On October 19, 2021, officers from the Reno Police Department's repeat offenders’ program were searching for Richard "Ricky" Thomas, who had an outstanding felony warrant. They located his red truck at a gas station on Golden Valley Drive. As the truck began to move, three unmarked police vehicles surrounded it. Officers attempted to remove Thomas from the vehicle, but he allegedly refused to comply and tried to drive away. During this attempt, one officer was pinned by the driver's door, and another was caught by the closing passenger door as the truck moved off. Both officers involved opened fire, killing Thomas.

On 10/18/2021, Metropolitan Police Department officers went to an apartment building on Congress Street Southeast in Washington, DC to serve a temporary protection order on Deandre Johnson and address a prior reported assault. Officers made contact with Johnson inside the building and attempted to place him under arrest. During the attempt to handcuff him, a physical struggle occurred. Police stated they believed Johnson was attempting to gain control of an officer’s firearm. Officers opened fire and shot Johnson. He was pronounced dead at the scene. The incident was referred for investigation and remained under review at the time of reporting.

On October 18, 2021, a Rochester Police Department officer observed a stolen vehicle on Bartlett Street in Rochester, New York. The officer reportedly did not activate emergency lights or engage in a pursuit, but the vehicle left the area.

Shortly afterward, the fleeing vehicle collided with another car at the intersection of Seward Street and Whittlesey Street. The occupants of the struck vehicle, a man and a woman, sustained severe injuries. The woman, identified as 36-year-old Tishara Pugh, was killed as a result of the crash.

On 10/18/2021, Pennsylvania State Police responded to a welfare check at a residence on the 200 block of Burket Drive in Bloomfield Township where Jacob Curry, 44, barricaded himself inside and fired a gun toward troopers. Police established a perimeter and deployed the Special Emergency Response Team as the situation escalated into an armed standoff. Reporting states Curry continued firing from the residence, and troopers opened fire, striking him. He died at the scene. The Bedford County District Attorney and coroner assisted with the response and the incident was reviewed by investigators.

On October 18, 2021, Shawn Delemore Dempsey was pursued by Mingo County Sheriff's deputies following the discovery of his parents, Shawn Dempsey, 52, and Karen Sue Dempsey, 52, who were found dead. During the pursuit, Dempsey allegedly pulled a gun and fired at the deputies. Officers shot and killed him during the confrontation.

On 10/17/2021, Springfield police officers encountered Douglas Knakmuhs at a gas station in Springfield, Missouri while attempting to make contact in connection with outstanding warrants. Police reported that gunfire occurred and Knakmuhs left the location. Officers later located him at a nearby residence where he entered the home and occupants were inside. A tactical response was initiated. During the police entry, officers encountered Knakmuhs inside the residence and a confrontation occurred. Police opened fire and fatally shot Knakmuhs at the scene. The incident was referred for investigation.

On 10/17/2021, Fort Smith police officers responded to reports of a fight involving weapons at a residence on Tilles Avenue in Fort Smith, Arkansas. Upon arrival, an officer encountered Christofer Conner and observed him striking a child with a brick. The officer ordered Conner to stop, and Conner initially complied. When the officer attempted to place Conner in handcuffs, police reported that Conner produced a sharp object and injured the officer. The officer opened fire, striking Conner. He was pronounced dead at the scene. The injured officer was treated for non life threatening injuries. The shooting was reviewed and the officer was cleared of wrongdoing.

Police responded around 2 a.m. to a 911 call about an armedman smoking crack. Two officers, with probable cause to arrest Allan Lorenzo Robb on a charge of grand theft auto, reportedly deployed less-lethal weapons without success before a third officer shot and killed him.

