On 11/06/2021, Will County Sheriff’s Office deputies responded to a domestic disturbance at 300 Middletree Road involving a family member reported to be in crisis and holding a knife. Deputies confronted Jabbar Muhammad during the encounter and observed him stab Eldred Wells Sr. Deputies fired their weapons, striking both men. Wells died from his injuries and Muhammad also died at the scene. Prosecutorial and internal reviews later concluded deputies would not face charges, and civil litigation followed the incident on behalf of family members.

On November 5, 2021, members of the Monmouth County Prosecutor’s Office attempted to execute a residential search warrant on Chelsea Avenue in Long Branch. During the operation, Mark D. Walker II shot and wounded one of the detectives. The detectives retreated, and Walker barricaded himself inside the residence with his two-week-old infant. Negotiations continued for approximately seven hours without success. Around 2 a.m. on November 6, a fire erupted in the apartment building. Walker fled the building holding the infant and fired several rounds at officers, striking a BearCat vehicle. In response, three MOCERT officers returned fire, fatally wounding Walker, who was pronounced dead at the scene. The infant was unharmed.

On 11/06/2021, Henrico County Police Officer Timothy Grant Million III shot and killed Tony Elliott Singleton, 53, of Norfolk, after responding to a single-vehicle rollover crash on Interstate 64 east of the I-295 interchange in eastern Henrico County. Initial police reports stated that Singleton's vehicle had rolled several times and that Million fired during an encounter with the driver. Singleton was transported to a hospital, where he died.

Two motorists who stopped to help after the crash later testified that Singleton appeared impaired and that they saw a knife in his waistband. A 911 dispatcher informed Million that the driver had a knife. Body-camera footage subsequently played in court showed Million repeatedly ordering Singleton to turn off the vehicle, show his hands and stop reaching around inside the vehicle.

A civilian riding with Million testified that Singleton eventually got out of the crashed vehicle and approached Million. She described a physical confrontation between the two immediately before Million opened fire. She testified that she did not see anything in Singleton's hands and did not see him reach for the knife.

Million fired seven times at Singleton. Prosecutors later argued that merely seeing the handle of a knife in Singleton's waistband did not justify the shooting and maintained that witnesses did not see Singleton reach for the knife. Million's defense maintained that Singleton moved his hands toward his waistband and that Million believed he faced an imminent deadly threat. A knife was recovered from Singleton's possession.

The shooting resulted in an unusually significant criminal review. In February 2022, a special grand jury unanimously found probable cause to indict Million for voluntary manslaughter. Henrico Commonwealth's Attorney Shannon Taylor characterized the charge as reflecting circumstances in which an officer may have observed something and overreacted. Million was the only officer who fired at Singleton.

Million's trial began in January 2023. Prosecutors argued that his use of deadly force was unreasonable, while the defense argued that he acted in self-defense. After the prosecution rested, the defense moved to strike the charge before the case went to the jury. On 01/25/2023, Judge Joseph Ellis granted the motion and dismissed the voluntary-manslaughter case, finding that the prosecution had not presented sufficient evidence for the charge to go to the jury.

On November 6, 2021, an officer attempted to pull over Chelsae Clevenger-Kirk, who was riding her motorcycle without a helmet and with no tags. She refused to stop and continued to her home, where she went inside. The officer tried to make contact but was unsuccessful. As the officer walked back to his patrol car, he reportedly heard gunshots from the residence. A standoff ensued, and Clevenger-Kirk allegedly set fire to her home. After some time, she exited the residence, reportedly holding a gun. Police shot and killed her. Tear gas canisters were deployed into the home, which startled her family members.

Police attempted to stop a vehicle connected to an aggravated assault, suspected abduction, and suspected DUI. The driver did not pull over and eventually entered the interstate. Virginia State Police knocked the vehicle off the road, causing it to overturn, resulting in the death of 47-year-old Amity Jo Grey. Brian Price, 45, the driver, fled on foot. After a brief pursuit, troopers caught up to him, and a fight ensued. During the struggle, a trooper shot and killed Price.

On November 5, 2021, Charlotte-Mecklenburg Police Department (CMPD) officers responded to a Walmart on Albemarle Road in Charlotte, North Carolina, following reports of a man threatening a store security officer with a firearm. The individual, identified as 33-year-old Derrell Lamar Raney, brandished a gun and refused commands to disarm.
Officers located Raney in the parking lot and repeatedly instructed him to drop his weapon. Body camera footage shows Raney pointing the firearm in the officers' direction. Officers James Longworth and Micah Edmunds, perceiving a lethal threat, opened fire, striking Raney. He was pronounced dead at the scene. No officers or bystanders were injured.
A firearm was recovered from Raney, and CMPD stated that his actions directly endangered public safety. The North Carolina State Bureau of Investigation (SBI) conducted a review of the incident, and in April 2022, the Mecklenburg County District Attorney's Office concluded that the officers' actions were justified.

