On 08/17/2024, Delfina Sanchez-Eusebio was killed when a car being pursued by the North Carolina State Highway Patrol crashed into the Honda Accord in which she was a passenger at North Bragg Boulevard and McKenzie Road in Spring Lake. The crash occurred shortly before midnight.

The pursuit began in Harnett County after a state trooper clocked a 2012 Acura traveling approximately 110 mph southbound on N.C. Highway 87. The trooper attempted to stop the Acura specifically for the 110-mph speeding violation, and police pursued when the driver continued driving away.

The pursuit continued south on N.C. 87 toward Spring Lake. At the intersection of North Bragg Boulevard and McKenzie Road, police said the Acura drove around vehicles stopped at a red light and crashed into the Honda Accord as the Honda was turning onto McKenzie Road.

Sanchez-Eusebio was a passenger in the Honda and died at the scene. The Honda's driver, Ismael Apolinar-Heredia, was seriously injured and transported to Cape Fear Valley Medical Center. The Acura then struck a utility pole and caught fire, and its driver was hospitalized in critical condition.

State Highway Patrol investigators alleged that alcohol was a contributing factor in the Acura driver's actions. The fatal crash occurred during an active State Highway Patrol pursuit initiated over the 110-mph speeding violation.

On 08/18/2024, Alex Angel Cortez, 16, was shot and killed by Chicago Police Department officers assigned to the Robbery Task Force in the Pilsen neighborhood. At approximately 1:16 a.m., the officers were already patrolling the area when they heard numerous gunshots near the 1800 block of South Blue Island Avenue and moved toward the gunfire.

Police alleged that officers encountered Cortez and another armed person after hearing the shots. Both officers fired their weapons, striking Cortez. Cortez was transported to a hospital and pronounced dead at approximately 1:56 a.m.

Nearby, police found 29-year-old Stephen Mancilla Carlos and a woman suffering from multiple gunshot wounds inside a vehicle. Both were initially hospitalized in critical condition. Mancilla Carlos died from his injuries on August 21.

Chicago police alleged that Cortez and another person had fired at the vehicle before officers encountered them. Police reported recovering a firearm at the scene. Cortez's father, Fulgencio Cortez, publicly disputed the police account that his son was armed and said the family wanted evidence and answers about the circumstances of the shooting.

A significant later development occurred in 2025, when prosecutors charged Christopher Ford, 24, in connection with the shooting of Mancilla Carlos and the woman. Prosecutors alleged that video showed Ford and Cortez firing approximately 35 rounds at the victims' vehicle in a case of mistaken identity. Prosecutors further alleged that immediately afterward police activated their emergency lights and pursued the two on foot, and that Cortez stopped and turned toward the pursuing officers while holding a firearm in his right hand. An officer then shot Cortez. Investigators attributed 16 of the shots fired at the vehicle to the gun police alleged Cortez was carrying.

The Civilian Office of Police Accountability opened an investigation into the officers' use of deadly force. COPA stated that Illinois juvenile-record law prevented it from publicly releasing the video and other materials normally released following a fatal police shooting. On 08/17/2024, Jeremy Jason Lundgreen, 46, was shot by Roy City police officers Cameron Hackworth, Tim Fulton and Patrick Brown following an attempted traffic stop and subsequent attempt to arrest him in Roy. Lundgreen died at a hospital on 08/20/2024 from injuries sustained in the shooting.

The encounter began at approximately 7:15 p.m. when Roy police stopped Lundgreen's vehicle. Police were attempting to arrest him in connection with an Internet Crimes Against Children investigation involving allegations that he had enticed a minor online. Police said Lundgreen was initially cooperative during the stop but then drove away.

Officers subsequently located Lundgreen near 5650 South 2775 West. According to police and the later Weber County Attorney's Office investigation, Lundgreen allegedly confronted officers with a firearm and began shooting. Officers Hackworth, Fulton and Brown returned fire during an exchange that continued for several minutes, striking Lundgreen. He was taken into custody and transported to a hospital.

