On April 17, 2021, Monroe County deputies escalated a chase and Malik Ali Malik was killed when the vehicle he was driving collided with a tree during the police pursuit. Deputies from the Monroe County Sheriff's Office chased Malik at speeds reaching up to 105 mph before the fatal crash occurred.

Officers from the Winter Haven Police Department responded to a domestic disturbance call around 10:10 p.m. after Larry Jenkins fired a gun in his apartment. Jenkins was shot and killed after he allegedly pulled a gun out of his pocket following being tasered at least twice.

On April 17, 2021, around 3:30 a.m., Spokane Police responded to a report of a suspected car prowling in the 500 block of South Cannon Street. A victim reported that Vadim A. Grishchuk had broken into his vehicle and pointed a gun at him when confronted. When officers arrived, Grishchuk pointed the gun at police after being given commands and allegedly fired a shot. Officer Brandon Roy shot and killed Grishchuk during the encounter.

On 04/17/2021, Weld County sheriff’s deputies shot and killed 60 year old Smoky Lynn Crockett outside a residence on Casler Avenue in Fort Lupton. Deputies responded to a call reporting that a man was armed with a gun, preventing another individual from leaving the home, and making threats to kill them.
When deputies arrived, they located Crockett outside the residence while he was armed. Deputies engaged with him for several minutes. During the encounter that followed, deputies opened fire and killed Crockett at the scene.

On April 16, 2021, Albuquerque Police Department officers responded to a domestic dispute at a residence on Coyote Canyon Place NW in Albuquerque, New Mexico. Juan James Cordova, 51, was reportedly armed and had fired a shot inside the house and was in mental health crisis per his girlfriend. Officers spent approximately an hour attempting to negotiate with Cordova, urging him to surrender peacefully.

Despite negotiations, Cordova remained uncooperative, holding a firearm and making statements suggesting he intended to provoke a lethal response from the police. At one point, he fired a round, which passed through a window and struck a neighbor's house. When Cordova allegedly pointed his firearm at the officers, Officer Bryce Willsey opened fire and killed him.

Officer Willsey, a member of the Albuquerque Police Department since 2015, had been involved in three prior officer-involved shootings before this incident. In October 2023, Cordova's family filed a lawsuit against the city of Albuquerque and Officer Willsey, alleging excessive force and claiming that Cordova was experiencing a mental health crisis and his death had been avoidable.

On April 15, 2021, at approximately 11:30 p.m., Roswell Police Department officers assisted the Chaves County Sheriff's Office during a pursuit involving a white Ford pickup truck that had fled from a deputy during an attempted traffic stop. Roswell officers deployed two sets of tire deflation devices and missed.
The vehicle then entered the parking lot of a bank at 1901 N Main St in Roswell, New Mexico. The driver, identified as 22-year-old Victor Ivan Barron, exited the truck holding a firearm and attempted to run away on foot. Officers Danny Flott and Abrem Atencio, both with the Roswell Police Department, along with Chaves County Sheriff’s Deputy Samuel Lueras, each discharged their firearms, striking and killing Barron.

On April 15, 2021, 56-year-old Stephen B. Swears of Boynton Beach, Florida, died after evading law enforcement. Swears was being pursued by the St. George Police Department and the South Carolina Highway Patrol on US 15 near Fox Run Road, Dorchester County. While fleeing, his vehicle crossed the center line and collided head-on with another motorist’s vehicle, resulting in his death at 1:30 p.m. No other fatalities were reported, and the crash remains under investigation by the Dorchester County Coroner’s Office and the South Carolina Highway Patrol.

On April 15, 2021, a high-speed pursuit by the California Highway Patrol ended when the driver of a white 2004 Toyota Camry fled southbound on Highway 1 from Dimeo Lane toward Morrissey Boulevard. The vehicle lost control on the off-ramp, struck a concrete curb, collided with a metal guardrail and caught fire. Nineteen-year-old Alyssa Roman of San Jose, a passenger in the vehicle, was pronounced dead at the scene. Three other passengers were injured and the driver was arrested for multiple felony offenses including DUI and evading a peace officer.