Deputies confronted her at the doorway and gunfire occurred during the encounter. A deputy was wounded and another deputy fired their weapon, striking Worsham. She died from her injuries. On November 4, 2021, Tulsa Police officers responded to a QuikTrip convenience store at 3050 S. Sheridan Road after receiving a call from the mother of Derek Collins, 36, reporting that he was suicidal and armed with a knife. Upon arrival, officers found Collins inside the store with self-inflicted cuts. Officers convinced him to exit the store into the parking lot, where he grabbed a passing cyclist and held a knife to the cyclist's throat. A Tulsa Police officer opened fire, fatally striking Collins.

On 11/04/2021, a Taylorsville police officer contacted Aaron Troy Michael Rehn, 29, at the Speedway gas station at 2187 W 4700 South after police described suspicious activity. Rehn ran from the officer. Police said Rehn turned and fired multiple rounds at the officer, and the officer opened fire. Police said neither was hit during the exchange. Multiple agencies searched the neighborhood and police issued a shelter in place order. Police said a 12 year old boy later spotted Rehn hiding in a basement window well and the family called 911. Police and K 9 units closed in nearby. Police said Rehn shot himself as officers approached. A later Salt Lake County District Attorney review stated the officer was justified in opening fire during the earlier exchange and that Rehn died by suicide after the search narrowed.

On November 4, 2021, at approximately 2:30 p.m., Louisiana State Police fatally shot 36-year-old Barry Rigsby at the Wisner Apartment Complex on Maple Street in Wisner, Louisiana. Rigsby was wanted for questioning in a triple homicide that had occurred earlier that day in Shreveport. Police located Rigsby's abandoned vehicle near the apartment complex. Franklin Parish Sheriff's deputies began evacuating residents. During the evacuation, Rigsby fired shots at the deputies and locked himself inside an apartment. Louisiana State Police, including SWAT and police negotiators, arrived. After a standoff lasting four hours, during which Rigsby continued to fire shots at police and threw a fire extinguisher at them, police shot and killed him.

On 11/04/2021, 25-year-old Ja’Marrius A. Rucker of Atlanta was killed while riding as a passenger in a Toyota Camry that crashed during a Georgia State Patrol pursuit in northwest Atlanta. Eighteen-year-old passenger Nadiyah S. McCants was also killed. The driver, 18-year-old La’Montez D. Winkfield, and another passenger, 19-year-old Tyreke M. Brookings, survived and were transported to Grady Memorial Hospital in critical condition.

At approximately 1:30 a.m., a Georgia State Patrol trooper encountered the Camry as it exited I-20 East onto I-75 South. GSP spokesperson Franka Young said the trooper clocked the car traveling more than 100 mph and attempted a traffic stop. Winkfield continued driving away and the trooper pursued the Camry through multiple Atlanta streets and along I-75.

On November 4, 2021, U.S. deputy marshals attempted to serve a warrant on a man who had two outstanding warrants for armed burglaries and felony firearm charges. As deputies approached the man, he allegedly pulled out a firearm and shot and killed himself.

At approximately 1:45 a.m. on November 3, 2021, Kentucky State Police troopers responded to a 911 call at the 800 block of Chipman Ridge Road in Williamstown, Kentucky. Upon arrival, troopers encountered 48-year-old Gary Wayne Bressler Sr., who was reportedly in a mental health crisis. Bressler was holding a decorative samurai sword. Despite commands to drop the weapon, Bressler did not comply, leading to the troopers firing their weapons, fatally wounding him. The KSP Critical Incident Response Team is handling the investigation into the incident, and the identities of the involved officers have not been released.

On 11/03/2021, Eau Claire Police Department Officer Kristopher O’Neill shot and killed LeKenneth Q. Miller, 30, inside a residence in the 400 block of Selma Street in Eau Claire. Police responded at approximately 2:41 p.m. after dispatch received a 911 hang-up call and called the number back. A woman reported that she and another woman were hiding in a locked bedroom because Miller, the other woman’s former boyfriend, had entered the residence.

The caller told dispatch that Miller had allegedly strangled the other woman approximately one week earlier. During the 911 call, she reported that Miller had entered the home, was armed with a knife and was attempting to reach the women. One of the women escaped before police entered the residence.

Officers Kristopher O’Neill and Jason Kaveney entered the residence. Police reported that Miller had stabbed the remaining woman multiple times. Investigators subsequently recovered a broken knife in the bathroom and another knife near where Miller fell after being shot.

