On 10/13/2021, deputies from multiple Idaho sheriff’s offices encountered Buddy Byron McKenzie in the McCammon area while attempting to locate him in connection with a prior shooting reported earlier in the month. A deputy observed McKenzie inside a pickup truck and attempted to make contact. McKenzie continued driving and police chased the vehicle through the area. During the pursuit, deputies reported that gunfire occurred involving McKenzie. The pursuit later ended near the interchange of U.S. Highway 30 and Interstate 15. Deputies opened fire and shot McKenzie, killing him at the scene. The incident was referred for external investigation.

On 10/13/2021, Los Angeles Police Department officers responded to a report of an assault with a deadly weapon at a residence on Keystone Avenue in the Palms area of Los Angeles shortly before 6 a.m. Officers encountered Carlos Arias inside the residence during a family disturbance. Police reported that family members were present and that Arias was holding a knife during the encounter. An officer opened fire, striking Arias. He was pronounced dead at the scene. The incident was reviewed by investigators, and the case was pending review at the time of reporting.

On October 12, 2021, at approximately 11:40 a.m., members of the Knoxville Police Department's Special Operations Squad were assisting agents from the Tennessee Bureau of Investigation's Drug Investigation Division and the 5th Judicial Drug Task Force in executing a search warrant at a residence in the 2900 block of Sevier Avenue. Upon arrival, officers encountered 43-year-old Bryan Calvin with a gun. Preliminary reports indicate that Lee pointed the firearm at officers. They engaged him in a shootout and killed him. Lee was struck and pronounced dead at the scene.

On October 12, 2021, at approximately 9:33 a.m., Las Vegas Metropolitan Police Department officers responded to an apartment on E Lake Mead Blvd for an assault and battery call. The victim's current boyfriend reported that her ex-boyfriend, Mario Lawrence Martinez, had been harassing them, fired shots, and forcibly entered the apartment to kidnap the woman at gunpoint. Martinez attempted to escape in her car, which wouldn't start, leading him to flee with her on foot. They were located near a shopping center at about 10:50 a.m. Martinez, still armed, did not comply with police orders to surrender, moving through the shopping center, and eventually breaking into a box truck in an alley, where he also shot out its window. During this standoff, Officer Ramon Hiestand and other officers engaged Martinez. After Martinez pointed his gun at officers, the woman escaped, and Hiestand shot him. Martinez continued to fire while on the ground. He fired until he shot and killed Martinez.

On 10/12/2021, deputies from the Anderson County Sheriff’s Office attempted to stop a vehicle on Interstate 85 near South Carolina Highway 8 in Anderson County after believing the vehicle was stolen. The driver, Jonathan Combs, continued driving and police escalated a chase. Deputies later stopped the vehicle. According to authorities, Combs was armed during the encounter. At least one deputy opened fire, striking Combs. He was pronounced dead at the scene. The South Carolina Law Enforcement Division was requested to investigate the shooting.

On October 11, 2021, Baltimore County Police officers responded to an incident at the intersection of Englewood Avenue and Gilmore Street in Woodlawn, Maryland. During the encounter, officers opened fire, killing 36-year-old Jovan Lewis Singleton.

Officers pursued a man reportedly wanted for several felony offenses through multiple counties before he fatally shot himself in the head. Police said the man in his 20s was found in his vehicle with a self-inflicted gunshot wound at the end of the chase around 10 p.m.

On 10/11/2021, Richmond County sheriff’s deputies stopped a vehicle in Augusta, Georgia, citing a license plate cover violation. Jermaine Jones Jr. was riding in the rear passenger seat along with family members. Deputies requested consent to search the vehicle, and another deputy arrived with a K9 that police said alerted to narcotics. Deputies reported finding a handgun and a crack pipe under the rear seat where Jones had been sitting. Jones exited the vehicle and ran from deputies. An officer deployed a Taser during the arrest. Jones was taken into custody and later experienced a medical emergency. He was hospitalized and died approximately one week later. The Richmond County District Attorney’s Office later cleared the involved officers. Jones’s family filed a civil lawsuit related to the incident.

