Officers responded to a 911 hang-up call from the Audubon Park Apartments around 7:45 a.m., where dispatchers heard screaming and the sounds of a struggle. Upon arrival, police were contacted by a victim who stated that a man was armed with a knife. Officers encountered Ashton Pinke at the side of the apartment building, where he reportedly charged at them while armed with a knife and a club. Two officers shot and killed Pinke.

Central Intelligence Agency and Federal Bureau of Investigation officers responded to a report of a "suspicious vehicle" outside of CIA headquarters in McLean, Virginia, around 6 p.m. Roy Gordon Cole emerged from his car holding a gun and was shot and killed by law enforcement officers.

On May 3, 2021, 32-year-old Brandon Lee Shockley Sr. was pursued by law enforcement officers from Indiana into Illinois. Shockley was wanted for attempted homicide.

The pursuit began on I-70 near the Indiana-Illinois state line, where Indiana State Police deployed stop sticks, causing Shockley's vehicle to suffer a flat tire. Despite this, he continued driving into Clark County, Illinois, eventually veering off the road and coming to a stop in a ditch near milepost 153.

Officers reported hearing a gunshot from inside the vehicle. In response, a Vigo County Sheriff's Deputy and a West Terre Haute Police Officer fired their weapons at the car. Officers found Shockley deceased in the driver's seat with a firearm present. An autopsy later determined that Shockley died from a self-inflicted gunshot wound.

At around 2:55 a.m., Ford County dispatch sent a be-on-the-lookout for a person wanted in Garden City who allegedly was involved in a stabbing. Gray County deputies located Luis Salgado's vehicle at around 3:17 a.m. The deputies attempted to stop the vehicle, but Luis Salgado continued driving. Deputies escalated a chase into Ford County, where Ford County deputies joined the chase at 3:24 a.m. Spike strips were deployed west of Dodge City, deflating one of the tires. Salgado continued driving east into Dodge City before veering off the roadway, striking a power pole, and rolling several times. He was ejected and died at the crash site.

On 05/02/2021, David Earle Hartenstein, 39, of Fitchburg, Massachusetts, was traveling northbound in the southbound travel lanes on NH-101. Police attempts to stop the wrong-way vehicle, including the use of tire deflation devices, were unsuccessful. Officers from multiple agencies set up rolling roadblocks to prevent other drivers from encountering the vehicle. Near the Route 101 split in Manchester, Hartenstein veered off the roadway and into the woods. The crash resulted in his death.

On May 2, 2021, at 6:18 p.m., Hamilton Township Police officers responded to a residence on the 1200 block of Adams Road in Loveland, Ohio, following a 911 call from a neighbor. The call reported that Anthony Williams, a 58-year-old male, was intoxicated, threatening suicide, and discharging a firearm in his home. The caller indicated that Williams’ partner had fled to the neighbor's house seeking help.

Officers Chris Wall and Josh Clift located Williams in the backyard of the residence at 6:55 p.m. Williams was allegedly holding a firearm. Officers opened fire, killing him.

On 05/02/2021, around 2 a.m., Berlynn Matthews stopped to assist Joshua Day, who had been struck by a car while crossing North Point BoulevaRd in Dundalk. While Matthews was helping Day, off-duty Officer William Collazo-Brown on a motorcycle struck both of them, leading to their deaths. Collazo-Brown was subsequently arrested and charged with DUI-related offenses.

On May 1, 2021, Cuba Police Officer Brandon Roybal Barber allegedly drove a black Ford F-150 pickup truck in the wrong direction on Interstate 25 near Roy Avenue in Albuquerque, New Mexico. Barber's vehicle collided with a black GMC Yukon, killing two occupants. Barber was charged with three counts of vehicular homicide, aggravated DWI, and possession of an open container.

On May 1, 2021, around 6 p.m., the Porter County Sheriff's Office responded to a 911 call regarding a disturbance at a residence located at 716 Long Run Road in South Haven, Indiana. After several hours of attempting to communicate with a man inside the home without success, officers entered the residence. Inside a bedroom, they encountered Alexander T. Tuzinski, who was reportedly armed with a handgun. Officers shot and killed Tuzinski during the confrontation.

Around 10:50 p.m. on May 1, 2021, officers in an unmarked patrol car attempted to stop Jose Astrain Jr. for an unspecified traffic violation. Astrain refused to stop and sped away at high speed. The officers turned off their emergency lights and did not pursue him. Astrain lost control of his Ford Fusion, fishtailed into oncoming traffic, and crashed into a pickup truck, resulting in his death.

