On 10/28/2021, Perth Amboy Police Lieutenant David Formeza died from complications of COVID-19 after contracting the virus in the line of duty. He died at his home in Perth Amboy. Formeza was a lifelong resident of the city and had served approximately 18 to 19 years with the department, including time assigned to patrol and the SWAT team. He worked nightshift patrol and was widely described by local officials and colleagues as deeply connected to the community. He was 46 years old.

On 10/28/2021, Senior Officer Mathew Leslie Lyons died from complications of COVID-19 after contracting the virus in the line of duty. He served with U.S. Customs and Border Protection, Office of Field Operations, for over 12 years and was the Port Director at the Port of Whitlash in Montana. His role required continuous contact in an active border environment during the pandemic. He was 48 years old and is survived by his son, daughter, and sister.

On 10/26/2021, Pontoon Beach Police Officer Tyler Timmins was shot while investigating a stolen vehicle in the parking lot of a Speedway gas station at the intersection of Chain of Rocks Road and Pontoon Road. During the encounter, a man inside the vehicle opened fire, striking Timmins. He was transported to a hospital where he died from his injuries. Timmins had served in law enforcement for approximately 14 years and was 36 years old. He is survived by his wife and daughter.

On 10/25/2021, Burns Police Chief Stephen Evans was killed in a vehicle crash on K-77 Highway near El Dorado while on duty. He observed a vehicle he believed to be stolen traveling in the opposite direction and attempted to make a U-turn to investigate. During the maneuver, another vehicle traveling southbound struck his patrol car. He died from injuries sustained in the crash. Evans also served with the Butler County Sheriff’s Office and had prior service with the Kansas Department of Corrections. He was 44 years old and is survived by his wife and three children.

On 10/20/2021, Corrections Officer Joshua E. Kristek contracted COVID-19 while serving at the Medina County Juvenile Detention Center at 655 Independence Drive in Medina. He died from complications of the illness on 10/24/2021. Kristek was a United States Army veteran and had served with the Medina County Juvenile Detention Center for three years. He is survived by his wife and three children.

On 10/24/2021, Supervisory Border Patrol Agent Rafael Gilberto Sanchez died from complications after contracting COVID-19 in the line of duty. Sanchez had served with the United States Border Patrol for 19 years and was assigned to the Hebbronville Border Patrol Station in the Laredo Sector. He entered on duty on 09/16/2002 as a member of the 527th Session of the Border Patrol Academy. His death was reviewed by an executive panel and the Commissioner of U.S. Customs and Border Protection, who determined that it occurred in the line of duty. Sanchez is survived by his wife, three children, mother, and three grandchildren.

On 09/15/2021, Police Officer Christopher William Cummins contracted COVID-19 in the line of duty while serving with the Bella Vista Police Department. He was hospitalized at Northwest Medical Center in Bentonville and remained on a ventilator for more than a month before dying from complications of the illness on 10/23/2021. Cummins had served with the Bella Vista Police Department for approximately 18 months and had served in law enforcement for a total of 12 years, including prior service with the Kellyville Police Department in Oklahoma and the Parsons Police Department in Kansas. He remained hospitalized for more than three weeks before dying from complications of the illness on 10/22/2021. On 09/12/2021, Deputy Sheriff Joshua Joseph Welge contracted COVID-19 in the line of duty while serving with the Sarasota County Sheriff's Office. He died from complications of the illness on 10/21/2021. Welge joined the Sarasota County Sheriff's Office in 1999 as a corrections deputy before becoming law enforcement certified. On 10/21/2021, Trooper First Class Dung Xuan Tran Martinez died from complications after contracting COVID-19 in the line of duty while assigned to Troop T at the King of Prussia Station located at 251 Flint Hill Road in King of Prussia. Martinez enlisted in the Pennsylvania State Police on 04/24/2000 and graduated as a member of the 107th Cadet Class. During his 21-year career, he served at Troop K in Media, Troop J in Lancaster, Troop M in Trevose, and was last assigned to Troop T in King of Prussia. He received the Blooming Grove Service Award for his role in the 2014 manhunt for Eric Frein. Martinez is survived by his wife and two children.