On October 10, 2021, at approximately 9 a.m., Indiana State Police responded to a reported domestic disturbance at a residence on East County Road 900 North in Sunman, Indiana. The disturbance involved Joshua "Josh" Edward Ebinger, 37, who was reportedly violating a protection order issued against him by a woman.
Two Ripley County deputies, an Indiana State Police trooper, and a Versailles Police officer arrived at the scene. The woman exited the residence and was moved to a safe location.

On October 10, 2021, at approximately 6 p.m., Brunswick County Sheriff's Office deputies responded to a domestic disturbance at 8627 Wanets Landing Road in Leland, North Carolina. Linda Beck, the caller, reported that her husband, 57-year-old Michael Robert Beck, had fired a firearm in her direction twice and expressed concerns about his volatility. Upon arrival, deputies located Beck on the back deck of the residence. He reportedly produced a firearm and pointed it toward the deputies. Despite repeated commands to drop the weapon, Beck allegedly failed to comply. Two deputies discharged their firearms, fatally striking him.

On 10/10/2021, Harris County sheriff’s deputies encountered Kenneth Anderson at the scene of a vehicle crash in Houston, Texas. Deputies believed Anderson was experiencing a drug related medical crisis and placed him into custody. While detained, deputies reported that Anderson became uncooperative and deployed a Taser multiple times. Deputies restrained him and placed him in a patrol vehicle. Emergency medical personnel responded and found Anderson unresponsive but did not transport him to a hospital. Deputies then drove Anderson to the county booking center, where he remained unresponsive. He was transported to a hospital and later died. The incident prompted internal and civil review. A federal civil rights lawsuit was later dismissed after a court granted qualified immunity to the involved deputy.

Deputies reported seeing Perry Lamar Stringfellow Jr. loading an air conditioning unit into a vehicle. Deputies ordered him to stop. Police stated that Stringfellow instead drove the vehicle toward deputies. Deputies opened fire, striking Stringfellow. He was pronounced dead at the scene. REASON WHY PURSUIT WAS INITIATED:
At approximately 2:45 a.m., Maryland Transportation Authority Police Officer Theodore Jeremenko was parked at I-395 and Conway Street when he saw Jawuan Ginyard make a left turn against a red light onto I-395. Jeremenko followed and observed the Monte Carlo repeatedly accelerate and slow sharply, weave and drive onto the shoulder. Believing the driver might be impaired, Jeremenko activated his emergency equipment and attempted a traffic stop.

OPEN RECORDS SUMMARY:
On 10/09/2021, Ginyard initially slowed on I-95, activated his hazard lights and pulled onto the right shoulder. Jeremenko stopped behind him and got out of his patrol vehicle. As the officer approached on foot, Ginyard drove away, and Jeremenko returned to his cruiser and initiated the pursuit.

The pursuit continued south on I-95 and onto I-695. Jeremenko repeatedly radioed Ginyard's speed as it increased beyond 85 mph. The subsequent crash investigation recovered much more precise information from the Monte Carlo's event-data recorder: Ginyard was traveling 111 mph as he entered the I-695 exit ramp toward eastbound Wilkens Avenue.

Ginyard attempted to negotiate the right-hand curve on the ramp but lost control. The Monte Carlo struck the left guardrail, then a concrete curb and grassy median separating the lanes of Wilkens Avenue before crossing the westbound lanes and striking another curb. Ginyard was ejected and suffered catastrophic head and chest injuries. Jeremenko stopped, approached the wreck and rendered aid until emergency medical personnel arrived, but Ginyard died at the scene.

The Attorney General's investigation also established where Ginyard had been going immediately before the police encounter. A longtime friend told investigators that he and Ginyard had met to get food approximately an hour before the pursuit. After eating, they left in separate cars and were traveling to the friend's Baltimore residence. The friend was behind Ginyard at Conway Street, saw him turn against the red light and subsequently saw the police vehicle follow him onto I-395.

