On 11/16/2025 at approximately 9:30 p.m. law-enforcement officers attempted to stop a vehicle driven by Benjamin Sink who was wanted on several charges related to a domestic relationship. When the vehicle failed to stop a pursuit ensued. The vehicle was disabled on the side of the road. Sink exited and allegedly opened fire on officers. Officers returned fire and struck him. He was transported to a hospital where he was pronounced dead.

On 11/10/2025, 28-year-old Kevin Fajardo died from a self-inflicted gunshot wound as officers executed a search warrant at a home on the 1200 block of Walnut Street in West Des Moines. Fajardo had been identified as the suspect in an October 26 shooting in Des Moines that seriously injured a 16-year-old girl. Officers announced their presence before entering and used a distraction device. After forcing entry, they found Fajardo dead with a firearm equipped with an automatic-fire conversion device nearby. The Iowa Division of Criminal Investigation is assisting in the investigation.

On 11/04/2025, the Carroll County Sheriff’s Office responded at approximately 9:15 a.m. to a field near 300th Street and Mahogany Avenue after a report of a male in possession of a firearm and expressing suicidal ideation. The individual was identified as 58-year-old Lance Allen Meaike of Audubon and was found deceased at the scene from a self-inflicted gunshot wound. Meaike had been charged in South Dakota with first-degree manslaughter and aggravated assault with a deadly weapon in connection to a June 12 shooting death. The scene remains under investigation.

On 08/27/2025 around 5:35 p.m., an Iowa State Trooper attempted a traffic stop near the 77-mile marker on Interstate 29. The driver, identified as 48-year-old Michael Broyles of Battlefield, Missouri, pulled a shotgun on the trooper and fled. Police pursued into Council Bluffs, where the chase ended near the Pottawattamie County Jail around 6:30 p.m. Broyles exited his vehicle with a firearm and fired at officers. Police opened fire, fatally striking him. He was transported to a hospital and pronounced dead.

Police opened fire and shot the man. He was transported to a Des Moines hospital where he was later pronounced dead. The woman was hospitalized and later released. On 08/10/2025, at approximately 1:13 p.m., Creston Police attempted to stop a 2022 Harley-Davidson Street Glide Special motorcycle driven by 55-year-old Tony D. Fizer for speeding over 70 mph in a 55 mph zone. The driver did not stop, and police chased him eastbound on Highway 34 at speeds exceeding 100 mph. About one mile east of Afton near mile marker 94, the motorcycle failed to navigate a curve, left the roadway, and overturned. Fizer was ejected into a ditch and sustained fatal injuries. He was transported to Greater Regional Health in Creston, where he died.

On July 13, 2025, at approximately 1:30 a.m., officers with the Iowa State Patrol attempted to stop a 2015 Ford Fusion driven by 22-year-old Zachary Hill, a resident of Forest City, Iowa. The pursuit, reportedly initiated for unknown reasons, involved multiple agencies and escalated into a high-speed chase across Cerro Gordo County. Officers did not disclose the precise speed reached or the reason for the attempted stop.
The pursuit ended at approximately 1:40 a.m. in a residential neighborhood near the intersection of 4th Avenue South and South Shore Drive in Clear Lake. Hill lost control of the vehicle and crashed into two parked vehicles, a tree, and a house. The impact caused severe damage to the car and surrounding property.
Emergency responders arrived quickly at the scene. Hill was transported to MercyOne North Iowa Medical Center in Mason City, where he was pronounced dead shortly afterward. No one else was reported injured during the crash, and no passengers were present in the vehicle.
The Iowa State Patrol is leading the ongoing investigation, which includes crash reconstruction and review of dashcam footage. On Sunday, July 13, 2025, around 7:15 p.m., Des Moines police officers responded to a call for assistance at a residence in the 2800 block of Grand Avenue, where a woman was reportedly experiencing a mental health crisis. Family members or neighbors had called 911 reporting that Black was armed with a knife and threatening to harm herself inside the home.
Responding officers arrived and attempted to negotiate with the woman, who was reportedly in a highly distressed and agitated state. According to initial police statements, officers spent several minutes attempting to de-escalate the situation verbally. Despite this, she allegedly advanced toward the officers with the knife.
Police then opened fire, striking her. She was provided emergency medical attention at the scene and then transported to a local hospital, where she was pronounced dead shortly afterward. No officers were physically injured during the incident.
The Iowa Division of Criminal Investigation (DCI) is conducting an independent investigation into the shooting.