On May 1, 2021, at approximately 7:27 p.m., dispatchers received a report of shots fired at the Oneida Casino in Green Bay, Wisconsin. Bruce K. Pofahl, a former restaurant employee, allegedly approached a waiter station and shot and killed 32-year-old Ian J. Simpson and 35-year-old Jacob T. Bartel. Pofahl then left the restaurant and shot and wounded 28-year-old Dan Mulligan. Green Bay police officers encountered Pofahl near the first floor of the parking ramp east of the Radisson Hotel. Officers Brian Jordan, Makayla Wolfe, and Ben Snyder confronted Pofahl, who was armed, and shot and killed him during the encounter.

Officers from the Miramar Police Department attempted to stop a black Mercedes sedan. The driver, Everette Orette Brown, continued driving and headed south on Southwest 64th Avenue, where the Mercedes struck a pedestrian. Officers pursued the car until it crashed into multiple light poles and a fire hydrant, overturning around 9 p.m. Brown was killed in the crash.

On April 28, 2021, at approximately 5:16 a.m., officers from the Lake Charles Police Department responded to reports of gunfire at a hotel located at 1000 North Lakeshore Drive in Lake Charles, Louisiana. Upon arrival, officers determined that shots had been fired from a hotel room toward Interstate 10, prompting the closure of the highway and diversion of traffic to Interstate 210 for safety reasons.

The individual inside the hotel room, identified as 44-year-old Kyner Gene Rollins of Los Angeles, California, had locked himself in the room. SWAT was called and forced entry into the room. They found Rollins dead from a gunshot wound. The circumstances surrounding his death and who killed him were not disclosed by police.

On 04/28/2021, a deputy from the Hillsdale County Sheriff's Office was dispatched to a home in the 6500 block of South Edon Road in Reading, Michigan, following a complaint about a dog at large. Upon arrival, the deputy was allegedly bitten by the dog. He shot and killed the animal. Following this, Oscar Herrera, the dog's owner, allegedly confronted the deputy while armed with a knife. The deputy used deadly force, shooting and killing Herrera. The incident is under investigation by Michigan State Police. The family of Oscar Herrera, represented by Attorney Ian Fallon, has viewed the bodycam footage but has called for its public release for transparency and accountability. They argue that Herrera was in emotional distress due to the death of his dog, and the situation could have been handled differently by the deputy.

On April 28, 2021, New York State Police identified Barry K. Stewart, 55, concerning a double homicide at BridgeView Real Estate Services in Watertown, New York. A State Police K9 unit spotted Stewart's vehicle, a gray 2020 Ford F-150 pickup truck, on Route 11B in Dickinson, Franklin County. When troopers attempted a traffic stop, Stewart's vehicle left the roadway. Troopers found Stewart inside the vehicle with a self-inflicted gunshot wound; he was pronounced dead at a nearby hospital.

The pursuit continued onto the 405 Freeway, then exited onto Manchester Avenue, with the driver racing through the streets of Inglewood. At the intersection of Glasgow Avenue and Olive Street, the pursued vehicle ran a stop sign and collided with a white sedan occupied by Ryan Davis and his partner, Asia Boatwright. The impact caused severe damage, trapping both victims in the wreckage. Davis was transported to a local hospital, where he was pronounced dead. The driver of the pickup truck fled on foot but was apprehended shortly thereafter and identified as 42-year-old Gustavo Ruelas. He was booked on multiple felony charges, including driving under the influence of a drug.

On April 28, 2021, Isaac Alton Barnes, 32, allegedly killed his mother and stepfather before engaging in a standoff with the Watauga County Sheriff's Office at a residence on 553 Hardaman Circle in Boone, North Carolina.

A trooper from the Florida Highway Patrol allegedly clocked a vehicle at 111 mph on a highway near Tampa. On April 24, 2021, officers with the Gary Police Department pursued a stolen U-Haul truck following a stolen vehicle alert in the area. The pursuit ended when the U-Haul collided with an SUV at 6th Avenue near Colfax Street, struck a utility pole, and rolled over. The male driver of the U-Haul was pronounced dead at the scene. One person in the SUV was transported to a hospital with non-life-threatening injuries. The crash caused power outages in the area as crews responded to repair downed utility lines.
