Late on June 24, 2025, Donte Melton Jr., 31, was in a mental health crisis when he approached a Baltimore Police officer in a marked cruiser at a red light on West Franklin Street near North Franklintown Road. He walked into traffic and allegedly resisted officers. Police engaged him in what the preliminary report calls a “violent scuffle” before they placed him in handcuffs and leg shackles. Officers requested medical assistance repeatedly—first at approximately 9:48 p.m. and again at 9:55 p.m.—but Baltimore’s CAD system was down, and no medic arrived. After Melton became unresponsive on the pavement, officers placed him in their vehicle and drove him to a hospital. It is unknown how long he was down restrained in cuffs and leg shackles on the pavement before he finally arrived at the hospital. He died about 5½ hours after the initial encounter, before midnight or in the early morning hours of June 25. Melton remained unresponsive on the pavement before being transported. While the exact duration has not been disclosed, his mother describes him lying helplessly on hot asphalt while officers discussed how to get him help. No EMS ever arrived during that time. His mother, Eleshiea Goode, has pressed for transparency and accountability, citing delayed or denied access to records including those from the medical examiner and hospital, and frustration over the lack of communication about body-worn camera footage or the circumstances of death. The Maryland Attorney General’s Independent Investigations Division named 10 officers involved and has delayed releasing the body-worn video due to the technical complexity and volume of footage involved. In January of 2026, all officers were cleared.

Around 5:48 a.m., officers responded to a report of an armed individual at a gas station on Winchester Road. Police say Patrick Shoptaw had already injured a civilian before retreating inside. When confronted by an officer, Shoptaw allegedly struck the officer with a screwdriver. The officer opened fire and killed him. A civilian injured before police arrived was treated and released. The Tennessee Bureau of Investigation is conducting an independent review.

Police responded to a welfare check and found Jason Arnold had left the area. A Millsboro officer attempted a traffic stop, but Arnold continued driving. After stopping at a residence on Blackwood Drive, Arnold exited the vehicle with a firearm and fired at police. A trooper and an officer returned fire, injuring him. Arnold barricaded himself inside the home. Hours later, police entered and found him dead from a self-inflicted gunshot wound. A 69-year-old woman nearby sustained minor injuries during the incident. The Delaware Department of Justice is investigating.

Just after 1:00 a.m., officers responded to gunfire reports at the Antero Apartments on Michelle Court. They encountered a man holding a gun. When ordered to drop the weapon, he allegedly fired at officers. Two officers opened fire and killed him. No officers or residents were injured. Nearby apartments were struck by bullets. The El Paso County Sheriff's Office is leading the investigation. Body camera footage is expected to be released within 21 days.

On June 23, 2025, deputies from the Pierce County Sheriff’s Office responded to reports of a person throwing items and attempting to set a building on fire at a Pacific Pride gas station on Pacific Avenue South. The person fled on foot. Deputies pursued him for three blocks and confronted him in a backyard near 112th Street South and C Street South. A deputy opened fire and killed the individual. The Pierce County Force Investigation Team is conducting the investigation.

On June 23, 2025, Utah County Sheriff’s Office SWAT officers served a search warrant at a home on East Duggan Drive in Eagle Mountain as part of an investigation into aggravated kidnapping and sexual assault. The man, accused of strangling and sexually assaulting a victim, reportedly pointed a handgun at officers during the operation. SWAT officers opened fire and killed him. The Utah County Officer-Involved Critical Incident Protocol Team is conducting the investigation.

On June 23, 2025, at approximately 5:14 p.m., Little Rock Police Department officers responded to the 600 block of Timber Hill Drive regarding a person observed holding a weapon on a porch. Upon arrival, officers encountered a male subject identified as 44-year-old Andrew Osborne. Officers attempted verbal engagement, issuing commands to drop the weapon. Officers opened fire, striking Osbourne. He was transported by MEMS to a local hospital, where he later died from his injuries.

