County : Maricopa
State : Arizona
Agency : Maricopa County Attorney’s Office
Officer(s) : John C. Byrd III
Cause of Death : Asphyxiation
Event : Strangled to death by off-duty husband, detective Byrd
OPEN RECORDS SUMMARY :
On 07/31/2024, 43-year-old Elizabeth Byrd was killed inside her home in the Crimson Creek neighborhood near Baseline Road and Crismon Road in Mesa. Investigators alleged that during an argument, her husband, Detective John C. Byrd III of the Maricopa County Attorney's Office, strangled her. Authorities said Byrd then locked her body inside a bedroom and left the residence, leaving the couple's three children at the home.
After family members became concerned and requested a welfare check, officers entered the residence and found Elizabeth Byrd dead. Investigators later located John C. Byrd III driving his wife's vehicle and took him into custody without incident.
According to court documents, Byrd told investigators that he "flipped a switch" during the altercation. He was charged with second-degree murder and child abuse. The Maricopa County Sheriff's Office led the homicide investigation because the accused was a law enforcement officer employed by the Maricopa County Attorney's Office. As of the latest public reporting, the criminal case remained pending.

Location : Unknown
City : Boynton Beach
County : Palm Beach
State : Florida
Agency : West Palm Beach Police Department
Officer(s) : Austin B. Danielovich, Pierre F. Etienne, Christopher C. Rekdahl, Michael E. Borgen, William L. Loayza, Brandan C. Stedfelt, Darien J. Thomas
Cause of Death : Vehicle
Event : Pregnant bystander killed by police chase crash
OPEN RECORDS SUMMARY :
On 07/31/2024, officers with the West Palm Beach Police Department pursued a vehicle driven by Neoni Copeland through Palm Beach County. During the pursuit, Copeland drove into the intersection of North Congress Avenue and Meadows Boulevard in Boynton Beach and crashed into the vehicle carrying 27-year-old Jenice Woods and her mother, Marcia Pochette. Woods, who was approximately two months pregnant, her unborn child, and Pochette all died from their injuries.
The Palm Beach County State Attorney's Office Public Corruption Unit investigated the pursuit. In June 2025, seven West Palm Beach police officers were criminally charged following the investigation. Officers Austin B. Danielovich, Pierre F. Etienne, and Christopher C. Rekdahl were each charged with two counts of leaving the scene of a crash involving death and one count of official misconduct. Officers Michael E. Borgen, William L. Loayza, Brandan C. Stedfelt, and Darien J. Thomas were each charged with one count of official misconduct. Prosecutors alleged the pursuit violated department policy and that officers failed to stop and render aid after the fatal collision. All seven officers were subsequently terminated by the department while the criminal cases proceeded. Police were reluctant to answer questions, but the chase was reportedly done for someone having too dark window tint.
Woods' family publicly questioned why the pursuit had been initiated and called for accountability, saying they were still seeking answers about the decisions that led to the crash.

On 07/31/2024, West Palm Beach Police Department officers attempted to stop a vehicle driven by Neoni Copeland. The reason for the attempted stop was never publicly confirmed by the department. Copeland continued driving while police pursued him from West Palm Beach into Boynton Beach. Brennan Russell
Age :29
Gender :Male
Race : White
Date : 07/31/2024
Location : McDonald’s on 545 North Hillside Street
City : Wichita
County : Sedgwick
State : Kansas
Agency : Wichita Police Department
Officer(s) : Not Disclosed
Cause of Death : Gunshot
Event : Killed by police during traffic stop
OPEN RECORDS SUMMARY :
On 07/31/2024, Wichita Police Department officers conducting surveillance near a location under investigation for drug trafficking observed a black Chevrolet Impala with an altered and expired registration tag leaving the area. Officers initiated a traffic stop, and the vehicle pulled into the McDonald's parking lot at 545 North Hillside Street in Wichita.
Officers contacted the three occupants and observed 29-year-old Brennan Russell, who was seated in the rear passenger seat, making movements consistent with concealing an object beneath the seat. Officers ordered Russell from the vehicle and advised him he was under arrest. Russell refused to comply, and officers requested additional units. A Taser was deployed through the open rear passenger door but was ineffective.
According to investigators, Russell then drew a handgun from his waistband and pointed it toward one of the officers. A second officer, positioned behind the vehicle, observed Russell point the firearm at the first officer before turning the gun toward his own head. Russell fired a single shot into his head. Almost simultaneously, the first officer fired five rounds, striking Russell four times in the arm and torso. Officers provided CPR until EMS arrived, but Russell was pronounced dead at the scene.
The Sedgwick County Sheriff's Office conducted the independent investigation. Body-worn camera footage, witness statements, forensic evidence, and the autopsy were reviewed by the Sedgwick County District Attorney. In March 2026, District Attorney Marc Bennett concluded there was insufficient evidence to disprove that the officer acted in lawful self-defense and declined to file criminal charges. The coroner determined Russell's self-inflicted gunshot wound would have been fatal on its own, but the officer's gunfire likely accelerated his death, resulting in the manner of death being classified as homicide.
The district attorney's report also stated that investigators recovered a Kimber Micro 9 pistol, suspected narcotics, scales, packaging materials, and other drug-related items from the vehicle. Toxicology testing found fentanyl and other controlled substances in Russell's system at the time of his death.

