Officer(s) : Not Disclosed
Cause of Death : Vehicle
Event : Bystander killed by police chase crash
OPEN RECORDS SUMMARY :
REASON FOR CHASE:
Peoria police officers heard gunfire and received a ShotSpotter alert reporting approximately 20 gunshots. Police saw a vehicle leaving the area and attempted a traffic stop, then escalated to chase, running police vehicle over pedestrian.
.
NARRATIVE:
On 08/22/2026 at approximately 2:45 a.m., an unidentified adult male pedestrian was killed during a Peoria Police Department pursuit on the city's South Side.
.
Peoria police officers were near the 1400 block of SW Jefferson Street when they heard gunfire. At approximately the same time, ShotSpotter registered 20 rounds fired in the area. Police saw a vehicle leaving the scene and attempted to conduct a traffic stop. The driver fled and police pursued the vehicle.
.
During the pursuit, the fleeing vehicle traveled west and collided with another vehicle at the intersection of Howett Street and Western Avenue. The driver continued after that collision and struck an adult male pedestrian. The vehicle then crashed into a utility pole. The pedestrian was pronounced dead at the scene.
.
The driver and passenger of the fleeing vehicle, both adult men, were hospitalized. One was reported in critical condition and the other in serious condition. Police said a firearm was recovered from their vehicle.
.
The pursuit originated from a separate shooting at the Jefferson Street location. Police subsequently found an adult male suffering from multiple gunshot wounds there. He was transported to a hospital and died shortly afterward. A second shooting victim arrived at a hospital in a private vehicle with a gunshot wound to his leg and was expected to survive. The shooting death is a separate homicide and is not included as a police-triggered fatality in this report.
.
The Illinois State Police are investigating the fatal pursuit crash, while the Peoria Police Department is investigating the original shooting. The Peoria County Coroner's Office had not publicly identified either deceased man as of the latest available reporting.
.
This is notably the second traffic-related death within a month connected to a Peoria Police Department action, according to WCBU.

Agency : Leon County Sheriff’s Office
Officer(s) : Not Disclosed
Cause of Death : Vehicle
Event : Pedestrian killed by police vehicle
OPEN RECORDS SUMMARY :
On 08/22/2026, a pedestrian was struck and killed by a Leon County Sheriff’s Office vehicle on the west side of Tallahassee.
.
According to initial reporting by the Tallahassee Democrat, the collision involved an LCSO SUV. The pedestrian died from injuries sustained in the collision.
.
The circumstances preceding the collision remain under investigation. Initial reporting does not establish that the deputy was involved in a pursuit, nor does it establish that the deputy was responding to an emergency call at the time of the collision. Those details should therefore remain open pending release of the crash investigation.
.
The Leon County Sheriff’s Office also has not publicly identified the deputy driving the SUV.
.
This is particularly worth following because another Leon County Sheriff’s Office SUV was involved in a separate crash Saturday evening, less than 24 hours after the fatal pedestrian collision. The Tallahassee Democrat reported the second crash occurred in northwest Tallahassee.

Cause of Death : Gunshot
Event : Killed by police during suspicious-person investigation
OPEN RECORDS SUMMARY :
NARRATIVE:
.
On 08/21/2026 shortly after 8:00 p.m., Cambridge Police Department officers responded to Temple Street in Central Square after a caller reported suspicious people who had been in a loading dock or parking lot behind area businesses for an extended period.
.
Officers contacted the people in the parking lot. Police claimed a man in his 60s who had been sitting inside a vehicle got out and pointed a loaded firearm at officers. Officers then attempted to physically disarm him, resulting in a physical struggle. Multiple shots were fired during the encounter, striking the man. Authorities have not yet disclosed how many officers fired or the number of shots.
.
Police provided medical aid, but the man was pronounced dead at the scene. Authorities said a loaded firearm was recovered from him. Several Cambridge police officers were taken to area hospitals as a precaution and later released.
.
The officers involved were wearing body cameras, and investigators are reviewing the recordings. The Middlesex District Attorney's Office is investigating the fatal shooting.
.
This was the second fatal Cambridge police shooting in just over three years. On 01/04/2023, Cambridge Officer Liam McMahon shot and killed 20-year-old Sayed Faisal during a mental health crisis. Faisal's killing led to substantial public criticism of the department and calls for greater transparency; Cambridge subsequently implemented department-wide body cameras in April 2025.

Location : Webster Court
City : Newington
County : Hartford
State : Connecticut
Agency : Hartford Police Department; Rocky Hill Police Department
Officer(s) : Not Disclosed
Cause of Death : Gunshot
Event : Shot and killed by police after vehicle pursuit
OPEN RECORDS SUMMARY :
REASON FOR CHASE:
Hartford police were investigating what they described as suspicious or illegal activity not otherwise specified on Fales Street in Hartford when they attempted to arrest a man. He fled on foot and, during a struggle with a pursuing Hartford officer, police alleged they believed he had a firearm in his waistband. He subsequently entered a vehicle and fled, and police pursued him from Hartford into Newington.
.
NARRATIVE:
On 08/21/2026 at approximately 11:07 p.m., an unidentified man was shot and killed by police after a pursuit that began in Hartford and ended on Webster Court in Newington, Connecticut.
.
Hartford police said officers were investigating what the department described as illegal activity on Fales Street when they attempted to arrest the man. He ran from officers. During the foot pursuit, an officer caught up with him and a struggle occurred. Police alleged that the officer believed the man had a firearm in his waistband.
.
The man escaped from the officer, entered a vehicle and drove away. Police pursued the vehicle out of Hartford. The pursuit eventually reached Webster Court in Newington, close to the Rocky Hill town line.
.
Police alleged that after the vehicle stopped, the man got out and exchanged gunfire with officers. The man was shot and pronounced dead at the scene. Authorities reported recovering a firearm. Available reporting has not yet established how many officers fired, which officers fired the fatal shots, or the number of rounds discharged.
.
Two police officers were wounded during the gunfire: Hartford Police Officer Alex Diaz and Rocky Hill Police Officer Jon Escobar. Both were hospitalized and subsequently released. Escobar reportedly suffered gunshot wounds to his arm and face and is expected to require additional surgeries.
.
The Connecticut Office of Inspector General has taken over the investigation into the fatal police shooting. The Inspector General is expected to issue a preliminary report, which in Connecticut police shootings commonly provides considerably more detail about the sequence of gunfire and identifies the officers who fired.

