On April 26, 2025, at approximately 5:07 p.m., deputies at the Santa Clara County Main Jail responded to a medical emergency and discovered a 56-year-old male unresponsive in his cell. Deputies and medical staff initiated CPR until emergency medical personnel arrived at 5:20 p.m. The man was pronounced dead at 5:45 p.m. He had been booked into jail the previous day on several outstanding warrants, including for firearm possession by a felon, DUI, possession of drug paraphernalia, and obstructing law enforcement. The Santa Clara County Sheriff's Office is investigating the incident in coordination with the District Attorney’s Office and Medical Examiner-Coroner’s Office. No foul play is currently suspected.

On 04/25/2025, New York State Police attempted to stop a 2015 GMC Sierra pickup truck on Route 9W in Newburgh for traffic violations. The driver did not stop. Troopers escalated a high-speed chase on Route 9W, then onto State Route 32, and finally westbound on Interstate 84. Troopers claimed that they discontinued the pursuit due to speeds, and shortly after, the pickup truck collided with a tractor (without a trailer) and crashed off the shoulder of I-84. Passenger Phelipe Figueroa, 25, of Otisville, was transported to Montefiore St. Luke's Cornwall Hospital in Newburgh, where he was pronounced dead.

On 04/25/2025, 64-year-old Richard Harvey Daugherty was fatally shot by law enforcement during a standoff in Okeechobee County, Florida. John Snyder, 37, was fatally shot by a Polk County Sheriff's deputy on April 25, 2025, after allegedly moving toward a deputy with a knife near a McDonald's on U.S. 92 in Lakeland. A family member reported being assaulted by 35-year-old Alexander Taylor, who was holding a butcher knife. Upon arrival, officers observed Taylor pacing with the knife and, as they approached, he charged toward them. Officers Dillon Arena and Thomas Risco fired their weapons, striking Taylor multiple times in the chest. He was transported to HCA Florida Lawnwood Hospital, where he was pronounced dead. On April 25, 2025, at approximately 1:55 p.m., Norfolk Police officers responded to a report of a disorderly person in the 800 block of East Little Creek Road. The individual, identified as 37-year-old Charlene Carey-Chavez, had reportedly been involved in an assault the previous day and had returned to the area.
Officers located Carey-Chavez in the 7600 block of Sewells Point Road. Upon contact, she produced a weapon and failed to comply with officers' commands to drop it. Officers discharged their service weapons, striking Carey-Chavez and inflicting life-threatening injuries. She was transported to the hospital for medical treatment.
As a result of the investigation, Carey-Chavez was charged with assault and battery, grand larceny, and brandishing a firearm. She remains hospitalized and will be taken to Norfolk City Jail upon her release. The Virginia State Police Bureau of Criminal Investigation is conducting an independent investigation into the incident.

On April 24, 2025, officers arrived at Devicco Moss's home in Thomson to execute a search warrant related to a homicide investigation. Moss fled in a blue Chevrolet Silverado. Police escalated a high-speed chase, during which a Georgia Department of Public Safety officer’s vehicle overturned, injuring the officer. Police continued the chase until Moss crashed into a tractor-trailer at the intersection of Ridge Road and Washington Highway, resulting in a fiery crash that killed Moss. On April 24, 2025, Richland police conducted a traffic stop on a recreational vehicle (RV) traveling along U.S. Highway 49. During the stop, the driver, 77-year-old Abdullah Chetin of Ocala, Florida, allegedly pulled out a firearm and began shooting at officers. An exchange of gunfire ensued, and Chetin barricaded himself inside the RV. After a standoff, additional gunfire was exchanged, resulting in Chetin being fatally shot. No officers were injured during the shooting; however, an officer responding to the scene was involved in a separate traffic accident and received medical attention at a local hospital. The Mississippi Bureau of Investigation is leading the investigation into the incident.

On 04/24/2025, Jacob Faulstich Jr., a 30-year-old White male from Marrero, Louisiana, died in a crash while fleeing from St. John the Baptist Parish Sheriff's deputies. Deputies made contact with Faulstich, who then drove away northbound on U.S. Highway 51 in a 2014 Chevrolet Camaro. Police escalated the situation into a high-speed pursuit. As the chase approached Woodland Drive, Faulstich attempted to overtake another vehicle, causing his Camaro to collide with the front left of the other car. The impact caused Faulstich to lose control of his vehicle, which then ran off the road and struck a traffic signal pole. Faulstich, who was unrestrained at the time, suffered serious injuries and died at the scene. The driver and passenger of the other vehicle were properly restrained and uninjured. The crash remains under investigation.

On April 23, 2025, Fairfax County police conducted a traffic stop near the 4000 block of Majestic Lane in the Fair Oaks area. Officers pulled over 36-year-old Jamal Wali for speeding and expired plates. Wali reportedly became hostile, refused to identify himself, and informed police he was armed. On April 23, 2025, at approximately 8:40 a.m., officers from the Vermillion County Sheriff’s Department and Indiana State Police responded to a report of a man allegedly pointing a firearm at another individual in the 700 block of Dana, Indiana. Upon arrival, officers established defensive positions as 57-year-old Michael Landers exited the residence while brandishing a gun. The Terre Haute Police Department’s Special Response Team was called to assist. According to preliminary investigations, Landers pointed his weapon directly at officers. An officer opened fire, striking Landers. Medical aid was administered at the scene, and Landers was transported to a hospital in Clinton, Indiana, where he later succumbed to his injuries. The officer involved has been placed on administrative leave, and the incident is under investigation by the Indiana State Police.