The friend later encountered the pursuit again while traveling toward I-695 and saw Ginyard's car with the police cruiser behind it and its emergency lights activated. He telephoned Ginyard; Ginyard answered but did not speak. The friend then heard the fatal crash occur through the telephone and arrived at the crash scene shortly afterward.

Ginyard was a 26-year-old Baltimore resident. The detailed state investigation provides unusually strong information about his final hour—he had been out getting food with a longtime friend and was following that friend toward his home when the police encounter began—but the publicly released investigative record contains little additional biographical information about his family, employment or personal life.

DISPOSITION:
The Maryland Attorney General's Independent Investigations Division conducted an independent investigation that included Jeremenko's dashboard-camera footage, police radio transmissions, witness interviews, crash reconstruction, the Monte Carlo's event-data recorder, medical-examiner findings and MDTA pursuit policies. The IID completed its investigation and forwarded its report to the Baltimore County State's Attorney's Office on 03/28/2022.

The Baltimore County State's Attorney's Office declined criminal prosecution on 04/06/2022. Prosecutors concluded there was no basis for criminal charges against Jeremenko for his role in initiating or continuing the pursuit.

Officers were responding to a DWI call a block away when they heard gunshots in the entertainment district at around 1 a.m. The officers arrived to find two groups of men shooting at one another. An officer began firing at the men. Michael "Mikey" Carothers was killed. It was not immediately known whether the teen was killed by officer-fired bullets or from the other group involved in the shooting.

At approximately 4:30 a.m. on October 9, 2021, a Middlesboro police officer shot and killed 35-year-old Timothy Houston. Houston was reportedly wanted in Claiborne County, Tennessee, on charges stemming from a previous shooting. The incident occurred in Middlesboro, Kentucky. During the stop, the driver, identified as 35-year-old Gustavo Angel Esparza, opened fire on the officers before fleeing the scene. On 10/09/2021, shortly before 7:30 p.m., 40-year-old Angel L. Acevedo Jr. and his wife, 35-year-old Daniela Correia Salles, were traveling in their 2020 Toyota Corolla eastbound on Asbury Avenue beneath the Garden State Parkway in Tinton Falls when an off-duty Hillside Police officer's pickup truck suddenly left the elevated Parkway and crashed into their car. Both Angel and Daniela suffered catastrophic injuries and died at the scene.

The officer, John P. McClave III, was driving a 2018 GMC Canyon northbound on the Garden State Parkway while on his way to work at the Hillside Police Department. Investigators determined that McClave's pickup left the travel lane, crossed the shoulder and continued for more than 500 feet across a grassy embankment without significantly slowing or changing direction. The pickup hit an embankment, became airborne above Asbury Avenue and came down onto the couple's Corolla before continuing across the road and striking a tree.

McClave was transported to Jersey Shore University Medical Center with serious but non-life-threatening injuries. Investigators subsequently determined that he had been driving recklessly while impaired. His blood tested positive for THC, toxicology evidence concluded he was impaired, and investigators found an open container of alcohol in his vehicle. He was also cited for reckless driving, failure to maintain his lane, failure to wear a seat belt and having an open alcohol container.

Angel was an accomplished engineer who grew up in Atlantic Highlands, graduated from Dartmouth College with an engineering degree and earned a master's degree from the Naval Postgraduate School. He began working as an engineer for the U.S. Army in 2006 and rose to become Chief of Mission Network Communications at Aberdeen Proving Ground. Colleagues remembered him for his reasoned judgment and infectious laugh.

Angel and Daniela were described by their family as soulmates. They loved traveling, hiking, skiing, photography and gathering friends and relatives at their home, and were remembered for their kindness, humor and positivity. Angel left behind his mother, sister, niece, three younger brothers and a large extended family.

DISPOSITION:
McClave was charged in March 2022 with two counts of second-degree vehicular homicide. Hillside Police suspended him without pay following the charges. The criminal case was later transferred from Monmouth County to Ocean County because of a potential conflict of interest.