At approximately 2:52 p.m. on October 17, 2021, a Kentucky State Police trooper observed a Yamaha motorcycle traveling eastbound on the Western Kentucky Parkway near the 132-mile marker. The trooper initiated a traffic stop, but the motorcyclist, later identified as 36-year-old Michael C. Beaver, did not stop and continued eastbound. Police escalated a chase. Beaver entered I-65 northbound at the 91-mile marker and continued for approximately 3 miles with police in hot pursuit. He exited I-65 at the 94-mile marker, lost control of the motorcycle on the offramp, and crashed. He was transported to a local hospital, where he was pronounced deceased. Deputies with the Clark County Sheriff’s Office located Kfin Karuo’s white Ford Expedition in a parking lot and attempted to arrest him for allegedly pointing a gun at a man on September 29. Karuo drove away, and deputies escalated a high-speed pursuit reaching 70 to 80 mph. Deputy Forrest Gonzalez performed a PIT maneuver, spinning Karuo’s SUV onto a berm. As Karuo began to climb out the driver’s window, Gonzalez saw a handgun in Karuo’s hand and ordered him to drop the weapon. Karuo pointed the gun at Gonzalez, who opened fire while backing away. Karuo ran toward a mobile home park, then turned and again pointed the gun at Gonzalez. Gonzalez fired more rounds. Deputy David Delin, who had arrived at the scene, also opened fire when Karuo appeared to square off. Deputies later found Karuo dead in a driveway with a loaded 9mm Glock in his hand. Gonzalez fired 10 rounds, and Delin fired 8. At least seven bullets struck a nearby occupied home, but no civilians were injured. The fatal shot struck Karuo in the torso. Deputy Forrest Gonzalez had previously been involved in two other fatal shootings.

About 4 a.m., officers responded to a report of shots fired. Officers arrived at the location and began to canvass the vicinity for a maroon sport utility vehicle firing a weapon. An officer spotted a maroon Lincoln SUV with paper plates and conducted a traffic stop along with assisting officers. Guillermo Gonzalez-Gonzalez was the lone occupant. The officers shot and killed Gonzalez. Gonzalez reportedly had two guns with him at the time of the incident: a semiautomatic handgun and a long rifle.

On 10/16/2021, the Minnesota State Patrol responded to an incident where Cody Wayne Pirila allegedly killed his roommate, Skylar Cody Hunter, 31, and roommate's father, Jamie Allen Hunter, 51. After the killings, Pirila took his own life following a police chase.

Joshua Dewayne Hammock allegedly robbed a convenience store in Lee County while displaying a knife around 11:20 p.m. Lee County deputies began searching for him in response to the theft. Around 7:30 a.m., another robbery took place at a 7/11, where Hammock allegedly stole a case of beer and flashed a knife before charging at the store clerk. When two responding deputies found him, Hammock allegedly tried to attack them with the knife, prompting one of the deputies to shoot and kill him.

Alexander King was carrying a pellet gun, which he allegedly pointed at officers. Police stated the pellet gun resembled a rifle, with only a faded orange tip that was not visible from a distance at night. The names of the two officers who shot King, along with five others present who did not fire, are being withheld under Marsy's Law.

On 10/15/2021, Milpitas police officers located Michael Edward Nelson Jr. inside a reported stolen vehicle at the Milpitas Square Shopping Center on Barber Lane in Milpitas, California. Officers approached the vehicle and Nelson exited briefly, looked toward officers, and then re entered the vehicle. Police reported that Nelson then fired a weapon toward officers from inside the vehicle. Officers opened fire, striking Nelson. He was pronounced dead at the scene. The incident was referred for independent investigation.

On October 14, 2021, at approximately 2 p.m., Edgecombe County Sheriff’s Office deputies responded to a residence on McNair Road in Tarboro, North Carolina, after receiving a report that 32-year-old Jermaine Harris was chasing a family member with a knife. Upon arrival, Harris lunged at one deputy and stabbed him twice in the back. A second deputy, who arrived shortly afterward, discharged their firearm, fatally striking Harris. The injured deputy was transported to Vidant Medical Center in Greenville, where they were treated and later released.

