On April 13, 2021, Pennsylvania State Police responded to a domestic violence call in Saltlick Township, Fayette County. A woman had fled the residence and reported being assaulted and strangled by her husband, Keith Robert Tafoya. Upon arrival, troopers attempted to make contact with Tafoya, who fired a shotgun through a door, grazing one trooper. After several hours of standoff, Tafoya exited the home and raised a shotgun. Officers shot and killed him.

On April 13, 2021, officers with the Tontitown Police Department attempted to stop a stolen Toyota 4Runner. The vehicle, driven by an unidentified individual, fled at high speed and failed to negotiate a turn on Bush Creek Road near US 412. The 4Runner crashed into a tree and burst into flames. Chassadee Costales, who was in the vehicle, died early that morning from her injuries. Officers noted one of their own was treated for minor injuries after entering the burning vehicle to assist survivors.

On April 13, 2021, at approximately 10:30 p.m., a McFarland Police officer attempted to pull over a vehicle driven by Cam-Ron P. M. Poelinitz for speeding. Shortly after the attempted stop, the vehicle clipped one house and crashed into another. Poelinitz was killed in the crash, and four other people in the vehicle were taken to the hospital. Fortunately, no one in either house was injured in the crash.

On 04/13/2021, at around 1: 30 p.m., Maryland State Police received two 911 calls regarding a person possibly armed with a gun. The first call provided only a phone number, while the second gave a street address in the 23000 block of Hollywood Road in Leonardtown. Upon arrival, Trooper Joseph Azzari confronted Peyton Ham in the driveway. Azzari shot Ham twice, resulting in his death.

On April 12, 2021, officers from the Georgia State Patrol attempted to initiate a traffic stop on a 2015 Nissan Sentra for speeding at 111 mph on Interstate 20 in Bremen, Georgia. The driver, Aaron JuJuan Shelton, fled, and the officers escalated a high-speed pursuit through Carroll County. During the chase, the passenger, Pier Alexander Shelton, 28, leaned out of the vehicle and fired an AK-47-style rifle at pursuing officers, striking and disabling a patrol car. The pursuit continued through Carrollton and into Villa Rica, where the vehicle crashed near Villa Rica Elementary School. Both occupants fled on foot. As officers searched the area, Pier Shelton fired upon them, injuring multiple officers. Officers returned fire, fatally shooting Pier Shelton.

On April 12, 2021, at approximately 1:36 a.m., Angelina Sue Raad, a 38-year-old white female, was driving her vehicle the wrong way on the inner loop of New Circle Road near Harrodsburg Road in Lexington, Kentucky. She collided head-on with a police cruiser driven by Officer Dawn Miller, who was off duty and heading home after her shift. Both drivers were extricated from their vehicles and transported to a local hospital. Raad was pronounced deceased at around 4 a.m., while Officer Miller sustained serious injuries but was reported to be in stable condition.

On 04/11/2021, Chouteau Police officers initiated a traffic stop on a truck allegedly involved in a larceny incident at approximately 4 a.m. The driver did not stop. Police escalated a multi-jurisdictional pursuit through Wagoner, Pryor, and Okay. During the chase, someone in the vehicle reportedly fired shots at police. Police engaged in a shootout. Police claim that they lost site of the vehicle during which time it crashed along one 38–1 64 N. 6 mile Road in Fort Gibson. The driver was ejected from the vehicle and pronounced dead at the scene.

On April 11, 2021, officers from the Ohio State Highway Patrol and Findlay Police Department initiated a high-speed pursuit of a vehicle driven by Trevor Clark, 22, following a domestic violence incident in North Baltimore, Ohio. The officers escalated the situation into a high-speed pursuit through Findlay, during which Clark turned off his headlights and reached speeds of 95 mph on surface roads. The pursuit was terminated after Clark gained distance and officers lost contact with the vehicle. Later that day, tire tracks leading into a retention pond near the 500 block of Marathon Blvd. were discovered. Divers located Clark's vehicle submerged 105 feet from shore at a depth of 23 feet. Clark was found inside the vehicle and pronounced dead at the scene.

On 04/11/2021, officers from the Brooklyn Center Police Department pulled over Daunte Wright for an alleged traffic violation related to expired registration tags. Officers then reportedly discovered that he had a warrant for his arrest. Police tried to arrest Wright, but he stepped back into his car, and officers struggled with him. On April 10, 2021, at approximately 7:00 p.m., deputies from the Jefferson Parish Sheriff's Office responded to a disturbance call in the 6500 block of Park Manor Drive in Metairie, Louisiana. A woman reported that her boyfriend, 30-year-old Joshua Alexander Mitchell, had sexually abused a child and threatened to kill her. She also informed deputies that Mitchell was armed.

Upon arrival, deputies located Mitchell at the apartment. Around 9:00 p.m., Mitchell exited the residence and pointed a firearm at a deputy, prompting the deputy to fire his weapon. Mitchell retreated into the apartment.

The Jefferson Parish Crisis Management Unit was activated and arrived at the scene. Mitchell emerged a second time, again pointing his firearm at deputies. A Crisis Management Unit deputy fired at Mitchell, forcing him to retreat into the apartment once more.

At approximately 10:30 p.m., the Crisis Management Unit breached the apartment door and found Mitchell deceased from multiple gunshot wounds. A gun was recovered near his body.