According to the subsequent investigation, O’Neill and Kaveney encountered Miller armed with a knife. O’Neill fired his service weapon, striking Miller. Police and emergency medical personnel attempted lifesaving measures, but Miller died at the scene. The wounded woman was initially transported to a local hospital and then flown to another hospital for treatment of serious injuries.

The Wisconsin Department of Justice Division of Criminal Investigation led the independent investigation with assistance from the Wisconsin State Patrol. The completed investigation was forwarded to the Eau Claire County District Attorney’s Office for review.

On 12/28/2021, the Eau Claire County District Attorney’s Office announced that O’Neill would not face criminal charges. Prosecutors concluded that his use of deadly force was legally justified as self-defense and defense of others under the circumstances documented by the DCI investigation.

At approximately 12:52 p.m., LaPorte County Sheriff’s officers identified a 2020 Infiniti QX60 as a stolen vehicle on Interstate 94 near mile marker 32–34 and attempted to stop it. The driver continued driving away and deputies escalated a high-speed pursuit.

On 11/03/2021, at approximately 12:52 p.m., LaPorte County Sheriff’s officers began pursuing the Infiniti on I-94. The vehicle was initially traveling eastbound, exited at U.S. Highway 421, then re-entered I-94 traveling westbound. Police described the Infiniti as traveling at a high rate of speed.

On 11/03/2021, Covington Police officers responded to a residence on Spillers Road after a 911 call reported domestic violence. Officers spoke with a woman who said Gilbert had left shortly before their arrival and instructed her to call again if he returned. Dispatch received a second call later that night reporting Gilbert back at the home, and officers returned and confronted him outside. Officers attempted to detain him, deployed a Taser, and physically struggled with him. During the encounter, officers reported he gained control of a Taser while rising from the ground, and an officer fired a handgun and shot him. EVENT NARRATIVE:
On 11/01/2021, shortly before 3:00 a.m., off-duty Newark Police Officer Louis A. Santiago was driving a 2005 Honda Accord northbound on the Garden State Parkway near Exit 151 in Bloomfield. Santiago had been drinking at a bar after completing his Halloween day shift and was later determined to have been driving while intoxicated. His cousin, Albert Guzman, was riding with him. Prosecutors said Santiago failed to maintain his lane, drove onto the right shoulder and struck 29-year-old Damian Z. Dymka, who was walking along the shoulder.

Dymka had spent Halloween night with friends at Club Feathers in River Edge. He left the club at approximately 1:00 a.m. in an Uber with a friend after accidentally leaving his cellphone behind. He subsequently ended up at a friend's home in Bloomfield; exactly how he came to be walking on the Garden State Parkway shortly before 3:00 a.m. was never established publicly.

Instead of immediately calling 911 or rendering aid, prosecutors said Santiago and Guzman left Dymka on the roadway and drove away, then returned to the scene multiple times. Santiago eventually placed Dymka in the Honda and removed him from the crash scene. He and Guzman drove to the Santiago family home in Bloomfield, where Santiago's parents were located.

The subsequent grand-jury investigation produced substantially more evidence of what occurred after Dymka was struck. On 11/03/2021, at approximately 3:55 p.m., off-duty Los Angeles County Sheriff’s Deputy Ricardo Castro drove his pickup truck eastbound on Firestone Boulevard in South Gate at an extraordinarily high speed. Investigators determined Castro was approaching 95 mph as he entered the intersection at San Juan Avenue. The normal speed limit was 35 mph and was reduced to 25 mph when schoolchildren were present.

Twelve-year-old Isaiah Suarez Rodriguez was riding in a car driven by his 19-year-old sister, Alexa, who was waiting to make a left turn at the intersection. Castro’s pickup struck their car with tremendous force. Isaiah suffered fatal injuries and died at a Long Beach hospital. Alexa suffered broken bones and other serious injuries. Castro and his passenger were also injured.

Isaiah had gone out with his sister that afternoon for an extraordinarily ordinary reason: he wanted to buy a ruler for a classmate whose ruler had been taken away while the child was being bullied. Three days before his death, Isaiah had decided that he wanted to be baptized. He loved superhero movies, playing Animal Crossing and swimming with his family, and dreamed of becoming a chef.

On 10/31/2021, Los Angeles police responded to calls about a man slashing car tires and acting erratically near West Victory Boulevard. Officers located Melkon Michaelidis armed with two knives. Police repeatedly ordered him to drop the knives as he walked toward them. He refused and continued toward officers while holding the blades. Officers then opened fire, using both less-lethal rounds and live ammunition, and shot Michaelidis, killing him at the scene. On October 29, 2021, Clinton police attempted to intercept a man accused of stealing a truck from Jackson and driving into Clinton.