At approximately 12:00 p.m. on October 14, 2021, the Trigg County Sheriff's Department responded to a disturbance on South Road in Cadiz, Kentucky, where 48-year-old Harold Floyd Jett allegedly fired a shot at a vehicle during an altercation. Jett drove away. Officers escalated a chase at high speeds that continued into the city of Cadiz. The chase ended at Hilltop Street, where Jett entered the residence of his aunt, 71-year-old Mary Dullenty.

On 10/14/2021, Crestview police officers responded to a residence in the 300 block of Hospital Drive after a neighbor reported hearing calls for help. Officers encountered Calvin Wilks Jr. inside the residence and attempted to have him step outside. When that did not occur, officers attempted to detain him. During the encounter, officers deployed a Taser and physically restrained Wilks. Wilks later became unresponsive and died. A medical examiner later cited restraint and Taser use as contributing factors. One Crestview police officer was charged with manslaughter in connection with Wilks’s death. The charge was later dismissed. Wilks’s family filed a civil lawsuit related to the incident.

On 10/14/2021, law enforcement attempted a traffic stop involving Russell Leggett in Riverside County, California. The driver continued driving and police escalated a pursuit that traveled through the area and onto Interstate 10. During the chase, police reported gunfire involving the vehicle. The pursuit ended near the Interstate 10 off ramp at North Indian Canyon Drive in Palm Springs. Officers opened fire on the vehicle, striking Leggett. He was pronounced dead at the scene. On 10/14/2021, deputies with the Anderson County Sheriff’s Office attempted to stop a motorcycle in the Anderson area of South Carolina. The driver continued driving and police chased him through local roads. During the encounter, deputies reported gunfire involving the motorcycle. Deputies opened fire. The motorcycle then crashed, and Robert Allen Maness was pronounced dead at the scene. The South Carolina Law Enforcement Division conducted the investigation. The case later resulted in a civil lawsuit, which was resolved with a financial settlement to Maness’s family.

Police arrived and confronted 34-year-old Dustin J. Paradis, who was reportedly holding a chef's knife. Officers alleged that he walked toward them. Two officers opened fire and killed him. On October 13, 2021, a Geary Police officer saw a 46-year-old man holding a knife near the I-40 and US-281 interchange in Hinton, Oklahoma. An off-duty officer driving past the scene stopped to assist, and a third officer arrived soon after. The third officer attempted to use a taser. Police allege that the man lunged at the off-duty officer with the knife when he saw the Taser. One of the officers opened fire, striking the man and fatally wounding him.

On October 13, 2021, off-duty NYPD officer Yvonne Wu was waiting at the Brooklyn apartment of her ex-girlfriend, Jenny Li, located near 19th Avenue and 79th Street. When Li returned to the apartment accompanied by 24-year-old Jamie Liang, Wu allegedly shot both women.

Jamie Liang was pronounced dead at the scene. Jenny Li sustained injuries but survived the shooting. Wu, who remained at the scene, was arrested by responding officers.

At approximately noon, officers were dispatched to an apartment complex for a family disturbance involving a gun. A caller reported seeing a man with a gun, shooting into the air and pointing the gun at a woman. The woman involved also called police and reported that Christopher Ryan Vasquez was trying to kill her. Officers arrived and Vasquez barricaded himself in his apartment. He occasionally approached the window and cursed at officers while holding one and sometimes two handguns. He also pointed the guns at his own head and at one point placed one in his mouth. He reportedly said he was not going to come out peacefully, threatened suicide, and stated he was going to kill himself. At approximately 6:41 p.m., officers sent gas into the apartment, and Vasquez fired his handgun toward an armored vehicle, and an officer fired one round at him. He fell away from the window and out of sight. Shortly after, additional gunfire could be heard from inside the apartment. A robot camera revealed Vasquez lying dead on the floor of the apartment with a gun nearby.

