On 08/22/2024, Tanner Smith, 36, was killed while riding as a passenger in a Nissan Altima that Elyria police pursued through Elyria. An Elyria officer reported seeing what the officer believed was a hand-to-hand drug transaction on Erie Street at approximately 3:15 p.m. and attempted to stop the Nissan. Dash-camera video later released by police showed the Nissan continuing away as police pursued it toward State Route 57.

Police reported that the Nissan accelerated north on State Route 57, passed traffic using the shoulder and continued through a red traffic signal at Midway Boulevard. Dispatch recordings captured an officer estimating the Nissan's speed at approximately 60 mph while the pursuit continued.

Seconds later, at approximately 3:22 p.m., the Nissan continued through another red light at State Route 57 and Griswold Road and collided with a Toyota Corolla that was turning onto Griswold Road. Smith and fellow passenger Meghan Incorvaia, 36, were killed at the scene. The Nissan's driver suffered life-threatening injuries and was flown to MetroHealth Medical Center. The 48-year-old driver of the Corolla suffered serious injuries.

Smith's mother, Brenda Smith, publicly questioned the Elyria Police Department's decision to continue the pursuit as speeds increased and the Nissan moved around other traffic. She said her son had struggled with addiction and questioned why police continued a pursuit that endangered people over an alleged drug transaction.

The Nissan's driver later pleaded guilty in Lorain County Common Pleas Court to multiple charges arising from the pursuit and fatal crash. In July 2025, a judge sentenced him to 24 to 28 years in prison.

Reporting from the scene indicates police pursued Randall's vehicle from the Altamonte Springs area into Winter Park. A witness who had just picked up her children from school in Eatonville described seeing Randall's vehicle traveling at high speed with police behind it. The pursuit reached the 900 block of Wymore Road near Lee Road, where Randall stopped in the roadway.

According to Altamonte Springs police, Randall abruptly exited his vehicle and ran toward officers while holding an unidentified object. Officers then fired multiple shots, striking him. Police did not identify the object Randall allegedly held. Randall was transported to a hospital, where he died from his gunshot wounds.

On 08/23/2024, Kentrail Small, 40, was shot and killed by Indianapolis Metropolitan Police Department SWAT officers while police served a narcotics search warrant at the Park Terrace Motel, 9025 Pendleton Pike, in Lawrence. IMPD narcotics detectives and SWAT officers arrived at approximately 10:35 a.m. Police said Small was the subject of the warrant and that they believed he was armed.

SWAT officers surrounded the motel room and used sirens and a public-address system to announce their presence and order Small to come outside. After receiving no response, officers forced entry into the room. Small and a woman then emerged from the bathroom. Body-camera footage released in November 2024 shows officers ordering both of them to the floor. The woman complied while officers moved toward Small.

As officers attempted to handcuff Small in the motel room's kitchenette, a physical struggle developed. IMPD alleged that Small broke away and attempted to grab a Draco-style firearm. Body-camera footage shows officers struggling with Small immediately before officers shouted warnings about a gun. Two SWAT officers then fired multiple rounds, striking Small.

Police and medics provided medical aid before Small was transported to Eskenazi Hospital, where he died. Police reported recovering three firearms from the motel room and said detectives seized fentanyl, cocaine, marijuana and prescription pills during execution of the warrant.

IMPD's Critical Incident Response Team investigated the shooting, with a separate administrative investigation by IMPD Internal Affairs. The Marion County Prosecutor's Office was consulted during the criminal investigation. IMPD subsequently released edited body-camera footage of the raid and shooting on November 8, 2024.

The Gwinnett County Police Department's Behavioral Health Unit was notified, and a licensed mental health clinician spoke with Hemphill by telephone while officers responded. According to police records, Hemphill told the clinician that he was depressed and suicidal. Officers encountered him near the roadway holding a knife and attempted for several minutes, along with the clinician, to persuade him to put it down.

According to the GBI and Gwinnett County police, Hemphill then ran toward two officers while holding the knife. The officers fired, and Hemphill fell to the ground. The GBI alleged that Hemphill then got back up while still holding the knife, and officers fired again. Hemphill was pronounced dead at the scene.

Although Hemphill had told 911 that he possessed both a handgun and a knife, the subsequent Gwinnett County police incident report stated that investigators found a knife but did not find a gun on him.

The Gwinnett County Police Department incident report classified the incident as an "assisted suicide." The Georgia Bureau of Investigation conducted an independent investigation of the shooting for submission to the Gwinnett County District Attorney's Office.

