On 08/19/2021, officers from the El Cajon Police Department were responding to reports that a person had been shot in the area of Ballard Street and Decker Street. While investigating, police stated they encountered Setha Phangdy standing in the roadway. Police alleged that Phangdy fired a gun toward an officer. An officer then opened fire, striking and killing him. Authorities later stated the shooting was reviewed by the district attorney’s office, which cleared the officer involved.

On 08/19/2021, at approximately 7:34 p.m., Ridgefield Park Police responded to a home near Main Street and Grand Avenue after receiving a report of a man barricaded inside and armed with a gun. Officers attempted to negotiate with Scott J. DeGraw, 58. After communication efforts, police breached the home and found DeGraw deceased from a self-inflicted gunshot wound.

On 08/18/2021, Terrence Bey allegedly shot a person and fled from an apartment complex in Bucks County. Police from the Middletown Township Police Department and the Philadelphia Police Department pursued Bey to Philadelphia. During an exchange of gunfire with police near Whitaker Avenue and Adams Avenue, Bey was shot and killed.

Around midnight, Pflugerville Police responded to a disturbance call after reports that a man with a gun was attempting to force his way into a bedroom. Ronald Zavaglia, a resident at the home, was reportedly trying to break down a door while armed. Moments after officers arrived, Officer Ian Clark shot and killed Zavaglia, who police said was still holding the weapon.

James Matalice Smith barricaded himself inside a home after shooting Deputy John Pollock while the deputy was serving a warrant. Around 3 a.m., Smith appeared to be surrendering as he exited his home. However, as officers attempted to take him into custody, Smith fired a gun at a trooper, hitting him in the arm. In response, police shot and killed Smith.

On 08/18/2021, San Bernardino police were tracking Ervin Olikong following a traffic stop earlier in which police reported that shots were fired toward an officer. When officers moved in to arrest Olikong in the Highland area, police reported that he exited a vehicle and fired toward approaching officers. Officers opened fire, killing Olikong at the scene. The San Bernardino County District Attorney later reviewed the incident and ruled the shooting justified.

On 08/18/2021, Cobb County police initiated a traffic stop in Marietta, Georgia, after officers reported observing a vehicle driving erratically. Salas continued driving and later stopped his vehicle near a golf course in the Thornton area. According to police, Salas exited the vehicle and produced a firearm. On August 18, 2021, a deputy with the Tippecanoe County Sheriff's Office responded to a report of a person in a ditch near 150 East and 430 South in Lafayette, Indiana. While investigating, the deputy heard a motorcycle revving repeatedly. Moments later, the deputy observed a motorcyclist riding with the front wheel elevated off the ground.
After the motorcycle returned to both wheels, the deputy used radar to clock the motorcyclist traveling at 85 mph. The deputy attempted to stop the motorcyclist, initiating a pursuit. The motorcycle crested a hill and crashed. Kyle Wade Plunkett, 35, was killed in the pursuit crash.

On 08/18/2021, Cobb County police attempted to stop Devonte Dawayne Brown after he refused to comply with a traffic stop. The driver refused to stop and police escalated a chase through Austell. During the pursuit Brown crashed into multiple vehicles and officers confronted him while he remained inside the car. Under unclear circumstances officers opened fire and killed him.

On August 18, 2021, deputies from the Raleigh County Sheriff's Office responded to a complaint of indecent exposure at 219 Rural Acres Drive in Beckley, West Virginia. Upon arrival, Kevin Eugene Norris was reportedly uncooperative with deputies. During the encounter, Norris allegedly pointed a pellet gun at deputies that resembled a semi-automatic handgun. Deputies shot and killed Norris during the confrontation.

On 08/17/2021, around 4:30 p.m., the Pennsylvania State Police responded to a report of an active shooter and a man threatening to harm himself at a residence on Third Street in Port Carbon, Pennsylvania. Police attempted to have the man exit the home voluntarily, but a gunshot was heard from inside. Officers later found the man deceased from a self-inflicted gunshot wound. The incident remains under investigation.

Destinee Thompson, a 27-year-old pregnant mother of three who was seven months pregnant, was on her way to lunch with her stepmother on August 17, 2021. Officers from the Arvada Police Department, including Officer Anthony Benallo, responded to a report of a woman who had stolen from a Target store and held a knife at an employee. A witness followed the individual to the American Motel at 10101 W. I-70 Frontage Rd. N., where officers were searching for a woman in a white tank top with a chest tattoo. Thompson was wearing a white tank top but did not have the specified chest tattoo. She repeatedly told officers she was not the person they were looking for and that she had no identification. Unable to convince the officers of her innocence, she locked herself in her minivan, locked the doors, and refused to exit, repeatedly stating, "It wasn't me."

An officer smashed the passenger window with a baton. Thompson panicked, reversed the vehicle, hitting a police car, and then drove forward over a curb onto the road. Officer Benallo opened fire, discharging eight shots and striking Thompson. She was pronounced deceased at the scene. Both Thompson and her seven-month-old unborn child died as a result of the shooting. The shooting was handled in an internal investigation by the Arvada Police Department and the First Judicial District Attorney's Office.

In August 2023, Thompson's family filed a wrongful death and excessive force lawsuit against five officers from the Arvada Police Department, alleging that her race—being part Latina and part Native American—played a role in her being targeted. The Arvada Police Department stated that the family's lawyer has mischaracterized the events surrounding Thompson's death and plans to mount a vigorous legal defense. Reports suggest that Thompson may have been mistakenly identified as the shoplifter.