On 10/13/2021, Michigan State Police fugitive team members located Steven David Schumann near the 9200 block of Lindsey Road while attempting to arrest him on a parole absconding warrant connected to a prior assault case. Troopers approached his vehicle and police reported Schumann produced an edged weapon, escalating the situation into a hostage confrontation involving a woman present. Troopers opened fire and struck Schumann, killing him at the scene while the woman sustained injuries during the encounter.

On 10/13/2021, Birmingham Police officers responded to a report of a woman shot during a domestic dispute on Brussels Circle and identified George Anthony Dowdell as the person involved. Police pursued him as he entered an apartment and established a perimeter after he barricaded himself and fired gunshots toward officers. Officers conducted hours of negotiations and tactical containment. When officers entered the residence, they found Dowdell dead from a self inflicted gunshot wound.

On 10/13/2021, Jim Rogers, a 54-year-old man experiencing a mental health crisis, was taken into custody by Pittsburgh police in the Bloomfield neighborhood. Officer Keith Edmonds deployed a Taser multiple times during the arrest. Rogers was placed face-down, handcuffed, and later became unresponsive in the back of a patrol vehicle. He was transported to a hospital, where he died the following day. The medical examiner ruled his death an accident, citing excited delirium and underlying health conditions following restraint and Taser use. No criminal charges were filed against Edmonds. The City of Pittsburgh fired Edmonds in 2022, but an arbitrator later ordered his reinstatement. In 2025, a Pennsylvania Commonwealth Court upheld that decision, ordering the officer reinstated with full back pay and benefits. Separately, the city settled a federal wrongful death lawsuit filed by Rogers’s family for $8 million.

On 10/13/2021, Davenport police officers responded to reports of a man in the area of East 9th Street and Iowa Street who was seen holding what appeared to be a handgun to his head. Officers encountered Bobby Jo Klum in public and attempted to stop him using two less lethal rounds, which did not end the encounter. Police stated that Klum moved toward nearby bystanders. One officer then opened fire, striking Klum. He was pronounced dead at the scene. Authorities later stated that the object Klum was holding was a pellet gun. The Scott County Attorney ruled the shooting justified, and a subsequent civil lawsuit was dismissed by a judge.

On 10/13/2021, deputies from multiple Idaho sheriff’s offices encountered Buddy Byron McKenzie in the McCammon area while attempting to locate him in connection with a prior shooting reported earlier in the month. A deputy observed McKenzie inside a pickup truck and attempted to make contact. McKenzie continued driving and police chased the vehicle through the area. During the pursuit, deputies reported that gunfire occurred involving McKenzie. The pursuit later ended near the interchange of U.S. Highway 30 and Interstate 15. Deputies opened fire and shot McKenzie, killing him at the scene. The incident was referred for external investigation.

On 10/13/2021, Los Angeles Police Department officers responded to a report of an assault with a deadly weapon at a residence on Keystone Avenue in the Palms area of Los Angeles shortly before 6 a.m. Officers encountered Carlos Arias inside the residence during a family disturbance. Police reported that family members were present and that Arias was holding a knife during the encounter. An officer opened fire, striking Arias. He was pronounced dead at the scene. The incident was reviewed by investigators, and the case was pending review at the time of reporting.

On October 12, 2021, at approximately 11:40 a.m., members of the Knoxville Police Department's Special Operations Squad were assisting agents from the Tennessee Bureau of Investigation's Drug Investigation Division and the 5th Judicial Drug Task Force in executing a search warrant at a residence in the 2900 block of Sevier Avenue. Upon arrival, officers encountered 43-year-old Bryan Calvin with a gun. Preliminary reports indicate that Lee pointed the firearm at officers. They engaged him in a shootout and killed him. Lee was struck and pronounced dead at the scene.

