On 11/04/2021, a Taylorsville police officer contacted Aaron Troy Michael Rehn, 29, at the Speedway gas station at 2187 W 4700 South after police described suspicious activity. Rehn ran from the officer. Police said Rehn turned and fired multiple rounds at the officer, and the officer opened fire. Police said neither was hit during the exchange. Multiple agencies searched the neighborhood and police issued a shelter in place order. Police said a 12 year old boy later spotted Rehn hiding in a basement window well and the family called 911. Police and K 9 units closed in nearby. Police said Rehn shot himself as officers approached. A later Salt Lake County District Attorney review stated the officer was justified in opening fire during the earlier exchange and that Rehn died by suicide after the search narrowed.

On November 4, 2021, at approximately 2:30 p.m., Louisiana State Police fatally shot 36-year-old Barry Rigsby at the Wisner Apartment Complex on Maple Street in Wisner, Louisiana. Rigsby was wanted for questioning in a triple homicide that had occurred earlier that day in Shreveport. Police located Rigsby's abandoned vehicle near the apartment complex. Franklin Parish Sheriff's deputies began evacuating residents. During the evacuation, Rigsby fired shots at the deputies and locked himself inside an apartment. Louisiana State Police, including SWAT and police negotiators, arrived. After a standoff lasting four hours, during which Rigsby continued to fire shots at police and threw a fire extinguisher at them, police shot and killed him.

On November 4, 2021, U.S. deputy marshals attempted to serve a warrant on a man who had two outstanding warrants for armed burglaries and felony firearm charges. As deputies approached the man, he allegedly pulled out a firearm and shot and killed himself.

At approximately 1:45 a.m. on November 3, 2021, Kentucky State Police troopers responded to a 911 call at the 800 block of Chipman Ridge Road in Williamstown, Kentucky. Upon arrival, troopers encountered 48-year-old Gary Wayne Bressler Sr., who was reportedly in a mental health crisis. Bressler was holding a decorative samurai sword. Despite commands to drop the weapon, Bressler did not comply, leading to the troopers firing their weapons, fatally wounding him. The KSP Critical Incident Response Team is handling the investigation into the incident, and the identities of the involved officers have not been released.

On November 3, 2021, Eau Claire Police responded to reports of a break-in at a residence where the man was allegedly armed with a knife. LeKenneth Q. Miller reportedly stabbed a woman inside the home. Upon arrival, officers confronted Miller and shot and killed him during the encounter.

On 11/03/2021, Glendale Police Department officers responded to reports of a man yelling and striking vehicles near N 62nd Avenue and W Ocotillo Road and located Jose Enrique Cortez in the area. Officers attempted contact and pursued him after he drove away and collided with a patrol vehicle. Cortez left the vehicle and officers confronted him on foot. Officers deployed a conducted energy weapon and fired their weapons during the encounter, striking him. He died at the scene. Reporting indicated he was not carrying a firearm when examined afterward, and prosecutors later reviewed the shooting and declined charges.

On 11/03/2021, John Kennedy Fitzgerald Bolden was driving a stolen vehicle on I-94 when law enforcement attempted a traffic stop and initiated a chase; the pursuit continued into Lake County where the fleeing vehicle struck a Porter County Sheriff’s cruiser, then the median wall and rolled over, ejecting Bolden, who was pronounced dead at the scene.

On November 3, 2021, a Wyoming Highway Patrol trooper stopped a Chrysler van on I-80 near Green River. A second trooper arrived, and after speaking with the driver, the individual jumped back into the van and fled. Troopers pursued the vehicle at speeds exceeding 120 mph. The pursuit ended when the van collided with a commercial belly dump truck, killing the driver.

On 11/03/2021, Covington Police officers responded to a residence on Spillers Road after a 911 call reported domestic violence. Officers spoke with a woman who said Gilbert had left shortly before their arrival and instructed her to call again if he returned. Dispatch received a second call later that night reporting Gilbert back at the home, and officers returned and confronted him outside. Officers attempted to detain him, deployed a Taser, and physically struggled with him. During the encounter, officers reported he gained control of a Taser while rising from the ground, and an officer fired a handgun and shot him. Officers transported Gilbert to a hospital where he died. The Georgia Bureau of Investigation conducted the case review and forwarded findings to the district attorney for evaluation.