Dispatch received several 911 calls reporting a man with a knife who had cut himself inside the Ross clothing store. When the first deputy arrived and saw the man outside the store, he ordered him to drop the knife. On 04/15/2021, Tuscaloosa police shot and killed 29 year old Roderick Inge following a police pursuit and confrontation in a wooded area near Skyland Boulevard. Officers responded to reports of a domestic dispute involving gunfire into a vehicle carrying a woman and two children, ages three and five. While officers were speaking with the woman, police saw Inge driving nearby and chased his vehicle through the area.
The pursuit ended in a parking lot, where Inge exited the vehicle and entered a nearby wooded area. Officers located him in the woods shortly afterward. During the encounter that followed, police opened fire and killed Inge.

On April 14, 2021, Aroostook County Sheriff's deputies responded to a 911 call at approximately 2:58 a.m. that reported a man, later identified as Jacob "Jake" Wood, holding a knife to a woman’s throat at a residence on Scovil Street in Mars Hill, Maine. Deputies arrived at the scene and made contact with Wood.

Despite repeated commands to drop the knife, Wood did not comply. He was reportedly holding a knife to the woman’s throat, and the situation quickly escalated. Deputy Isaac Ward, fearing for the woman’s safety and the potential for further escalation, discharged his firearm, fatally wounding Wood.

The woman was unharmed, and after the shooting, she was able to safely exit the residence. Following the incident, the Maine Attorney General's Office conducted an investigation and determined that the use of deadly force was justified under the circumstances, as Wood did not comply with law enforcement commands and posed a potential threat to both officers and the woman involved.

On April 14, 2021, a Henry County Sheriff’s Deputy attempted to stop a vehicle for a traffic violation and suspected impaired driving. The driver, identified as Zacery Kane Antrim, fled through Henry and Delaware Counties before crashing into another vehicle at McGalliard Road and Tillotson Avenue in Muncie. The passenger, 25-year-old Nicholas Wayne Wollum, was pronounced dead at the scene. Both Antrim and the other driver, 47-year-old Shanta Sharrel Schmiesing, were transported to a hospital in critical condition. The Indiana State Police are investigating the pursuit and crash.

On 04/14/2021, a Harris County sheriff’s deputy shot and killed 46 year old Marcelo Garcia at his home in northeast Harris County. Garcia’s wife called police requesting help, stating that her husband was experiencing a mental health crisis. While en route, the deputy requested a mental health response unit, but none was available.
The deputy arrived at the residence and made contact with Garcia at the front door. During the encounter, Garcia held a knife. The deputy issued commands for Garcia to drop the knife. The deputy deployed a Taser and then opened fire, striking Garcia multiple times. Garcia died at the scene.

On April 13, 2021, Pennsylvania State Police responded to a domestic violence call in Saltlick Township, Fayette County. A woman had fled the residence and reported being assaulted and strangled by her husband, Keith Robert Tafoya. On April 13, 2021, officers with the Tontitown Police Department attempted to stop a stolen Toyota 4Runner. The vehicle, driven by an unidentified individual, fled at high speed and failed to negotiate a turn on Bush Creek Road near US 412. The 4Runner crashed into a tree and burst into flames. Chassadee Costales, who was in the vehicle, died early that morning from her injuries. Officers noted one of their own was treated for minor injuries after entering the burning vehicle to assist survivors.

On 04/13/2021, San Diego police shot and killed 36 year old Christopher Templo Marquez following a prolonged standoff that moved through multiple parts of the city and ended near San Diego High School. Police reported that officers were searching for Marquez after reports of gunfire during an earlier encounter. Marquez took cover inside a dumpster on the school campus, where police maintained a perimeter for approximately 11 hours.
During the standoff, police stated that Marquez was accompanied by a woman. As the standoff continued, officers reported observing Marquez reposition the woman in a way they believed placed her at risk. Two police snipers opened fire, killing Marquez at the scene. The woman was not shot.
The San Diego County District Attorney later reviewed the incident and ruled the officers’ use of deadly force justified. No criminal charges were filed against the officers involved.