The man, identified as 34-year-old Vincent Morin, was dressed in battle fatigues with body armor and a communications headset, and had two additional magazines in his vest. Deputies issued multiple commands for Morin to drop his weapon, but he did not comply. Three deputies—Lt. William Gergel, Deputy Matthew Fluck, and Deputy Courtney Baldwin—then discharged their firearms, striking Morin. On April 22, 2025, around 7:30 a.m., U.S. Marshals and Nash County deputies attempted to serve a second-degree murder warrant from New York on 40-year-old Donald Fields at a residence on Crabtree Lane near Castalia, North Carolina. Fields fled into nearby woods with a 9mm pistol, told officers he would not be taken alive, and shot himself. He was pronounced dead at the scene. Fields had previously served 20 years in Connecticut for murder and was wanted for parole violations. Another individual at the residence, Lonnie Hawley, was arrested on unrelated federal weapons charges.

Officer Nathan Anderson opened fire, shooting the man. He was transported to a local hospital where he was pronounced dead. Another individual believed to be waiting in a vehicle fled the scene and has not been located. The Texas Rangers are conducting an independent investigation.

The driver, 50-year-old Victor Vigil of Pittsburg, did not stop. Deputies escalated a pursuit. Vigil drove southbound on Highway 160 and entered the northbound lanes. Deputies discontinued the pursuit, but Vigil continued driving. Shortly after, Vigil collided head-on with a Mercedes SUV at the Highway 4 and Highway 160 interchange. The crash also involved a Dodge Caliber and a Honda Civic. Vigil died at the scene. The driver of the Mercedes sustained major injuries and was airlifted to a trauma center. Two occupants of the Dodge were also hospitalized. The incident is under investigation by the California Highway Patrol and the Contra Costa District Attorney’s Office.

On April 22, 2025, at approximately 9:10 p.m., Berkeley County Sheriff's Office deputies conducted a traffic stop near the intersection of Clements Ferry Road and Rivers Reach Drive as part of an investigation into an uptick in shootings into homes in the Huger and Cainhoy areas. During the stop, 17-year-old Tyren J. Kinloch of Wando fled from the vehicle. Deputies reported that Kinloch pointed a firearm at one of them, prompting the deputy to discharge their weapon, striking Kinloch. Kinloch was transported to a hospital, where he later died from his injuries. All occupants of the vehicle were detained. No deputies were injured during the incident. The South Carolina Law Enforcement Division (SLED) is conducting an investigation into the shooting.

On April 22, 2025, at approximately 9:00 p.m., a Maxton Police Department officer attempted to stop a vehicle whose driver refused to exit and allegedly tried to run over the officer. The officer escalated a chase and Scotland County deputies also engaged in chasing 36-year-old Michael McCartney of Laurinburg, who was then driving the wrong way on Highway 74, fleeing from numerous police vehicles until he collided head on with a van. McCartney was pronounced dead at the crash site. The female driver of the van sustained minor injuries and was transported to the hospital. The North Carolina State Highway Patrol is investigating the incident.

On 04/22/2025, at approximately 8:05 a.m., 45-year-old White female Rena Rae Flaherty was found unresponsive in a holding cell at the City of Kent Corrections Facility by a corrections officer. Flaherty had been in custody since 04/19/2025. Despite life-saving efforts by jail staff and medics, she was pronounced dead at the scene. The King County Medical Examiner’s Office has identified Flaherty, but the cause and manner of death have not yet been determined. The Valley Independent Investigative Team is conducting an independent investigation.

On 04/22/2025, Torrance Bernard Bishop, a 39-year-old Black male inmate at the Athens-Clarke County Jail in Athens, Georgia, was found unresponsive in his cell around 7:55 a.m. during routine morning checks. He was pronounced dead shortly before 10:00 a.m. Bishop had been in custody since November 2024 on charges including entering auto, trespassing, and loitering. In January 2025, a Superior Court judge ordered a mental evaluation prior to his trial; it is unclear if the evaluation had occurred. Decatur police responded twice on April 15, 2025, to calls involving John Daniel Scott Jr., who was experiencing a mental health crisis. During the second encounter at Brookridge Apartments, officers attempted to arrest Scott due to an outstanding warrant. Scott appeared confused and panicked. Deputies responded to reports of a man firing from a balcony at 5550 Dancy Road in northwest Houston. The man barricaded himself inside a third-floor apartment and fired shots at deputies. SWAT was called in and attempted negotiations for over two hours using drones, robots, and chemical agents. Around 10:00 a.m., the man stepped onto the balcony with a pistol pointed at officers. Four SWAT deputies opened fire and killed him. The Harris County Sheriff's Office and District Attorney's Office are investigating.

Officers arrived and found the man and woman outside the residence. The man was armed with a handgun and refused multiple commands to drop it. One officer opened fire and killed him. The man died at the scene. The Davis County Officer Involved Critical Incident Protocol Team is investigating the shooting. The officer was placed on administrative leave.

On April 20, 2025, Louisville Metro Police responded to reports of a man believed to be intoxicated, yelling at neighbors and smashing windows in the 3301 block of Arterburn Avenue. Upon arrival, officers encountered the man armed with a knife and instructed him not to grab it. After a struggle, during which a taser was deployed without effect, the man grabbed the knife and stabbed an officer in the chest. The officer was unharmed due to his protective vest. During the altercation, another officer shot the man in the head. He was transported to UofL Hospital, where he was pronounced dead.