On 08/17/2024, a juvenile boy was killed when off-duty Lancaster Police Officer Derrick A. Glover crashed a department Dodge Durango into the rear of the boy's off-road motorcycle on U.S. 27 near Old Norwood Road in Pulaski County. The collision occurred shortly before 11:45 p.m.

Kentucky State Police reported that Glover was driving south on U.S. 27 in a Lancaster Police Department Dodge Durango when he struck the rear of the red Coleman 196cc off-road motorcycle. The juvenile was also traveling on U.S. 27 and was not wearing a helmet.

The collision fatally injured the juvenile, who was pronounced dead at the scene by the Pulaski County Coroner's Office. Glover was off duty at the time of the collision.

Kentucky State Police initially assigned the collision investigation to Trooper Cory Jones. The investigation was subsequently transferred to the Kentucky State Police Critical Incident Response Team. On August 17, 2024, at approximately 6:13 p.m., Paulding County Sheriff's Office deputies, including 30-year-old Deputy Brandon Cunningham, responded to a domestic disturbance at a residence on Foggy Creek Lane in Hiram, Georgia. James Samuel Atkins, 42, ambushed the responding deputies, fatally shooting Deputy Cunningham.

During the incident, Atkins also shot his wife, 52-year-old Kim Thao Vu, as she attempted to flee the home. She was transported to an Atlanta-area hospital in serious but stable condition.

Following the shootings, Atkins locked himself inside the residence. Police escalated to a standoff, which lasted several hours, at which point officers forced entry into the home and discovered Atkins deceased from an apparent self-inflicted gunshot wound.

On 08/17/2024, Joseph Nehemiah Powell, 34, of Orange Park was killed when he crashed his motorcycle while Florida Highway Patrol troopers pursued him on Interstate 10. The pursuit began at approximately 3:20 p.m. in Washington County and continued east into Jackson County.

The initiating reason for the attempted traffic stop was specifically no license plate/tag displayed on Powell's motorcycle. An FHP trooper attempted to stop Powell's green 2007 Kawasaki on eastbound I-10 between mile markers 102 and 104. Powell continued driving and the trooper pursued.

During the pursuit, FHP reported that Powell reached speeds approaching 142 mph and at times traveled on the center grass median and emergency shoulder. The pursuit continued from Washington County into Jackson County.

Powell attempted to exit I-10 onto State Road 71 in Jackson County. According to FHP, he attempted to change direction while negotiating the exit, lost control of the Kawasaki and crashed. The pursuing trooper arrived at the crash scene, requested EMS and began CPR, but Powell died from his injuries.

FHP subsequently reported that the motorcycle was unregistered and uninsured and that Powell did not have a driver's license or motorcycle endorsement. Those facts were discovered in connection with the investigation; the stated reason for the original attempted stop was the motorcycle displaying no tag.

The Florida Highway Patrol Traffic Homicide Division investigated the fatal pursuit and crash.

On 08/17/2024, Derek Pickett, 44, was shot and killed by Albuquerque Police Department SWAT Officer Robert Sanchez following a police foot chase and several-hour barricade in a backyard shed in the 400 block of Arno Street SE. The encounter began at approximately 2:20 p.m. at the Family Dollar at 613 Broadway Boulevard SE. An officer was already at the store on an unrelated call when employees ran after Pickett and told the officer that he had stolen merchandise. Later police reporting valued the merchandise at approximately $47.

The officer pursued Pickett on foot over the shoplifting accusation. During the chase, Pickett dropped the merchandise. Police alleged that Pickett displayed a handgun from his waistband and said that he could not go back to jail before continuing away from the officer. A landscaper subsequently told police that Pickett had entered a backyard shed on Arno Street.