On April 15, 2025, Dallas County Sheriff's deputies conducted a traffic stop on Interstate 80 near Earlham for a vehicle speeding. On March 6, 2025, at approximately 4:30 a.m., deputies from the Shelby County Sheriff's Office conducted a welfare check at a residence on 12th Street in Harlan, Iowa. Upon arrival, they discovered the body of 31-year-old Alexis Martin inside the home. During the investigation, deputies encountered 36-year-old David Plagmann, who was armed. A standoff ensued, during which Plagmann made aggressive movements toward law enforcement. In response, a deputy shot Plagmann. Despite lifesaving measures, Plagmann was pronounced dead at a local hospital. The deputy involved has been placed on administrative leave, pending an investigation.

David Leroy Kintz Jr., 43, was shot and killed by Des Moines Police Department officers Neal Shaffer and Titus DeFord on February 6, 2025. Officers responded to an incident at the 1200 block of Maury Street around 8:40 p.m. Details on the encounter leading to the shooting remain limited. Authorities have not disclosed if Kintz was armed or the specific circumstances of the officers’ use of deadly force.

On 12/08/2024, at approximately 3:15 p.m., Muscatine police responded to reports of a woman firing shots at individuals while driving through the area. Brittany Dale Miles, 36, of Cedar Rapids, fled from officers when they attempted a traffic stop on Park Avenue in Muscatine.

Police escalated a high-speed pursuit, which extended beyond city limits and onto U.S. Highway 218 in Washington County. During the chase, a Muscatine Police Department vehicle crashed near the Cedar River, resulting in non-life-threatening injuries to the officer.

The pursuit continued until Miles drove into oncoming traffic on U.S. Highway 218 and collided head-on with another vehicle, driven by Olivia Marie Alvarez, 27, of Cedar Rapids. Both Miles and Alvarez died as a result of the collision.

The Iowa State Patrol is leading the investigation into the fatal crash. No officers were injured in the initial shooting incidents reported during the pursuit.

Officers Cade Moritz, Chad Steffen, Jacob Boekhoff, and Frankie Contreras fatally shot Joshua Dean Greene, a 37-year-old White man, on 09/17/2024. Upon their arrival, officers reported encountering Greene armed with a handgun. Greene allegedly fired in the direction of officers, and Officer Cade Moritz returned fire, striking Greene. Officers Chad Steffen, Jacob Boekhoff, and Frankie Contreras also fired their guns during the confrontation.

The incident began when Des Moines Police attempted to stop Greene for a traffic violation near Easton Boulevard and Hubbell Avenue. Greene did not stop. The officers escalated a "brief pursuit" that ended when his vehicle crashed into a semi-truck near East 24th Street and Easton Boulevard. After the crash, Greene allegedly revved the engine and displayed a firearm. Tasers deployed by officers were ineffective, and Greene reportedly fired at them, striking Officer Cade Moritz in the head and Officer Jacob Boekhoff in the arm.

Officers returned fire, discharging a total of 51 rounds. Officer Moritz underwent emergency surgery for his injuries, and Officer Boekhoff required surgery for his arm injury. Both officers were transported to a local hospital and are currently on administrative assignment as they recover. greene was pronounced dead at the scene.