Patrick Frank Whitten
Age :31
Gender :Male
Race : White
Date : 07/31/2024
Location : Unknown
City : Price
County : Carbon
State : Utah
Agency : Price Police Department, Helper Police Department, and the Carbon County Sheriff's Office
Officer(s) : Not Disclosed
Cause of Death : Gunshot
Event : Killed by police during domestic call
OPEN RECORDS SUMMARY :
On 07/30/2024 at approximately 11:36 p.m., officers from the Price Police Department, Helper Police Department, and the Carbon County Sheriff's Office responded to a reported domestic violence incident at a residence just outside the Price city limits. During the investigation, officers learned that 31-year-old Patrick Frank Whitten had left the residence before they arrived. Officers were informed that Whitten was potentially armed, and a search of the surrounding property was initiated.
According to investigators, officers searched approximately three acres of property for about three hours before locating Whitten hiding inside a small outdoor lean-to or animal enclosure near the residence. Officers ordered him to drop his firearm. Investigators said Whitten ignored repeated commands, held the handgun in a low-ready position with both hands, and then raised it toward the officers. Five officers from the responding agencies fired their weapons, striking Whitten multiple times. He was treated at the scene and transported to a local hospital, where he died from his injuries.
The Utah State Bureau of Investigation conducted the Officer Involved Critical Incident investigation. Investigators reported that the original domestic violence call alleged Whitten had been under the influence of a controlled substance and had threatened to kill his spouse before leaving the residence armed.
In September 2024, Carbon County Attorney Christian Bryner completed his review of the investigation and determined that the five officers who fired their weapons were legally justified in using deadly force because they reasonably believed Whitten posed an imminent threat of death or serious bodily injury after raising the firearm toward them. No criminal charges were filed against the officers. Their identities have not been publicly released.

Weston Dean Derby
Age :45
Gender :Male
Race : White
Date : 07/30/2024
Location : E Army Post Road and SE 45th Street
City : Des Moines
County : Polk
State : Iowa
Agency : Des Moines Police Department
Officer(s) : Jesse Schneider, Anthony Lampman
Cause of Death : Gunshot
Event : Killed by police during pursuit
OPEN RECORDS SUMMARY :
On 07/30/2024 at approximately 3:30 a.m., Des Moines Police Department officers responded to a report that 45-year-old Weston Dean Derby had violated a recently issued protective order by appearing at his former girlfriend's residence in the 5800 block of SE 35th Circle. The caller reported that Derby was acting erratically and left the area in what appeared to be a stolen SUV. Officer Jesse Schneider located the vehicle nearby and attempted a traffic stop. Derby continued driving while officers pursued.
The pursuit ended when Derby drove off East Army Post Road into dense vegetation, where his vehicle became disabled. Derby exited and ran on foot while holding what appeared to be a handgun. Officers repeatedly ordered him to stop and drop the weapon. Body-worn camera footage showed Derby briefly placing the weapon against his own head before turning and pointing it toward Officers Schneider and Anthony Lampman. Both officers then fired, striking Derby multiple times. Derby died at the scene. The object he was carrying was later determined to be a Gamo P-27 .177-caliber pellet pistol that investigators said was nearly indistinguishable from a semi-automatic handgun.
During the exchange of gunfire, Officer Jesse Schneider was struck in the back shoulder by a round fired by Officer Anthony Lampman. Schneider was hospitalized for approximately one week before being released to recover. Investigators concluded the injury resulted from friendly fire.
The Iowa Division of Criminal Investigation investigated the shooting. Polk County Attorney Kimberly Graham and the Iowa Attorney General's Office independently reviewed the evidence, including body-worn camera footage, witness statements, forensic evidence, and scene reconstruction. Both concluded that the officers reasonably believed Derby posed an imminent threat when he pointed the weapon toward them and determined that the use of deadly force was legally justified. No criminal charges were filed, and the Attorney General formally closed the investigation in September 2024.

