On 09/25/2021, Coshocton County sheriff’s deputies responded to reports of a man kicking in the door of a building on North Whitewoman Street. Police stated officers entered the building and attempted to subdue Luke J. Longeretta. Police alleged that during the struggle, Longeretta gained control of an officer’s firearm. Deputies opened fire and killed him at the scene.

On 09/25/2021, Huntington Beach police responded to reports of a man with a gun along Pacific Coast Highway. Police stated officers attempted to speak with Ronnie Andrew Garcia and give commands. Police alleged Garcia did not comply, and officers opened fire, killing him at the scene.

On 09/25/2021, DeKalb County police encountered Christian Smith after responding to reports of a person walking in the roadway. Police stated an officer offered Smith a ride to a nearby gas station. Police stated that at the gas station they learned Smith had an active warrant and attempted to arrest him. Police alleged Smith produced a firearm during the encounter. Officers fired their weapons. An autopsy later determined Smith died from a self-inflicted gunshot wound rather than police gunfire.

On 09/25/2021, Delano police responded to reports of a verbal disturbance at a residence on Oxford Street. Police stated an officer encountered Sandra Barajas and believed she made threatening movements toward a girl. Police stated the officer ordered Barajas to drop a weapon and alleged she did not comply. The officer opened fire and killed her in the home.

On September 24, 2021, at approximately 1:30 p.m., Indianapolis Metropolitan Police attempted to stop a white Honda Pilot driven by a 36-year-old white woman, later identified as Jessie Leonard. The vehicle fled the traffic stop, and officers did not pursue.

About an hour later, officers located a vehicle matching the description in the 500 block of Eugene Street. While preparing to tow the vehicle, police saw Leonard on a nearby porch with a gun. Police alleged she reached for the gun. Four officers opened fire, shooting and killing her.

At 2:25 p.m., McKinney police received a 911 call from a relative of a man reported to be suicidal and armed in a home. Officers arrived and tried to make contact with the man for almost an hour, trying to get him to come out of the home. Once he did come out of the home, he pointed a shotgun at officers who shot and killed him.

On September 23, 2021, a deputy from the Tate County Sheriff's Office attempted to stop Keith Cole, who was wanted on a domestic violence warrant, on Highway 4. Cole pulled over, exited his vehicle, and fled into the woods. The deputy pursued and found Cole hiding behind a tree. Police allege that Cole emerged with his “hand on his waistband” and the deputy shot and killed him. No weapon was found on Cole after the incident. Police did not provide video or other evidence to document the narrative.

On 09/23/2021, Elk Grove police responded after being called by the Cosumnes Fire Department regarding a man reported to be acting erratically next to a vehicle stopped in the roadway. Police stated officers attempted to detain David Anthony Bocci, which led to resistance and a physical struggle. Officers restrained him with handcuffs while he was seated on the ground. Police alleged Bocci repeatedly struck his head on the concrete while restrained. He was transported to a hospital, where he died later that afternoon. Bocci’s mother had contacted authorities and stated her son had been diagnosed with bipolar disorder and schizophrenia and had had interruptions in his medication.

On September 22, 2021, 19-year-old Daisha Reynolds was killed during a high-speed police pursuit that began in Wasatch County and ended in Summit County, Utah. A Wasatch County deputy located a stolen vehicle on State Route 113 and attempted a traffic stop. The driver did not stop. Police escalated a chase through Midway. The chase was temporarily halted twice when deputies lost visual contact. The pursuit resumed on River Road and continued onto U.S. Route 40, with speeds exceeding 95 mph.
Utah Highway Patrol troopers deployed a tire-deflating device on U.S. 40 to slow the fleeing vehicle. A Summit County Sheriff's deputy attempted a second deployment of tire spikes near mile marker 2. The driver swerved to avoid the spikes, lost control, and the vehicle rolled multiple times.
Reynolds, the passenger, was ejected from the vehicle and pronounced dead at the scene.
A multi-agency critical incident investigation was initiated, involving the Utah Attorney General’s Office, West Valley Police Department, and Park City Police Department.