On 04/10/2021, at approximately 11:00 p.m., San Fernando Police Department officers responded to a 911 call reporting gunfire in the alley behind the 600 block of S. Brand Boulevard. Witnesses described the shooter as a heavyset male adult leaving the area in an older model minivan. At around 11:01 p.m., officers located a vehicle matching this description on Harps Street near Fourth Street, with Guillermo "Memo" Amezcua, 48, seated in the driver’s seat. Officers issued multiple commands over approximately five minutes for Amezcua to show his hands. Police alleged that Amezcua was uncooperative and observed manipulating an unknown object. To gain compliance, an officer deployed three less-lethal stun bags aimed at the rear window area of the vehicle, but these attempts were unsuccessful. At approximately 11:06 p.m., police reported observing a muzzle flash consistent with a gunshot from inside the vehicle. Officers opened fire, fatally striking Amezcua, who was pronounced dead at the scene.

The Los Angeles County Sheriff's Department investigated the officer-involved shooting. According to an autopsy report dated 04/14/2021, Amezcua sustained five gunshot wounds, with three being fatal. The report also documented tattoos on Amezcua, including "Memo" and "Jennifer Marie" on his neck and "Guillermo Amezcua Jr." on his left shoulder.

San Marcos Police Department officers responded to a report of a man walking in traffic on southbound I-35 around 12:19 a.m. Upon arrival, officers saw Rescue Eram walking along the retaining wall and crossing traffic. Officers attempted to arrest him and reportedly observed him holding a knife or similar object. On April 9, 2021, at approximately 6:57 a.m., Georgetown Police Department officers responded to reports of a man attempting to break into cars in a shopping center parking lot near Connector Road in Georgetown, Kentucky. Upon arrival, officers encountered 31-year-old Deshund Andrea Tanner, who fled on foot and attempted two carjackings during his escape. When officers confronted Tanner during the second carjacking attempt, he produced a firearm. Officers opened fire, killing him. The officers involved—Sgt. San Bernardino deputies escalated a chase in Apple Valley. The driver did not stop and drove eastbound on Yucca Loma Road. Police continued to chase him into a residential area near Iroquois Road. Around 3:50 p.m., while still being chased, the vehicle crashed into a tree and a block wall. The driver was pronounced dead at the crash site. A female passenger sustained injuries and was airlifted to a trauma center. The pursuit was initiated after deputies attempted a traffic stop for a vehicle code violation.

Two men, a woman, and a child were parked at the entrance of an apartment complex when officers stopped to investigate. While the driver was outside the car speaking with officers, the other man in the car drove off, reportedly hitting an officer. Police later found the car at the Valencia Square Apartments. Someone from an apartment reportedly informed officers that a man inside had hostages. As police waited outside, Raheem Reeder reportedly jumped from the second story and waved a gun at officers, who shot and killed him.

On April 7, 2021, around 6:45 p.m., Philadelphia Police officers conducted a traffic stop on a blue Kia Optima in the 1500 block of West Somerville Avenue after it failed to stop at a stop sign. The vehicle had four occupants. Upon running their information, officers found that two individuals, including 24-year-old James "Poppie" Alexander, had outstanding warrants. As officers attempted to remove Alexander from the vehicle, he allegedly drew a firearm and fired at them, striking Officer Matthew Ponente in the foot. Alexander then exited the vehicle and reportedly fired additional rounds at the officers. In response, Officers Christopher Burton, Charles McCairns, Michael Braun, David Tamamoto, and Matthew Ponente discharged their weapons, fatally wounding Alexander.

On April 7, 2021, officers with the New Orleans Police Department attempted to stop a white sedan for a traffic violation. At about 4 a.m. on April 7, 2021, a Dodge truck, driven by off-duty Officer Christopher Carson, was traveling southbound on U.S. 75 in a portion of the roadway under construction. The vehicle struck Noah Joseph Ford, who was in the main lanes of the highway. Ford was killed in the incident.

Alexandria PD escalated a chase of a vehicle that they felt was involved in nearby gunfire. The vehicle did not stop and drove northbound on Interstate 295. Police continued to chase the driver into Southeast Washington, D.C., where he exited the vehicle and jumped over a jersey barrier, falling approximately 23 feet onto Malcolm X Avenue. While still being chased, a U.S. Park Police officer deployed a stun gun, and Alexandria Police officers took him into custody. He was pronounced dead at the crash site.

On April 6, 2021, Oneonta Police Department Sergeant Ralph Pajerski and an unidentified partner responded to a domestic incident at 48 River Street in Oneonta, New York. Upon arrival, they encountered Tyler R. Green, 23, who was allegedly threatening his girlfriend with a knife while holding their two-year-old son. On April 5, 2021, Louisiana State Police Troop C engaged a pursuit of a 2007 Toyota Highlander reported stolen in Houma. The driver, later identified as 19-year-old Frederick Howard of Gretna, fled eastbound in the wrong direction in the westbound lanes of U.S. Highway 90, crossing the Des Allemands Bridge near Raceland. Troopers discovered the vehicle crashed and empty at the top of the bridge. On 04/04/2021, Michigan State Police troopers were conducting a welfare check at around 8 p.m. when they discovered a 62-year-old woman dead from an apparent homicide in her home in Lake Township. The house was empty except for the victim at the time of discovery. Through investigation, troopers identified a man, Adam Briggs, a 30-year-old relative of the victim. A broadcast alert was issued for Briggs. Shortly after 1 a.m., the Cadillac Police Department spotted the vehicle associated with Briggs and attempted a traffic stop. Briggs did not stop. Police escalated a with multiple agencies. During the chase, Briggs allegedly fired shots at the pursuing officers. No one was hit. Missaukee County Sheriff's deputies used stop sticks to disable Briggs' vehicle. Briggs then ran away on foot. His body was found shortly after 6 a.m. with a self-inflicted gunshot wound, confirming his death as a suicide.