Cause of Death : Gunshot
Event : Shot and killed by police while driving vehicle
OPEN RECORDS SUMMARY :
On 08/21/2026 at approximately 11:35 p.m., an unidentified adult man was shot and killed by a Prince George’s County Police Department officer in the 4500 block of Reamy Drive in Suitland, Maryland.
.
Prince George’s County police said officers in marked cruisers saw two vehicles near Suitland Road, Randolph Road and Reamy Drive and began following them. The vehicles turned onto Reamy Drive, which police described as a narrow, dimly lit dead-end street. Current reporting does not explain why officers began following the vehicles, and NBC Washington reported that the department had not answered that question.
.
Police alleged that one of the vehicles then left the roadway, drove through a residential yard and moved toward officers. One officer fired a service weapon, striking the driver. Authorities have not yet explained precisely what the officer perceived, whether any weapon was recovered from the driver or vehicle, or how many rounds were fired.
.

Investigators said the officers involved were wearing body-worn cameras. Under the IID’s normal process, the names of the decedent and involved officer are generally released within two business days.

Event : Driver killed in multi-vehicle crash involving police vehicle
OPEN RECORDS SUMMARY :
On 08/20/2026, three people were killed and a Chickasaw Nation Lighthorse Police Department officer was injured in a multi-vehicle crash on northbound Interstate 44 near SW 119th Street in Oklahoma City.
.
Police said traffic on I-44 had slowed for an unknown reason when the driver of a northbound semi-truck failed to slow and struck another vehicle, triggering a chain-reaction crash involving multiple vehicles, including a Chickasaw Nation Lighthorse Police Department patrol vehicle.
.
The semi left the roadway and rolled into a ditch. The semi driver was killed. Another vehicle caught fire following the collision, killing two people inside. The three people killed have not yet been publicly identified in the reporting reviewed.
.
A Chickasaw Nation Lighthorse police officer whose patrol vehicle was caught in the crash was injured and transported to a hospital.
.
Current reporting does not indicate that the Lighthorse officer was pursuing a vehicle, conducting a traffic stop, traveling in emergency response, or otherwise initiated the events leading to the collision. Police currently attribute the chain reaction to the semi driver failing to slow for traffic ahead.

Paul William Sokoloski, MD
Age :57
Gender :Male
Race : White
Date : 08/19/2026
Location : Carlisle Airport, 228 Petersburg Road
City : Carlisle
County : Cumberland
State : Pennsylvania
Agency : Pennsylvania State Police
Officer(s) : Bryce Corman; Jason Mills
Cause of Death : Vehicle
Event : Pilot killed in collision with police helicopter during police training exercise
OPEN RECORDS SUMMARY :
NARRATIVE:
On 08/19/2026, 57-year-old Dr. Paul William Sokoloski of Hershey was killed when the Cessna 150 he was piloting collided with a Pennsylvania State Police Bell 407GX helicopter during a police aviation training exercise at Carlisle Airport in Cumberland County, Pennsylvania.
.
The collision occurred at approximately 6:58 p.m. The Pennsylvania State Police helicopter was hovering several feet above a grassy area north of the runway while Cpl. Bryce Corman and Trooper Jason Mills were conducting routine aviation training.
.
Sokoloski, the sole occupant of the Cessna, was attempting to land. According to preliminary information from investigators, the Cessna approached along the runway before suddenly pitching sharply upward, turning and leveling off before striking the rear of the hovering police helicopter.
.
The impact caused catastrophic damage to both aircraft. The Cessna broke apart and crashed to the ground. The helicopter lost its rotor blades and tail boom before crashing and rolling onto its side. Sokoloski was pronounced dead at the scene by the Cumberland County Coroner's Office.
.
Corman and Mills were trapped inside the wrecked police helicopter but survived. Corman escaped and helped Mills from the aircraft. Both were airlifted to Penn State Health Milton S. Hershey Medical Center. Corman was subsequently released from the hospital, while Mills remained hospitalized in stable condition.
.
Sokoloski was an emergency medicine physician with Penn State Health and a certified flight instructor. He was an experienced pilot and was flying the Cessna alone when the collision occurred.
.
Carlisle Airport does not have an air traffic control tower. Aircraft operating at the airport coordinate their movements through a common radio frequency. Investigators determined that the Cessna and police helicopter pilots had communicated before the collision and were aware of each other's presence, although those radio communications were not recorded.
.
National Transportation Safety Board investigators recovered data cards from the wreckage and are examining video, aircraft wreckage, flight paths, pilot communications, maintenance information and the operation of both aircraft. Investigators have not yet determined why Sokoloski's Cessna made its abrupt maneuver immediately before striking the police helicopter.
.
The National Transportation Safety Board is leading the investigation in cooperation with the Federal Aviation Administration. The Pennsylvania State Police Troop H Major Case Team is also assisting with the investigation.
.
OFFICER BACKGROUND:
Cpl. Bryce Corman and Trooper Jason Mills were Pennsylvania State Police Aviation Patrol Unit pilots aboard the Bell 407GX helicopter during the training exercise.
.
Mills had previously served as a Pennsylvania State Police fire marshal in northeastern Pennsylvania before becoming a pilot with the State Police Aviation Patrol Unit.
.
Both Corman and Mills survived the collision. Corman was released from the hospital, while Mills remained hospitalized in stable condition as of 08/20/2026.