On 11/03/2021, McAllen Police officers responded to a disturbance reported at a restaurant on South 10th Street where employees said Garza was arguing inside. Officers confronted him outside and chased him on foot through the parking area. He fell during the encounter, and officers struck him with batons, placed him in handcuffs, and restrained him on the ground using body weight. Garza lost consciousness while officers held him and later died after transport from the scene. His family filed a wrongful death lawsuit alleging the beating and restraint caused fatal injuries. On November 1, 2021, at approximately 4:11 p.m., the Houma Police Department responded to a disturbance call at Downtown Court and East Main Street in Houma, Louisiana. Upon arrival, the first officer encountered 36-year-old Johnny McGee, who reportedly charged at the officer while holding a screwdriver. The officer issued commands to stop and attempted to create distance; however, McGee continued to advance. A second officer arrived on the scene as McGee allegedly raised his arm with the screwdriver toward one of the officers, prompting an officer to discharge his service weapon. McGee was struck by gunfire and transported to a nearby hospital, where he succumbed to his injuries. Family members later revealed that McGee had been diagnosed with bipolar disorder and schizophrenia and was experiencing a mental health crisis at the time of the encounter.

On 11/2021, a 12-year-old boy was killed in South Gate after a high-speed crash involving an off-duty Los Angeles County Sheriff’s deputy. Authorities said Deputy Ricardo Castro was driving at a high rate of speed when his vehicle collided with another car, killing the child. Investigators determined Castro was not responding to a call at the time of the crash. He was later charged with murder and reckless driving in connection with the child’s death.
Prosecutors said Castro had been involved in several previous vehicle collisions before the South Gate crash. He had also received multiple traffic citations — including for speeding — during his time before the fatal crash. The prosecutor’s narrative and the fact that it was included in official comments suggests a pattern of risky driving and citations before the deadly collision. On 10/31/2021, Los Angeles police responded to calls about a man slashing car tires and acting erratically near West Victory Boulevard. Officers located Melkon Michaelidis armed with two knives. Upon arrival, troopers encountered Glenn William Custer, a 59-year-old white male, who was behaving erratically and armed with a knife. On October 30, 2021, at approximately 1:30 p.m., Cleveland officers received a report that 50-year-old Matthew James Riggs was threatening to harm himself and was armed with a weapon. Officers located Riggs' vehicle in the 1000 block of Cottage Stone Lane and initiated a traffic stop. During the encounter, officers allegedly heard a gunshot. Police open fire killing Riggs. It remains unconfirmed, whether he fired any shots or not. The Tennessee Bureau of Investigation (TBI) conducted an independent investigation into the circumstances leading to the shooting. On 10/29/2021, an off-duty Massachusetts State Police trooper drove a vehicle on Interstate 93 in Boston while intoxicated. The trooper struck a highway median and came to a stop in the roadway. At approximately 4 a.m. on October 29, 2021, dispatchers received a 911 call reporting individuals in a Jeep SUV threatening to shoot the caller. Terre Haute Police attempted to stop the Jeep, but the driver, 35-year-old Shawn Michael Passwater, allegedly fled. Officers pursued the vehicle until it crashed at S 13th Street and Lockport Street. At about 11 p.m., Montgomery County deputies chased Justin Frostad, who was traveling eastbound on County Road 3875 in a 2007 Kia Optima. Police never disclosed the reason for the pursuit. Frostad lost control of his vehicle after crossing the railroad tracks, veered off the road, and flipped several times. On October 29, 2021, Clinton police attempted to intercept a man accused of stealing a truck from Jackson and driving into Clinton. The man refused to stop, leading to a chase that ended with him returning to Jackson. On October 28, 2021, Colorado Springs Police Department officers responded to 911 reports of a man firing a handgun into an occupied vehicle near the 1600 block of South Nevada Avenue. When officers arrived, the suspect fled on foot and was tracked by police across multiple locations, including near the Vanguard School campus and later in the 1700 block of South Wahsatch Avenue. During the encounter, officers said the suspect repeatedly pointed a firearm at them. Officers fired on the suspect, identified by law enforcement as 30-year-old Ahmad Akeem Abdul Muhammad. He was struck by gunfire and taken to a hospital with life-threatening injuries. According to investigators, Muhammad died from his injuries on October 31, 2021. The El Paso County Sheriff’s Office led the investigation, and a subsequent review by the 4th Judicial District Attorney’s Office determined the use of deadly force by police was justified. No officers were injured in the incident.

