On 09/16/2021, Wichita Falls police officers responded to a residence on 8th Street in Wichita Falls, Texas, after a call from a woman who told dispatchers her son had struck her, threatened her with a gun, and pointed the weapon at her. Police stated that upon arrival, officers attempted to engage Gabriel Michael Varela but that he refused to negotiate. Police stated officers deployed chemical agents before entering the home. Police alleged Varela fired at officers from inside. Police stated he then exited the residence armed and fired at officers again. Officers opened fire and shot and killed Varela outside the home.

On September 15, 2021, members of a U.S. Marshals' task force attempted to serve an arrest warrant at 6 Vinewood Place in Rochester, New York, for 24-year-old Dedrick Jamon James. As officers attempted to apprehend him, James allegedly returned back into the residence and struggled with officers.

On 09/15/2021, officers attempted to take a man into custody near 24th Street and Thomas Road in Phoenix, Arizona. Police stated Kenzie Keyes was with the man at the time officers approached. On 09/15/2021, Independence police officers responded to a residence on Northern Boulevard in Independence, Missouri, around 11:30 a.m. Police stated officers were met by Cody Levi Harrison, who they alleged fired a gun toward them, striking one officer. On September 14, 2021, the Waukesha Fire Department responded to a medical call about Paul J. Bruner, 58, who was having difficulty breathing. Police were attempting to arrest Bruner for domestic abuse as he was under a domestic abuse injunction order. Police said Bruner was uncooperative and refused medical attention. The woman involved in the incident had visible minor injuries. After determining that domestic violence had occurred, officers attempted to arrest Bruner. He allegedly became combative, and officers used a stun gun (electronic control device) and physical force to take him into custody. Bruner later died during the arrest.

On 09/14/2021, at approximately 1:35 a.m., two Mantua Township police officers responded to a 911 call at a residence on Elm Avenue. During an encounter outside the home, one of the officers shot and killed Charles Frederick Sharp III, 49. Police reported that a .45-caliber handgun was recovered near Sharp. An FBI agent deputized as a U.S. Marshal was attempting to serve federal and state arrest warrants on Michael Jonathan Cortez for unknown reasons. The agent arrived at a store, exited his vehicle with his weapon drawn, and entered the store. Surveillance footage does not capture events inside the store. According to reports, the agent gave Cortez unknown commands. Cortez attempted to flee, bumped into the agent, and allegedly pulled a gun, resulting in the agent shooting him dead. Inconsistencies were noted between the FBI agent’s report and physician accounts of the entry wound. The FBI has not disclosed the agent's identity or provided the store’s security footage.

Deputies arrived after 1:30 a.m. at a game room near Channelview. One man was killed near the entrance. It was uncertain if he was wounded during an exchange of gunfire with a deputy or earlier during a possible encounter with a security guard, but since he was alive during the exchange, police were present during his death.

On September 13, 2021, a Georgia State Patrol trooper attempted to stop a vehicle driven by a neighbor transporting 12-year-old Le’Den Boykins and another child home from church. The driver refused to comply and continued on Interstate 20. Troopers escalated a chase that reached speeds of approximately 90 miles per hour. Despite knowing minors were in the car, one trooper performed a Pursuit Intervention Technique (PIT) maneuver, causing the vehicle to flip several times.
The driver was Charlie Moore, a neighbor who regularly took Le’Den and another child to church. According to the Georgia State Patrol incident report and coverage by 11Alive and WXIA, troopers originally stopped Moore for reckless driving and suspected DUI after he allegedly refused to lower his window and refused to produce ID. Troopers claimed that they reported smelling alcohol and seeing “erratic movements” in the vehicle.
When Moore refused to comply, one trooper said:
“If he drives off, we’ll chase him.”
A few seconds later, Moore accelerated away, initiating the chase.
Le’Den Boykins was killed instantly. The dash-cam video released in January 2023 showed troopers acknowledging that children were in the vehicle moments before the PIT maneuver.

No blood-alcohol test results were ever released.
Court records do not show a confirmed BAC in the case file, and the family’s attorney has said repeatedly that Moore was not intoxicated. The state did not pursue a DUI conviction; instead, he was charged with felony murder, vehicular homicide, and fleeing and eluding.
Notoxicology evidence proving the allegations of intoxication has appeared in any of the filings reviewed or in media coverage through 2024. The state cleared the troopers of criminal wrongdoing.
The footage sparked statewide outrage and renewed scrutiny of Georgia’s pursuit tactics, particularly the use of high-risk maneuvers on vehicles carrying passengers. Boykins’ family publicly demanded criminal accountability and filed notice of intent to sue the state. The Georgia State Patrol stated that its internal investigation found troopers acted within current pursuit policy, though the agency confirmed that policy review discussions are ongoing.