Christopher Ford
Age :45
Gender :Male
Race : Native Pacific Islander
Date : 08/18/2026
Location : 1700 block of West Beaver Street near Stockton Street
City : Jacksonville
County : Duval
State : Florida
Agency : Jacksonville Sheriff's Office
Officer(s) : Not Disclosed
Cause of Death : Gunshot
Event : Killed by police after approaching officer at police stop station
OPEN RECORDS SUMMARY :
NARRATIVE:
On 08/18/2026, 45-year-old Christopher Ford was shot and killed by a Jacksonville Sheriff's Office sergeant outside a police stop station in the 1700 block of West Beaver Street near Stockton Street in Jacksonville. Ford was transported to UF Health Jacksonville, where he died.
.
At approximately 3:30 p.m., the sergeant was sitting in a parked patrol vehicle at the stop station when Ford approached the vehicle. JSO Chief of Investigations Michael Paul said the sergeant got out and had a brief conversation with Ford. Police have not publicly disclosed what Ford said to the sergeant or why Ford approached him.
.
JSO alleged that during the interaction Ford pulled out a handgun and began walking backward. The sergeant drew his firearm and pointed it at Ford. Police said Ford then fired one shot toward the sergeant. The bullet missed the sergeant and struck the building behind him.
.
The sergeant returned fire and shot Ford. After the shooting, the sergeant provided medical aid and Ford was taken into custody. Jacksonville Fire and Rescue Department personnel transported him to UF Health Jacksonville, where he was pronounced dead.
.
JSO said Ford had no local criminal history. Police disclosed that Ford had been held under Florida's Baker Act in February 2026, approximately six months before his death, indicating a recent mental health crisis. No additional information about the circumstances of that intervention has been publicly released.
.
The sergeant was wearing a body camera and activated it during the encounter. Chief Paul cautioned that because the encounter unfolded very quickly, the recording may not have captured the entire interaction. JSO has not yet released the body-camera footage.
.
JSO's Cold Case Unit is investigating the shooting. The sergeant was placed on administrative leave following the killing. This was the 14th shooting by Jacksonville Sheriff's Office personnel during 2026.
.
OFFICER BACKGROUND:
The Jacksonville Sheriff's Office has not publicly identified the sergeant who killed Ford, citing Marsy's Law. JSO disclosed, however, that this was the sergeant's second shooting involving police gunfire. The identity of the sergeant and the circumstances and outcome of his previous shooting therefore remain important outstanding issues for this case.

Lora Brady
Age :53
Gender :Female
Race : Native Alaskan
Date : 08/18/2026
Location : Adelaide Apartments, 200 block of East 9th Avenue
City : Anchorage
County : Anchorage
State : Alaska
Agency : Anchorage Police Department
Officer(s) : Not Disclosed
Cause of Death : Gunshot
Event : Killed by police during mental health crisis and SWAT standoff
OPEN RECORDS SUMMARY :
On 08/18/2026, an unidentified woman was shot and killed by an Anchorage Police Department officer during a mental health crisis and approximately hour-long standoff at The Adelaide, a supportive-housing complex for formerly homeless people in the 200 block of East Ninth Avenue in downtown Anchorage.
.
At approximately 1:34 p.m., a staff member at The Adelaide called 911. Police Chief Sean Case said the caller reported that the woman was intoxicated, threatening suicide, had a firearm, and had pointed the gun at two staff members.
.
Anchorage police arrived approximately six minutes later. The response included patrol officers, the department's Mobile Intervention Team, crisis negotiators, and SWAT personnel.
.
By approximately 2 p.m., police had deployed a drone inside the building. The drone hovered in the third-floor hallway outside the woman's apartment door while police attempted to communicate with her.
.
Police Chief Sean Case said officers outside the building could see the woman through a third-floor window. Police alleged that she repeatedly pointed the gun toward the apartment's front door and later pointed it from the window toward officers on the ground and toward nearby Denali Montessori School.
.
The standoff and negotiation effort lasted approximately one hour and three minutes. Case said police established some contact with the woman during that period but characterized the communication as not meaningful.
.
Denali Montessori School, directly across from The Adelaide, was being used as a polling place for Alaska's primary election. People inside the school were advised to shelter in place for roughly 40 minutes during the police operation. Students had not yet begun the new school year.
.
Police alleged that at approximately 3 p.m. the woman pointed the firearm from her third-floor window toward officers below and toward the school.
.
A single Anchorage police officer positioned outside the building fired a single round. The bullet struck the woman in the upper body. Police entered the apartment after the shot and found her dead.
.
The incident was captured on police body-worn cameras and patrol-vehicle dash cameras. Police Chief Case said his initial public account was preliminary and that the officer who fired the fatal shot had not yet been interviewed when Case gave his first briefing.
.
The following day, residents of The Adelaide described the woman very differently from the anonymous armed-person description in initial police reporting. Neighbor Christopher Williams described her as a sweet and kind person who shared food with neighbors and was often seen with her large dog, Hercules. Williams said he was surprised to hear police allege that she had been armed.
.
Another Adelaide resident, Brandon Moore, described the police response as extensive and aggressive, recalling the block being closed and heavily equipped SWAT personnel at the scene.
.
NeighborWorks Alaska, the nonprofit organization operating The Adelaide, declined to identify the woman or provide additional information, saying it would not comment on details that could affect residents, staff, or the ongoing investigation.
.
As of the latest reporting located through 08/22/2026, neither Anchorage police nor local news organizations had publicly identified the woman. No reliable public identification was located through obituary, local-news, or other indexed searches.
.
The Anchorage Police Department also had not publicly identified the officer who fired the fatal shot as of the latest material located.
.
This incident was Anchorage's seventh police shooting of 2026 and the 19th police shooting since the beginning of 2024. Importantly, it was the city's fourth fatal police shooting of 2026; the earlier report's wording could incorrectly imply that all seven shootings were fatal.
.
The shooting occurred amid continuing scrutiny of Anchorage police use of deadly force. Following a series of police shootings in 2024 and 2025, including the killings of Easter Leafa and Kristopher Handy, Anchorage officials commissioned outside reviews of APD use-of-force, de-escalation, training, and critical-incident practices.
.
OFFICER BACKGROUND:
The Anchorage Police Department has not publicly identified the officer who fired the single fatal shot in the latest reporting located through 08/22/2026.
Because the officer remains unidentified, no individual disciplinary, complaint, shooting, lawsuit, or employment-history search can yet be completed.
The shooting was recorded by body-worn cameras and police-vehicle dash cameras. Chief Sean Case said the officer had not yet been interviewed when he gave the initial public briefing following the shooting.

