On or about 09/24/2025, deputies responded to a report of battery at Links Edge Condominiums on Saxony Circle, Deep Creek. While deputies were speaking with the alleged victim, 50-year-old Daniel Burch reportedly approached and a verbal altercation escalated. A scuffle ensued, and at least one deputy discharged at least three shots. Burch was hit at least twice and transported by air to a hospital. Overnight, his condition declined and he died on the morning of 09/25/2025. The deputy involved was taken to hospital for heart issues (not from gunfire) and was later released. The Sheriff’s Office has placed the deputy on administrative leave, and the incident is under investigation.

On 09/25/2025, Sheridan police responded to reports of a man threatening passersby with a machete on the Bear Creek Bike Path near South Clay Street and North Drive. Witnesses called 911, and officers encountered the man, issuing repeated commands to drop the machete. He refused and advanced toward them. Officers first fired less-lethal munitions, then opened fire, striking him. The man was transported to Swedish Medical Center, where he was pronounced dead. The 18th Judicial District Critical Incident Response Team is investigating, and involved officers have been placed on administrative leave.

On 09/24/2025 at approximately 8:40 p.m., Gary police responded to a domestic battery call at a home on Connecticut Street. A 24-year-old man exited the residence armed with a gun and confronted officers. Police opened fire, and the man was transported to a hospital, where he died. A 57-year-old man was also hospitalized with injuries related to the domestic battery incident. The Lake County Sheriff’s Department is conducting an independent investigation.

On 09/24/2025, at approximately 6:40 a.m., 29-year-old Joshua Jahn allegedly opened fire at the U.S. Immigration and Customs Enforcement facility at 8101 North Stemmons Freeway in Dallas. He fired at the building and a transport van in the sallyport, striking three ICE detainees inside. One detainee was killed, and two others remain in critical condition. Jahn was later found on a nearby rooftop, dead by suicide under police duress. Ammunition at the scene was claimed by the FBI to have been engraved with “anti-ICE” messages. Oddly, no INCE officers were shot in an event which only targeted migrants. The FBI, Dallas Police Department, and ICE confirmed the incident and are conducting an investigation.

On 09/24/2025, 21-year-old Yahir David Martinez, a resident of Brownsville, died at the scene after crashing into a semi-trailer during a high-speed pursuit involving Border Patrol on State Highway 4 east of Indiana Avenue. The driver was carrying two passengers and had been transporting narcotics. The semi-trailer driver and the male passenger were hospitalized, treated, and released; the female passenger remains hospitalized. Martinez was pronounced dead at the crash site.

On 09/24/2025 Oklahoma City Police responded at about 3:10 a.m. to a disturbance call in a southwest neighborhood near the 3000 block of Southwest 64th Terrace. Sgt. Christopher Skinner encountered Jason Redshirt, who was shouting outside a residence and may have been intoxicated. Redshirt threw wood and bricks at Officer Skinner. The officer fired his weapon, striking Redshirt. Redshirt was hospitalized after the shooting. More than two months later on 12/08/2025, Redshirt, age 45, died from his injuries. Police classified the death as a homicide under department policy. The shooting is under review by the homicide unit and district attorney’s office.

On 09/23/2025, officers in Johnston County attempted to stop a stolen vehicle on State Highway 199. The driver did not stop. Police escalated a chase. On 09/23/2025, at approximately 7:55 p.m., deputies with the Calvert County Sheriff’s Office attempted to stop a motorcycle driven by 39-year-old David Martin Sweet Jr. of Lothian. The operator refused to stop, and police escalated a chase with speeds exceeding 120 mph while traveling the wrong way on Route 4. The motorcycle struck a vehicle near Chaneyville Road in Dunkirk, killing Sweet at the scene. A 14-year-old female passenger on the motorcycle was flown to Johns Hopkins Hospital with serious injuries.

On 09/23/2025, 62-year-old Trina Jones of Alexandria was first struck by a hit-and-run driver while crossing Richmond Highway at Belford Drive in Hybla Valley. As Fairfax County police responded to the scene, an officer driving a marked patrol vehicle struck Jones while she was still alive in the roadway. She was transported to a hospital where she died from her injuries. Both criminal and administrative investigations were launched by the Fairfax County Police Department.

On September 23, 2025, officers of the San Diego Police Department responded to a 911 call from the area of Fay Elementary School in City Heights. Upon arrival they encountered 41-year-old Huy Ly holding what appeared to be a firearm. Ly pointed the object at officers. The driver’s object was later determined to be a replica firearm. Officers fired and struck Ly, who was pronounced dead at the scene. The United States Department of Justice is reviewing use-of-force policies in connection with the incident.

