On April 4, 2021, Michael Ray Wagner, a 47-year-old white male from Geneva, Indiana, was killed during a police chase in Shelbyville, Kentucky. After reports that Wagner had stopped his vehicle on the roadway and pointed a gun at another motorist, officers located him. He drove eastbound on Frankfort Road (US-60), and police escalated a high-speed chase. Approximately two to three miles into the pursuit, Wagner lost control of his 1999 Pontiac Firebird near the intersection of US-60 and Ski Area Road. He crashed, was ejected, and was pronounced deceased at the scene by the Shelby County Coroner.

Gabriel Casso, 21, was running toward New York City Police officers while being pursued by two unidentified men near East 166th Street and Findlay Avenue in the Bronx on April 3, 2021. Police opened fire during the encounter, resulting in Casso being fatally shot. It remains unclear whether the bullets that killed Casso or injured a nearby bystander were fired by police or the men they engaged. The bystander sustained injuries and was transported to a local hospital for treatment.

On April 3, 2021, just after 6:00 a.m., officers with the Vallejo Police Department responded to a call of a man slumped behind the wheel of an idling car. When officers approached, the 32-year-old drove away in a stolen vehicle, reached speeds around 85 mph in a 40 mph zone, and covered just over one mile before losing control and crashing into a tree. First responders pronounced him dead at the scene. The vehicle was stolen from Walnut Creek. The Solano County Major Crimes Task Force and the California Highway Patrol’s Multidisciplinary Accident Investigation Team are leading investigations.

Around 3:30 a.m., Corvallis Police responded to a call from the Days Inn motel staff reporting a man acting aggressively with a knife. Officers encountered Jeffrey W. Appelt, 32, who reportedly had a knife. An officer fired a Taser, but Appelt retreated toward the motel entrance. A second officer attempted to take him into custody, but Appelt evaded the officer. As a third officer arrived, Appelt continued approaching while holding the knife. Officer Daniel Granillo then shot and killed Appelt.

On 04/03/2021, a standoff occurred at a residence located at 601 6th Street in Carlstadt, New Jersey, resulting in the lockdown of two city blocks. Police entered the home around 8 p.m. and discovered a deceased female victim inside. James T. Cox, 43, was also found dead of a single, self-inflicted gunshot wound.

On April 2, 2021, at approximately 5:15 a.m., Long Beach Police Department officers responded to a 911 call reporting a man who appeared to have had a heart attack and a woman threatening suicide at a residence in the 2800 block of Golden Avenue. Upon arrival, officers observed a large amount of blood inside the residence and were immediately confronted by 38-year-old Lauren Nicole Archibeque, who was armed with a rifle. Officers quickly exited the residence, set up a perimeter, and attempted to de-escalate the situation by making repeated attempts to communicate with Archibeque, urging her to surrender. She did not respond.

After some time, Archibeque emerged at the doorway of the residence, and officers observed her pointing the rifle at them. At that point, three officers discharged their firearms, fatally wounding her. She was pronounced dead at the scene. Upon re-entering the residence, officers discovered a 49-year-old man on a bedroom floor with a serious laceration to his neck. He was also pronounced dead at the scene. The Los Angeles County Coroner’s Office later determined that the man's death was a suicide.

The Long Beach Police Department is conducting a full and thorough multi-level review of the incident, and the Los Angeles County District Attorney’s Office is conducting an independent investigation, as is standard protocol for officer-involved shootings resulting in injury or death. On April 2, 2021, deputies with the Hamilton County Sheriff’s Office pursued a blue Chevrolet driven by 38-year-old Daniel Peak at speeds up to 90 miles per hour in a 55-mile-per-hour zone. The chase continued onto Highway 60 near the Blythe Ferry boat ramp, where the vehicle left the roadway and plunged into the Tennessee River. The passenger, 36-year-old Dewleen Self of McCaysville, Georgia, drowned after the vehicle overturned in the water. The driver was taken into custody.

On April 2, 2021, deputies from the Coweta County Sheriff’s Office attempted to stop a vehicle traveling approximately 70 mph in a 45 mph zone. The driver, Jamaal Willis-Powell of Covington, refused to comply and the pursuit covered about 1.7 miles before the vehicle crossed into Fulton County and struck a tree near Fayetteville Road and Blossom Street. Willis-Powell was killed instantly. His passenger, 28-year-old Jasmine Simone Hayes of Palmetto, also died in the crash. Deputies released body-camera footage as part of an investigation into the high-speed chase.

On April 2, 2021, around 10:13 p.m., a deputy from the Person County Sheriff's Office responded to an incomplete 911 call at a residence in the 1300 block of Clay Long Road in Hurdle Mills, North Carolina. Upon arrival, the deputy encountered Jackie Cameron Capps, 58, holding three people hostage at gunpoint.
The deputy called for backup and ordered Capps to drop his weapon. Capps refused and allegedly pointed the firearm at the hostages. The deputy discharged their weapon, fatally shooting Capps. The Person County Sheriff's Office is investigating the circumstances of the shooting.