Albuquerque officers surrounded the shed and attempted to persuade Pickett to surrender. Officer Truitt Bushnell communicated with Pickett from outside. Police alleged that Pickett fired a shot from inside the shed approximately 16 minutes into the encounter. Bushnell fired back. Neither Pickett nor Bushnell was struck during that exchange.

APD SWAT took control of the scene at approximately 4:16 p.m. Police attempted dozens of times over approximately two hours to establish communication with Pickett and placed a telephone near the shed for him to use.
.
At approximately 6:25 p.m., Pickett opened the shed door and reached for the police telephone while holding a handgun in his other hand. Video released by APD showed Pickett bringing the handgun to his own head. At approximately the same time, SWAT Officer Robert Sanchez fired one rifle round, striking Pickett in the chest, while other officers deployed a less-lethal projectile and a flashbang.

Sanchez later told investigators that he believed Pickett was beginning to raise the firearm toward SWAT personnel. Police subsequently reported that the handgun recovered after the shooting had no magazine inserted and no cartridge in the chamber. APD said the gun had been fired earlier during the standoff.

Pickett died at the scene. APD's Multi-Agency Task Force investigated the shooting, with additional administrative review by Internal Affairs and the Force Review Board.

APD's own account of that shooting identified Sanchez and noted that he had already been involved in another police shooting in 2018.

On 08/16/2024, an unidentified male passenger was killed when a pickup truck crashed at Commerce Way and Atlantic Avenue in Woburn shortly after a Massachusetts State Police trooper attempted to stop the truck on Interstate 93 North. Two other people in the pickup were hospitalized. The driver of a sedan involved in the collision was not injured.

The police encounter began at approximately 9:30 p.m. when a Massachusetts State Police trooper patrolling I-93 North saw the pickup pass the cruiser at a high rate of speed. The trooper activated the cruiser's emergency lights and attempted to stop the pickup specifically because of the speeding.

The pickup continued toward Exit 30 and left I-93. State Police said the trooper terminated the pursuit as the pickup exited the highway. Shortly afterward, the pickup crashed into a sedan at Commerce Way and Atlantic Avenue.

The collision severely damaged both vehicles and knocked out electrical equipment near the intersection. One person traveling in the pickup died, while two other people from the pickup were conscious and alert when transported to hospitals.

On 08/16/2024, Marcus Dunbar, 19, was killed along with two teenage passengers when the Mercury Sable he was driving crashed head-on into a pickup truck and caught fire on Deans Bridge Road in Richmond County during a pursuit by Blythe Police Officer Thomas Ray. Dunbar was not officially identified as the driver until September 2024.

The encounter began at approximately 7:53 p.m. when Officer Thomas Ray clocked the Mercury traveling 71 mph in a 55-mph zone on Deans Bridge Road. Ray attempted to stop the Mercury specifically for the 16-mph speeding violation. Dunbar continued driving and Ray pursued.

The pursuit continued north on Deans Bridge Road. As the Mercury approached Bath Edie Road, Dunbar entered the southbound lanes while continuing north against traffic. Georgia State Patrol reported that Ray did not follow the Mercury into the wrong-way lanes.

After traveling approximately another mile against traffic, the Mercury collided head-on with a southbound pickup truck just north of Etterlee Road. The Mercury caught fire and became engulfed in flames. Dunbar and his two passengers, brothers Barry Fields Jr., 15, and Casey Fields, 13, died at the scene.