On 08/23/2024, Billy Ray Factor, 25, was shot and killed by Edmond police officers Garrett Hill and Ismael Fiorina at a townhome complex in the 1900 block of Three Stars Road. At approximately 10:30 p.m., police responded after a resident reported seeing an unknown person around the back porch of her residence and believed the person was still nearby.

Officers searched the area and found Factor crouched on a second-floor balcony. Body-camera footage released the following month shows officers illuminating Factor with flashlights and asking what he was doing and how he had gotten onto the balcony. Factor stood and moved toward the balcony railing.

According to Edmond police, Factor raised a handgun and fired toward officers. Body-camera footage captured an officer shouting for him to put the weapon down, followed by gunfire. Hill and Fiorina fired at Factor, striking him. Factor died at the scene.

Police records later established that the balcony was outside the apartment of Factor's former girlfriend. A woman was inside the bedroom adjoining the balcony during the shooting. Investigators also recovered a backpack near the scene containing Factor's belongings, including a loaded firearm magazine.

Edmond police released body-camera footage of the shooting in September 2024. Later reporting identified the shooting officers as Garrett Hill and Ismael Fiorina. The Oklahoma County District Attorney subsequently reviewed the shooting and cleared the officers of criminal wrongdoing.

Factor was born August 6, 1999, in Oklahoma City. He graduated from Edmond Memorial High School in 2018 and later attended Northeastern State University. His family remembered him as passionate, adventurous and devoted to his family.

On 08/22/2024, Juan Barbosa Jr., 41, was shot and killed by Mustang Police Officer Jazreale Wright during a mental health crisis near South Mustang Road and Southwest 89th Street. Police were called shortly after 7:00 p.m. regarding Barbosa, who had reportedly jumped from a vehicle while experiencing the crisis.

Wright arrived alone and spoke with Barbosa's nephew, who attempted to explain what was happening. According to Mustang police, Barbosa approached Wright despite her commands. Barbosa's nephew attempted to restrain him, and Wright fired her Taser. Police later said only one of the two probes made contact, preventing the Taser from functioning as intended.

Barbosa remained on the ground for approximately 30 seconds before getting up. Wright attempted to use the Taser again as Barbosa approached her. The second attempt also failed to stop him. Wright then drew her handgun and fired two shots, killing Barbosa.

Video released in May 2025 showed that Barbosa was holding an object as he approached Wright. Mustang Police Chief Mike Wallace acknowledged after the investigation that the object was a pair of gloves.

The Oklahoma State Bureau of Investigation investigated the shooting. The investigation was completed in late 2024 and presented for prosecutorial review. Wright was not criminally charged and returned to duty in December 2024. Mustang Police Chief Mike Wallace later publicly defended the shooting as justified.

On 01/07/2026, Barbosa's estate filed a federal civil-rights lawsuit in the U.S. District Court for the Western District of Oklahoma against the City of Mustang and Wright in both her individual and official capacities. The lawsuit was filed under 42 U.S.C. § 1983.

On 08/22/2024, Julio César Cordero, 56, was shot and killed by an El Paso Independent School District police officer outside the Franklin High School Magnet Center in El Paso. Shortly before the shooting, police received a 911 call reporting a man breaking windows at the school. Cordero's son was a senior at Franklin High School.

According to an EPISD custodial death report subsequently obtained by the El Paso Times, an officer responded to the 911 call and encountered Cordero. Police alleged that Cordero did not comply with verbal commands. The report states that the officer attempted to detain Cordero using unspecified less-lethal methods before Cordero allegedly picked up a large rock and moved toward the officer. The officer then shot Cordero. He died at the scene. The report did not state how close Cordero was to the officer or how many shots the officer fired.

Cordero's family said he had been experiencing serious mental health problems following his retirement from the FBI, including paranoia, periods of sleeplessness and PTSD symptoms. His brother Marco Cordero said Julio's mental health deteriorated after a suicidal man jumped in front of his vehicle and died in 2014, an event that also resurfaced trauma surrounding his sister's death in a 1993 crash. His family described episodes in which Cordero would stop taking medication and begin behaving erratically, including damaging property, but said he was not ordinarily violent.

Cordero was a retired FBI special agent who served in the FBI's El Paso Division from 1996 until 2019. He was the lead agent in Operation Poisoned Pawns, a major public-corruption investigation that resulted in numerous convictions of public officials and others in El Paso. The FBI publicly mourned Cordero following his death and described him as an important former member of its El Paso office.