Around 6:30 p.m., Salem Police officers responded to a report of a suicidal man who had injured himself. Firefighters arrived first, and Natzeryt Layahoshua Viertel allegedly threatened them with a firearm. When police arrived, Viertel was still seen with the gun. An officer opened fire, killing him. A Marion County grand jury later found that the police shooting was a lawful and justified use of deadly force.

On April 1, 2021, a high-speed pursuit involving the California Highway Patrol and Los Angeles Police Department occurred on the 405 and 118 freeways in Sherman Oaks. The driver of a Honda, fleeing police at speeds reaching 117 miles per hour, entered the wrong lanes of traffic and crashed head-on into a Subaru. The Chicago Police Department attempted a traffic stop on Alvarez on Laramie Avenue at about 12:18 a.m.; when Alvarez refused to stop, officers escalated a foot pursuit. Alvarez fled through a parking lot and alley, holding a gun in his right hand and a cellphone in his left, then stumbled in the front yard of a residence near 5200 W. Eddy Street. Officer Solano confronted him, ordered him to drop the gun and fired five shots, shooting Alvarez in his back and thigh. Alvarez died at the hospital. Alvarez final words are "Why are you shooting me?" to which Solano replied "You had a gun." The Cook County State’s Attorney’s Office reviewed the case and found insufficient evidence to charge the officers.

At approximately 6:45 p.m., a 2003 Mack truck tractor towing a pole trailer was traveling east on Highway 94. A 2017 DPS Chevrolet patrol vehicle was traveling westbound. The DPS Trooper, Trooper Brady Germeroth, identified a traffic violation on another eastbound vehicle and made a U-turn. As the patrol vehicle re-entered the eastbound lane, the Mack truck driver swerved into the westbound lane, striking a 2019 Jeep Wrangler head-on. The truck continued to swerve and struck the back right side of the DPS vehicle, which then overturned and caught fire. The driver of the truck and the trooper were not injured. Melanie Painter, the driver of the Jeep, was killed in the crash.

On 03/31/2021, police responded to a home at 247 Sullivan Street in Claremont, New Hampshire, following reports of gunshots being fired. Jeffrey Ely had barricaded himself inside the residence. After an extended standoff, gunfire was exchanged between Ely and New Hampshire State Police. Troopers Nicholas Cyr, Stefan Czyzowski, Gary Ingham, Shane Larkin, William Neilsen, and Noah Sanctuary fired at Ely, who was subsequently killed. Officer Noah Sanctuary had been involved in a previous shooting incident in February of 2022. Sergeant Stefan Czyzowski and again Sergeant Noah Sanctuary were among six troopers who fired their weapons during a standoff in Plymouth, where Kevin Steinfeldt, died from a self-inflicted gunshot wound under police duress.

Shortly after 10 p.m. on March 31, 2021, gunshots were reported to the police. Approximately 25 minutes later, officers located a pickup truck and attempted a traffic stop. The driver did not stop, leading officers on a pursuit that ended in a dead-end parking lot near Southeast 122nd Avenue and Mill Street. Police initiated a standoff, during which a shot was heard from within the vehicle. Officers deployed gas into the truck, after which the man died by suicide.

On March 30, 2021, Helen Marie Quarles was driving home from her job at the state probation office. As she attempted to make a left-hand turn onto eastbound Route 3, her vehicle was struck by a Culpeper County Sheriff's Office-issued Chevy Tahoe SUV driven by Lt. Ashleigh Baughan. The officer, who was allegedly driving 75 mph in a 55 mph zone, was not responding to an emergency or pursuing another vehicle, and her emergency lights and sirens were not activated at the time of the crash.

On March 30, 2021, at approximately 11:00 a.m., officers from the Bossier City Police Department responded to a report of an allegedly stolen vehicle near a business in the 500 block of Barksdale Blvd, Bossier City, LA. Upon arrival, officers encountered 30-year-old Billy Ray Hill, who reportedly fled on foot but was apprehended by police after a brief chase. Details of the arrest are unknown. After the arrest, Hill became unconscious and died shortly thereafter.

The Louisiana State Police conducted an investigation into the incident. The Bossier Parish Coroner's Office performed an autopsy, which indicated no external or internal injuries or diseases. The coroner suggested that Hill may have suffered a life-threatening cardiac arrhythmia; however, this diagnosis could not be confirmed, and the final cause of death was listed as undetermined.

A Clay County deputy attempted to stop Gage Allen Salsman, who was on a motorcycle, in Henrietta. During the stop, Salsman reportedly threw something from his pockets and began speeding away, yelling for the officers to stop chasing him. Wichita Falls police and the sheriff's office set up ahead to wait for the chase. Salsman eventually collided with an SUV and was killed on impact.

A domestic disturbance occurred at the Quality Inn, starting around 6:30 a.m. A woman called officers there several more times, with the last call shortly after 12:30 p.m. Michael Leon Hughes had forced his way back into the room after having left earlier. When two officers arrived, Hughes refused to leave. As officers attempted to take Hughes into custody, a struggle ensued lasting more than 60 seconds with one of the officers. Hughes reportedly gained control of Officer J. H. Wing's Taser, stunned him once, and was then shot and killed by the officer.