Gawon Kaerey Benson
Age :31
Gender :Male
Race : Black
Date : 07/30/2024
Location : Intersection of East Campus Drive and Kettler Circle, Purdue University Fort Wayne Campus
City : Fort Wayne
County : Allen
State : Indiana
Agency : Fort Wayne Police Department
Officer(s) : Officer Jason Fuhrman
Cause of Death : Gunshot
Event : Killed by police during apprehension for community corrections violation
OPEN RECORDS SUMMARY :
On 07/30/2024, officers with the Fort Wayne Police Department were asked by DeKalb County Community Corrections to locate 31-year-old Gawon Kaerey Benson after he failed to return to custody from a community release program. Officers located Benson on the Purdue University Fort Wayne campus near the intersection of East Campus Drive and Kettler Circle.
Body-worn camera footage released by the Fort Wayne Police Department shows officers repeatedly ordering Benson to drop an object in his hand. Investigators determined the object was a loaded handgun. According to investigators, Benson manipulated the slide of the firearm while officers continued issuing commands. Officer Jason Fuhrman then fired multiple rounds, striking Benson. Officers rendered medical aid until paramedics arrived, but Benson died at the scene.
Following the shooting, investigators reported finding a handwritten note in Benson's pocket that they said suggested he intended to provoke a fatal encounter with police. Fort Wayne Police publicly stated the shooting appeared consistent with "suicide by cop." Benson's family disputed that conclusion, arguing that the released body-camera footage did not conclusively support the claim and criticizing the department for releasing only edited video. Family members also stated Benson had no history of violent offenses and said he had been incarcerated because of unpaid traffic-related violations and subsequent community corrections violations. They questioned whether deadly force was necessary under the circumstances.
The Allen County Prosecutor's Office reviewed the Indiana State Police investigation, body-camera footage, witness statements, forensic evidence, and the firearm recovered at the scene. In April 2025, Allen County Prosecutor Michael McAlexander concluded Officer Jason Fuhrman's use of deadly force was legally justified, finding no evidence of criminal liability. The prosecutor determined Benson's actions with the handgun created an imminent threat that justified the use of deadly force under Indiana law. No criminal charges were filed against Officer Fuhrman.
Officer Jason Fuhrman has served with the Fort Wayne Police Department since 2004. Prior to this incident, he had received multiple commendations and had also been involved in several civil lawsuits alleging excessive force, none of which resulted in findings of liability against him.

Location : Jackson Highway at Murieta D
City : Rancho Murieta
County : Sacramento
State : California
Agency : California Highway Patrol (CH
Officer(s) : Not Disclosed
Cause of Death : Vehicle
Event : Driver killed by police chase
OPEN RECORDS SUMMARY :
On 07/29/2024 at approximately 8:15 p.m., California Highway Patrol officers attempted to stop a Hyundai that had been reported stolen. The driver, later identified as 36-year-old James William Toomey, continued driving while CHP officers pursued the vehicle through Sacramento County. During the pursuit, investigators reported the vehicle reached speeds exceeding 120 mph and traveled more than 15 miles before entering the Rancho Murieta area.
The pursuit ended at the intersection of Jackson Highway and Murieta Drive when Toomey's vehicle crossed into the intersection and broadsided a Rivian SUV occupied by three people. The Hyundai then struck a utility pole and burst into flames. Two CHP officers pulled Toomey and his female passenger from the burning vehicle. Both sustained critical injuries and were transported to area hospitals. Toomey died from his injuries the following morning. His female passenger survived with major injuries. The three occupants of the Rivian were transported to hospitals with moderate injuries.
The Sacramento County Coroner's Office identified the deceased driver as James William Toomey. CHP later identified the two pursuing officers who rescued the occupants from the burning Hyundai as Officers Brandon Rodgers and Jim Dann of the Amador Area office. Witnesses described the officers entering the burning vehicle despite heavy flames to remove both occupants.
As of the latest publicly available information, CHP has not publicly released the results of an internal pursuit review, and no public finding regarding compliance with pursuit policy has been issued. Court records indicate that at least one occupant of the struck Rivian later alleged that CHP negligently continued the high-speed pursuit through populated areas and subsequently filed legal claims arising from the collision.