Cordero's family questioned the police response and sought access to video and additional information about the shooting. Reporting after the shooting also raised questions about what less-lethal equipment was available to EPISD officers. The district would not specify what non-lethal weapons its officers carried, while sources reported that EPISD officers did not have Tasers.

The Texas Rangers led the criminal investigation with assistance from the El Paso Police Department. EPISD also opened an administrative investigation. The school district stated that the completed criminal investigation would be referred to the 34th Judicial District Attorney's Office.

On 08/22/2024, Matthew Bruce Coleman, 45, died from an apparent self-inflicted gunshot wound during a SWAT standoff with Polk County sheriff’s deputies in Lakeland. Deputies were searching for Coleman because he had an active warrant and received information that he was at a property along West Socrum Loop Road.

When deputies arrived, Coleman was believed to be inside or near a backyard shed. According to the Polk County Sheriff’s Office, deputies reported that gunfire came from the shed area as they attempted to locate and arrest him. Deputies established a perimeter and summoned the agency’s SWAT team and crisis negotiators.

Law enforcement spent several hours attempting to persuade Coleman to surrender. Deputies used a public-address system and other negotiation efforts before SWAT officers deployed a chemical agent into the structure.

SWAT personnel later entered the shed and found Coleman dead from what authorities described as an apparent self-inflicted gunshot wound. Deputies did not report shooting Coleman.

On 08/22/2024, Thomas Chase, 92, was killed in Brighton when his vehicle was struck by a stolen Kia that Rochester police were pursuing. The incident began at approximately 3:30 p.m. when Rochester police responded to a ShotSpotter activation reporting multiple gunshots near Maria and Bernard streets.

Police located a man near the scene and alleged that he discarded a loaded 9mm handgun and ran toward Friedrich Park. He entered another stolen Kia containing two other people. Police said the Kia then backed toward an officer before driving away, and Rochester police began pursuing it.

The pursuit continued from Rochester into Brighton for approximately eight minutes. Police reported that the Kia reached speeds exceeding 100 mph. As it traveled south on East Avenue, Chase was driving his own vehicle and attempting to turn left from East Avenue onto Elmwood Avenue. The Kia struck Chase's vehicle at the intersection. Chase died at the scene. The three people in the Kia were hospitalized with serious injuries.

Rochester Police Chief David Smith said an initial review of the pursuit found no violation of the department's pursuit policy. Body-camera footage reviewed by the department showed an officer bracing for impact when the Kia allegedly backed toward the officer shortly before the pursuit began.

On 08/21/2024, Zachary Johnson, 32, died in the Tennessee River after an approximately hour-long police pursuit that began in Chester County and continued through McNairy County into Hardin County. At approximately 10:15 a.m., Johnson drove down the Saltillo Boat Ramp and entered the Tennessee River. Law-enforcement officers followed him into the water in an attempt to take him into custody. When officers arrived, they encountered one injured woman outside the house and entered to locate additional victims. Police found two more women suffering from stab wounds inside. On 08/21/2024, shortly after midnight, Oskar Hernandez Martinez Jr., 20, was killed when his black 2000 Ford Mustang crashed off North Division Street in Harvard after a Harvard police officer attempted to stop the vehicle. Hernandez Martinez was pronounced dead at the scene. The McHenry County Coroner later determined that he died from blunt-force trauma.

The encounter began at approximately 12:02 a.m. when a Harvard police officer saw a dark-colored sedan disobey a stop sign at South Park Avenue and South Division Street, also known as U.S. Route 14. Police reported that the same vehicle then continued northbound, disregarded a traffic signal at Brink Street/Route 173 and rapidly accelerated. The officer, who had been traveling southbound, turned around and attempted to stop the vehicle.

The officer briefly lost sight of the vehicle as it crossed the North Division Street viaduct over the railroad tracks and then completely lost visual contact near North Division Street and Diggins Street. Continuing north, the officer stopped another dark-colored sedan at North Division Street and Crowley Road but determined after speaking with its driver that it was not the vehicle the officer had originally attempted to stop.

Two additional Harvard police officers responding as backup drove north on Division Street and struck a low-hanging braided metal utility guy wire in the 1500 block. Both police vehicles were damaged. Police subsequently determined that the wire had fallen across the roadway after Hernandez Martinez's Mustang struck and broke a utility pole.

Police searched the area and discovered Hernandez Martinez's Mustang approximately 100 feet off the roadway in a heavily wooded area. He was the vehicle's sole occupant and was pronounced dead at the scene by the Harvard Fire Protection District.