On March 29, 2021, at approximately 9:11 a.m., officers responded to a domestic incident at a home involving Pierce L. Flinchum. When approached by police, Flinchum ran into the house and barricaded himself inside. Police obtained an arrest warrant for him on charges of assault and battery, and possession of a firearm as a felon. Throughout the day, police made multiple attempts to contact him by telephone. At approximately 10:30 p.m., officers entered the home and discovered Flinchum dead from a self-inflicted gunshot wound.

Nathaly Medina, 19, was a passenger in a car that Tulsa police attempted to stop for reckless driving. The driver, Jose Lara-Garcia, did not stop, leading officers to initiate a pursuit. The chase continued until the vehicle crashed into several poles before entering the Arkansas River. Medina died in the crash. The incident is under investigation by the Tulsa Police Department.

On 03/29/2021, at approximately 3:50 a.m., LAPD officers attempted to stop Matthew Blaylock near 7th and Bixel Streets. Blaylock fled on foot, and officers escalated a chase that ended near 8th and Bixel Streets, where Blaylock allegedly produced a handgun. Officers opened fire, killing him. A handgun was recovered at the scene. Body camera footage of the incident has been released and is under review. The LAPD's Force Investigation Division is investigating the shooting, and the Los Angeles County District Attorney's Office will review the case.

On March 29, 2021, Chicago Police Officer Eric Stillman shot and killed 13-year-old Adam Toledo in an alley near the 2300 block of South Sawyer Avenue in the Little Village neighborhood. Officers had responded to reports of gunfire when Toledo fled on foot. Body camera footage later showed Toledo discarding a handgun behind a fence and turning toward the officer with his hands raised. Stillman fired a single shot that struck him in the chest. Adam was a seventh grader who loved video games and was close to his family. Officers responded to a call about a shooting with injuries at Rich's Billiards. Witnesses provided a description of the gunman, who was last seen running toward an apartment complex. Officers located Robert Lee Canchola, and he attempted to flee. Canchola allegedly pulled out a handgun, and in response, officers Charles Tanner, Armando Cisneros, Remington Strickland, and Gerry Montelongo shot and killed him.

On March 29, 2021, at approximately 4:30 p.m., Deputy Justin Conley of the Fleming County Sheriff's Office observed a vehicle on Kentucky Route 11 near Maysville, Kentucky, with a male driver and a female passenger who appeared to be involved in a physical altercation. Deputy Conley attempted to initiate a traffic stop, but the driver, 48-year-old Terry Darnell Jackson, exited the vehicle and fled on foot into a wooded area. During a physical struggle, Deputy Conley opened fire, killing Jackson. Jackson was airlifted to the University of Cincinnati Hospital, where he was pronounced deceased. Deputy Conley was treated for minor injuries at Meadowview Regional Medical Center and was subsequently placed on administrative leave, per department policy.

On March 29, 2021, troopers from the Wyoming Highway Patrol attempted to stop a vehicle that was already being pursued. Despite the attempt to stop the vehicle, the driver, identified as 24-year-old Josiah Griffith, fled from the trooper. The vehicle entered Blairtown Connection Road before coming to a stop on 5th Avenue West. The vehicle then moved forward again, went down an embankment, and came to a stop. When officers approached the vehicle, they discovered Griffith dead from a self-inflicted gunshot wound.

On March 29, 2021, Springfield Police Department officers responded to a reported shooting at the Bass Pro call center on East Kearney Street. Upon arrival, officers discovered 33-year-old Troy Rapp deceased from gunshot wounds. Witnesses identified 28-year-old Joshua A. Anderson as the shooter. Anderson fled the scene, leading to a police pursuit that ended when Anderson crashed his vehicle near Glenstone Avenue and Kearney Street. Officers found Anderson with an apparent self-inflicted gunshot wound. He was transported to a local hospital, where he died from his injuries on April 1, 2021.

On March 28, 2021, deputies with the Greensville County Sheriff's Office responded to a disturbance call at the Quality Motel on U.S. Route 301. The caller complained of a white Ford Crown Victoria playing loud music in the parking lot. When a deputy arrived, the vehicle, driven by Daiquan Tarez Wright, was seen leaving the motel at a high rate of speed. The deputy attempted to stop the vehicle, but Wright fled down Liberty Road, reaching speeds of 60 mph. Wright's vehicle ran through standing water, lost control, struck a mailbox and fire hydrant, and then crashed into a tree, resulting in his death.

On March 28, 2021, in Norman, Oklahoma, police responded to a standoff involving a man firing weapons on his property near 58th Avenue Northeast and East Robinson Street. The chase continued north on Interstate 95 and into Fairfax County, where the fleeing driver, 24-year-old Curtis Armstead Jr., lost control of the car on Interstate 495 near the Route 123 interchange. The vehicle struck a barrier and burst into flames. Armstead died at the scene.
