On 10/25/2021, DeKalb police responded to a reported domestic disturbance at a residence on Tilton Park Drive. Officers reported that Kristopher Kramer had been drinking and was inside the home during the response. Police said Kramer had injured himself prior to their arrival. After several minutes of communication, Kramer opened the door. Police reported that Kramer held a bladed object and moved toward officers. An officer deployed a Taser and a bean bag round, and another officer then opened fire, fatally shooting Kramer. The shooting was reviewed and later cleared.

On 10/25/2021, Jacob Bergquist opened fire inside the Boise Towne Square shopping mall, killing two people and wounding four others. Officers responded to reports of an active shooter and confronted Bergquist outside the mall. Police reported that Bergquist exchanged gunfire with responding officers in the parking lot area. Officers shot and killed him at the scene. The Ada County Coroner later identified Bergquist as the shooter. The incident prompted a large law enforcement response and evacuation of the shopping center.

Erik Black allegedly stole an 18-wheeler with a flatbed trailer. He fled police and jumped out of the truck as it was still in motion. The truck struck a home. Two people who were in bed inside the residence were struck by the truck. A man was injured, and Aylene "Nicole" Heaney was killed. Black was charged with murder and reckless aggravated assault. The company that owns the truck was able to use a GPS to locate the truck.

On October 25, 2021, a 78-year-old man contacted Mitchell County dispatch to report that he had been kidnapped at gunpoint and held for 26 hours by an acquaintance, John Roudybush, 59. When officers attempted to serve a search warrant at Roudybush's home located at 2446 190 Road in Glen Elder, Kansas, he refused to exit the residence. After an hours-long standoff, officers heard a gunshot from inside the home. Upon entry, they found Roudybush deceased from a self-inflicted gunshot wound.

Corey Stanley was wanted in connection with a home invasion and assault, accused of breaking into a couple's home and beating them with a rock. Police found him outside a white sedan parked in a parking lot. As officers approached, Stanley got back into the driver's seat and allegedly drove toward them, prompting police to open fire, killing him.

On October 25, 2021, Oklahoma City Police attempted to stop a vehicle for a traffic violation near I-44 and SW 104th Street. The driver, identified as 16-year-old Milton Neal, fled. Police escalated a high-speed pursuit. Police allegedly called off the chase immediately before Neal ran a red light at the intersection of SW 104th Street and S Pennsylvania Avenue, colliding with bystander Sean Donahue's vehicle. Donahue, 35, was driving a company vehicle for Diagnostic Laboratory of Oklahoma at the time and was killed instantly. Neal and his two passengers sustained non-life-threatening injuries and were transported to a local hospital.

On 10/24/2021, Conway police attempted to stop a vehicle at the intersection of College Avenue and Fairway Drive after receiving information linking the driver to a nearby residential disturbance. During the stop, Nicholas Smith exited the vehicle. Police reported that gunfire occurred and that an officer was shot during the encounter. Smith moved away from the initial stop location, and officers then opened fire, killing him. The officer was transported for treatment. The Arkansas State Police were requested to investigate the shooting.

At approximately 12:00 p.m. on October 24, 2021, an officer from the Hopkinsville Police Department responded to a welfare check on a man in a vehicle parked near the intersection of South Main Street and West 19th Street. A computer check revealed that the individual, later identified as 44-year-old Arthur Hunt of Hendersonville, Tennessee, was wanted on an extraditable warrant from Florida related to a DUI crash. During the encounter, police engaged in an altercation with Hunt. He responded by pointing a gun at the officer and fleeing on foot into a nearby vacant apartment. Law enforcement established a perimeter and deployed tear gas. After a six-hour standoff, officers located Hunt hiding in the attic between two walls. Upon his emergence, police claim that he pointed a firearm at the officers, prompting them to fire, resulting in his death. The Kentucky State Police Critical Incident Response Team conducted the investigation, and the identities of the involved officers were not publicly disclosed.