According to the Hardy County Sheriff's Office, as the deputy attempted to arrest Hart, Hart produced a firearm and fired at the deputy. The deputy returned fire and wounded Hart. Police said Hart was nevertheless able to get back into his vehicle and drive away.
.
The deputy pursued Hart as he drove into the Monongahela National Forest near the end of Waites Run. Hart eventually eluded police. Authorities then obtained an additional warrant charging him with attempted murder of a law-enforcement officer.
.
A multi-agency manhunt followed involving the Hardy County Sheriff's Office, Hampshire County Sheriff's Office, West Virginia State Police and West Virginia Division of Natural Resources/Natural Resources Police.
.
At approximately 3:30 p.m. on 08/18/2026, officers located Hart inside a residence near the end of Waites Run Road that authorities alleged he had entered unlawfully. A Hardy County deputy, Hampshire County deputy and West Virginia Natural Resources Police officer approached the residence to take him into custody.
.
Police alleged Hart again opened fire on the officers. Law-enforcement personnel returned fire and fatally wounded him. The available reporting does not identify which officer or officers fired the fatal rounds.
.
The West Virginia State Police Crime Scene Team was called to process the scene and investigate the fatal shooting.
.
One important date correction: some secondary labeling refers to this as an 08/17 Wardensville case because that is when the initial traffic stop, first shooting and pursuit began. Hart was actually killed on 08/18/2026, so I would use 08/18/2026 as the database Date.
.
OFFICER BACKGROUND:
Authorities have not publicly identified the Hardy County deputy involved in the initial traffic stop and first exchange of gunfire, nor the Hardy County deputy, Hampshire County deputy or Natural Resources Police officer who fired during the fatal encounter on 08/18/2026.

City : San Antonio
County : Bexar

Officer(s) : Rafael Perez
Cause of Death : Gunshot
Event : Killed by police during mental health crisis
OPEN RECORDS SUMMARY :
NARRATIVE:
On 08/17/2026, 56-year-old Gregory Allan Boaz was shot and killed by San Antonio Police Officer Rafael Perez outside Boaz's home in the 6700 block of Spring Front Drive during a mental health crisis. Boaz was a Marine Corps veteran, father and longtime member of San Antonio's cycling community.
.
The encounter began late on August 16 when Boaz himself called 911 and reported that someone armed with a gun was in his backyard. Police subsequently said investigators found no evidence that another armed person had been there and believed Boaz was experiencing a mental health crisis.
.
Acting Police Chief Jesse Salame said Perez did not know when he arrived that Boaz was experiencing a mental health crisis. Perez met Boaz's girlfriend at the front door and accompanied her along the side of the house to investigate the backyard.
.
Police alleged that Boaz then appeared carrying two large kitchen knives. Salame alleged that Perez ordered Boaz to drop the knives and that Boaz continued toward the officer. These descriptions are police claims pending completion of the investigation and release of the available video evidence.
.
Perez fired multiple rounds, striking Boaz. Boaz died at the scene.
.
The Bexar County Medical Examiner subsequently identified Boaz and determined that he died from multiple gunshot wounds. His death was ruled a homicide. The identification corrected initial reporting that had described him as 57 years old; Boaz was 56.
.
Boaz had served in the United States Marine Corps and was deeply involved in San Antonio's cycling community. He participated in several cycling organizations, including Team Taco Cycling and Team Left Hand, an organization that raises money for multiple sclerosis research.
.
Friends described Boaz as someone who organized training rides, encouraged other cyclists and regularly helped fellow riders. Members of the cycling community said they were heartbroken by his death and began organizing a memorial bicycle ride in his honor.
.
Boaz's killing was the seventh San Antonio Police Department shooting of 2026.
.
The shooting remains under investigation. Available police video and other evidence will be important in independently evaluating the circumstances immediately preceding Perez's gunfire.
.
OFFICER BACKGROUND:
Rafael Perez had been a San Antonio Police Department officer for approximately one year when he shot and killed Gregory Allan Boaz.
.
Perez fired multiple rounds during the encounter. He was placed on administrative duty while SAPD conducts its investigation.

Event : Killed by police during mental health/high-stress response
OPEN RECORDS SUMMARY :
On 08/16/2026, 39-year-old Juan Osornio was shot and killed by Mission Police Department officers inside his family's home in the 1600 block of Oblate Avenue in Mission, Texas.
.
At approximately 12:04 a.m., Mission officers responded to what the department described only as a report involving a person in a “high-stress situation.”
.
Police alleged Osornio was armed with a knife and moved toward another person and responding officers, causing officers to fire.
.
Police subsequently released photographs they said showed Osornio holding a knife and holding an unidentified woman by the neck before moving toward officers.
.
Osornio's family gives a sharply different account. His sister, Blanca Osornio, said her mother and Juan were sitting together on a couch when someone knocked loudly on the door.