Twiggs County deputies attempted to stop a vehicle occupied by Christopher Holmes and another individual after observing suspicious activity. Holmes refused to stop, prompting deputies to escalate to a high-speed pursuit onto Interstate 16. The pursuit reached speeds exceeding 80 mph before the vehicle crashed near the Sgoda Road exit.
As deputies approached the disabled vehicle, Holmes and his passenger exited and opened fire on the officers, discharging multiple rounds. Deputies returned fire, fatally striking Holmes. The passenger surrendered and was taken into custody without further incident.
During the chaos, a separate vehicle involved in the unfolding scene crashed into a patrol car, injuring Deputy Michael Hatcher, who was treated and released from a local hospital. The Georgia Bureau of Investigation has launched a full probe, including forensic ballistics, body camera footage review, and interviews with all deputies involved. The investigation also covers the circumstances leading to the pursuit and the events at the crash site.
Community leaders have expressed concern over the violence and are calling for increased transparency in the investigation. Holmes had a prior criminal record including non-violent offenses. The other occupant’s identity has not been publicly released.

The man was detained for questioning, and police claim he ran from the scene. Officers pursued him on foot. Police claim he was armed and fired at them. Officers opened fire and shot him. He was transported to Denver Health Medical Center, where he was pronounced dead. A woman who was with him at the beginning of the encounter left the scene and has not been identified. Denver Police, the Colorado Bureau of Investigation, and Colorado State Patrol are conducting the investigation. No officers were reported injured.

On June 22, 2025, around 2 a.m., officers with the St. Louis Metropolitan Police Department responded to reports of a shooting on Manchester Avenue near Sarah Street in The Grove neighborhood. Upon arrival, officers encountered a person who reportedly opened fire on them. Police returned fire and shot the individual, who died at the scene. On 06/22/2025 at approximately 3:00 a.m., Jose Enrique Cartagena Chacon, 25, was shot and killed by Lenexa Police Officer Nicholas Gaul at the Lenexa Crossing Apartments in Lenexa, Kansas. Officers had been dispatched after a Spanish-speaking woman called 911 and reported a man with what appeared to be a handgun near the apartment complex pool.

Officers Nicholas Gaul and Alexander Clark located Cartagena Chacon sitting in the driver’s seat of a Volkswagen Jetta in the parking lot. Gaul approached the driver’s side while Clark approached the passenger side. Police said they saw what appeared to be a handgun in Cartagena Chacon’s lap. The weapon was subsequently determined to be a .177-caliber Umarex Walther PPQ CO2-powered pellet gun. It was not loaded with live ammunition and closely resembled a semiautomatic handgun.

The encounter lasted approximately 10 seconds. According to the Johnson County District Attorney’s investigation, officers repeatedly ordered Cartagena Chacon to show his hands and not touch the gun. Police alleged that he picked up the pellet gun and moved it toward Clark on the passenger side. Gaul, positioned at the driver’s side, then fired multiple rounds, fatally wounding Cartagena Chacon. Body-camera footage of the shooting was subsequently released.
The Johnson County District Attorney’s Office investigated the shooting for approximately four months. District Attorney Steve Howe announced in October 2025 that criminal charges would not be filed, concluding that the officers’ use of deadly force was legally justified.
The family disputes that account.
In June 2026, Cartagena Chacon’s estate filed a federal wrongful-death and civil-rights lawsuit against the City of Lenexa, Gaul, Clark and additional unidentified officers. The lawsuit publicly identified Gaul and Clark for the first time. It alleges Cartagena Chacon was asleep in his vehicle when the officers approached with their guns already drawn, that he posed no threat, and that the officers did not identify themselves as police.

The complaint further alleges that Cartagena Chacon spoke very little English but that the officers issued their commands in English. According to the family, he was given only seconds to comprehend what was happening. The lawsuit alleges Gaul fired at least eight rounds into the vehicle and disputes the police assertion that Cartagena Chacon pointed the pellet gun at either officer.

The lawsuit asserts nine claims, including excessive deadly force, unlawful seizure, failure to intervene, assault and battery, wrongful death, survival action and intentional infliction of emotional distress. The family is seeking compensatory and punitive damages and a jury trial. The City of Lenexa has declined to comment on the allegations while the litigation is pending.