On 08/15/2024, at approximately 10:00 p.m., a Graves County sheriff’s deputy located a man at the Wingfield Inn, 1101 West Housman Street in Mayfield, after the Benton County Sheriff’s Office in Tennessee notified Kentucky authorities that he had left the Benton County Courthouse during jury deliberations and might be in the Mayfield area. The Tennessee court had been trying him on charges involving rape of a child, incest and domestic assault. After he left the courthouse, authorities obtained a nationwide extraditable warrant for felony failure to appear. He also reportedly sent communications stating that he intended to kill himself.
The Graves County deputy identified the man after he emerged from a motel room near the vehicle described by Tennessee authorities. The deputy attempted to arrest him, and the two struggled outside the room. The man broke away, returned through the doorway and barricaded himself inside. Additional Graves County deputies and Mayfield police officers arrived, creating a brief standoff outside the room.
The man subsequently opened the motel-room door and came outside holding a handgun. Graves County Sheriff Jon Hayden reported that he pointed the weapon toward the assembled deputies and Mayfield officers. Police fired multiple rounds, striking him. The Graves County coroner pronounced him dead at the motel. Body cameras worn by officers recorded the encounter, and investigators secured the recordings as evidence.
Both the Graves County Sheriff’s Office and Mayfield Police Department requested an independent investigation by the Kentucky State Police Critical Incident Response Team. On 08/15/2024, at approximately 2:23 a.m., Morrisville police responded to a reported armed robbery and carjacking at the Camden Westwood Apartments on West Shore Court. The victim reported being assaulted by four individuals, one reportedly armed with a handgun, before his 2016 BMW SUV was taken. About eight minutes later, a Morrisville police officer located the BMW traveling on Chapel Hill Road toward Durham and initiated a pursuit.
The pursuit continued toward Durham before officers lost sight of the BMW near the Chapel Hill Road/Page Road area. Morrisville police terminated the pursuit. Approximately two hours later, at about 4:41 a.m., a passing motorist discovered the BMW crashed in a field off the 5900 block of South Miami Boulevard in Durham and notified authorities. Durham officers found the 16-year-old driver had been ejected from the vehicle and pronounced him dead at the scene. Investigators determined he was the only person inside the SUV when it crashed.
The crash investigation was conducted by the Durham Police Department while the Morrisville Police Department conducted an internal review of the pursuit. One Morrisville officer involved in the pursuit was placed on administrative leave pending that review. On 08/14/2024, at approximately 10:23 p.m., officers with the Virginia Beach Police Department responded to a residence in the 1200 block of Quarter Way after receiving a report of a possible drug overdose. The first officer to arrive heard a disturbance inside the home, entered the residence, and found 61-year-old Robert "Bobby" Charles Miller and a woman struggling over a knife in an upstairs bedroom.
According to police, the officer repeatedly ordered Miller to drop the knife. Police reported that Miller instead moved toward the officer while still holding the knife. OPEN RECORDS SUMMARY :

On 08/13/2024, League City police responded to a reported aggravated robbery involving an armed man. Officers identified 34-year-old Jonathan Velasquez as the robbery suspect and attempted to stop his vehicle. Velasquez continued driving away while League City officers pursued north toward Houston. The reason for the pursuit was the reported aggravated robbery.

On 08/13/2024, Kansas City police attempted to stop a Kia Soul after determining it had been reported stolen. The reason for the pursuit was the stolen vehicle. On 02/10/2023, 28-year-old Delmy “Patty” Alvarez was killed when a Lexus being pursued by Orlando Police Officer Esequiel Colon crashed into the Honda Civic she was driving at South Parramore Avenue and West Anderson Street in Orlando. Alvarez was completely uninvolved in the police encounter. Her girlfriend, Gina Mustacchio, was riding in the passenger seat and survived with devastating injuries.

The pursuit began at approximately 10:18 p.m. when Colon saw a Lexus run a red light at Westmoreland Drive and South Street in the Parramore neighborhood. Colon later told an Orlando Police internal-affairs investigator that he considered running the red light reckless driving. This traffic violation—not a violent felony—was the specific initiating reason for Colon's decision to follow the vehicle.

Colon began following the Lexus in a marked Ford Explorer. As its driver increased speed, Colon continued behind him through residential city streets. Internal police records subsequently established that the vehicles reached speeds exceeding 60 mph.

Critically, Colon did not activate his emergency lights or siren during the pursuit. He later acknowledged this during the internal investigation. Consequently, Alvarez and other motorists approaching the eventual crash intersection received no emergency lights or siren warning that a high-speed police pursuit was entering the intersection.