The Iowa Attorney General's Office reviewed the case and determined the officers' use of deadly force was legally justified. Body camera footage released by the Des Moines Police Department provided detailed visuals of the encounter, including the crash and the ensuing confrontation.

On September 5, 2024, at approximately 6:30 p.m., officers from the Mount Pleasant Police Department responded to a domestic violence call. During their response, a man drove by the scene, and officers attempted to initiate contact. The man failed to stop and continued driving through intersections.

Near the corner of West Henry Street and North Jefferson Street, the man, while still in his vehicle, fired multiple shots at a Mount Pleasant police officer inside a patrol car. The officer was not injured. The man fled, and officers pursued.

On August 28, 2024, at approximately 1:00 a.m., Iowa State Patrol troopers attempted to stop a speeding Hyundai Elantra on Interstate 235 near 31st Street in Des Moines. The driver, a 16-year-old male, did not stop. This escalated a pursuit during which the vehicle exited the interstate and proceeded eastbound on Grand Avenue, traveling on the wrong side of the road.

On July 30, 2024, at approximately 3:30 a.m., Des Moines Police Department officers responded to a report of a protective order violation in the 5800 block of SE 35th Circle. Weston Dean Derby, a 37-year-old male from Bellevue, Nebraska, was located driving near the scene. Officers attempted to stop him, but Derby did not comply.

Police escalated a chase that ended in the 4300 block of East Army Post Road when Derby's vehicle left the roadway and became stuck in dense vegetation. Derby exited the vehicle and attempted to flee on foot. During the pursuit, he produced what appeared to be a gun, initially pointing it at his own head before directing it toward the pursuing officers. Officers Jesse Schneider and Anthony Lampman opened fire, striking Derby.

Officer Schneider sustained a gunshot wound to the shoulder, believed to be the result of friendly fire. He was hospitalized in stable condition.

Upon arrival, officers located 35-year-old Kelvin Lee Plain Jr. in Sullivan Park, matching the provided description. As officers approached, Plain fled on his bicycle and then on foot through the park. Officers fired a Taser, causing Plain to fall to the ground. While on the ground, Plain fired a handgun at the officers, striking one officer inside a patrol vehicle and another who was on foot. Both officers returned fire, fatally wounding Plain. The injured officers were transported to a local hospital in stable condition; one was expected to be released the following day. The incident was captured on a police vehicle's dash camera. On June 24, 2024, at approximately 1:30 p.m., the Taylor County Sheriff's Office responded to a reported robbery at the Iowa State Savings Bank in Lenox, Iowa. Steven Trent Sapp, a 45-year-old male from Hooper, Nebraska, demanded money and fled the scene on a motorcycle. Law enforcement initiated a pursuit, which extended into Adams County.

During the chase, Sapp lost control of his motorcycle and crashed into a farm field. He fled on foot, and as officers closed in, they heard a single gunshot. Officers discovered Sapp deceased from a self-inflicted gunshot wound.

A forensic autopsy conducted by the Iowa State Medical Examiner's Office confirmed that Sapp died from a single gunshot wound, ruling his death a suicide.

On 06/15/2024, the Iowa State Patrol attempted to stop 24-year-old Donovan Burrell on Old Lakeport Road in Sergeant Bluff, Iowa. Burrell did not stop and continued driving. Troopers escalated a high-speed pursuit, during which the vehicle reached high speeds.

On June 1, 2024, off-duty Waterloo Police Officer Keaton Northup was driving his motorcycle with 28-year-old Jenna Mills as a passenger. Investigators say Northup was intoxicated with a blood alcohol level of .292—more than three times the legal limit—and had been drinking throughout the day. He was traveling at approximately 100 mph when he lost control and crashed into a metal barrier on Interstate 380. Jenna Mills was killed in the crash. Northup was hospitalized with injuries. He was later charged with homicide by vehicle (OWI) and homicide by vehicle—reckless driving. He remains employed by the Waterloo Police Department on administrative leave.