Location : 2630 S Halm Avenue
City : Los Angeles
County : Los Angeles
State : California
Agency : LAPD, SWAT
Officer(s) : Timothy Jang
Cause of Death : Gunshot
Event : Killed by SWAT during standoff
OPEN RECORDS SUMMARY :
On 07/29/2024 at approximately 3:20 a.m., Los Angeles Police Department officers responded to a report of a burglary in progress in the 2600 block of South Halm Avenue. According to investigators, 31-year-old Lavell Adams attempted to break into one residence before entering a neighboring home occupied by an elderly woman, who fled the residence. A fire subsequently started inside the home, prompting a response from the Los Angeles Fire Department.
Firefighters entered the residence but encountered Adams holding a knife and withdrew. LAPD officers attempted to take Adams into custody, but investigators reported that Adams lunged toward officers while armed. An officer fired two beanbag rounds, after which Adams barricaded himself inside the residence. Metropolitan Division SWAT officers responded and established containment around the home.
According to the LAPD, Adams later exited through a side window covered with a blanket. SWAT officers repeatedly ordered him to show his hands, drop the blanket, and comply with commands. Investigators stated that Adams then revealed a large butcher knife and advanced toward the officers. Officer Timothy Jang fired his weapon while another officer simultaneously deployed a TASER. State : Illinois
Agency : Carroll County Sheriff's Department
Officer(s) : Matthew Herpstreith
Cause of Death : Vehicle
Event : Pedestrian believed killed by officer
OPEN RECORDS SUMMARY :
On 07/28/2024, at approximately 3:30 a.m., off-duty Carroll County Sheriff's Deputy Matthew Herpstreith and off-duty dispatcher Amy Hubble reported finding 18-year-old Jackson Kradle lying along Illinois Route 78 near Lowden Road outside Mount Carroll.
An autopsy determined Kradle died from blunt force trauma after being struck by a motor vehicle. During the Illinois State Police investigation, Kradle's DNA was recovered from the underside of Herpstreith's pickup truck. Investigators also obtained surveillance video showing Herpstreith washing the underside of his truck at a car wash shortly after leaving the scene. During a second interview with investigators, Herpstreith acknowledged he may have struck Kradle after initially stating he had swerved to avoid an object in the roadway.
In January 2025, a Carroll County coroner's inquest heard testimony from investigators, forensic experts, and witnesses. The jury determined that Kradle's manner of death was homicide. Herpstreith and Hubble, both of whom had resigned from the sheriff's office, invoked their Fifth Amendment rights during the inquest.
On 03/24/2025, former Deputy Matthew Herpstreith was charged with reckless homicide, reckless conduct, obstructing justice, attempted obstructing justice, and failure to reduce speed to avoid an accident. The criminal case remains pending.

Location : The Pinnacle apartments on Main Street
City : Fort Lee
County : Bergen
State : New Jersey
Agency : Fort Lee Police Department
Officer(s) : Tony G. Pickens, Jr.
Cause of Death : Gunshot
Event : Killed by police during mental health crisis
OPEN RECORDS SUMMARY :
On 07/28/2024, Fort Lee Police Department officers responded to The Pinnacle apartments after Victoria Lee's brother called 911 requesting medical assistance because she was experiencing a mental health crisis. During the call, he advised dispatch that Lee had access to a small folding knife. Before officers entered the apartment, Lee and her mother told them not to come inside and closed the door.
Officers forced entry into the apartment. Shortly after the door was breached, Officer Tony G. Pickens Jr. fired a single shot, striking 25-year-old Victoria Lee in the chest. She was transported to Englewood Hospital, where she was pronounced dead.
The Attorney General later released body-worn camera footage, conducted energy device video, radio traffic, and 911 recordings from the incident. Lee's family disputed the police account, stating she was holding only a plastic water bottle when she was shot and that officers unnecessarily escalated the encounter by forcing entry instead of continuing de-escalation efforts. They have maintained that Victoria was experiencing a mental health crisis and primarily posed a danger to herself rather than to others.
The Attorney General's Office conducted the investigation and presented the case to a state grand jury. In July 2025, the grand jury voted not to indict Officer Tony G. Pickens Jr., concluding no criminal charges would be filed.
In July 2026, Victoria Lee's family filed a federal wrongful death and civil rights lawsuit against the Borough of Fort Lee and several police officers, alleging excessive force, failure to use appropriate de-escalation techniques, and violations of Lee's constitutional rights. The civil litigation remains pending.
