Drive in the Village of Maine. After the vehicle came to a stop, deputies approached and found Wallace suffering from a gunshot wound. Deputies rendered aid until emergency medical personnel arrived, but Wallace died at the scene.
The Wood County Sheriff's Office conducted the independent investigation into the incident. Investigators determined Wallace died from a self-inflicted gunshot wound after the pursuit ended. The Marathon County Medical Examiner ruled the death a suicide. Authorities stated no law enforcement officer fired a weapon during the incident.
The pursuit began in Lincoln County and concluded in Marathon County, resulting in a multi-agency response involving the Lincoln County Sheriff's Office, Marathon County Sheriff's Office, Wisconsin State Patrol, and emergency medical personnel. The Wisconsin Department of Justice Division of Criminal Investigation was not assigned because the death was determined to be self-inflicted rather than the result of police gunfire.

Jesse Fred
Age :42
Gender :Male
Race : White
Date : 07/28/2026
Location : 2529 West Campbell Avenue
City : Phoenix
County : Maricopa
State : Arizona
Agency : Phoenix Police Department
Officer(s) : Not Disclosed
Cause of Death : Unknown
Event : Died from medical emergency during police detention
OPEN RECORDS SUMMARY :
NARRATIVE:
On 07/28/2026, 42-year-old Jesse Fred died after Phoenix Police Department officers detained and handcuffed him at an apartment complex at 2529 West Campbell Avenue. Fred was incoherent, injured and experiencing an extreme medical emergency; Phoenix Fire personnel measured his body temperature at 107°F. He was transported to a hospital and died.
.
At approximately 3:50 p.m., Phoenix police responded after a woman called 911 and reported that a man had broken an apartment window. The caller described the man as acting erratically. Officers located Fred near the front gate of the apartment complex, bleeding from his face and partially concealed in bushes.
.
Body-camera video subsequently released by Phoenix police shows Fred appearing sluggish and largely unresponsive to officers' questions. An officer asked whether he was OK and whether he spoke Spanish, but Fred did not respond. Officers detained and handcuffed him. Police said Fred had apparently injured himself and was making incoherent statements, and officers requested Phoenix Fire personnel.
.
As Fred's condition deteriorated, one officer told the others that Fred appeared to be losing consciousness. Three officers removed him from the bushes, with one carrying his legs and two supporting his upper body. They moved Fred into the shade beside a marked police vehicle while awaiting firefighters.
.
Phoenix Fire personnel arrived approximately three minutes after officers moved Fred from the bushes. Paramedics determined that Fred's body temperature was 107°F and that he was in severe physical distress. Emergency personnel began aggressive cooling treatment, placing Fred in a plastic bag filled with ice and cold water in an attempt to lower his temperature.
.
Fred was transported by ambulance to a hospital, where he died. The Phoenix Police Department Homicide Unit assumed responsibility for the death investigation. The Maricopa County Office of the Medical Examiner was tasked with determining the cause and manner of death.
.
Phoenix police classified Fred's death as a critical incident and released an edited Critical Incident Briefing containing body-camera footage approximately two weeks later. Police said an internal investigation was also underway into the officers' actions. As of the latest material located, no officers had been charged and a final medical cause of death had not been publicly reported.
.
The circumstances make Fred's death particularly easy to miss in conventional police-death searches because police did not shoot him and the initial police account characterized the incident as a death investigation involving a man suffering from self-inflicted injuries and extreme physical distress. Fred nevertheless died after officers physically detained and handcuffed him during an active police encounter.
.
OFFICER BACKGROUND:
Phoenix Police Department has not publicly identified the officers who detained and handcuffed Fred. Body-camera footage from at least one officer was released in the department's Critical Incident Briefing. An internal investigation into the officers' actions remained underway when the briefing was released.