The Lexus ran another red light at Parramore Avenue and West Anderson Street and struck the driver's side of Alvarez's Honda. Alvarez died from her injuries. Mustacchio suffered a broken jaw, multiple rib and pelvic fractures, a broken arm and injuries that ultimately cost her a kidney.

Orlando Police Department's own investigation determined that Colon's pursuit violated department policy. OPD policy permitted vehicle pursuits when an officer had reasonable suspicion that the fleeing person had committed or attempted a specified forcible felony. Running a red light did not qualify.

The internal investigation established that Colon himself acknowledged he did not have reasonable suspicion that the driver had committed or attempted a forcible felony and that the incident did not meet OPD's criteria for a vehicle pursuit.

The department nevertheless stopped short of attributing Alvarez's death to Colon. Colon received a 40-hour suspension and additional training for violating the pursuit policy. Under the police union agreement, he was permitted to surrender five days of paid leave instead of losing pay during the suspension.

No independent law-enforcement agency investigated Colon's role in Alvarez's death. The Orlando Police Department investigated both the fatal crash involving its own officer and Colon's policy violations. OPD later said there was no procedure requiring an outside investigation for a crash of this type.

The department's handling of the aftermath became a major part of the case. OPD did not issue a public news release explaining that Alvarez had been killed during a police pursuit and did not tell her mother, Marta Lopez, the full circumstances of her daughter's death. Lopez initially understood only that a reckless driver had run a red light and killed Delmy.

For months, OPD also denied an Orlando Sentinel public-records request by citing an active-investigation exemption even though the administrative investigation of Colon had already concluded. When records were eventually produced, substantial portions were redacted. Unredacted internal records became available through legal discovery in Mustacchio's civil litigation.

Lopez said she first began learning that police had been pursuing the Lexus through Mustacchio rather than through the department. When Lopez later confronted the crash investigator about the existence of a police pursuit, she recalled his response as asking who had told her.

The family has consequently been outspoken in challenging OPD's handling of Alvarez's death. Lopez said she believed the crash could have been prevented and explicitly assigned responsibility both to the pursued driver and to police. She criticized the department for failing to contact her or keep her informed.

Mustacchio likewise became an outspoken critic of the department. She argued that although the pursued driver was responsible for his actions, Colon's decision to chase him without emergency lights or siren materially contributed to the circumstances that killed Alvarez.

Mustacchio sued the City of Orlando, alleging that Colon's unauthorized pursuit caused or contributed to the collision. The lawsuit specifically cited Colon's violation of OPD's restrictive pursuit policy and his failure to use emergency lights and siren.

The City of Orlando settled the litigation in 2025 for $300,000. The settlement did not require the city to admit legal responsibility. Mustacchio said her purpose in bringing the case was accountability and exposing what she believed OPD had attempted to keep from public view.

The criminal investigation of the pursued driver was also marked by an extraordinary delay. Although police possessed evidence from the crash, charges were not submitted to the Orange-Osceola State Attorney's Office until September 2024, approximately 19 months after Alvarez was killed and only days after investigative reporting publicly exposed OPD's role in the pursuit.

Records showed that DNA evidence from the Lexus airbag and suspected marijuana recovered from the vehicle had been submitted to FDLE in April 2023. OPD did not report receiving the DNA result until September 2024. The suspected marijuana was not submitted for testing until September 11, 2024, the day after other charges were forwarded to prosecutors.

The driver was ultimately arrested in February 2025 and prosecuted for the fatal crash. At his 2025 sentencing proceedings, Mustacchio took the unusual position of speaking partly in his defense because she believed Colon and OPD also bore substantial responsibility. Alvarez's mother likewise told the court she believed there were two responsible parties: the driver and the pursuing police officer.