On 09/13/2021, deputies with the San Juan County Sheriff’s Office were serving a search warrant at a residence on County Road 3935 in Farmington, New Mexico. Police stated the warrant was related to an alleged assault reported earlier that day. Police alleged Tedman Werito threw an object described as a non functioning grenade from the residence. Police stated deputies deployed flashbang devices and tear gas. Police alleged Werito later pointed an object they believed to be a firearm toward deputies. The man, Rodolfo Gonzales, noticed officers approaching, and he reportedly tried to avoid arrest. Gonzales allegedly tried to pull a handgun from his waistband, and the officers shot and killed him.

On September 11, 2021, around 3:30 a.m., Lawton Police officers attempted to stop a black Chevrolet Camaro for alleged reckless driving. The driver, 21-year-old D'monte L. Harris, did not stop. Police escalated a high-speed chase that continued until Harris lost control of the vehicle at the intersection of SW Sheridan Road and SW Jefferson Avenue, resulting in a fatal crash.

On September 11, 2021, 28-year-old Tyrik Mott allegedly fled from police while driving the wrong way on Gates Avenue in Brooklyn, New York, after running a red light at a high rate of speed. On September 10, 2021, at approximately 12:45 p.m., Corporal John Diggs of the Prince George's County Police Department, who resided at an apartment complex in the 1100 block of Owens Road in Oxon Hill, Maryland, was informed by a complex employee that Cedric Ricardo Williams, 29, who had been previously banned from the property, was on the premises. In full uniform, Corporal Diggs approached Williams and instructed him to leave. Williams refused, leading to a verbal confrontation that escalated into a physical struggle outside the building and onto a roadway.

During the altercation, Williams retrieved a handgun from his bookbag. The weapon allegedly malfunctioned which was not specified further. Williams then discarded the handgun into a wooded area. Corporal Diggs fired his Taser twice in an effort to subdue Williams. Williams retrieved what appeared to be a gun from the wooded area and pointed it at Corporal Diggs. Corporal Diggs discharged his service weapon multiple times, fatally striking Williams.

Upon investigation, both weapons possessed by Williams were all determined to be realistic replicas. The Prince George's County Police Department's Internal Affairs Division is leading the investigation, with oversight from the Prince George's County State's Attorney's Office.

On 09/10/2021, Phoenix police officers responded around 1 a.m. to reports of a man walking naked in the roadway near North 7th Avenue in Phoenix, Arizona. Police stated officers encountered Jessee Rickman and described his behavior as erratic and his speech as difficult to understand. Police stated officers placed Rickman in handcuffs, put him on the ground, and turned him onto his side. Police reported that Rickman later went into medical distress and died at the scene after being restrained.

On 09/10/2021, Georgia State Patrol troopers stopped a vehicle on GA-22 in Douglasville, Georgia, for a traffic violation. On 09/10/2021, Officer Thomas Ouellette, 44, of the Bow Police Department, was driving his personal Ford Expedition southbound on I-89 when his vehicle drifted from the right-hand travel lane into the breakdown lane. The vehicle struck the rear of a parked, disabled tractor-trailer truck. Two of Officer Ouellette’s children, Nicholas Ouellette, 16, and Gavin Ouellette, 6, were killed in the collision. While the media did not confirm his on-duty status, it is assumed he was off-duty as he was driving his personal vehicle.

William Escano Yah was believed to be involved in a Sept. 5 quadruple murder. The bodies of 37-year-old Jairo Escano; his wife Elvira, 46; and their two kids: 13-year-old Isabel and 10-year-old Jairo Jr., were found after fire crews extinguished a fire at their home. He was killed by an apparent self-inflicted gunshot wound after officers attempted to arrest him about 8:20 a.m.

On September 9, 2021, at approximately 1:33 p.m., a 911 call was made to report a disturbance at West Lanes Bowling Center involving Eric Liao, an autistic adult man, who was reportedly out of control and biting his caregiver. Bowling alley staff were restraining him when an officer from the Omaha Police Department arrived at 1:46 p.m. The officer found Liao being held down and proceeded to handcuff him. Shortly thereafter, it was observed that Liao was unresponsive, and he was pronounced dead a short time later.