On 10/24/2021, the Lyndhurst Police Department received a call at approximately 8:07 a.m. to conduct a welfare check on a 36-year-old man. A uniformed officer arrived at the residence on Post Avenue and encountered the man outside his home. The man was armed with a handgun and allegedly fired at the officer. The officer returned fire but did not hit the man. The man then turned the gun on himself and died on the sidewalk.

On 10/24/2021, Curtis Armstead Jr. was driving on the Capital Beltway in Fairfax County when Virginia State Police attempted to stop the vehicle and escalated a pursuit that continued onto Interstate 495; during the chase the vehicle crashed and caught fire, killing Armstead at the scene, with police confirming the pursuit immediately preceded the fatal crash.

On 10/24/2021, officers from the Kentwood Police Department responded to reports of a man, Kim Douglas Ropp, stabbing his roommates at 4500 GrantwoodAvenue SE in Kentwood. Upon arrival, police found Ropp struggling with a roommate. Officers attempted to de-escalate the situation by firing bean bag rounds at both Ropp and the roommate. However, Ropp reportedly charged towards the officers, leading to them using deadly force, resulting in Ropp's death.

At 3:45 a.m., a Virginia State Police trooper attempted to stop a Dodge Charger traveling 72 mph in a 55 mph zone. The driver, 24-year-old Curtis Armstead, fled and lost control of the vehicle on the Route 123 ramp. The car struck a Jersey wall and then hit the bottom of the overpass for the I-95 Express Lanes, causing it to catch fire. Both the driver and passenger, 23-year-old Miquel D. Jenkins, were pulled from the wreckage but were killed in the crash.

On October 23, 2021, Tulsa Police Department officers responded to a 911 call from Leon Perry, who was incoherent during the call and allowed a child to speak, indicating they needed help. Upon arrival at the Seminole Hills apartment complex, officers encountered a 7-year-old boy with bound wrists and neck, who informed them of an argument and assault on his mother. Officers then heard a gunshot from the apartment. As they entered, Perry pushed the lifeless, bound body of Tyanna Blades down the stairs. During the ensuing confrontation, officers shot and killed Perry and rescued a 4-year-old girl who was also bound.

On 10/22/2021 at about 12:29 a.m., a Rialto police officer stopped a Chevrolet pickup truck for a vehicle code violation at North Encina Avenue. Christian Valdez exited the truck and moved away from the vehicle while the officer attempted to detain him. As the encounter continued, Valdez returned to the driver’s seat while officers attempted to arrest him at the open door. A second officer arrived during the struggle. The truck began moving forward while officers were still at the driver’s side. An officer fired multiple shots, striking Valdez. The truck continued forward, went through a chain link fence, and struck a residence. Valdez was transported to a hospital and pronounced dead. The San Bernardino County District Attorney later ruled the shooting justified.

On 10/22/2021, around noon, officers from the Detroit Police Department attempted to stop a man who was wanted. He fled to a nearby gas station where he encountered an uninvolved citizen, asking to borrow a phone due to a domestic incident. The citizen took him to his home, allowing him to use the phone, where the man admitted he was on the run from police. The citizen left unharmed, but the fugitive stayed inside the house. A Crisis Intervention Team, including social workers specialized in mental illness, was called to assist. The standoff concluded around 7 p.m. when the man was found deceased from a self-inflicted gunshot wound.

Police officers surrounded the vehicle after receiving the 911 call. Shortly after, a single gunshot was heard from inside the vehicle. Upon investigation, officers found Hines deceased from a self-inflicted gunshot wound.

On October 22, 2021, at approximately 3:39 a.m., Oklahoma City Police officers responded to a reported robbery involving two men fleeing in a red Saturn. An officer spotted the vehicle minutes later and initiated a traffic stop. The vehicle initially pulled over in a parking lot but then sped away eastbound on a westbound service road. The vehicle entered the westbound lanes of I-240, driving against traffic. The pursuing officer paralleled the vehicle by driving eastbound in the eastbound lanes to maintain visual contact. The man vehicle collided head-on with another vehicle, resulting in the death of the driver, identified as Melvin Eugene Simpson, 51. His passenger sustained injuries from the crash.