On 09/22/2021, the Ramsey County Sheriff's Office responded to a call for a felony domestic assault. Troy Allen Engstrom, who was the man identified, shot at pursuing officers, hitting one officer's vehicle just above the driver's side windshield and a Ramsey County deputy's vehicle just below the driver's side windshield. Ramsey County Deputy Donald Rindal rammed Engstrom with his car, killing him.

On September 21, 2021, at approximately 11:29 a.m., the Cayuga County 911 Center received a report of shots fired in Auburn, New York. Officers from the Auburn Police Department responded to the scene at 12 Wheeler Street. Upon arrival, officers were met with gunfire coming from an upstairs window of the residence. In response, officers opened fire, fatally shooting 30-year-old Brandi Rene Baida.

After the exchange of gunfire, a perimeter was established around the home to secure the location. At approximately 3 p.m., Baida was found dead inside the house.

Upon police arrival, two individuals fled the scene. The female driver initially stopped but allowed the male passenger, identified as 39-year-old Noah Daniel Boswell, to drive off. Police escalated a chase and placed stop sticks. Boswell drove through the stop sticks. When he stopped, police escalated a 15-hour standoff on the Natcher Bridge near Maceo, Kentucky. Police alleged that he eventually pointed a gun at them. Boswell allegedly died from a self-inflicted gunshot wound. The Kentucky State Police Critical Incident Response Team investigated the shooting.

The man, Trent Xavier Leach, ran into an abandoned building. Per police, Leach allegedly began shooting indiscriminately into the surrounding area. On 09/21/2021, Henderson Police Department responded to a call for a person with a gun. Officers saw a man holding a gun. The man walked in and out of a home, pointing the gun at people. He locked himself inside. Henderson officers opened fire. The individual was pronounced deceased on the balcony.

Officer William "Bill" Jeffrey and Sgt. Michael Vance arrived at an apartment complex to arrest 30-year-old Deon Ledet just before 7:30 a.m. The officers were serving a warrant for Ledet's arrest for skipping bond on two felony drug charges from November 2020. After the officers announced their presence, Ledet opened fire from inside the apartment. Ledet reportedly was armed with a handgun and an assault rifle. Ledet was killed. Jeffrey, 54, died of multiple gunshots. Vance, 53, was wounded and expected to survive.

On 09/20/2021, at approximately 12:55 p.m., Signal Hill Police Department officers responded to a report of a stabbing at a grocery store in the 1600 block of East Willow Street in Signal Hill, California. Upon arrival, officers found a man who had been stabbed in the upper torso. He was transported to a hospital in fair condition. Witnesses provided a description of the attacker, later identified as 42-year-old Phillip Lopez, who was reportedly fleeing westbound on Willow Street. An officer located Lopez and attempted to detain him.

The officer exited his vehicle to pursue on foot. Lopez produced a large knife and moved toward the officer. The officer opened fire, striking Lopez. He was transported to a hospital, where he was pronounced dead.

The Los Angeles County Sheriff's Department is assisting the Signal Hill Police Department in the investigation of the officer-involved shooting. The names of the involved officer(s) have not been disclosed.

On September 19, 2021, officers from the Jackson Police Department were dispatched to the Olive Tree Hotel where they discovered a man dead from a gunshot wound. Upon arrival, Dishawn Sanders allegedly opened fire on the officers. An officer shot and killed Sanders.

Adrian Lamont Cameron Sr., a 47-year-old Black male, was wanted for a state parole violation and was being questioned in a homicide case. Police located him at a Days Inn motel in Nashville, Tennessee, on September 18, 2021. After a woman exited the motel room and was escorted away, Cameron emerged with an AR-15 and fired at officers, who shot and killed him.

James Andrew Black, aged 27, died on September 18, 2021, following a high-speed police chase in Phippsburg, Maine. During the pursuit, Black's vehicle crashed into a utility pole near 610 Main Road during a high-speed chase for undisclosed reasons conducted by Sagadahoc County Sheriff's Office. He was pronounced dead at the scene.

