On 08/14/2023, Walla Walla County deputies attempted to serve a warrant at a residence in the 13970 block of Merry Lane in Burbank. Philip Stroh, 65, barricaded himself inside the home, initiating an overnight standoff that began on 08/13/2023 and continued into the next day. Deputies established a perimeter and attempted to negotiate while Stroh remained inside and armed. During the confrontation with law enforcement outside, Stroh shot himself. On 08/14/2023, at approximately 3:06 p.m., Wayne County 911 received a call reporting that 48-year-old Chad DeWayne Pike was inside a residence at 219 Capps Bridge Road near Princeton carrying a loaded shotgun. The caller told the dispatcher Pike was demanding to see her husband, who had died several years earlier. During the 16-minute 911 call, the caller reported that Pike walked through the residence with the shotgun, pushed her aside, and continued searching the home. She eventually locked herself inside a bedroom with a handgun while waiting for deputies to arrive.
Wayne County Sheriff's Office deputies responded to the residence and encountered Pike outside the home while he was carrying the shotgun. Deputies repeatedly ordered him to put the weapon down. According to the Sheriff's Office, Pike did not comply and instead raised the shotgun toward the deputies. At least one deputy fired, striking Pike. Deputies requested emergency medical assistance, but Pike died at the scene. No deputies were physically injured during the encounter.
The North Carolina State Bureau of Investigation assumed responsibility for the criminal investigation. The Wayne County Sheriff's Office placed the deputies involved on administrative leave pending completion of the SBI investigation, consistent with department policy.

On 08/13/2023, at approximately 1:30 a.m., a Dothan Police Department officer observed 25-year-old Jordan Nicholas Griffin drive through a red traffic signal at the intersection of Denton Road and Montgomery Highway. The officer attempted to catch up to Griffin to conduct a traffic stop for the traffic violation. Griffin continued driving away as the officer closed the distance.
Griffin traveled south along Montgomery Highway and drove through the parking lot of a Family Dollar store. His vehicle left the roadway near Chinook Street, struck a utility pole and a tree, and caught fire. Officers and emergency medical personnel responded, but Griffin died at the scene from injuries sustained in the crash.
Dothan Police Captain Lynn Watkins stated that the department did not classify the incident as a police pursuit because the officer had not initiated a sustained chase before the collision. Watkins said Griffin may have been preparing to stop once the patrol vehicle got immediately behind him, but the crash occurred almost immediately afterward.

On 08/13/2023, at approximately 5:20 a.m., Los Angeles Police Department officers responded to the 6300 block of South Hoover Street after the owner of a vehicle reported that 27-year-old Olban Perdomo Rodas was sitting inside a car that had been stolen from the property. When officers arrived, the victim directed them to the driveway where Perdomo remained seated in the driver's seat of the vehicle.
Officers gave repeated commands for Perdomo to get out of the vehicle. He did not comply. Officers approached, opened the driver's door, and attempted to make contact. As they did so, officers saw Perdomo's arm extending from the rear seat while holding a black handgun. The officers backed away, requested additional units, and established distance from the vehicle.
Perdomo then got out of the vehicle while still holding the handgun. Officers repeatedly ordered him to drop the weapon. One officer fired a 40 mm less-lethal launcher, but the projectile did not stop him. According to LAPD, Perdomo continued holding the handgun and pointed it toward the officers. Five officers then fired their service weapons, striking him multiple times. Officers recovered a loaded semiautomatic handgun at the scene, handcuffed Perdomo, and requested Los Angeles Fire Department paramedics. He died before he could be transported to a hospital. No officers or bystanders were injured.
On 08/21/2023, LAPD identified the officers who fired as Police Officer III Eric Darling, Police Officer II Shuhei Fuchigami, Police Officer II Steven Valenzuela, Police Officer II Alfred Foca, and Police Officer II Julian Lozano, all assigned to 77th Street Division. LAPD's Force Investigation Division conducted the criminal and administrative investigation.
LAPD later released body-camera video, audio, photographs, and investigative materials documenting the encounter. The released recordings showed officers attempting verbal commands, the deployment of the less-lethal launcher, and the subsequent shooting.

