On July 13, 2025, at approximately 1:30 a.m., officers with the Iowa State Patrol attempted to stop a 2015 Ford Fusion driven by 22-year-old Zachary Hill, a resident of Forest City, Iowa. The pursuit, reportedly initiated for unknown reasons, involved multiple agencies and escalated into a high-speed chase across Cerro Gordo County. The pursuit ended at approximately 1:40 a.m. in a residential neighborhood near the intersection of 4th Avenue South and South Shore Drive in Clear Lake. On Sunday, July 13, 2025, around 7:15 p.m., Des Moines police officers responded to a call for assistance at a residence in the 2800 block of Grand Avenue, where a woman was reportedly experiencing a mental health crisis. Family members or neighbors had called 911 reporting that Black was armed with a knife and threatening to harm herself inside the home.
Responding officers arrived and attempted to negotiate with the woman, who was reportedly in a highly distressed and agitated state. According to initial police statements, officers spent several minutes attempting to de-escalate the situation verbally. Despite this, she allegedly advanced toward the officers with the knife.

The vehicle had struck a curb, left the roadway, and collided with a tree, resulting in all occupants being ejected. The driver, identified as 15-year-old James "JT" Taylor III, and two passengers, 18-year-old Jamarion Jackson and 15-year-old Eric Vortes Jr., sustained fatal injuries. An 18-year-old, Tarez Lee Matthew Williams, was critically injured and transported to a local hospital where he died later.