Date : 08/16/2026
Location : 115 Ludlow Street
City : New York
County : New York
State : New York
Agency : New York City Police Department
Officer(s) : Not Disclosed
Cause of Death : Gunshot
Event : Killed by police during mental health crisis
OPEN RECORDS SUMMARY :
On 08/16/2026, 54-year-old Wei Chan was shot and killed by New York City Police Department officers inside a residential building at 115 Ludlow Street on Manhattan's Lower East Side. Officers had responded at approximately 4 p.m. to a report of an assault.
.
Police said officers entered the building and encountered blood on the staircase. They followed the blood trail upstairs to an apartment, where they found Chan's 75-year-old father, Chung Chan, suffering from a head injury. Police alleged that Wei Chan had struck his father in the head with a butcher knife before officers arrived. The older man was subsequently taken to Bellevue Hospital in stable condition.
.
Police alleged that Wei Chan was holding a large butcher knife when officers encountered him. According to the NYPD account, officers repeatedly ordered him to drop the knife, but Chan continued moving toward them. Officers then fired multiple rounds, striking Chan.
.
Officers and emergency personnel provided medical aid at the scene. Chan was transported to Bellevue Hospital, where he was pronounced dead. Police recovered the knife from the scene.
.
Police said Chan had no prior criminal record but did have a documented mental health history. The circumstances preceding the confrontation, including the reported attack on his father and his behavior when officers arrived, indicate that a mental health crisis was a significant component of the fatal encounter.
.
Body-worn cameras captured the police encounter. New York City officials said the footage will be released as the investigation proceeds.
.
The fatal shooting remains under investigation. Because a civilian died following the use of force by police, the case is also subject to review under New York's procedures for investigating deaths caused by law-enforcement officers.
.
OFFICER BACKGROUND:
The NYPD has not publicly identified the officer or officers who shot Wei Chan.

.
At approximately 10 p.m., Monroe police were dispatched to 208 Alda Drive after a woman called 911 and reported that someone was attempting to enter her home. The caller provided a description of the person. Police later said Montes-Hernandez and the woman knew each other and described the incident as targeted, although authorities have not publicly disclosed the nature of their relationship.
.
The first officer arrived alone and encountered Montes-Hernandez in the street. Police said Montes-Hernandez matched the description provided by the caller and had a bicycle with him. Investigators subsequently confirmed that a bicycle found in the roadway belonged to Montes-Hernandez.
.
Police alleged that Montes-Hernandez was carrying a handgun and pointed it at the officer. According to Monroe police, the officer repeatedly ordered Montes-Hernandez to drop the gun before shooting him. Police said less than 30 seconds elapsed between the officer's arrival in the neighborhood and the shooting.
.
After the shooting, officers provided CPR and other emergency medical aid until paramedics arrived. Montes-Hernandez was transported to a Charlotte-area hospital with life-threatening injuries. Police initially announced that he was not expected to survive and confirmed on August 18 that he had died.
.
Police said a handgun belonging to Montes-Hernandez was recovered at the scene. The publicly available reporting reviewed so far does not indicate that Montes-Hernandez fired the weapon.
.
Montes-Hernandez was identified as being from Mexico and was reportedly staying in the Monroe area at the time of his death.

The encounter from the officer's arrival until the shooting lasted less than 30 seconds. The officer remains on administrative leave while the North Carolina State Bureau of Investigation investigates. Identification of the officer is needed to determine any prior shootings, complaints, disciplinary history, lawsuits or other use-of-force incidents.

City : Council Bluffs
County : Pottawattamie
State : Iowa
Agency : Council Bluffs Police Department; Pottawattamie County Sheriff’s Office; Iowa State Patrol; U.S. Marshals Service; Metro Area Fugitive Task Force
Officer(s) : Not Disclosed
Cause of Death : Suicide
Event : Suicide under police duress during fugitive standoff
OPEN RECORDS SUMMARY :
NARRATIVE:
On 08/16/2026, James Leroy Perdue III died from what police described as a self-inflicted gunshot wound after an approximately six-hour standoff with local, state and federal law enforcement at a residence at 1201 Madison Avenue near Autumn Circle in Council Bluffs, Iowa. Police specifically reported that no officers fired their weapons during the incident.
.
Council Bluffs police officers and members of the Metro Area Fugitive Task Force arrived at approximately 1:45 p.m. while attempting to locate Perdue. Police said he had active Iowa parole-violation warrants. Officers had also been advised by the FBI that Perdue was being sought in connection with a homicide in South Dakota and was considered armed and dangerous.
.
About an hour after police arrived, authorities determined that Perdue was inside the residence. Council Bluffs police, the Pottawattamie County Sheriff’s Office, Iowa State Patrol and U.S. Marshals Service established a perimeter around the house. Police closed part of Madison Avenue and directed nearby residents to shelter in place.
.
Police negotiators repeatedly attempted to persuade Perdue to come out of the house, but he remained inside. The standoff continued for several hours.
.
At approximately 7:30 p.m., officers heard gunshots coming from inside the residence. Police subsequently entered or otherwise approached the residence and found Perdue dead from what they said was a self-inflicted gunshot wound. Authorities said no law-enforcement officer discharged a firearm during the standoff.
.
Perdue had also been identified as a person sought in connection with a fatal shooting in the Wagner North Housing area near Wagner, South Dakota. Yankton Sioux tribal authorities said that shooting occurred at approximately 9:04 p.m. on 08/14/2026. The FBI and tribal police were investigating that homicide.
.
Perdue’s death occurred while his residence was surrounded by multiple law-enforcement agencies, after hours of attempted negotiations intended to secure his surrender. Because police reported that he shot himself during that active armed standoff, the death is classified here as suicide under police duress.
.
OFFICER BACKGROUND:
No individual officer has been publicly identified as having primary responsibility for the operation. The response involved Council Bluffs police, the Pottawattamie County Sheriff’s Office, Iowa State Patrol, U.S. Marshals Service and the Metro Area Fugitive Task Force. Police reported that negotiators made repeated attempts to obtain Perdue’s surrender and that no officer fired a weapon.