On 10/21/2021, Sierra J. Stricker was killed on US-77 near I Avenue when a Thurston County Sheriff’s Office deputy driving an unmarked patrol vehicle struck her while traveling south on the highway. Stricker died from injuries sustained in the collision. The case was reviewed by a grand jury, which returned a no bill, and no criminal charges were filed against the deputy.

On October 21, 2021, at approximately 10:00 p.m., Salisbury Police Department OfficerChristopher Denny observed a vehicle operating without headlights and swerving near the intersection of Delaware Avenue and West Salisbury Parkway in Salisbury, Maryland. Suspecting the driver was impaired, Officer Denny attempted a traffic stop, but the driver, identified as 35-year-old Jamaal "Smurfee" Parish Mitchell, failed to stop and accelerated, initiating a pursuit.

During the pursuit, Mitchell's vehicle collided with a Wicomico County Sheriff's Office cruiser driven by Deputy David Munir, who then joined the pursuit. Mitchell continued driving and struck another vehicle at the intersection of Queen Avenue and Duchess Drive, causing him to lose control. His vehicle veered into a yard on Duchess Drive, hitting a parked car and a tree. Mitchell was pronounced dead at the scene.

The Independent Investigations Division (IID) of the Maryland Attorney General's Office conducted a review of the incident. In April 2022, the Wicomico County State’s Attorney’s Office declined to file charges against the involved officers, determining that their actions were not grossly negligent and did not cause Mitchell's death.

Officers and deputies spotted a car traveling along the Appalachian Highway that was the subject of a BOLO (Be On the Lookout) in connection to a robbery that occurred about 7 p.m. in Jacksboro. Officers escalated a pursuit onto the freeway, I-75, and chased the vehicle at high speed until the driver crashed into several vehicles stopped in a construction zone, including a cruiser driven by a trooper with the Tennessee Highway Patrol who was parked behind a disabled vehicle on the highway. During reported efforts to take the driver into custody, a Caryville officer and a Campbell County deputy shot and killed him.

On October 21, 2021, Memphis and Bartlett police attempted to stop a teenage driver with active warrants for aggravated assault. The driver briefly stopped but then hit a Bartlett squad car while fleeing. Memphis police pursued the vehicle but alleged that they lost sight of him on Elvis Presley Blvd. near Winchester. As the driver approached Raines Road from Elvis Presley Blvd., he crashed into another vehicle driven by bystander Marvin Honey who was on his way to the grocery store, and after the massive crash was transported to the hospital where he died from his injuries.

On 10/20/2021, officers assigned to a fugitive apprehension task force with the Colorado Department of Corrections attempted to arrest Aaron Leo Lang in Aurora, Colorado for an alleged parole violation. During the attempted arrest near South Parker Road, officers reported that Lang appeared to raise or aim a weapon toward them. Officers opened fire, striking Lang. He was pronounced dead at the scene. The shooting was referred for independent investigation and remained under review at the time of reporting.

On October 20, 2021, officers from the Syracuse Police Department responded to a residence at 216 Ulster Street in Syracuse, New York, following reports of a disturbance. attempted to communicate with Allison Lakie, 33, who was inside the home.

Around 2:30 p.m., dispatch received a call from a passerby who said an SUV was stopped in the middle of the road, and the driver appeared to be slumped over the wheel. Two Arlington officers responded to the call. One of the officers tried to talk to the driver and told him to stay where he was. The driver fled. The driver stopped briefly, at which point one of the officers told him he was under arrest and that he should shut off his vehicle. The driver again fled. The officers again pursued. The driver drove to a cul-de-sac and made a U-turn and allegedly began driving in the direction of one of the officers, who had stopped, exited his vehicle, and was standing near his open door. As the vehicle moved towards him, the officer shot and killed him. Officer Robert Phillips was fired from the agency two days later for violations of policy.

