Heath of Aiken, South Carolina, was killed when a Chrysler 300 being pursued by Richmond County Sheriff’s Office deputies crossed into his lane and struck his Nissan Versa head-on on Old Savannah Road near Roselle Street in Augusta. Heath was an uninvolved driver returning from work and had no connection to the police pursuit.

The pursuit began at approximately 7:40 p.m. when a Richmond County deputy attempted to stop the Chrysler at East Boundary and East Walker streets for failure to maintain lane, a misdemeanor traffic offense. The driver continued driving away while the deputy pursued. The Chrysler reached speeds exceeding 100 mph.

Records associated with the case listed additional traffic offenses including driving too fast for conditions, an open-container violation, failure to wear a seat belt, driving on the wrong side of the roadway, an expired tag, failure to provide proof of insurance and driving under the influence. The immediate reason reported for the deputy's attempted stop, however, was failure to maintain lane.

The pursuit continued along Old Savannah Road. According to the Georgia State Patrol, the Chrysler vaulted over a railroad crossing near Roselle Street, causing the driver to lose control. The Chrysler crossed into the opposite lane and struck Heath’s Nissan Versa head-on.

On 08/26/2022, Oakley Police Department Officer Brett Jackson shot and killed 58-year-old Frank Anthony Correa after police responded to a domestic violence call at a residence in the 1800 block of Teresa Lane. Officers arrived and attempted to communicate with Correa, who remained inside the residence. Police deployed a drone and reported seeing Correa inside with a handgun. According to Oakley Police Chief Paul Beard, officers continued attempting to establish communication while Correa remained inside. Police reported hearing multiple gunshots from within the residence during the standoff.

At approximately 1:18 a.m., police alleged that Correa began firing toward officers outside the residence. Initial reporting stated that Correa fired three rounds at officers during the final confrontation. A subsequent police review reported that investigators recovered 16 spent 9mm cartridge casings associated with shots Correa allegedly fired during the overall incident in the direction of either the domestic violence victim or police.

Jackson was the only Oakley officer who fired his weapon. He returned fire and struck Correa. Officers subsequently approached Correa and provided medical aid, but he died at the scene.

The Contra Costa County District Attorney's Office and Oakley Police Department investigated the shooting under the county's law-enforcement-involved fatal incident protocol. Oakley police released partial body-worn-camera audio several days after the killing.

The Contra Costa County District Attorney's Office later completed an independent criminal review of Jackson's use of deadly force. The review included the autopsy, coroner's inquest and investigative evidence and concluded that Jackson's shooting of Correa was legally justified.

The encounter began shortly after 5:30 p.m. when three CMPD officers went to Donahue's residence in the 8900 block of St. Croix Lane specifically to serve an involuntary commitment order. A man at the residence opened the door and allowed the officers inside. The commitment order indicates that the police encounter was connected to concerns about Donahue's mental or behavioral condition rather than a criminal arrest warrant.

After entering the residence, officers called out to Donahue. According to CMPD, Donahue responded by firing a gun toward the three officers. Police reported that one bullet struck a female officer in the chest, but her protective vest stopped the round.

On 08/25/2022, 24-year-old Gavin Lee Stansell died from a self-inflicted gunshot wound during an eviction at Lind Commons Apartments, 3493 East Lind Road in Tucson. Pima County Constable Deborah Martinez-Garibay and apartment manager Angela Fox-Heath had gone to Stansell’s apartment to enforce a court-ordered eviction when Stansell opened fire.

The eviction was not based on nonpayment of rent. Court records show the landlord filed the eviction action after allegations that Stansell had threatened and intimidated another resident with a firearm and otherwise disturbed the peace. An eviction hearing was held on 08/22/2022, and the court issued the eviction order on 08/23/2022.

According to Tucson police, Martinez-Garibay and Fox-Heath were attempting to contact Stansell when he opened fire, fatally shooting 28-year-old Fox-Heath. Responding officers found Fox-Heath in the apartment complex courtyard.
.
Police could not initially locate Martinez-Garibay and believed she remained inside Stansell’s apartment. The Tucson Police Department SWAT team was called to the complex and subsequently entered the apartment. Officers found Martinez-Garibay suffering from gunshot wounds and Stansell dead from a self-inflicted gunshot wound. Martinez-Garibay died at the scene.

Investigators determined that Stansell had also entered a neighboring apartment and fatally shot 25-year-old Elijah Miranda. Miranda was visiting a friend at the complex and had no involvement in Stansell’s eviction. Stansell therefore killed three people — Martinez-Garibay, Fox-Heath and Miranda — before killing himself.

Residents later described a history of threatening behavior preceding the killings. One neighbor said Stansell had repeatedly caused disturbances and threatened residents and that police had previously been contacted about his behavior. Court documents independently documented an allegation that Stansell had threatened another resident with a firearm, which became part of the basis for his eviction.