Following a weeklong trial, an Ocean County jury convicted McClave in March 2026 of two counts of second-degree vehicular homicide. On 05/08/2026, Superior Court Judge David M. Fritch sentenced McClave to two consecutive 8½-year prison terms—one for killing Angel and one for killing Daniela—for a total sentence of 17 years. He must serve 85 percent of the sentence before becoming eligible for parole.

They explored hiking trails together, traveled internationally, skied, enjoyed photography and regularly brought people together at their home. Friends and relatives remembered them for their kindness, humor and positive outlook. Daniela left behind her parents, two sisters, a niece and extended family in Brazil.

On 10/09/2021, Lompoc Police Department officers responded to reports of an assault with a deadly weapon in the 700 block of West Cypress Avenue in Lompoc, California. Officers located Travis Daniel Carlon inside a residence and established a perimeter after he remained inside for an extended period. Police reported that Carlon later came to the doorway holding a firearm. Officers opened fire and fatally shot him at the scene. The Santa Barbara County District Attorney’s Office later reviewed the incident and ruled the shooting justified.

On October 8, 2021, Officer Dustin Broughton of the Mexico Police Department responded to an unknown incident at 1167 Main Street in Dixfield, Maine, involving 29-year-old Matthew Allen Marston. During the unspecified confrontation, Officer Broughton opened fire and killed Marston. The State Medical Examiner determined that Marston's death resulted from a self-inflicted gunshot wound. On 10/08/2021, around midnight, Tulsa Police officers saw Tommy Carmichael, 46, driving a white truck in West Tulsa. Carmichael was a person of interest in the death of his mother, Carol Longacre, who had been found dead in her bathtub earlier that day. Officers followed Carmichael as he drove toward the home where Longacre’s body was discovered.
Carmichael arrived at the house near South Utica Avenue and East 61st Street and tried to enter the property. Before officers could apprehend him, Carmichael shot himself and was pronounced dead at the home.

On 10/08/2021, the Pennsylvania State Police and Coaldale Police Department were involved in a standoff with 47-year-old Richard Lee Sweet in Coaldale, Pennsylvania. Sweet was wanted in connection with the fatal shooting of his girlfriend, Joan Cressman, earlier that day in East Allentown. After the shooting, Sweet fled to his home on the 100 block of Fisher Avenue in Coaldale. Police escalated a large standoff that lasted over four hours. They used loudspeakers while Sweet locked himself inside. Around 4 p.m., neighbors heard a single gunshot from inside the residence. When police entered, Sweet was found with a self-inflicted gunshot wound and was pronounced dead.

On October 8, 2021, at approximately 8:51 p.m., Winston-Salem Police officers responded to a 911 call reporting that 46-year-old Gilbert Lee Collison Jr. had broken into a residence on South Main Street. The caller reported that Collison was armed with a shotgun and had threatened to shoot any law enforcement officers who attempted to intervene. Officers learned that Collison had previously pointed the shotgun at his girlfriend during an argument.

The Winston-Salem Police Department, along with SWAT and crisis negotiators, surrounded the residence and made repeated attempts to communicate with Collison. Despite these efforts, Collison fired shots at the officers. At approximately 2:30 a.m., Collison appeared in the front doorway with the shotgun. Officers used a less-lethal impact baton, striking him in the leg, but Collison responded by firing on the officers. Three officers returned fire, fatally striking Collison.

The North Carolina State Bureau of Investigation is conducting an independent investigation into the incident. The three officers involved have been placed on administrative leave, as is standard protocol for officer-involved shootings.

On October 8, 2021, deputies from the Chippewa County Sheriff's Office conducted a traffic stop on Lance Richard Stelzer, who was riding a moped in Goetz, Wisconsin. During the stop, a physical altercation broke out between Stelzer and Deputy Martin Folczyk. Stelzer allegedly sprayed Folczyk with wasp/hornet killer during the struggle. In response, Deputy Folczyk shot and killed Stelzer.