On June 22, 2025, Honolulu Police responded around 6:30 a.m. to reports of shots fired from a boat off Sand Island Access Road. Officers located a man barricaded on the vessel. Police attempted two taser deployments and spent nearly six hours negotiating. At 12:51 p.m., the man exited the boat’s hatch and pointed a shotgun at Specialized Services Division officers. One officer shot him. The man was transported to a hospital and pronounced dead. The incident remains under investigation.

On June 22, 2025, police escalated a standoff during which they discovered James Scott Grigory dead from a self-inflicted gunshot wound after a standoff at a home on County Road 456 in Leon County. Earlier that evening, Grigory allegedly shot and killed his mother in Harris County, then traveled to his father's home in Normangee. Deputies responded to a welfare check and encountered gunfire. A Leon County deputy was shot twice. DPS SWAT and Texas Rangers secured the scene and found Grigory, his father, and two dogs dead inside the residence. Investigators confirmed Grigory’s death was self-inflicted during the standoff.

Gloria Zamora, a 40-year-old fitness influencer and mother of seven, was fatally shot along with her date, Hector Garduno, in a parking lot outside a sushi restaurant in Fontana. Zamora’s estranged husband, Thomas Alberto Tamayo Lizarraga, ambushed the pair and shot both of them. An off-duty deputy with the San Bernardino County Sheriff’s Department heard the gunfire, responded to the scene, and fatally shot Lizarraga. Zamora and Garduno later died at local hospitals. Zamora had recently spoken publicly about her divorce and domestic abuse.

On June 22, 2025, Delta Police responded to a reported shooting at 1572 G96 Lane involving 75-year-old Steven Grubb. Officers arrived to find that Grubb had allegedly shot and wounded his 57-year-old son and the son's 35-year-old girlfriend before barricading himself inside the home. A Special Response Team was deployed and attempted negotiation. When officers entered, they found Grubb dead from a self-inflicted gunshot wound. The two shooting victims were hospitalized in stable condition. The incident remains under investigation by the Delta Police Department.

Victoria Police attempted to stop a red Nissan after a 911 caller alleged a man in a red hoodie pointed a gun. The driver refused to stop. Officers escalated an aggressive chase through Victoria that ended when the vehicle lost control and struck a utility pole in the 3800 block of North Ben Jordan Street. Rear passenger Aidan Hewer, 18, was pronounced dead at the scene. A gun was never found.

A Victoria police chase on June 22, 2025, ended in a crash that killed rear passenger T’Andre Kitchens, 17. On 06/22/2025, 45-year-old Byron Jackson was pursued by Irmo police after officers responded around 2 a.m. to a report of a possible altercation inside a vehicle in the New Friarsgate area. Police said Jackson drove away when officers approached his utility van. Officers chased him to 7812 Broad River Road, where the van crashed into pallets behind a vacant building.
Police said Jackson fled on foot and struggled with officers, with Jackson and an officer falling approximately 20 feet down a ravine. Officer Tyler Baker deployed a drive-stun Taser while Officer John Parker was attempting to restrain Jackson. Jackson was then handcuffed.
Shortly after being restrained, Jackson developed difficulty breathing. Police initially moved his handcuffs from behind his back to the front and then removed them when EMS began treating him. Jackson suffered cardiac arrest while being transported to the hospital and was placed on life support. He died on 06/25/2025, three days after the police chase, Taser deployment and restraint.
Irmo police initially said Jackson died in police custody, then retracted that characterization and claimed he had never been physically in custody. Police records, however, show that Jackson had been handcuffed, and the incident report listed resisting arrest and a traffic offense. Jackson's family and attorney Bakari Sellers disputed portions of the police account and sought release of the video.

Officer Tyler Baker joined the Irmo Police Department in April 2024 and deployed the Taser. Officer John Parker joined the department in May 2023 and was the other primary officer involved in restraining Jackson.

On June 21, 2025, an off-duty deputy with the San Bernardino County Sheriff’s Department shot and killed a man in the parking lot of Falcon Ridge Town Center in Fontana. Police say the 45-year-old man had fatally shot a 43-year-old man and a 40-year-old woman moments earlier. The off-duty deputy intervened and shot him and he was pronounced dead at the scene. The investigation is ongoing.