On October 12, 2021, at approximately 9:33 a.m., Las Vegas Metropolitan Police Department officers responded to an apartment on E Lake Mead Blvd for an assault and battery call. The victim's current boyfriend reported that her ex-boyfriend, Mario Lawrence Martinez, had been harassing them, fired shots, and forcibly entered the apartment to kidnap the woman at gunpoint. Martinez attempted to escape in her car, which wouldn't start, leading him to flee with her on foot. They were located near a shopping center at about 10:50 a.m. Martinez, still armed, did not comply with police orders to surrender, moving through the shopping center, and eventually breaking into a box truck in an alley, where he also shot out its window. During this standoff, Officer Ramon Hiestand and other officers engaged Martinez. After Martinez pointed his gun at officers, the woman escaped, and Hiestand shot him. Martinez continued to fire while on the ground. He fired until he shot and killed Martinez.

On 10/12/2021, deputies from the Anderson County Sheriff’s Office attempted to stop a vehicle on Interstate 85 near South Carolina Highway 8 in Anderson County after believing the vehicle was stolen. The driver, Jonathan Combs, continued driving and police escalated a chase. Deputies later stopped the vehicle. According to authorities, Combs was armed during the encounter. At least one deputy opened fire, striking Combs. He was pronounced dead at the scene. The South Carolina Law Enforcement Division was requested to investigate the shooting.

On October 11, 2021, Baltimore County Police officers responded to an incident at the intersection of Englewood Avenue and Gilmore Street in Woodlawn, Maryland. During the encounter, officers opened fire, killing 36-year-old Jovan Lewis Singleton.

Officers pursued a man reportedly wanted for several felony offenses through multiple counties before he fatally shot himself in the head. Police said the man in his 20s was found in his vehicle with a self-inflicted gunshot wound at the end of the chase around 10 p.m.

A trooper pulled his marked cruiser into the path of a motorcyclist on I-4 around 7:30 p.m. The motorcyclist crashed into the front right side of the cruiser and was ejected from his bike. After falling into the road, the motorcyclist was struck by a semi-truck and killed.

On 10/11/2021, Richmond County sheriff’s deputies stopped a vehicle in Augusta, Georgia, citing a license plate cover violation. Jermaine Jones Jr. was riding in the rear passenger seat along with family members. Deputies requested consent to search the vehicle, and another deputy arrived with a K9 that police said alerted to narcotics. Deputies reported finding a handgun and a crack pipe under the rear seat where Jones had been sitting. Jones exited the vehicle and ran from deputies. An officer deployed a Taser during the arrest. Jones was taken into custody and later experienced a medical emergency. He was hospitalized and died approximately one week later. The Richmond County District Attorney’s Office later cleared the involved officers. Jones’s family filed a civil lawsuit related to the incident.

On October 10, 2021, at approximately 9 a.m., Indiana State Police responded to a reported domestic disturbance at a residence on East County Road 900 North in Sunman, Indiana. The disturbance involved Joshua "Josh" Edward Ebinger, 37, who was reportedly violating a protection order issued against him by a woman.
Two Ripley County deputies, an Indiana State Police trooper, and a Versailles Police officer arrived at the scene. The woman exited the residence and was moved to a safe location.

Officers issued commands for Ebinger to drop the gun. He allegedly pointed it at officers. A trooper and one of the deputies opened fire, shooting and killing Ebinger.

On October 10, 2021, at approximately 6 p.m., Brunswick County Sheriff's Office deputies responded to a domestic disturbance at 8627 Wanets Landing Road in Leland, North Carolina. Linda Beck, the caller, reported that her husband, 57-year-old Michael Robert Beck, had fired a firearm in her direction twice and expressed concerns about his volatility. Upon arrival, deputies located Beck on the back deck of the residence. He reportedly produced a firearm and pointed it toward the deputies. Despite repeated commands to drop the weapon, Beck allegedly failed to comply. Two deputies discharged their firearms, fatally striking him.

The Brunswick County District Attorney's Office, in coordination with the North Carolina State Bureau of Investigation, reviewed the case and determined that the deputies were legally justified in their use of deadly force.