On 08/06/2021, Sergeant Miguel Rodriguez, Jr. contracted COVID-19 while working at the Florida Agricultural Checkpoint located at 752300 Interstate 95 in Yulee. He died from complications of the illness on 10/21/2021. On 09/01/2021, Deputy Sheriff Joshua James Sieman contracted COVID-19 while serving at the Harris County Sheriff's Office Tomball Substation located at 23828 Tomball Parkway in Tomball. He died from complications of the illness on 10/21/2021 after being hospitalized for more than a month. Sieman had served with the Harris County Sheriff's Office for five years and was assigned to the evening shift in Patrol District 5, where he was a member of the Crime Reduction Unit and the High-Water Rescue Team. On 10/14/2021, Trooper Theodore Lawrence Benda was deploying stop sticks during a pursuit on Interstate 80 near the 42-mile marker outside Avoca when he was struck by a passing vehicle. The pursuit had been initiated after a traffic stop for speeding. Benda was transported to Nebraska Medical Center in Omaha, where he died from his injuries on 10/20/2021. On 10/17/2021, Police Officer Ryan Andrew Hayworth was investigating a single-vehicle crash on Interstate 540 near Mile Marker 22 with his field training officer when their parked patrol vehicle was struck from behind by an impaired driver who failed to move over or slow down. Hayworth suffered fatal injuries in the collision. His field training officer, the driver from the original crash, and the impaired driver were seriously injured. In 2022, the driver who struck the patrol vehicle was charged with second-degree murder. Hayworth was a United States Army National Guard veteran and had served with the Knightdale Police Department for three months. He is survived by his father, mother, brother, and two stepbrothers. His father is retired Zebulon Police Chief Tim Hayworth.

On 10/17/2021, Police Officer Yandy Chirino responded to reports of a suspicious person checking vehicle door handles in the 4000 block of North Hills Drive in Hollywood. While attempting to detain the man, a physical struggle ensued and the man produced a concealed handgun and shot Chirino. Fellow officers took the gunman into custody and transported Chirino to Memorial Regional Hospital, where he died from his injuries a short time later. Chirino had served with the Hollywood Police Department for four years after joining the agency in 2017. During his career he received five supervisor's commendations and was named Officer of the Month in June 2020. He is survived by his parents, sister, two nephews, and girlfriend.

Mooneyham was a United States Army veteran. He had served with the Denton Police Department for almost 11 years after previously serving with the Van Zandt County Sheriff's Office, the Kaufman County Sheriff's Office, and the Dallas Area Rapid Transit Police Department. During his tenure with the Denton Police Department, he served in General Investigations before transferring to the Major Crimes Unit in 2019, where he led numerous homicide investigations and received nine departmental commendations, including a Chief's Commendation. On 09/10/2021, Officer Thomas Ouellette, 44, of the Bow Police Department, was driving his personal Ford Expedition southbound on I-89 when his vehicle drifted from the right-hand travel lane into the breakdown lane. The vehicle struck the rear of a parked, disabled tractor-trailer truck. Two of Officer Ouellette’s children, Nicholas Ouellette, 16, and Gavin Ouellette, 6, were killed in the collision. While the media did not confirm his on-duty status, it is assumed he was off-duty as he was driving his personal vehicle.

NARRATIVE:
On 08/04/2021, 24-year-old Brooklyn Police Officer Brian Russell Pierce Jr. was struck and killed by a Dodge Charger fleeing other Brooklyn police officers while Pierce was deploying spike strips on the McKinley Bridge between Illinois and Missouri.

At approximately 3:00 a.m., Brooklyn police began pursuing the red Dodge Charger following an incident at the Bottoms Up nightclub in Brooklyn. Contemporary reporting states that authorities did not disclose what occurred at the nightclub that prompted the pursuit.