City : Allen Township
County : Northampton
State : Pennsylvania
Agency : Philadelphia Police Department
Officer(s) : Kenroy Anthony Cummings
Cause of Death : Gunshot
Event : Killed by off-duty police officer during family dispute
OPEN RECORDS SUMMARY :
On 07/24/2026, Shakair T. Moncrieffe, 27, was at a residence in the 1200 block of Old Gate Road in Allen Township when a dispute developed involving family members. According to the criminal complaint, a juvenile family member told investigators that Moncrieffe had verbally threatened him, causing the juvenile to contact several relatives for help, including off-duty Philadelphia Police Officer Kenroy Anthony Cummings, 25. Cummings, who was Moncrieffe’s nephew, came to the residence.
.
According to witness statements described in the criminal complaint, Moncrieffe and Cummings became involved in a physical confrontation inside the home. The juvenile reported seeing Moncrieffe push Cummings as Cummings attempted to stand. The juvenile then entered and punched Moncrieffe twice in the face. Investigators said Cummings subsequently drew a Taurus G2C 9mm handgun and fired repeatedly at Moncrieffe. Court records state that Moncrieffe was shot six times in the head, with investigators alleging that Cummings fired additional gunfire after Moncrieffe had fallen.
.
Cummings told investigators that he acted in self-defense because he believed Moncrieffe was attempting to reach for Cummings’ concealed firearm. Moncrieffe was transported by Northampton Regional EMS to Lehigh Valley Hospital-Muhlenberg, where he was pronounced dead at 2:45 p.m. An autopsy determined that he died from multiple gunshot wounds, and the Lehigh County Coroner’s Office ruled the death a homicide.
.
After the shooting, Cummings left in an orange 2023 Kia Forte. Pennsylvania State Police later located him near Race Street and Airport Road in Hanover Township and attempted to stop him. Police alleged that Cummings fled, at times exceeding 100 mph through residential areas, failed to stop at a stop sign, and continued driving away while state troopers pursued. Cummings eventually drove back to the Old Gate Road residence, where he surrendered and was taken into custody.
.
Cummings was charged with criminal homicide and fleeing or attempting to elude an officer and was jailed without bail. The Pennsylvania State Police and Northampton County District Attorney’s Office are investigating the killing. The Philadelphia Police Department Internal Affairs Bureau opened a separate administrative investigation.
.
OFFICER BACKGROUND
Kenroy Anthony Cummings, 25, was a Philadelphia Police Department officer assigned to the 24th District and patrolled the Kensington area. Cummings had approximately two years with the department and held an entry-level officer rank.
.
Philadelphia Police Commissioner Kevin Bethel said the department had begun the process of firing Cummings following his arrest. Cummings remained subject to both the Pennsylvania State Police criminal investigation and a Philadelphia Police Department Internal Affairs investigation.

Cause of Death : Gunshot
Event : Killed by police during mental health crisis
OPEN RECORDS SUMMARY :
On 07/24/2026 at approximately 3:51 a.m., Austin Police Department officers responded to a 911 call reporting a person with a handgun near a playground in the 10100 block of Baden Lane in northeast Austin. The caller reported the individual repeatedly reached toward his waistband and appeared to be armed. Officers Jake Pitcher, Alexis Cruz, and Avery Zucchi responded to the call.
At approximately 4:02 a.m., officers located 17-year-old Anthoneil Williams II near the playground. Body-worn camera video released by APD shows officers ordering Williams to keep his hands raised. Seconds later, Williams lifted his sweatshirt with one hand and reached toward his waistband with the other. Officers Pitcher, Cruz, and Zucchi fired multiple rounds, striking him. Williams was pronounced dead at the scene at approximately 4:25 a.m.
During the investigation, detectives determined the object in Williams' waistband was a plastic water bottle rather than a firearm. Investigators also determined the 911 call originated from Williams' own cellular phone. APD Chief Lisa Davis stated those findings led investigators to believe the incident was potentially a "suicide by police."
Williams was publicly identified by his father, Anthoneil Williams Sr., several days after the shooting. Family members disputed the characterization of the incident and questioned the officers' decision to use deadly force after learning their son had been carrying only a water bottle. Community members and elected officials called for additional scrutiny of the shooting and broader reforms concerning police responses to mental health crises involving juveniles.
The Austin Police Department released body-worn camera footage, surveillance video, 911 recordings, and dispatch audio within days of the shooting. The officers were placed on administrative leave in accordance with department policy while the department's Special Investigations Unit and Internal Affairs Division continue separate criminal and administrative investigations.

The driver continued driving away while the trooper pursued through Kansas City. The pursuit entered the area of East 18th Street and Prospect Avenue, where the fleeing vehicle struck John Thompson Sr. as he crossed the street in a marked crosswalk.