On September 18, 2021, at approximately 3:20 a.m., an off-duty U.S. Park Police officer and a female companion were confronted by a group of four individuals, allegedly led by John Shortt, as they left a nightclub near 1440 Utica Avenue in Brooklyn, New York. Shortt and another man wearing a balaclava reportedly approached the officer and his companion and pointed a handgun at them.
The officer drew his own weapon and fired, striking Shortt. Two other individuals, identified as Tyshawn Whidbee and Joshua Cooper, then approached from another direction. Whidbee allegedly pulled out a gun and began firing at the officer and the woman, hitting both of them. The officer fatally shooting Cooper.

On 09/18/2021, the Union Township Police Department responded to a situation involving Christopher E. Rush on the 3800 block of West Main Street in Hillsville, Pennsylvania. Around 8:45 p.m., officers and troopers from several local police departments arrived to find Rush armed with a knife. Rush reportedly stabbed several victims and assaulted a Mahoning Township police officer. An officer from the Union Township Police Department opened fire, fatally shooting Rush. The incident is under investigation by local authorities.

At approximately 4:31 a.m. on September 18, 2021, officers from the Gallatin County Sheriff's Office responded to a call on Montgomery Road in Ghent, Kentucky. A male individual allegedly fired in the direction of several officers. Officers opened fire and shot and killed him. He remains unidentified. The names of the officers were shielded. The Kentucky State Police Critical Incident Response Team investigated the shooting.

On 09/18/2021, Smyrna police responded to reports of a man with a gun making suicidal remarks at an apartment complex near N Lincoln Trace Avenue. Police stated officers tried to talk with Thurmond for several minutes. Police stated he raised a gun toward officers and an officer opened fire, striking him. He was taken to a hospital and died. On 09/18/2021, Charleston County Sheriff’s Office deputies responded to reports of gunfire in the 2500 block of Mona Avenue in West Ashley. Deputies said the subject was firing a weapon and struck a patrol vehicle with gunfire. Officers then exchanged fire with the subject. Deputy Nicholas Vecchione fired at and struck Jason Husted, who later died from a gunshot wound at a hospital. The incident was investigated by the South Carolina Law Enforcement Division.

On 09/17/2021, Cobb County police responded just after midnight to a domestic disturbance call described as involving a suicidal man in the 4500 block of N Landing Drive in Marietta. Police stated Wilbanks was holding a knife when officers encountered him outside. Police stated he moved toward officers while still holding the knife, and an officer opened fire, killing him. The Georgia Bureau of Investigation investigated the shooting.

On 09/17/2021, Escondido police said Jonathan Carroll had wounded a motorist in a reported car to car shooting. Police said they located Carroll driving and attempted a traffic stop. Carroll did not stop and police chased the vehicle until it crashed near Bear Valley Parkway and Encino Drive. Police stated Carroll got out of the vehicle holding a gun, and Officer Chandler Hoppal opened fire and killed him.

Andrew "Drew" Brandon Christian, a 34-year-old White male, was killed by police during an arrest attempt on September 16, 2021, at a residence on Ptarmigan Trail in Memphis, Tennessee. At approximately 12:40 p.m., officers from the Memphis Police Department and Shelby County Sheriff's Office attempted to serve arrest warrants at the home. Upon entering the residence, officers were fired upon. Officers opened fire in response. Officers exited the house, with two sustaining injuries after jumping through a window. Deputies later entered the home and found Christian dead from a gunshot wound.

Deputies were dispatched after receiving a call about a disturbance and a person with a weapon. When deputies arrived, they spoke with the 911 caller who confirmed that the man was a family member and still inside the home. Dan Ross Boyd allegedly had already discharged a weapon several times with people inside the home. While trying to make contact, Boyd fired a weapon at deputies outside the home. The deputies shot and killed him.