The case gained renewed significance because OPD subsequently experienced another fatal pursuit involving an officer violating the department's pursuit restrictions. In February 2025, Detective Christopher Moulton attempted to stop a driver over an unreadable license plate; the ensuing pursuit ended with 49-year-old Gerald Neal being killed. Prosecutors declined to charge Moulton.

In March 2026, previously unseen video obtained through records requests provided additional visual documentation of the Alvarez pursuit and crash. The footage showed the pursued Lexus running the red light and striking Alvarez's car while Colon had been following without emergency lights or siren.

OFFICER BACKGROUND
Officer Esequiel Colon was the Orlando police officer whose unauthorized pursuit culminated in the crash that killed Delmy Alvarez.

OPD's own internal investigation sustained the pursuit-policy violation. Colon acknowledged that he did not have reasonable suspicion of a qualifying forcible felony and that the circumstances did not meet department pursuit criteria. On 08/12/2024, the Vermillion Police Department received information that 40-year-old Kevin Ray Kirchner was suicidal, was armed with a rifle, and might be violating a protection order. On 08/12/2024, Indianapolis Metropolitan Police Department officers responded to a domestic violence call involving a weapon at a residence on Watersonway Circle. While officers were en route, dispatchers remained on the line with a caller inside the home and responding officers heard multiple gunshots as they arrived. Officers entered the residence to stop the ongoing violence and locate the victims.
Inside the home, officers found two teenage girls suffering from gunshot wounds. Officers then located 41-year-old Ashley Nolan and her husband, Thomas Joseph Nolan, dead in the backyard. Investigators determined that Thomas Nolan fatally shot Ashley before shooting the couple's two daughters and then killing himself with a self-inflicted gunshot wound.
Thomas Nolan was a former Indianapolis Metropolitan Police Department reserve officer who served with the department for more than 20 years before retiring in May 2024. He had also previously served as a Marion County Sheriff's Office reserve deputy and later worked as a school resource officer with Perry Township Schools. Ashley and Thomas Nolan were married, making the shooter both her husband and a former law enforcement officer.
The two teenage daughters survived after being transported to area hospitals with serious gunshot injuries. Indianapolis Metropolitan Police investigators classified the incident as a domestic violence-related murder-suicide. Following the shooting, family members established a fundraising campaign to assist the daughters with their recovery.

On 08/12/2024, Alexandria police responded to the Alexandria Overlook Apartments at 5101 Maris Avenue after one of Paul G. Behan Jr.'s coworkers reported that Behan was intoxicated, despondent over a recent job suspension, experiencing marital problems, armed with a firearm, and threatening suicide. Officers responded to conduct a welfare check. When Behan opened the apartment door, he was holding a Beretta CX4 Storm 9 mm carbine under his chin and repeatedly stated that he wanted to kill himself. Officers immediately backed away, created distance, and began negotiating with him.
Over the next approximately 24 minutes, patrol officers and a trained crisis negotiator repeatedly attempted to persuade Behan to surrender peacefully. Body-worn camera footage later released by the Alexandria Police Department showed officers encouraging him to put the rifle down, reminding him that he had reasons to live, and attempting to de-escalate the encounter. During the standoff, Behan twice raised and pointed the rifle toward officers. After the second time he aimed the weapon at police, a Special Operations officer fired three rounds, striking Behan. He was pronounced dead at the scene.
The Alexandria Police Department requested an independent criminal investigation by an outside law enforcement agency. The investigation was presented to Alexandria Commonwealth's Attorney Bryan Porter, who concluded in October 2024 that the officer's use of deadly force was legally justified and declined to file criminal charges. Following that decision, the department released body-worn camera footage and investigative materials. The officer who fired was described as a nine-year veteran assigned to the Special Operations Team, but Alexandria Police did not publicly identify him by name.
Following release of the body-camera footage, Interim Chief Raul Pedrosa stated the department would conduct a separate administrative review examining policy compliance, training, supervision, and crisis-response practices. Public reporting through 2026 identified no civil lawsuit filed by Behan's family and no disciplinary action against the involved officer.