Corey Durell Ruiz
Age :38
Gender :Male
Race : Black
Date : 07/22/2026
Location : South Baldwin Street and Williamson Street
City : Madison
County : Dane
State : Wisconsin
Agency : Madison Police Department
Officer(s) : Kiel Baitinger-Peterson
Cause of Death : Gunshot
Event : Killed by police during arrest
OPEN RECORDS SUMMARY :
On 07/22/2026 shortly after 1:30 p.m., Madison Police Department officers responded to the Marquette neighborhood after callers reported a man checking the doors of parked vehicles. Officers encountered 38-year-old Corey Durell Ruiz, who rode away on a bicycle when police attempted to stop him. Officers located Ruiz near Williamson Street and South Baldwin Street and attempted to take him into custody. Ruiz was under Wisconsin Department of Corrections community supervision and had been listed as absconded.
Four Madison police officers became involved in a physical struggle with Ruiz in the middle of the intersection. Multiple bystanders stopped and recorded the encounter. Video shows several officers surrounding Ruiz and attempting to restrain him on the pavement. Police Chief John Patterson said Ruiz produced a fixed-blade knife during the struggle and injured one of the officers. Another officer deployed a Taser, but the electrical discharge did not stop the struggle.
The officer who sustained the knife injury fired three gunshots at Ruiz from close range. Bystander recordings captured the shots and showed Ruiz collapsing while officers remained around him. Officers subsequently placed Ruiz in handcuffs. Ruiz died at the scene from multiple gunshot wounds. The four officers who participated in the attempted arrest were removed from field duty while the shooting was investigated.
The Wisconsin Department of Justice Division of Criminal Investigation assumed responsibility for the criminal investigation under Wisconsin’s law requiring an outside agency to investigate deaths caused by law enforcement. DCI investigators collected the knife, firearms, Taser evidence, bystander recordings, physical evidence and witness statements. The City of Madison’s Office of the Independent Monitor also opened a separate review of the officers’ conduct.
Ruiz’s family retained civil-rights attorneys Ben Crump, Steve Hart and B’Ivory Lamarr. His mother, siblings, 13-year-old daughter Kah’liyah Aliomar and other relatives appeared at a news conference at the Wisconsin State Capitol on 07/24/2026. The family described Ruiz as a father, brother and son who worked in construction, made music and was known for making people laugh. His daughter said his criminal record did not erase his humanity or his relationship with his family.
Crump argued that police escalated an investigation into suspected vehicle break-ins into a fatal encounter and called for the officer who fired to face criminal charges. The family’s attorneys questioned why four officers could not restrain Ruiz without killing him and pointed to the bystander recordings showing officers holding him down when the shots were fired. The legal team demanded a complete independent investigation, preservation and release of all evidence, and accountability for the officer’s use of deadly force.
The shooting triggered immediate demonstrations at Williamson and South Baldwin streets. Hundreds of people marched from the shooting scene to the Wisconsin State Capitol on the evening of 07/22/2026. Demonstrators entered the Capitol Square and caused organizers to cancel a scheduled Wisconsin Chamber Orchestra “Concerts on the Square” performance. Protesters demanded that the officer who shot Ruiz be identified and prosecuted.
Residents created a memorial of flowers, candles, photographs and signs at the intersection. A candlelight vigil was held there on 07/23/2026, followed by additional marches and gatherings. Ruiz’s daughter and other relatives attended community events mourning him. Protesters maintained a continuous presence at the intersection, blocking Williamson Street and South Baldwin Street to vehicle traffic during extreme summer heat.
Protesters entered Chief Patterson’s 07/23/2026 press conference at the City-County Building, took control of the microphone and demanded that police disclose the officer’s name and make an arrest. Madison Common Council President Sabrina Madison said the bystander footage left her angry and questioned why the officer who fired was able to leave rather than being taken into custody. Mayor Satya Rhodes-Conway said Ruiz’s past did not justify his death during an encounter with police.
The continuing demonstrations disrupted normal activity across Madison. The city postponed the Madison Public Market grand opening, while organizers canceled or altered AtwoodFest, the Dane County Farmers’ Market and other public events. Protesters continued occupying the shooting location for several days, and city crews placed traffic barriers around the intersection. The sustained blockade affected nearby residents, bus routes and businesses while demonstrators insisted that Madison could not return to normal activity without accountability for Ruiz’s death.
The shooting carried particular weight in the Marquette neighborhood because Madison police had previously killed Paul Heenan nearby in 2012 and Tony Robinson several blocks away in 2015. Community organizations, elected officials and the NAACP Legal Defense Fund described Ruiz’s death as part of a recurring pattern in which low-level police encounters involving marginalized residents escalate into fatal force. The Legal Defense Fund called for transparency, de-escalation and a full investigation of the officers’ actions.