Police later determined the vehicle belonged to 45-year-old John Andreyo, information that was not available to responding officers when they arrived.
Two Tempe police officers located Andreyo in a stairwell near the shopping center and repeatedly ordered him to show his hands. The East Valley Critical Incident Response Team (EVCIRT), led by the Mesa Police Department, conducted the criminal investigation. Investigators collected body-camera video, witness statements, forensic evidence, dispatch recordings, and scene evidence before forwarding the case to the Maricopa County Attorney's Office for review. Under Tempe's officer-involved shooting protocol, the department also opened a separate administrative investigation.
Tempe Police released edited body-camera footage on 10/30/2023 showing the officers confronting Andreyo, the exchange of gunfire, and officers rendering aid after the shooting. The city has not publicly identified the officers who fired.

Before officers arrived, 28-year-old Zachary Joseph Johnson left the scene in a Subaru Forester.
At approximately 12:52 a.m. on 08/14/2023, an off-duty Muskegon County Sheriff's Office deputy located Johnson's vehicle and attempted to conduct a traffic stop. Johnson continued driving away while deputies, Michigan State Police troopers, Muskegon Township officers, and Fruitport Township officers pursued him for several miles.
The pursuit ended when Johnson's Subaru entered a ditch on South Slocum Road near Neil Road outside Ravenna. Deputies approached the vehicle and attempted to arrest Johnson at gunpoint. During the encounter, Johnson fired multiple shots, striking Muskegon County Deputy Joshua Hankins several times. Other deputies and officers returned fire, striking Johnson. He died at the scene. Deputy Hankins was transported to Trinity Health Muskegon, treated for multiple gunshot wounds, and released from the hospital later that morning.
The Michigan State Police assumed responsibility for the investigation under Michigan's officer-involved shooting protocol. On 08/21/2023, the Muskegon County Sheriff's Office identified the deceased as Zachary Joseph Johnson.

On 08/14/2023, shortly before noon, Philadelphia Police Officers Mark Dial and Michael Morris observed 27-year-old Eddie Irizarry driving erratically in the Kensington neighborhood. Officers followed Irizarry after he drove the wrong way on East Willard Street and stopped behind his Toyota Corolla after it came to a stop.
Philadelphia police initially told the public that Irizarry had exited his vehicle and advanced toward officers with a knife. Within days, Police Commissioner Danielle Outlaw publicly acknowledged that the original account was inaccurate after body-camera footage showed Irizarry remained seated inside his vehicle when Officer Mark Dial rushed to the driver's side window and fired six shots through the closed window within seconds of exiting the patrol vehicle. The video contradicted the department's original public narrative.
Investigators determined that Irizarry was holding a folding knife inside the vehicle. Officer Dial later stated that he believed the knife was a firearm and that he feared for his life. Officer Michael Morris did not fire his weapon. Dial fired six rounds, striking Irizarry multiple times. Officers removed Irizarry from the vehicle and transported him to Temple University Hospital, where he died from his injuries.
Philadelphia Police released body-camera footage after intense public criticism. The recordings became central evidence in the criminal case because they contradicted the department's initial statements describing the shooting. Commissioner Outlaw stated that police should not have released inaccurate information before reviewing the video evidence.
On 09/08/2023, the Philadelphia District Attorney charged Officer Mark Dial with murder, voluntary manslaughter, aggravated assault, reckless endangerment, possession of an instrument of crime, simple assault, and official oppression. Dial surrendered and was suspended with the intent to dismiss from the Philadelphia Police Department.
On 09/26/2023, Philadelphia Municipal Court Judge Wendy Pew dismissed all criminal charges at the preliminary hearing. The District Attorney immediately appealed. On 10/25/2023, Philadelphia Common Pleas Judge Lillian Ransom reinstated every charge, including murder, allowing the prosecution to proceed.
Following additional pretrial proceedings, prosecutors withdrew the first-degree murder charge before trial. In May 2025, a jury acquitted Dial of third-degree murder but convicted him of voluntary manslaughter, possession of an instrument of crime, and reckless endangerment. On 07/17/2025, Judge Glenn Bronson sentenced Dial to 9½ months to two years in prison. Because Dial had already served approximately ten months while the case was pending, the court granted him immediate parole. The sentence drew criticism from Eddie Irizarry's family, District Attorney Larry Krasner, and community organizations.
Mark Dial joined the Philadelphia Police Department in approximately 2018 and had about five years of service when he killed Eddie Irizarry. A search of publicly available records located no verified prior fatal police shootings before this incident. Following the shooting, Philadelphia Police suspended Dial with the intent to terminate his employment. He refused to provide an internal statement during the department's administrative investigation. Dial became one of the few American police officers convicted of voluntary manslaughter for an on-duty shooting. He served approximately ten months in custody before receiving immediate parole following sentencing. On 08/10/2023, shortly after 12:45 a.m., Leavenworth Police Department officers responded to a domestic violence call in the 900 block of Columbia Avenue in Leavenworth, Kansas. Officers learned a woman reported being raped and identified the driver as the assailant. As officers searched the residence, they saw the driver's pickup pass the scene and attempted to stop it. The driver continued driving away while officers pursued across the Kansas-Missouri state line.
The pursuit ended on Missouri Highway 45 Spur near Stillings Road in Platte County after the pickup became disabled. Officers surrounded the vehicle and attempted to take the driver into custody. According to investigative reports, officers spent several minutes giving commands, attempting to de-escalate the situation, and deployed a beanbag shotgun before the encounter turned deadly. Officers reported the driver repeatedly stated they would have to shoot him and refused commands to surrender.
Investigators reported the driver removed a black object from a backpack and pointed it toward officers. Multiple Leavenworth police officers opened fire. Jonathan Heath-Taylor was struck multiple times and transported to North Kansas City Hospital, where he died on 08/14/2023. An autopsy determined the cause of death was multiple gunshot wounds. Toxicology testing detected fentanyl and norfentanyl.
The Platte County Sheriff's Office investigated the shooting with assistance from the Kansas City, Missouri, Police Department Crime Scene Unit. In March 2024, Platte County Prosecuting Attorney Eric Zahnd issued a 15-page report concluding the officers' use of deadly force was legally justified and declined to file criminal charges. The report identified the officers who fired as Andrew Beard, Caleb Bonar, Cody McKinney, Tyler Danner, Nathaniel DesJardins, Brandon Eddinger, and Tanner Schuler of the Leavenworth Police Department.