On 04/13/2021, Garden City police shot and killed 58 year old Thomas Andrew Bunde during a response to a domestic disturbance at a residence on North Quinella Court. Around 12:52 a.m., three patrol officers arrived at the home. Bunde exited the residence while holding a handgun and spoke to officers. During the encounter, officers opened fire. Bunde was shot by two officers and died at the scene.
The Ada County Prosecutor reviewed the shooting and ruled the officers’ use of deadly force justified. No criminal charges were filed against the officers involved.

On 04/13/2021, Honolulu police shot and killed 29 year old Lindani Myeni during an encounter outside a residence on Coelho Way. Officers responded to a call reporting a man sitting in a vehicle outside a woman’s home. Officers approached Myeni and attempted to detain him. During the encounter, an officer deployed a Taser, and other officers opened fire. Myeni was struck and died at the scene.
Body camera footage released later showed officers converging on Myeni at close range during the encounter. The Honolulu Prosecuting Attorney reviewed the case and ruled the use of deadly force justified. No officers were charged. Myeni’s family filed a civil wrongful death lawsuit against the City and County of Honolulu. The lawsuit was later dismissed by a judge.

On April 13, 2021, at approximately 10:30 p.m., a McFarland Police officer attempted to pull over a vehicle driven by Cam-Ron P. M. Poelinitz for speeding. Shortly after the attempted stop, the vehicle clipped one house and crashed into another. Poelinitz was killed in the crash, and four other people in the vehicle were taken to the hospital. Fortunately, no one in either house was injured in the crash.

On 04/13/2021, at around 1: 30 p.m., Maryland State Police received two 911 calls regarding a person possibly armed with a gun. The first call provided only a phone number, while the second gave a street address in the 23000 block of Hollywood Road in Leonardtown. Upon arrival, Trooper Joseph Azzari confronted Peyton Ham in the driveway. Azzari shot Ham twice, resulting in his death.

On 04/12/2021, Knoxville Police Department officers responded to Austin-East Magnet High School after a report the 17-year-old student Anthony Thompson Jr. was armed and involved in an altercation. Officers located Thompson inside a school bathroom. During a struggle, shots were fired and Thompson was struck and killed by police. A Knoxville District Attorney’s Office later determined officers would not face criminal charges for the shooting. Thompson’s family filed a federal civil rights lawsuit alleging denial of medical care and other claims related to the incident and the aftermath. In January 2026, court filings indicated the family reached an agreement with the city and officers to resolve the remaining claim in that lawsuit. Claims over the adequacy of medical response and treatment following the shooting were central to the litigation. The settlement has not been finalized, and details of any financial terms have not been publicly released.

On April 12, 2021, officers from the Georgia State Patrol attempted to initiate a traffic stop on a 2015 Nissan Sentra for speeding at 111 mph on Interstate 20 in Bremen, Georgia. The driver, Aaron JuJuan Shelton, fled, and the officers escalated a high-speed pursuit through Carroll County. During the chase, the passenger, Pier Alexander Shelton, 28, leaned out of the vehicle and fired an AK-47-style rifle at pursuing officers, striking and disabling a patrol car. The pursuit continued through Carrollton and into Villa Rica, where the vehicle crashed near Villa Rica Elementary School. Both occupants fled on foot. As officers searched the area, Pier Shelton fired upon them, injuring multiple officers. Officers returned fire, fatally shooting Pier Shelton.

On April 12, 2021, at approximately 1:36 a.m., Angelina Sue Raad, a 38-year-old white female, was driving her vehicle the wrong way on the inner loop of New Circle Road near Harrodsburg Road in Lexington, Kentucky. She collided head-on with a police cruiser driven by Officer Dawn Miller, who was off duty and heading home after her shift. Both drivers were extricated from their vehicles and transported to a local hospital. Raad was pronounced deceased at around 4 a.m., while Officer Miller sustained serious injuries but was reported to be in stable condition.

Multiple agencies, including the Airway Heights Police Department, Kalispel Tribal Police Department, Spokane Police Department, Spokane County Sheriff’s Office, Spokane Valley Police Department, and Washington State Patrol, were involved in the incident. After the chase concluded, Peterson was reportedly involved in a standoff with law enforcement during which he shot and killed himself.