Khylin Stevens
Age :28
Gender :Male
Race : Black
Date : 08/15/2026
Location : 2300 block of Creole Street
City : Lake Charles
County : Calcasieu
State : Louisiana
Agency : Lake Charles Police Department
Officer(s) : Not disclosed
Cause of Death : Gunshot
Event : Killed by police during mental health crisis and SWAT barricade
OPEN RECORDS SUMMARY :
On 08/15/2026, 28-year-old Khylin Stevens of Lake Charles was shot and killed by a Lake Charles Police Department officer following an extended barricade and mental health crisis at a residence in the 2300 block of Creole Street. Louisiana State Police are investigating the fatal shooting.
.
Lake Charles police responded to the residence following a 911 call. Police determined that Stevens had barricaded himself inside the residence and began extended negotiations with him. Louisiana State Police said officers also used several less-lethal measures during the encounter.
.
Louisiana State Police alleged that during negotiations Stevens did not comply with officers' commands and “produced a weapon.” LSP said a Lake Charles police officer then fired their service weapon, striking Stevens. The initial LSP release did not identify the alleged weapon, state that Stevens fired a weapon, or provide additional details establishing what Stevens allegedly possessed or what he was doing immediately before police opened fire.
.
Stevens' father publicly disputed the police account after his son's death and posted video of the encounter on Facebook. In an accompanying statement, he said Stevens was experiencing serious mental health problems and described his son as mentally unstable during the encounter. He said the video made Stevens' condition apparent and argued that police could have detained him rather than killing him.
.
Stevens' father specifically disputed the state's assertion that his son had a weapon, stating that Stevens “had no gun.” He further alleged that Stevens had his back turned when police shot him. He said the shooting was unnecessary and publicly called for justice for his son. The father's account directly conflicts with Louisiana State Police's preliminary statement that Stevens “produced a weapon.”
.
The video posted publicly by Stevens' father is potentially significant evidence in determining what occurred immediately before the shooting, including Stevens' behavior during the mental health crisis, his position relative to officers, whether a weapon is visible, the commands and negotiations preceding the gunfire, the use of less-lethal measures, and Stevens' position when the officer fired. Louisiana State Police had separately asked members of the public possessing photographs or video of the incident to provide that evidence to investigators.
.
After the shooting, Stevens was transported to a local hospital, where he was pronounced dead. Louisiana State Police Bureau of Investigations is conducting the investigation. The Lake Charles police officer who shot Stevens has not been publicly identified.

Timothy Cromwell
Age :23
Gender :Male
Race : Black
Date : 08/15/2026
Location : Werk Road and South Road
City : Green Township
County : Hamilton
State : Ohio
Agency : Cincinnati Police Department; Green Township Police Department
Officer(s) : Not Disclosed
Cause of Death : Gunshot
Event : Killed by police after abduction and 45-minute police chase
OPEN RECORDS SUMMARY :
At approximately 4 p.m., police received a report that a woman had been abducted from her workplace at Mercy Health - West Hospital. Cincinnati Interim Police Chief Adam Hennie alleged that Cromwell pushed the woman from the driver's seat into the passenger seat of her vehicle and drove away with her still inside. The woman was able to contact family members, who notified police and provided information about the vehicle.
.
Cincinnati police subsequently located the vehicle in the area of Boudinot and Ramona avenues and attempted to stop it. Cromwell continued driving with the woman inside, beginning a pursuit that police said lasted approximately 45 minutes. Interim Chief Hennie said Cincinnati's pursuit policy generally restricts pursuits to violent felonies and that officers continued this pursuit because they were responding to a reported abduction with the victim still inside the vehicle.
.
Police used stop sticks during the pursuit but were unable to bring the vehicle to a stop. The pursuit eventually reached Green Township, where police used a SWAT vehicle to stop Cromwell near Werk and South roads, in the area of Our Lady of the Visitation School.
.
After the vehicle stopped, Cromwell got out. Police alleged that he was carrying a knife and approached officers. According to Hennie, officers repeatedly ordered Cromwell to drop the knife. Police body-camera footage reportedly shows Cromwell continuing toward an officer with the knife.
.
Two Cincinnati Police Department officers and one Green Township Police Department officer fired at Cromwell. Police said Cromwell was struck and fell, then got back up while still holding the knife and again moved toward officers. Officers fired again. Police said this sequence occurred more than once before Cromwell was finally incapacitated.
.
Cromwell was transported to a hospital and subsequently pronounced dead.

Gaelan Pinchot
Age :28
Gender :Male
Race : White
Date : 08/15/2026
Location : Sharon Woods Metro Park, near Cleveland Avenue entrance
City : Columbus
County : Franklin
State : Ohio
Agency : Genoa Township Police Department; assisting law-enforcement agencies not yet fully disclosed
Officer(s) : Not Disclosed
Cause of Death : Suicide
Event : Suicide under police duress after being shot by police during armed standoff
OPEN RECORDS SUMMARY :
On 08/15/2026, 28-year-old Gaelan Pinchot died from a self-inflicted gunshot wound after a confrontation with Genoa Township and Westerville police at Sharon Woods Metro Park near Westerville, Ohio. Before Pinchot shot himself, a Genoa Township police officer fired one gunshot at him and a Westerville officer deployed a Taser.
.
The incident began shortly after 1 p.m. when a woman called Delaware County dispatch and reported that she had been sexually assaulted and that the man who assaulted her had used a gun. Police identified Pinchot and his vehicle from information provided during the investigation. A Genoa Township officer located the vehicle on Cleveland Avenue and followed it south while waiting for additional officers.
.
Westerville police joined the response. Police records show that a bulletin concerning Pinchot was entered at 1:04 p.m. and that officers received information that he was suicidal. At 1:08 p.m., Westerville records indicate that a pursuit was authorized. Pinchot then drove into Sharon Woods Metro Park and stopped in the first parking area.
.
Police alleged that Pinchot got out of his vehicle holding a handgun to his head. Officers surrounded the area and spent approximately 45 minutes negotiating with him. During that period, police said Pinchot repeatedly placed the handgun on the ground and then picked it back up, at times again holding it to his head.
.
Shortly before 2 p.m., Pinchot again placed the handgun on the ground. Four officers, including three Westerville officers, began approaching him. Police alleged that Pinchot bent down and picked the handgun back up. A Westerville officer fired a Taser at him, but police said it was ineffective.
.
A Genoa Township officer then fired one gunshot at Pinchot. According to Genoa Township Police Chief James McMillin, Pinchot remained standing after the officer's shot, raised his own handgun to the side of his head and fired. Pinchot died at the scene. The available reporting does not establish whether the police officer's bullet struck Pinchot or, if it did, what injury it caused. Police have identified the self-inflicted gunshot as the fatal wound.
.
Police and medics provided emergency aid. Westerville dispatch records show the Taser deployment at approximately 1:58 p.m., followed within about a minute by a request for medics. The Ohio Bureau of Criminal Investigation arrived later and is conducting the independent investigation of the police gunfire. Genoa Township police are separately investigating the reported sexual assault.
.
OFFICER BACKGROUND:
The Genoa Township officer who fired the gunshot and the Westerville officer who deployed the Taser have not been publicly identified in the sources located. Three Westerville officers were among the four officers who approached Pinchot immediately before the gunfire. Identification of the Genoa Township officer is particularly important because that officer fired immediately before Pinchot's fatal self-inflicted gunshot.