Clay County deputies sought to arrest Dajshon Travis Asbell on multiple outstanding warrants. Officers found Asbell with another person, who allegedly fled with him into a building where they barricaded themselves. The second person later exited, but Asbell remained. Police deployed tear gas and breached the building using a tactical vehicle. Asbell allegedly exited holding an edged weapon and moved toward officers. Police released a K9, which Asbell allegedly threw aside. Officers then shot him, deployed less-lethal rounds as he continued to resist, and arrested him. Asbell died three days later from gunshot wounds.

Seattle Police Department officers responded around 4:51 a.m. to a domestic violence call at 32nd Avenue East and East Madison Street. Officers found a woman with a gunshot wound to her lower back who collapsed into their arms. While officers provided aid, Daniel Jolliffe fired multiple shots from an upstairs window at officers and the victim. Officer Leonardo Bassi Rodriguez returned fire and struck Jolliffe. SPD set a perimeter and engaged SWAT and Hostage Negotiation Team efforts. Jolliffe fired at a drone, barricaded inside, and ignored commands. After roughly an hour, SWAT used a robot and explosive charge to breach. Officers found Jolliffe unresponsive in a bedroom. King County Medical Examiner later ruled the death as suicide by self-inflicted gunshot wound, not by police fire. The woman was transported to Harborview Medical Center in serious but stable condition. SPD Force Investigation Team, Office of Police Accountability, and Office of Inspector General opened investigations. Officer Rodriguez was placed on administrative leave and body-worn video was released within 72 hours.

On 06/20/2025, deputies from the St. Tammany Parish Sheriff's Office responded to a report of a woman being shot at a home on Jasmine Street. Richard Skyles III exited the residence armed and fired at deputies. Deputies returned fire and struck Skyles. He was transported to a hospital and later pronounced dead. On June 20, 2025, at approximately 11:17 PM, a Lodi Police Department officer observed a vehicle driving recklessly near South Cherokee Lane and East Vine Street. The officer activated emergency lights and sirens to initiate a traffic stop. The driver did not stop. Police escalated a chase that ended when the vehicle entered the on-ramp to southbound Highway 99 at Century Boulevard. The vehicle lost control, rolled over, and came to rest on its roof. Emergency personnel extracted the 19-year-old driver, who was pronounced dead at the scene. A passenger exited independently and was transported to a local hospital with non-life-threatening injuries. The San Joaquin County Officer Involved Critical Incident Protocol was activated. An investigation is being conducted by Lodi PD, the San Joaquin County District Attorney’s Office, the California DOJ, the San Joaquin County Medical Examiner, and CHP is leading the crash reconstruction.

On 06/20/2025 at approximately 4:45 PM, Greenville County deputies responded to reports of an armed robbery at the Quick & Fast convenience store on White Horse Road. Inside the store, deputies say Trey Rashon Lovett displayed a knife and demanded cash. Deputies located Lovett nearby and a K-9 handler deployed the K-9 to apprehend him. The K-9 bit Lovett while he was restrained. Deputies applied first aid while awaiting EMS. Lovett was transported by ambulance to Greenville Memorial Hospital, where he became unresponsive and later died. The Greenville County Coroner’s Office reported he suffered cardiac arrest following the K-9 bite. The nature and severity of the dog bite injuries were not disclosed. The South Carolina Law Enforcement Division (SLED) and Coroner’s Office are investigating. The officer was not named.

On 06/20/2025 at approximately 6:14 p.m., a Chula Vista police officer was speeding westbound on Olympic Parkway with lights and sirens activated, responding to a radio call. Simon Robledo was traveling eastbound on his motorcycle, approaching Windingwalk Street. The police vehicle turned left into Robledo’s path, resulting in a forceful collision with the passenger side of the patrol car. Robledo suffered fatal injuries and was pronounced dead at a local hospital. The officer was evaluated at the same hospital and released. The California Highway Patrol has taken over the collision investigation.