On 10/20/2021, Pennsylvania State Police and Pocono Township police pursued Betty Jane Tibaldi, 54, after officers attempted to contact her outside a closed business and she drove away. Police disabled the vehicle with stop sticks along PA-611 near Shine Hill Road, leading to an hours-long armed standoff involving negotiators and the state police Special Emergency Response Team. Tibaldi remained inside the vehicle with her husband while displaying signs of severe mental distress. After her husband surrendered and left a handgun in the vehicle, Tibaldi opened the door and fired a shot toward troopers. Police returned fire, striking and killing her at the scene. The Monroe County District Attorney’s Office conducted the investigation.

At about 5:30 p.m., police received information that a person wanted on several warrants, including grand theft, auto theft, aggravated fleeing and eluding, and driving with a suspended license, was in the area. Officers arrived and found the person inside his vehicle. Police blocked the vehicle, at which point the person drew a handgun. An officer fired a single shot, missing him. After the shot was fired, the person pointed the gun at his head and shot and killed himself.

On October 20, 2021, around 11:37 p.m., a Missoula Police Department officer observed a white Honda Civic speeding at approximately 60 to 70 mph in a 25-mph zone. An attempt to stop the vehicle led to a pursuit after the driver, Kemari D. Parks, aged 20, made a quick turn and initially evaded the officer. The vehicle was later spotted, and as police approached, Parks accelerated through a parking lot, with a second person, J.M., in the passenger seat. Police continue to chase including into oncoming traffic and through red lights, until Parks crashed into a parked car, leading to further collisions. J.M., the passenger, died in the crash.

Officers from the Falmouth Police Department responded to an incident at the intersection of Middle Road and Lunt Road in Falmouth, Maine. The individual, identified as Daniel Louis DiMillo, was reportedly armed with a knife. Police opened fire during the confrontation, killing DiMillo. On October 19, 2021, at approximately 11:40 p.m., Suffolk County Police officers in a marked police cruiser attempted a traffic stop on a 2004 Toyota Tacoma with stolen license plates. The alleged driver, Robert Hengeveld, 50, failed to comply and fled from officers, leading to a pursuit.

During the chase, the Toyota Tacoma collided with a Honda at the intersection of North Ocean Avenue and Expressway Drive South in Holtsville, New York. The driver of the Honda, 19-year-old Starlin Manuel Diaz Felipe, was killed in the crash. Hengeveld was arrested and charged with reckless driving, unlawfully fleeing a police officer, criminal possession of stolen property, and multiple bench warrants.

On 10/19/2021, Georgia State Patrol troopers attempted to stop Shavoz Levon Seals near Abernathy Road NW and Brandon Mill Road while police were trying to arrest him on active warrants tied to a murder investigation. Seals did not stop and continued driving. Troopers performed a PIT maneuver near Brandon Mill Road and Brandon Ridge Drive to stop the vehicle. Authorities reported Seals then shot himself inside the vehicle. Police transported him to a hospital where he died, and investigators reported finding a firearm in the vehicle.

On October 19, 2021, Indiana State Troopers and Evansville Police responded to a residence at 1801 Stinson Avenue following a report of a woman discovering a man’s body inside the home. Upon arrival, officers found 36-year-old Heidi Carter standing outside near a vehicle. Carter informed officers there were additional individuals inside the home and stated that she was armed.

Police took Carter into custody and surrounded the residence, ordering all occupants to exit. Carey Hammond, 46, reportedly rushed out of the home holding what appeared to be a firearm. Officers opened fire, shooting and killing Hammond.
Inside the residence, police discovered a tied-up woman with visible injuries and the body of another deceased man. Investigators have not disclosed further details about the sequence of events or the identities of the individuals found inside the home.

Around 7 p.m., deputies were searching for a homicide suspect when they began speaking with a man unrelated to the case. The man reached inside his waistband, prompting a scuffle between him and a deputy. During the struggle, the man retrieved a weapon and shot the deputy in the leg. The deputy then shot and killed the man.