On 08/16/2021, Loveland police responded after Alex Domina’s grandmother, his legal guardian, called 911 reporting that he was acting erratically inside her home, breaking items and experiencing a mental health crisis. She told dispatch that Domina could potentially be calmed but that she could not physically manage him. When the first officer arrived, Domina’s grandmother told the officer that Domina had a knife. According to police, Domina moved toward the officer while holding the knife, and the officer fired, critically wounding him. Domina was hospitalized and died from his injuries approximately two weeks later. The shooting was reported as under investigation, and no officers were charged.

On 08/16/2021, officers attempted a traffic stop on a 2014 Kia Forte near 1900 block of US 321 Business South in Newton. The driver pulled over then fled. The pursuit traveled through Maiden and Lincolnton. The vehicle lost control, crashed into a concrete bridge guardrail and caught fire. Kyle Alan Wagner was pulled from the vehicle but died from his injuries en route to hospital.

Upon arrival, officers encountered Shelton, who was armed with a gun. Police reportedly ordered him to drop the weapon, but Shelton allegedly fired his gun at officers. Officers shot and killed Shelton during the confrontation.

Katelyn L. Harris, a 29-year-old White female, was shot and killed by deputies with the Sullivan County Sheriff's Office on August 15, 2021, in Chuckey, Tennessee. Deputies recognized Harris, who reportedly had outstanding felony warrants, driving on John B. Dennis Highway and attempted a traffic stop. Harris fled, leading deputies on a pursuit onto Interstate 81 and into Greene County. She turned north on Jearoldstown Road before going off the roadway. A passenger exited the vehicle and was taken into custody. Deputies reportedly observed Harris holding a gun. Harris allegedly put the vehicle in reverse and accelerated toward deputies, at which point three deputies opened fire, killing her.

On 08/15/2021, Barstow police responded to a call reporting a man who appeared to be searching for a vehicle that had recently been towed. Officers contacted Victor Richard Carevic and attempted to question him. According to police, Carevic moved away on foot, and during the encounter he was reported to have pointed a firearm toward an officer, who fell and fired. Police stated that Carevic continued moving away before officers caught up to him near a locked business. Officers reported that Carevic again turned toward them with a firearm, and officers fired, killing him at the scene. The San Bernardino County District Attorney later reviewed the case and ruled the shooting justified.

On 08/15/2021, deputies from the Newberry County Sheriff’s Office responded to reports that Phillip John Walden had carjacked a vehicle in the Chapin area of South Carolina. Deputies located the vehicle and chased it along Interstate 26. According to limited reporting, the pursuit ended when deputies used force against Walden. He was shot and killed at the scene. Authorities reported that a Taser was also deployed during the encounter. The shooting was reported as under investigation, and no deputies were charged.

On 08/15/2021, Newberry County deputies responded to an overturned vehicle on Interstate 26 near the Newberry and Laurens County line. Phillip John Walden reportedly carjacked a Good Samaritan who stopped to help, and police escalated a chase southbound on Interstate 26. During the chase he struck multiple vehicles including a deputy’s cruiser. Near mile marker 91 in Lexington County, he exited the vehicle and moved toward officers, and deputies opened fire and killed him.

On August 15, 2021, at approximately 5:00 p.m., officers from the Albuquerque Police Department (APD) observed a stolen vehicle occupied by a male and female near Lomas Blvd. and Louisiana Blvd. The vehicle fled from a traffic stop, prompting officers to deploy a StarChase GPS tracking device, which successfully attached to the vehicle. The vehicle was tracked to Morrow Ave. and Pennsylvania Blvd. NE, where officers discovered the tracking device had been removed. The vehicle was later found unoccupied in a Walmart parking lot at Northeastern Blvd. and Wyoming Blvd. NE. Officers located a male and female matching the descriptions of the individuals who had exited the stolen vehicle. During questioning, the male suspect, identified as Eric Padilla, handed over a knife from his pocket but refused to be patted down. As officers insisted on conducting a search, Padilla fled on foot. Witnesses and video footage indicate that as Padilla ran, he turned slightly to his left and produced a handgun. Both officers fired their weapons, striking Padilla, who succumbed to his injuries at the scene. A firearm was recovered near Padilla. The incident is under investigation by the Albuquerque Police Department.

On August 14, 2021, deputies from the Kalamazoo County Sheriff's Office encountered Kyle Anthony Goidosik at a gas station. Goidosik was believed to have been involved in an earlier pursuit in a nearby community. Upon being approached by law enforcement, Goidosik allegedly drew a firearm and pointed it at the deputies before attempting to escape in his vehicle.

During the subsequent chase, Goidosik reportedly fired several shots from his car, one of which struck Deputy Ryan J. Proxmire, causing him to crash his patrol vehicle. The chase escalated until Goidosik lost control of his vehicle, drove into a field, and his car became immobilized. After exiting his vehicle, Goidosik continued to shoot at the approaching deputies. Officers opened fire and killed Goidosik.

On 08/14/2021, Kalamazoo County sheriff’s deputies attempted a traffic stop in the Galesburg area for a vehicle registration issue. The driver continued driving, and deputies later located the vehicle the same day. Police chased the vehicle at high speed before it crashed in a field. According to police statements, Kyle Anthony Goidosik fired shots toward deputies before the crash and continued shooting afterward. Deputies opened fire, killing him at the scene. The incident was reported as under investigation at the time, and no deputies were charged.

On 08/14/2021, Nebraska State Patrol officers responded to a disturbance involving neighbors in St. Paul, Nebraska, after reports that a firearm had been discharged. Mauricio Luna, a 27-year-old Latino male, was killed in a traffic collision involving Officer Cody Klingmann of the Knoxville Police Department on August 13, 2021, in Knoxville, Tennessee.