The deputy escalated the pursuit on Highway 450, during which Jonathan Townsend’s motorcycle, carrying passenger Amber M. Cisneros, collided with a pickup truck in the 5300 block area. On June 19, 2025 at approximately 7:20 p.m., a Phoenix Police Department officer was traveling northbound on Cave Creek Road when a collision occurred at the intersection with Pinnacle Peak Road. Erik Sosteno Yrigollen was driving westbound and ran a red light, striking the passenger side of the marked police Tahoe. Yrigollen was transported to a nearby hospital in critical condition, where he later died. The officer received non-life-threatening injuries and was also evaluated at the hospital. Phoenix PD detectives, along with the Arizona Department of Public Safety and Maricopa County Medical Examiner, are conducting the investigation.

On the morning of June 19, 2025, at approximately 10:45 a.m., officers assigned to a paid detail were flagged down in the 1900 block of Tchoupitoulas Street regarding a shoplifting incident at Walmart. Officers located a man matching the description near the store. When they attempted to intervene, he fled on foot into the street. As they pursued him for a short distance, he produced two knives and stabbed himself in the neck multiple times. Officers fired a taser at the dying man. The man was transported to a hospital, where he succumbed to his injuries later that day. The death is being treated as a suicide and is classified as an "in-custody death," and both the Independent Police Monitor and Federal Monitor were present on scene.

On June 19, 2025 just before 3 p.m., Escambia County deputies received a tip that a man with warrants was working inside the Arby’s on US 98. Deputies located Daniel Lewis Hayes, 37, in the dining area. He fled behind the counter when they approached. Two deputies pursued him; Hayes knocked one deputy down. He then retrieved a knife and confronted three deputies who closed in on him. All three deputies opened fire, striking and killing Hayes in the parking lot. The Florida Department of Law Enforcement is conducting an independent investigation. Hayes was an Arby’s employee for about 12 years, described by family as humble, smiling, and generous.

On May 8, 2025, deputies from the Kershaw County Sheriff’s Office attempted to stop Antwan Washington for a moving violation of failure to yield. Washington did not stop. Kershaw County deputies escalated a high-speed pursuit that reached speeds up to 85 mph, traversing dirt roads, highways, and wooded terrain. The chase ended near Providence Road when Washington crashed his vehicle. Deputies secured the scene, and Washington was transported by air ambulance to a hospital. He remained hospitalized for over a month before dying from his injuries on June 19, 2025. The South Carolina Law Enforcement Division is leading the investigation.

On June 18, 2025 around 3:30 p.m., Chautauqua County deputies responded to a reported domestic dispute at a residence on Mina French Creek Road. Deputies encountered Mark W. Bemis, 66, holding a shotgun outside the home. He fired a round that struck a deputy in the hand. Deputies then opened fire and struck Bemis. He was pronounced dead at the scene. The injured deputy was taken to a hospital and treated for non-life-threatening injuries. The New York Attorney General’s Office and District Attorney’s Forensic Identification Team are investigating. Deputies involved were wearing body cameras, which were active at the time.

On June 18, 2025, at approximately 8:12 p.m., Greeley Police tried to stop Theodore Rybus on 18th Street near the university for an active warrant. Rybus drove away in his car. Police escalated chase and moments later, they located Rybus on foot near the Garden Theatre area of campus. Officers ordered him to stop and drop his gun. Rybus turned toward officers and officers reported being fired upon. Two officers opened.fire, striking Rybus. After being wounded by officers, Rybus shot himself in the head. Officers rendered first aid and summoned EMS. Rybus was transported to a hospital, where he was pronounced dead. The Weld County Coroner ruled the death a suicide due to a self-inflicted gunshot, with additional wounds from officer fire. The Colorado 19th Judicial Critical Incident Response Team and Indiana State Police (for collaborative oversight) are investigating.

On June 18, 2025, deputies tracked a black Toyota via license-plate reader after Raymond Benedetti’s mother reported it stolen. Deputies located the vehicle and initiated a traffic stop on 38th Place. Benedetti exited the car, raised his hands, then allegedly "reached into his waistband." One deputy fired one round as Benedetti "made a motion." Both deputies then opened fire, striking Benedetti multiple times. Benedetti was taken to Cleveland Clinic Indian River Hospital and died there. The passenger, who remained inside the vehicle, was not injured. Benedetti had been unarmed and there is no indication so far that he had been a threat to police. The Florida Department of Law Enforcement is conducting an independent criminal investigation. The case will proceed to a grand jury. No names of the deputies have been released as of now.