Pierce positioned himself on the McKinley Bridge near the Illinois-Missouri state line and deployed spike strips across the roadway. The fleeing Charger, driven by Caleb L. Campbell, approached at high speed and struck Pierce. Evidence presented at Campbell's later trial established that he reached speeds of up to approximately 98 mph while fleeing police.

Pierce sustained fatal blunt-force injuries. The Charger continued across the bridge into St. Louis, Missouri, and was subsequently found abandoned. Illinois State Police investigated the fatal collision.

Pierce had served approximately nine months with the Brooklyn Police Department after previously serving for about two years with the Spillertown Police Department. He was also a lieutenant and fire investigator with the Makanda Township Fire Department. He was the son of Tammy and Brian Pierce Sr.

DISPOSITION:
Caleb L. Campbell was charged with first-degree murder, reckless homicide, aggravated fleeing or attempting to elude police, and offenses related to leaving the fatal crash. In December 2022, a judge convicted Campbell of first-degree murder. He was subsequently sentenced to life imprisonment plus 13 years.

On 08/03/2021, Officer George Gonzalez of the Pentagon Force Protection Agency was attacked at approximately 10:35 a.m. on a bus/metro platform outside the Pentagon in Arlington. A man, identified as Austin William Lanz, 27, ambushed Gonzalez and stabbed him in the neck. Responding officers engaged the attacker and fatally shot him. Gonzalez died from his injuries and multiple bystanders were also wounded in the incident.

On 08/01/2021, Deputy Michael C. Hoy, 49, of the Wyandot County Sheriff's Office, was involved in a fatal motorcycle accident on State Route 67 near Marseilles, Ohio. While riding his 2008 Harley Davidson motorcycle, Deputy Hoy collided with a deer, resulting in a crash. He was transported to Wyandot Memorial Hospital in Upper Sandusky, where he was pronounced dead.

On July 31, 2021, at approximately 12:20 a.m., Supervisory Border Patrol Agent Daniel Paul Cox, a 24-year veteran of the agency and a U.S. Army veteran, was killed in a head-on collision on State Route 86 near Sells, Arizona. The crash involved his Border Patrol vehicle and another car. Both Cox and the other driver were pronounced dead at the scene. The incident is under investigation by the Tohono O’odham Police Department.

On 07/29/2021, Officer Gunther Hashida, age 43, of the Metropolitan Police Department, was found dead in his residence from a self-inflicted gunshot wound. He had been assigned to the Emergency Response Team and was among those who defended the U.S. Capitol during the January 6 attack. His death was one of four suicides among officers who served during or immediately after the riot.

On 07/10/2021, Officer Kyle DeFreytag, age 26, of the Metropolitan Police Department, died by suicide. He was one of several officers who had responded to the U.S. Capitol on January 6, 2021. His death was confirmed by the department in early August 2021 and came shortly after that of Officer Gunther Hashida. Both were recognized as part of the rising toll of suicides among officers who defended the Capitol during the attack.

On June 17, 2021, Officer Joseph William Burson of the Holly Springs Police Department attempted to stop a vehicle during a traffic stop on Hickory Road. The driver refused to comply and dragged Officer Burson with the vehicle. During the incident, Burson discharged his weapon and fatally shot the driver. Officer Burson died from injuries sustained during the dragging. He had served with the department for approximately 18 months.

On 05/19/2021, Liberty County deputies located Harris County Precinct 1 Sgt. Robert Johnson after he became the focus of a multi-agency investigation into child sexual abuse allegations and threatened to take his own life. Deputies attempted a traffic stop after identifying his vehicle near a cabin community; Johnson drove away and triggered a pursuit that ended when officers stopped his vehicle on the FM 787 bridge over the Trinity River. Police surrounded the vehicle, established containment, and held a standoff lasting several hours while negotiators communicated with him. During the confrontation Johnson made phone calls admitting to sexual abuse of minors and implicated coworkers, information that later led to arrests and charges against two employees. Negotiations continued for roughly six hours before Johnson shot himself inside the vehicle and died before officers took custody.