On 10/08/2021, police responded to a report of a man acting strange and standing on a vehicle in the 900 block of Harlem Street in Altoona, Wisconsin. Officers from the Altoona Police Department, the Eau Claire County Sheriff’s Office, and the Eau Claire Police Department encountered Demetrio Jackson, who was found to be resisting arrest and running from police. Officers deployed a Taser multiple times and restrained Jackson in a prone position. Jackson complained that he could not breathe during the restraint. Emergency medical personnel were summoned, provided care, and injected Jackson with a sedative before transporting him to a hospital. Jackson remained minimally responsive and died at a hospital approximately two weeks later.

On 10/08/2021, law enforcement officers from the Evans County Sheriff’s Office and the Hagan Police Department were operating a license checkpoint in Evans County, Georgia, when Ramone Javaris Dwight drove through the area and continued driving instead of stopping. Police chased the vehicle through the area at high speeds. During the chase, police reported gunfire involving the vehicle. The pursuit ended near Ponderosa Road and Firetower Road, where Dwight exited the vehicle and additional gunfire occurred between Dwight and officers. An Evans County deputy and a Hagan police officer opened fire, striking Dwight. He was pronounced dead at the scene. The Georgia Bureau of Investigation took over the case for independent review.

On 10/08/2021, deputies from the Lancaster County Sheriff’s Office were conducting a welfare check at an apartment on 3000 Fast Lane in Indian Land, South Carolina, after family members reported Derrick Clinton missing. Officers obtained a search warrant and entered the residence to locate Clinton. Deputies said Clinton was inside the apartment and fired a weapon at officers during the encounter. A Lancaster County deputy returned fire, striking Clinton. Clinton was pronounced dead at the scene. A sheriff’s deputy was also shot and taken to a hospital with injuries that were reported as serious but not expected to be fatal. The South Carolina Law Enforcement Division is investigating the shooting.

On 10/08/2021, personnel from the Alaska State Troopers and the Fairbanks Police Department, including members of Northern and Southcentral Special Emergency Reaction Teams, arrived at a residence on Richardson Highway near Salcha, Alaska, to serve multiple outstanding warrants for Scott W. Hottinger. As officers approached, Hottinger exited the home armed with a firearm and began shouting. Troopers and local officers identified themselves as law enforcement. Hottinger then discharged a weapon toward officers. Two law enforcement officers returned fire, striking Hottinger. Hottinger was pronounced dead at the scene. Deputies attempted to communicate with Isaac Soria by phone and text message, but he refused to surrender and made threats toward law enforcement. Upon arrival, three officers encountered 71-year-old Lloyd Grant McClung Jr., who was armed with a shotgun. Despite repeated verbal commands to drop the weapon, McClung advanced toward the officers and fired a round from his shotgun. Two officers opened fire, striking McClung. McClung was transported to a local hospital, where he was pronounced dead.

Upon arrival, gunshots were exchanged, and a Brockton police officer was reportedly struck several times. The gunman, Kevin M. Serpa, then retreated into the home, where he locked himself for several hours while police attempted negotiations. At approximately 9: 30 p.m., Serpa was outside the home when he shot and killed himself.

On October 6, 2021, at approximately 6:00 a.m., correctional officers with the Hardeman County Sheriff's Office were outside the Hardeman County Criminal Justice Complex when they were allegedly threatened by an armed individual. Three officers from the Bolivar Police Department responded, but the man had already fled. Shortly thereafter, the individual returned, exited his vehicle while still armed, and confronted the officers. The officers shot and killed 43-year-old Andra Davon Murphy.

Additional Information: The family of Andra Davon Murphy has retained attorney Ben Crump, asserting that Murphy was not the individual who initially threatened the correctional officers and is seeking the release of video footage to clarify the events leading to his death.

On October 6, 2021, U.S. Marshals attempted to serve an arrest warrant on Cody James Herman, who was wanted for charges including aggravated assault and weapons violations. As officers approached the home, they were fired upon, resulting in one agent being shot and wounded. Officers fell back, and negotiations began. Several children and adults exited the residence, and after continued negotiations, police entered the home and found Herman dead from a self-inflicted gunshot wound.