Chad Hickman
Age :39
Gender :Male
Race : White
Date : 08/14/2026
Location : Wooded area near Whitlock Lake
City : Lake City area
County : Missaukee
State : Michigan
Agency : Michigan State Police
Officer(s) : Not Disclosed
Cause of Death : Suicide
Event : Suicide under police duress
OPEN RECORDS SUMMARY :
On 08/14/2026, 39-year-old Chad Hickman died by suicide in a wooded area near Whitlock Lake in Missaukee County while Michigan State Police were conducting a large-scale armed search for him following a series of shootings that left five other people dead and a 13-year-old girl wounded.
At approximately 11:40 a.m., Michigan State Police troopers responded to a residence on South LaChance Road in Lake Township after a 911 caller reported that someone had been shot. When troopers arrived, they found three people dead from gunshot wounds and a fourth person critically wounded.
The surviving victim was a 13-year-old girl. Subsequent reporting identified two of those killed at the first location as 40-year-old Amanda Everitt and her 16-year-old son Eric. The girl survived multiple gunshot wounds and was able to call 911.
Michigan State Police identified Hickman as the suspected shooter and reported that he had left the residence before troopers arrived. Police considered him armed and dangerous and launched a large-scale search involving the Michigan State Police Emergency Support Team, aviation resources and canine units.
During the search, investigators discovered a fourth deceased victim, a 53-year-old man, at a separate residence in Missaukee County. The expanding investigation therefore involved multiple shooting scenes while police continued searching for Hickman.
Troopers subsequently located Hickman's vehicle near a wooded area by Whitlock Lake in Caldwell Township. Police concentrated their search in that area and ultimately found Hickman and a 29-year-old woman dead in the woods. Both had suffered gunshot wounds.
The available reporting does not indicate that Michigan State Police fired at Hickman or exchanged gunfire with him during the search. Subsequent reporting describes Hickman as having taken his own life. There is therefore no evidence presently establishing his death as a fatal police shooting.
Hickman's death occurred while an intensive police operation was underway to locate and apprehend him. Michigan State Police had deployed specialized emergency-response personnel, aircraft and canine teams and had warned the surrounding community that Hickman was armed and dangerous.
Five people other than Hickman were killed during the series of shootings. The victims included a 16-year-old boy, a 40-year-old woman, a 45-year-old man, a 53-year-old man and a 29-year-old woman. The 13-year-old girl survived and was reported in stable condition after initially being described as critically injured.
Michigan State Police continued investigating the shootings across three separate locations, including the relationships between Hickman and the victims and the motive for the killings. Authorities said the discovery of Hickman's body ended the immediate threat to the public.
Hickman's death qualifies for inclusion as because he died by suicide while police were actively conducting a large-scale armed manhunt to locate and apprehend him. His death was not caused by police gunfire.

Lorenzo Devonte Gray
Age :33
Gender :Male
Race : Black
Date : 08/14/2026
Location : South Main Street and Bethpage Road
City : Kannapolis
County : Cabarrus
State : North Carolina
Agency : Kannapolis Police Department
Officer(s) : Justin Smith; Matthew Hoehman
Cause of Death : Gunshot
Event : Killed by police during attempted carjacking
OPEN RECORDS SUMMARY :
On 08/14/2026, 33-year-old Lorenzo Devonte Gray of Kannapolis was shot and killed by Kannapolis Police Department Lt. Justin Smith and Sgt. Matthew Hoehman at the intersection of South Main Street and Bethpage Road in Kannapolis after police responded to a report of a man firing a gun and encountered Gray during an alleged attempt to enter occupied vehicles.
.
At approximately 11:40 a.m., Kannapolis police were dispatched to South Main Street near Bell Street after a 911 caller reported that a man was firing a gun. As officers approached South Main Street and Bethpage Road, police alleged that they saw Gray holding a firearm and attempting to carjack vehicles at the intersection.
.
A woman who owns a coffee shop at the intersection witnessed the events preceding the shooting and provided WSOC-TV with a more detailed account. She said Gray first attempted to enter one vehicle and then approached a second vehicle directly behind it. According to the witness, the woman driving the second vehicle had locked her doors, preventing Gray from entering.
.
The witness said the second driver then accelerated away while Gray was holding onto or attempting to access the rear portion of her vehicle. Gray was pulled along briefly before being thrown from the vehicle. He then moved back in the direction of a nearby apartment complex.
.
According to the witness, police had arrived by that point and ordered Gray to stop. She said Gray continued running and she then heard approximately five gunshots.
.
Kannapolis police subsequently identified Smith and Hoehman as the officers who fired at Gray. Police have alleged that Gray was armed during the encounter, but the public information released to date does not establish the precise actions that immediately caused Smith and Hoehman to fire. Authorities have not publicly established whether Gray pointed the firearm at either officer, fired at police, was still holding the weapon when he was shot, or what distance separated Gray and the officers when they opened fire.
.
Gray was struck by police gunfire and transported to Atrium Health Cabarrus, where he died from his injuries.
.
On 08/17/2026, Kannapolis police publicly identified Gray and the two officers who fired. Both Smith and Hoehman were placed on administrative leave while the department conducts an internal review.
.
The North Carolina State Bureau of Investigation is conducting the independent investigation into the fatal shooting. The SBI investigation is expected to document the exact sequence immediately preceding the gunfire, the location and handling of the firearm attributed to Gray, the number of rounds fired by Gray if any, the number of rounds fired by each officer, the officers' positions and distances from Gray, and available body-camera, surveillance and other video evidence.
.
OFFICER BACKGROUND:
Sgt. Matthew Hoehman is a Kannapolis Police Department supervisor who was publicly identified as the second officer who shot Gray. Public court records document Hoehman's work with the department by at least January 2020 and identify him as a sergeant in subsequent investigations. A 2023 federal civil-rights lawsuit named Hoehman and another Kannapolis officer over their involvement in a January 2020 encounter at a Kannapolis residence; the existence of the lawsuit does not establish that the allegations against Hoehman were proven.