On 08/14/2023, at approximately 3:59 a.m., Long Branch Police Department officers responded to a 911 report of an armed home invasion on Elmwood Avenue. A marked police SUV driven by Patrolman Lucas Brito was traveling to the call with emergency lights and siren activated. At the same time, a BMW believed to be leaving the reported break-in entered the intersection of MacArthur Avenue and Elmwood Avenue. The BMW was driven by 46-year-old Altonia Williams, and 38-year-old Tracee Blount was riding in the front passenger seat.
Officer Brito's patrol vehicle collided with the BMW in the intersection. The impact caused catastrophic damage to both vehicles. Blount suffered critical injuries and was transported to Jersey Shore University Medical Center in Neptune, where she was pronounced dead at approximately 7:12 a.m. Williams was taken into custody and transported to the same hospital for treatment. Officer Brito also sustained injuries and was treated and released.
Under New Jersey law, the Office of Public Integrity and Accountability (OPIA) in the Attorney General's Office assumed responsibility for the investigation because the death occurred during an encounter involving a law enforcement officer. Investigators reviewed witness statements, physical evidence, autopsy findings, police radio transmissions, dashboard-camera video, body-worn camera recordings, and 911 calls. On 10/03/2023, the Attorney General publicly released the videos and audio recordings after first providing them to Blount's family.
The investigation determined that Officer Brito was responding to an emergency call when the collision occurred. The evidence was presented to a New Jersey state grand jury as required by state law. On 02/05/2024, the grand jury returned a "no bill," declining to indict Officer Lucas Brito on any criminal charge arising from the collision. The Attorney General announced that no criminal charges would be filed against him.
Altonia Williams was identified as a participant in the reported home invasion and was taken into custody following the collision. The publicly available records reviewed through August 2026 do not indicate that Blount's estate obtained a civil settlement or wrongful-death judgment against Long Branch, Officer Brito, or the State of New Jersey.
Dashboard-camera footage from Officer Brito's patrol vehicle, body-camera footage, police radio traffic, and 911 recordings were publicly released by the Attorney General's Office as part of New Jersey's police transparency policy.