On 09/22/2025 shortly after 11:00 p.m., a man identified as Patrick O. Cardew Jr. was found suffering from gunshot wounds at the intersection of Northeast 102nd Avenue and Sandy Boulevard in the Parkrose neighborhood of Northeast Portland. Police responded to reports of a shooting. Cardew Jr. was declared dead at the scene after officers discovered his body. On 09/22/2025 around 4:14 a.m., a Hampton police officer went into a Royal Farms convenience store in the 4000 block of West Mercury Boulevard while an attempted robbery was in progress. Inside, the officer encountered a man armed with a firearm. The officer opened fire, striking him. The man was transported to a hospital and later died. A female suspect involved in the robbery was arrested nearby.

On 09/22/2025, just before 4:00 a.m., a gunman opened fire into the Amazon facility in Columbus, Georgia from outside and damaged windows of the associate entrance. He then walked a short distance away. Responding officers confronted him, and he brandished a firearm, leading to a brief standoff. The incident ended when he died by a self-inflicted gunshot wound. No other people were injured.

On 09/22/2025 at about 5:00 a.m., Brunswick County Sheriff’s deputies responded to a call at the Minuteman convenience store on Mercantile Drive in Leland. Deputies encountered Brandon Wayne Taylor, 34, of Collinsville, Virginia. According to officials, Taylor drew a firearm and fired, striking a deputy twice. Deputies opened fire, killing him. The deputy was taken to a hospital and later released. Taylor’s body was transported to the State Medical Examiner’s Office for autopsy.

On 09/22/2025, Fort Wayne police responded to a domestic disturbance call at 2100 St. Mary’s Avenue. During the response, a suspect stabbed an officer. Other officers then shot and killed him. The officer who was stabbed was transported to a hospital in serious condition. On 09/22/2025, Lafayette Police arrested Lennis Mitchell, 36, on a domestic battery charge. During the arrest process, Mitchell suffered a medical emergency and collapsed. Police responded to reports of gunfire at a home on the 5100 block of North 10th Street. James Harris shot a woman inside the house and then barricaded himself for several hours. He fired at officers and medics from inside. When police entered, Harris was found dead from a self-inflicted gunshot wound. The woman escaped through a second-story window and was hospitalized with gunshot and fall injuries.

Police in Miami, Oklahoma, responded to reports of a shooting following a vehicle collision. The driver, identified as 37-year-old Cody Ryan Mayfield, allegedly shot another driver before leaving the scene. Officers located Mayfield’s vehicle near Peoria and E Street N.W. When police surrounded a nearby residence, they said Mayfield emerged holding a gun to his head and threatening suicide. On 09/20/2025, Officer Jacob Buckwald was involved in a two-vehicle collision on Butler Avenue in Flagstaff. The driver of the second vehicle was pronounced dead at the scene. Buckwald was hospitalized with minor injuries and later released.
This fatal crash follows a previous incident in July 2024 when Buckwald was involved in another serious crash documented at well over 70 miles per hour, which caused devastating injuries. He was suspended after that crash. He was reinstated to limited duty in late 2024 and only cleared back for full patrol duty in August 2025.
The department has suggested that Buckwald was allegedly “impaired” due to seizure medication in the earlier wreck. The department has not explained the cause of the possible impairment and repeat crashes caused by this officer. Allowing him to resume patrol duties despite prior dangerous crashes raises serious questions about oversight and accountability. Following the September 2025 crash, Officer Buckwald resigned from the Flagstaff Police Department.

On 09/20/2025, deputies from the Harris County Sheriff’s Office responded to reports of a disturbance at apartments on 719 Sheldon Road in Channelview involving broken windows and vehicle break-ins. They encountered a man holding a sharp object who fled. Deputies later reported that he moved toward them, and they opened fire. The man was struck, transported to a hospital, and later pronounced dead.

On 09/20/2025, police believe Andre Anthony Matijasevic, 31, fatally shot a woman in Carlsbad and stole her SUV on Friday night. Police located the vehicle through license plate readers and pursued it into Orange County. The chase ended at Avenida de la Carlota and El Toro Road outside an In-N-Out Burger in Laguna Hills. Matijasevic exited the vehicle and, according to police, fired at officers. Officers opened fire, striking him. He was transported to a hospital where he was pronounced dead.

On 09/20/2025 at approximately 3:30 p.m., Hobart Fire Department paramedics and New Chicago Police responded to a residence at 216 Huber Boulevard after a 911 call reporting a deceased person inside who had died from natural causes of cardiac arrest. While at the residence, 64-year-old Benjamin Scott, the co-owner of the home (also the 911 caller) became agitated with police. Officers requested an ambulance to forcibly take him for a psychological evaluation. Around 4:30 p.m., two New Chicago police officers fired inside the residence for undisclosed reasons, killing Scott. Scott’s death was ruled a homicide (i.e. caused by another) by gunshot. The Lake County Coroner will identify the deceased and conduct an autopsy. The case is under investigation and will be reviewed by the Lake County Prosecutor’s Office.