The McHenry County Coroner's Office identified Hernandez Martinez several days later. An autopsy determined that his cause of death was blunt-force trauma. The Major Crash Assistance Team investigated the crash, and Harvard police requested an outside review by the McHenry County Sheriff's Office under the Illinois Police and Community Relations Improvement Act.

Hernandez Martinez was born in Harvard. His obituary described him as someone who loved football, cooking and driving his Mustang and said that family was particularly important to him. A pursuing Pierce County sheriff’s deputy then performed a PIT maneuver against the Tahoe. The Tahoe spun, struck a curb and flipped into roadside brush. Bagnariol, who was riding in the front passenger seat, was ejected, and the Tahoe landed on top of her. Emergency personnel freed her from beneath the vehicle and transported her to a hospital in critical condition. She died five days later.

The driver later pleaded guilty to DUI vehicular homicide. On 10/29/2024, Pierce County Superior Court Judge Timothy Ashcraft sentenced him to seven years in prison.

Kansas Highway Patrol reported that Masterson had his emergency lights and siren activated as he approached the intersection. The intersection was described in later reporting as an intersection of two dirt roads that apparently did not have stop signs.

At approximately 4:00 p.m., deputies responded after a crisis intervention team had gone to the residence to assist Cochran and police alleged that he made death threats against his wife and members of the crisis team.

Cochran’s wife and the crisis intervention workers took shelter at a neighbor’s home across the street. Bartow County deputies responded there to protect them. Authorities reported that Cochran had access to numerous rifles inside his residence.

According to the GBI, Cochran returned inside his home, broke windows and allegedly continued threatening his wife, the crisis intervention workers and deputies. The Bartow County Sheriff’s Office deployed its SWAT team to remove the people sheltering across the street and take Cochran into custody.

The GBI alleged that Cochran pointed a long gun from a second-story window while repeatedly threatening SWAT personnel, deputies and witnesses outside. A Bartow County SWAT officer then fired, striking Cochran. Cochran was pronounced dead at the scene by the Bartow County Coroner’s Office.

The Georgia Bureau of Investigation conducted the independent investigation and said the completed case would be submitted to the Cherokee Judicial Circuit District Attorney’s Office for review.

The encounter followed a shooting shortly before 9:00 a.m. in an overflow parking lot across from the Hardin County Justice Center in Elizabethtown. Police alleged that Elder shot his girlfriend, Erica Riley, 37, Riley’s mother, Janet Riley, 71, and another male relative shortly before a scheduled court hearing concerning an emergency protective order. Erica Riley died at the scene and Janet Riley later died at a hospital. The male relative survived.

Kentucky State Police Post 4 issued an attempt-to-locate alert for Elder and his silver 2008 Hyundai Elantra in connection with the shootings. Authorities subsequently spotted the Hyundai near Greenville in Muhlenberg County. Muhlenberg County sheriff’s deputies and Greenville police attempted to stop Elder specifically because he was wanted in connection with the fatal Hardin County shootings. Elder continued driving and police initiated a vehicle pursuit.

The pursuit continued through western Kentucky toward Christian County. Police alleged that Elder repeatedly pointed a rifle toward pursuing officers while driving. This was not a traffic stop or police pursuit; law enforcement encountered Redondo-Zelaya because of the crash.

According to police, a St. Clair officer approached the overturned vehicle and saw Redondo-Zelaya inside holding a knife. Police alleged that the officer repeatedly ordered him to drop the knife. On 08/18/2024, Howard Theodore Coleman Jr., 42, died from a self-inflicted gunshot wound during a Charles County Sheriff’s Office tactical operation at a residence in the 9300 block of Brechin Place in Waldorf. Officers initially went to the home for a welfare check after the family of Coleman’s girlfriend, who lived out of state, reported that they had not heard from her for several days and were concerned for her safety. The family told police that she was in a physically abusive relationship with Coleman.

Patrol officers were unable to contact the woman. After officers learned additional information about the situation, detectives from the Criminal Investigations Division took over the investigation and obtained a search warrant for the residence. Coleman did not cooperate when the tactical team arrived. Officers deployed a drone and entered the residence while hostage negotiators established telephone contact with Coleman. During the negotiations, police learned that the woman had suffered multiple stab wounds and severe lacerations. Authorities reported that Coleman had access to firearms and other weapons inside the house.

While negotiators continued speaking with Coleman, members of the Emergency Services Team entered the house and reached the injured woman. Officers removed her from the residence and transferred her to waiting emergency medical personnel. She was flown to a hospital in critical condition with multiple stab wounds to her head and upper body and a possible broken jaw.