On 08/12/2023, at approximately 12:44 p.m., Polk County dispatch received multiple 911 calls reporting that 25-year-old Bradley Robert Van Heeswyk was firing an AK-47-style rifle in the 400 block of Pleasant Circle in Pleasant Hill. Callers reported that Van Heeswyk was naked, shooting into a residence, and had killed two family dogs before police arrived. Officers responded immediately to an active-shooter situation in a residential neighborhood.
Pleasant Hill Police Officer Steven Burroughs located Van Heeswyk in the backyard of a residence while he continued holding the rifle. Burroughs repeatedly ordered him to drop the weapon. According to the Iowa Division of Criminal Investigation and the Polk County Attorney, Van Heeswyk instead raised the rifle above his head while continuing to hold it. Burroughs fired a single shotgun round, striking Van Heeswyk. Officers immediately rendered medical aid and requested an ambulance. Van Heeswyk was transported to MercyOne Medical Center, where he died from his injuries.
The Iowa Division of Criminal Investigation investigated the shooting. Investigators reviewed body-camera video, witness statements, physical evidence, dispatch recordings, forensic evidence, and interviewed Officer Burroughs. Both the Polk County Attorney's Office and the Iowa Attorney General independently reviewed the investigation.

The vehicle ran multiple red lights before entering the intersection of North 68th Street and West Silver Spring Drive, where it collided with a vehicle occupied by two women. The Hyundai then caught fire.
Jamillian Brown, who was riding as a passenger in the Hyundai, suffered fatal injuries and died at the scene. Seven other teenage passengers in the Hyundai survived with injuries. On 08/11/2023, at approximately 8:49 a.m., San Bernardino Police Department officers responded to multiple 911 calls reporting that an armed man was holding a gas station clerk at gunpoint at the Arco station located at Base Line Street and H Street. Callers described the man as armed with a handgun and associated with a Mazda parked near the fuel pumps.
A responding officer arrived within minutes and located a man matching the description walking away from the convenience store toward the fuel pumps while holding a handgun. The officer repeatedly ordered him to drop the weapon. The officer then fired two rounds, striking the man. Officers recovered a handgun at the scene, took him into custody, and requested emergency medical assistance. He was transported to a local hospital, where he died from his injuries.
Investigators later determined through witness interviews and surveillance footage that the man had committed an armed robbery inside the gas station immediately before officers arrived. Several bystanders were present in the fueling area during the shooting. The San Bernardino County District Attorney's Office responded to the scene and conducted a parallel investigation alongside the San Bernardino Police Department.

As officers approached, a gunfire exchange occurred. Investigator Celtain Batterson was struck in the right arm. Officers Celtain Batterson, Nicholas Ayres, and Christopher Dyess fired their weapons, striking Heard. He was transported to a hospital, where he died from his injuries. Batterson was treated and released. The Tennessee Bureau of Investigation assumed responsibility for the shooting investigation.
On 08/20/2023, Hamilton County District Attorney Coty Wamp released surveillance video from the shooting. According to the district attorney, the video showed the encounter at the gas station and was obtained from a private business because the investigators involved were not equipped with body-worn cameras. The release followed public requests from Heard's family and community members seeking greater transparency.
The Tennessee Bureau of Investigation completed its investigation and submitted the findings to the Hamilton County District Attorney's Office. According to the Tennessee Bureau of Investigation’s preliminary account, Greer grabbed the agent while the agent searched the rear of the SUV and gained control of the agent’s firearm during the struggle. Greer then returned to the SUV and backed it into the task-force vehicle.
A second task-force agent entered the SUV while attempting to take Greer into custody. The TBI reported that Greer grabbed the second agent’s drawn firearm during the struggle inside the vehicle. The agent fired, striking Greer. Greer died at the scene.
The Tennessee Bureau of Investigation opened an independent investigation at the request of 23rd Judicial District Attorney General Ray Crouch. TBI agents collected physical evidence, interviewed witnesses and reconstructed the sequence of events for prosecutorial review. On 08/10/2023, at approximately 12:50 p.m., New York State Police troopers and members of the St. Lawrence County Sheriff's Office searched a wooded area along Donnerville Road in the Town of Russell for 34-year-old Shawn Sheridan, who had failed to appear for sentencing after pleading guilty in the shooting of his former fiancée. Investigators located Sheridan at a campsite concealed in the woods.
As officers and a police K9 approached, Sheridan opened fire. A New York State trooper returned fire, striking Sheridan. He was pronounced dead at the scene. The trooper was not injured, but the police K9 was shot and treated for non-life-threatening injuries.
On 08/16/2023, New York State Police released additional details from the investigation. Investigators reported that Sheridan was carrying two long guns, wearing a tactical vest and a bandolier of ammunition, and had additional firearms and ammunition stored in a tent at the campsite. Lexington County Coroner Margaret Fisher identified the deceased as Bernice Junior Smith III of Aiken. An autopsy was performed at the Medical University of South Carolina. On September 29, 2023, the coroner ruled Smith's death accidental, with the cause of death listed as multi-drug toxicity.