On 09/20/2025, a man driving a white Mercedes-Benz entered the Peace Arch Port of Entry in Blaine, Washington, traveling the wrong way and crashing into a curb before running toward inspection lanes. Officers reported seeing a gun fall from his waistband as they ordered him to the ground. He was restrained and placed in a wheelchair but soon became unresponsive. On 09/19/2025, Isaiah Joseph Dionne, age 4, was found unresponsive in his home in Walhalla, South Carolina. The county coroner ruled the death a homicide and described Isaiah as severely underweight and malnourished, with visible ribs and bones. He weighed approximately 21 pounds. Two individuals, Joshua Emanuel Lamar Latimer, a then-South Carolina Highway Patrol trooper, and Destiny Shay Dionne (the boy’s mother), were charged with unlawful conduct toward a child and homicide by child abuse after allegedly placing him (and a 2-year-old child) at unreasonable risk of harm. Latimer was terminated from his law enforcement post on Sept. 19 upon his arrest.

On 09/18/2025 around 6:45 p.m., Brunswick Police responded to a disturbance at the BP station at Boston Road and Pearl Road. Officers took Jacob Derkin, 41, into custody. He was transported to a hospital where he was pronounced dead. The Medina County Coroner ruled the cause of death was cardiac arrest, categorized as a medical emergency. Officials said Derkin had sustained injuries prior to his arrest. Autopsy and toxicology tests remain pending.

On 09/22/2025 at about 5:00 a.m., Pasadena Police attempted to stop a vehicle. The driver refused to stop, and police escalated a chase that lasted less than one minute before being terminated. Nearly a mile later, the vehicle crashed into the garage of a home in the 300 block of Orange Grove Avenue near Columbia Street in South Pasadena. The car flipped onto its hood, partially inside the garage. Andrew Gonzalez, 20, was pronounced dead at the scene.

On 09/19/2025 at about 6:35 p.m., Fort Lauderdale Police responded to a domestic disturbance in the 3800 block of Northeast 34th Avenue. Officers were told the person inside might be armed and experiencing a mental health crisis. A perimeter was established. SWAT and the Crisis Negotiation Team responded. A single gunshot was later heard inside the home. A drone was used to check inside, and the person was found dead. No contact was made with them before the gunshot.

On September 19, 2025, an LAPD officer driving a marked patrol car with lights and sirens activated was responding to a separate emergency call. The officer entered the median on Figueroa Street near Woodside Drive to pass traffic and struck 30-year-old Gerardo Estrada, who was reportedly riding a skateboard. Estrada was killed in the collision.

On September 19, 2025, Rapid City police attempted to arrest a 43-year-old man on outstanding warrants under unclear circumstances. Police allege that he fled on foot and was caught after a short pursuit. Officers stated that he became combative and was placed into a WRAP restraint device, spit hood, and helmet. After being taken to jail, the man was found unresponsive and died shortly after.

On 09/18/2025, around 9:50 a.m., Frisco police responded to a welfare concern at the 8000 block of McKinney Road. A caller reported a man armed with a knife who had harmed himself. Officers found 31-year-old Logan Holliday in a wooded area north of Frisco Commons Park. Holliday advanced toward officers with the knife despite commands and less-lethal efforts. Officers opened fire, striking him. He was transported to a local hospital where he was pronounced dead. The Texas Rangers are conducting an independent investigation.

On 09/18/2025, around 12:40 a.m., Glendale police responded to a welfare check near 89th Avenue and Maryland Avenue. Police said 28-year-old Adrian Green was armed with a gun and standing outside a home. Officers established a perimeter and deployed a drone. Green fired at officers and then pointed the gun at them. Officers opened fire and killed him at the scene. The West Valley Incident Response Team, led by Buckeye Police, is conducting the investigation.

On 09/18/2025, around 6:20 p.m., law enforcement officers with Pennsylvania State Police, ATF, FBI, and the Beaver County Municipal Police Task Force attempted to contact 18-year-old Kendrick Curtis Jr. in the Linmar Terrace housing complex at Waugaman Street and Tyler Street. Curtis fled on foot and fired a gun toward officers. Police opened fire and struck him. He was transported to Allegheny General Hospital where he died from his injuries. No officers were injured. The Pennsylvania State Police are conducting the investigation and will present findings to the district attorney.