OPEN RECORDS SUMMARY :
NARRATIVE:
On 08/14/2026, 20-year-old Ryan Herrera of Yonkers was killed when the red Toyota Supra he was driving left the roadway and struck a tree in the Mount Pleasant/Briarcliff Manor area of Westchester County, New York. His passenger, 21-year-old Jaden D. Sanabria of the Bronx, was also killed.
.
At approximately 4:00 a.m., a Mount Pleasant Police Department patrol officer saw Herrera's Supra traveling at approximately 100 mph. The officer pulled onto Route 9A and initiat4ed a pursuit, attempting to catch up to the vehicle. Police said the officer activated the patrol vehicle's emergency lights as the Supra approached the intersection of Route 9A and North State Road in Briarcliff Manor.
.
According to police, the Supra had no visible license plates and extremely dark window tint. Police alleged Herrera accelerated away and the officer lost sight of the vehicle. The officer subsequently encountered debris in the roadway and saw headlights in the brush near the Route 100 northbound entrance ramp, where the Supra had crashed.

Investigators determined that the Supra had been rented to Herrera through a social-media platform. The person who rented the vehicle provided investigators access to its GPS data. According to police, the GPS showed that the Supra had exceeded 100 mph multiple times that morning, reached a maximum recorded speed of 134 mph, and was recorded at approximately 108 mph shortly before the Mount Pleasant officer encountered it.
.
Police reported finding two stolen semiautomatic pistols inside the wrecked vehicle, one near Sanabria and another between Herrera's legs. Police also said both men were wearing balaclavas and were not wearing seat belts. These allegations concern evidence discovered after the fatal crash and were not the stated reason the officer initially attempted to catch the Supra.
.
The New York Attorney General's Office of Special Investigation is reviewing the deaths because of the police involvement immediately preceding the crash.
.
OFFICER BACKGROUND:
The Mount Pleasant Police Department has not publicly identified the patrol officer who observed Herrera's vehicle, pulled onto Route 9A to catch up to it, and activated emergency lights before the fatal crash.

Date : 08/13/2026
Location : North parking deck, UAB St. Vincent’s Birmingham, near 2700 block of University Boulevard
City : Birmingham
County : Jefferson
State : Alabama
Agency : Birmingham Police Department
Officer(s) : Not Disclosed
Cause of Death : Gunshot
Event : Killed by police during attempted detention
OPEN RECORDS SUMMARY :
On 08/13/2026, 38-year-old Matthew Sean Massey of Birmingham was shot and killed by a Birmingham Police Department officer inside the north parking deck at UAB St. Vincent’s Birmingham. The encounter began after a McDonald’s security guard flagged down a patrol officer and reported that Massey had assaulted an employee at the restaurant.
.
At approximately 12:35 p.m., the officer was patrolling near University Boulevard and 28th Street South when the security guard approached him. Police said the security guard identified Massey, who had crossed University Boulevard. The officer drove down University Boulevard, stopped his patrol vehicle and attempted to contact Massey to investigate the reported assault.
.
Police alleged that when the officer attempted to detain Massey, he ran. The officer pursued him on foot into the north parking deck at UAB St. Vincent’s Birmingham.
.
Birmingham Police Chief Michael Pickett said the officer encountered Massey inside the parking deck and issued verbal commands. Police alleged that Massey did not comply, and the officer deployed a Taser. According to police, Massey then attempted to take control of the officer’s Taser.
.
Police said the officer then drew his firearm and shot Massey. Emergency medical personnel responded, but Massey died at the scene. The Jefferson County Coroner’s Office subsequently identified him.
.
Police had not claimed that Massey possessed a weapon. At an evening news conference on the day of the shooting, Chief Pickett said investigators had not established that Massey was armed. The police justification for the shooting instead centered on the allegation that Massey attempted to gain control of the officer’s Taser.
.
The Birmingham Police Department Homicide Division is investigating the shooting. Police said the completed investigation will be submitted to the Jefferson County District Attorney’s Office for independent review.
.
Police said body-worn camera footage exists and indicated that it intends to release footage from the encounter when investigators determine that doing so will not compromise the investigation. UAB also issued an emergency alert after the shooting directing people to avoid the North Tower and Level A of the parking deck. Massey was reportedly unhoused and originally from the Pensacola area.
.
OFFICER BACKGROUND:
The Birmingham Police Department has not publicly identified the officer who killed Massey. Chief Michael Pickett described him as a seven-year veteran of the department.