On 08/09/2023, at approximately 10:15 p.m., a Northern Berks Regional Police Department officer observed several motorcycles traveling southbound on Route 61 at what police described as a high rate of speed. The officer attempted to stop the motorcycles near the intersection of Route 61 and Route 73. The motorcycles accelerated away. Because of their speed, the officer did not continue the attempted stop.
A few minutes later, officers came upon a crash at the intersection of Route 61 and Cross Keys Road. Amilcar J. Cortez-Ramos, 29, was riding one of the motorcycles when it struck the rear of a sedan that was stopped at a red traffic signal. Cortez-Ramos was thrown from the motorcycle and pronounced dead at the scene by the Berks County Coroner. The coroner ruled the death an accident caused by multiple traumatic injuries. On 08/10/2023, at approximately 11:20 p.m., Atlanta Police Department Officer Kiran Kimbrough responded to a minor traffic collision at Cunningham Place SW and Joseph E. Lowery Boulevard SW. After investigating the crash, Kimbrough attempted to issue Johnny Hollman Sr., a 62-year-old church deacon, a traffic citation. A confrontation followed when Hollman objected to the citation and requested a supervisor. During the encounter, Kimbrough attempted to arrest Hollman, and the two struggled on the ground. Kimbrough deployed his Taser while attempting to restrain Hollman. Hollman repeatedly stated that he could not breathe before becoming unresponsive. Officers requested EMS, and Hollman was transported to Grady Memorial Hospital, where he was pronounced dead.
The Georgia Bureau of Investigation conducted the independent criminal investigation. The Fulton County Medical Examiner ruled Hollman's death a homicide, finding that an abnormal heart rhythm triggered during the restraint and Taser deployment caused his death, with underlying heart disease listed as a contributing factor. Body-camera video released in November 2023 showed Hollman repeatedly asking for a supervisor before the confrontation escalated.
Atlanta Police Chief Darin Schierbaum terminated Officer Kiran Kimbrough in October 2023, concluding that department policy required the officer to honor Hollman's request for a supervisor rather than arrest him over the citation dispute. The Atlanta Civil Service Board later upheld Kimbrough's termination.

Event : Killed by police during disturbance call

OPEN RECORDS SUMMARY :

On 08/09/2023, at approximately 8:45 p.m., Amarillo Police Department officers responded to the area of Southwest 48th Avenue and Cline Road after receiving a report of a domestic disturbance. Officers were told that 39-year-old Daniel M. Smith was in an alley south of the residence.
Officers located Smith in a vehicle and maneuvered patrol units around it in an attempt to prevent him from leaving. According to police, Smith reversed his vehicle into one patrol unit, nearly striking an officer who was inside. Smith then drove forward, pushing another patrol vehicle into a shed and fence. During the encounter, one Amarillo police officer fired at Smith. Officers removed Smith from the vehicle and began emergency medical treatment until Amarillo Medical Services arrived. Smith was transported to a hospital, where he was pronounced dead.
The Amarillo Police Department Homicide Unit investigated the shooting and processed the scene. Smith's next of kin was notified before his identity was released. Public reporting identified Smith as the only person struck by gunfire during the encounter.

OPEN RECORDS SUMMARY :

On 08/10/2023, shortly after 3:00 p.m., Wilson County Sheriff's Office deputies responded to a residence in the 100 block of Vesta Road after receiving reports that 60-year-old Kevin Withrow had assaulted a family member and was armed with a handgun. Deputies attempted to contact Withrow, who remained inside the residence. The encounter developed into an armed standoff.
During the standoff, deputies negotiated with Withrow for several hours in an effort to secure a peaceful surrender. According to the Tennessee Bureau of Investigation, Withrow fired at deputies from inside the residence. Deputies returned fire, striking him. Emergency medical personnel responded, but Withrow was pronounced dead at the scene. No deputies were reported to have been struck during the exchange.
The Tennessee Bureau of Investigation assumed responsibility for the independent investigation, as required for fatal shootings involving law enforcement in Tennessee. The investigative findings were submitted to the District Attorney General for review. Public reporting reviewed through August 2026 did not identify the deputies who fired their weapons.