Upon arrival, deputies encountered an 85-year-old male who was suicidal and armed with a firearm. After approximately nine minutes, the individual fatally shot himself. The incident is under investigation by the Santa Fe Sheriff's Office.

On 10/27/2021, Boise police responded to reports of a man threatening to jump from an overpass near West Myrtle Street and South Capitol Boulevard. Officers located Zachary Snow on the overpass. Police stated that Snow moved away from officers while pointing a dark object in their direction. Officers then opened fire, shooting and killing Snow at the scene. Prosecutors later cleared the officers of criminal wrongdoing. Snow’s family filed a federal civil rights lawsuit alleging wrongful death, and the City of Boise later agreed to a financial settlement with Snow’s mother.

Police were executing a search warrant at a home. Someone in the home allegedly shot at them. Robert Richart fired more shots at officers and started a fire inside the home. Richart allegedly exited the home through the garage with a weapon in hand. Officer Eric Strnad shot and killed Richart.

On October 26, 2021, Dulce Maria Castro Perez, 21, of Hudson, Colorado, was riding in a vehicle struck by a stolen car during a Brighton police pursuit. The drive, ran a stop sign at the intersection of Bridge Street and Mt. Bierstadt Street and crashed into the vehicle she was in. Perez was ejected and died from her injuries at the hospital.

On October 26, 2021, Gustavo Mosqueda Ortega, 25, of Montrose, Colorado, was a pedestrian struck and killed during a Brighton police chase. Police had chased a vehicle they believed to be stolen which hit Ortega along Mt. Bierstadt Street. Ortega was taken to a hospital but died from his injuries.

Officers were sent to an apartment on River Bend Drive in response to a domestic disturbance call at approximately 12:35 a.m. Foster had a gun. Officers arrived, and during the encounter, Foster became agitated and reached into a pile of clothing on a dresser for a gun. He fired at Officer Tom Seabolt, striking his police radio. Officer Timothy Lanham opened fire and killed Foster. The incident occurred in the presence of Foster's wife and children. The Kentucky State Police Critical Incident Response Team handled the investigation and shielded the names of the officers. The family expressed concern about the lack of transparency and the absence of body camera footage.

On 10/26/2021, Lima Police Department officers escalated a chase to speeds exceeding 100 mph while traveling westbound on Koop Road following a reported shoplifting incident at Walmart. Police then alleged that they "called off the chase" just before the vehicle missed a curve on West Koop Road and crashed into a tree at Koop Road and N Eastown Road.

The driver, Marva Ackles, was killed in the crash. A passenger in the vehicle was injured and transported to a nearby hospital in unknown condition. The incident is under internal self-investigation by the Lima Police Department.

On 10/26/2021, Lake County Sheriff’s Office deputies were dispatched to a residence near Leroy Center Road and Taylor Road after Scott Wright reportedly told dispatchers he was going to kill someone. Upon arrival, deputies encountered Wright allegedly restraining a 60-year-old woman at gunpoint. Wright released the woman and returned inside the home.

Deputies held the home for the next 4 to 5 hours. They stated that Wright appeared intermittently in the windows and the garage armed with a long gun and a handgun. Around 9 a.m., six hours into the standoff, Wright allegedly began setting fires inside the home. He exited the smoke-filled residence into the backyard with a long gun. Deputies fired beanbags and alleged that Wright raised his rifle. Deputies opened fire, killing him.

On October 26, 2021, Philadelphia Police officers responded to a reported dispute between a landlord and tenant at a residence on the 5700 block of Overbrook Avenue in Philadelphia, Pennsylvania. Upon arrival, officers encountered Koffi Dzima, a 31-year-old Black male, who was allegedly holding a pickaxe and a hammer. One officer deployed a Taser, but Dzima reportedly struck an officer over the head with the hammer. In response, officers opened fire, fatally shooting Dzima. The Philadelphia Police Department handle the case and internal self investigation.