The firearm used in the killings later became the subject of a federal investigation. Federal authorities determined that a lower receiver used in Stansell’s firearm had been purchased by Josue Lopez Quintana in November 2021. Quintana was accused of falsely representing that he was purchasing the receiver for himself and later making false statements to federal investigators about acquiring it for Stansell. Quintana pleaded guilty and was sentenced to time served and three years of supervised release.

The circumstances of the eviction subsequently produced wrongful-death litigation by Fox-Heath’s surviving husband. The lawsuit alleged that Martinez-Garibay knew Stansell possessed a firearm but nevertheless had Fox-Heath accompany her to enforce the eviction. The litigation also alleged failures involving Martinez-Garibay’s training and the procedures used to conduct the eviction. The Arizona Court of Appeals later addressed immunity issues arising from those claims.

On 08/25/2022, Muncie Police Department officers shot and killed 45-year-old Richard Allen Skinner during a standoff at a residence in the 1500 block of East Andover Avenue. Officers remained outside the residence and made contact with Skinner, who was inside. Police began negotiating with him and subsequently activated the Muncie Police Department SWAT team. Skinner was the brother of Delaware County Sheriff Tony Skinner, but the Delaware County Sheriff's Office stated that it was not involved in the operation.

According to Indiana State Police, Muncie officers heard gunshots coming from inside the residence during the standoff. Police then alleged that Skinner appeared at a window and pointed what appeared to be a firearm toward officers outside. Muncie officers responded by firing department-issued rifles at Skinner.

Skinner was struck by police gunfire. Officers provided medical treatment, but Delaware County Coroner Gavin Greene pronounced him dead at the scene. The initial Indiana State Police account described Skinner as having been struck by officers firing their department-issued rifles.

At the time of his death, Skinner had three pending Delaware County criminal cases involving allegations of domestic battery or domestic violence. Those cases had not been adjudicated. The police response that resulted in his death, however, originated with the neighbor's report of a domestic disturbance at the Andover Avenue residence that morning.

Skinner was born on 11/06/1976. His obituary identified his surviving family as including his father Walter H. Skinner and brothers Troy Skinner and Delaware County Sheriff Tony Skinner. Memorial donations were directed to the National Alliance on Mental Illness organization in Delaware County.

On 08/25/2022, Tucson Police Department SWAT officers Barrie Pedersen and Ernest Ortiz shot and killed 49-year-old Francisco Javier Galarza outside the Circle K at 6090 South Park Avenue in Tucson. SWAT officers and detectives had been conducting surveillance on Galarza before approaching him to make an arrest.

Police were seeking Galarza on active felony arrest warrants connected to a home invasion and multiple bank robberies in the Tucson area. Officers had located Galarza and were watching him at the Circle K rather than responding to an unfolding crime at the store. At approximately 11:51 a.m., officers moved in as Galarza exited the business.

Body-camera footage released by the Pima Regional Critical Incident Team shows officers rapidly exiting an unmarked vehicle with weapons drawn and ordering Galarza to get on the ground. Galarza immediately ran across the parking lot. Police reported that as he ran, he removed a black handgun from a bag he was carrying.

Pedersen, a Tucson police K-9 officer assigned to SWAT, released his police dog Kiro to apprehend Galarza. Kiro chased Galarza and brought him to the ground. Video released after the shooting shows Galarza producing the handgun as the dog takes him down.

Police alleged that while Galarza was on the ground, he pointed the handgun toward Pedersen and Ortiz. The released footage appears to show Galarza raising the weapon in the officers' direction immediately before they opened fire. Pedersen and Ortiz both fired their weapons, striking Galarza.

Investigators recovered a Taurus G3 9mm semiautomatic handgun from beside Galarza. Police reported that the magazine contained ammunition and that a round was in the chamber. The available accounts of the shooting do not report Galarza firing the handgun during the encounter.

Officers provided medical aid until Tucson Fire Department personnel arrived. Galarza died at the scene. Pedersen was involved in another police shooting less than six months after Galarza's death. On 02/14/2023, Tucson police responded to a mental-health crisis involving 29-year-old Jacques Taylor. Police found Taylor walking on South Kolb Road while armed with a handgun. After approximately 15 minutes of telephone negotiation, Pedersen fired one rifle round and struck Taylor. Taylor survived with injuries described as non-life-threatening.

Ernest Ortiz had approximately 15 years with the Tucson Police Department when he killed Galarza. Galarza was not Ortiz's first fatal police shooting.

On 09/10/2018, Ortiz and Tucson Police Sergeant William Corrales shot and killed 53-year-old Joel R. Andrade during a domestic-violence response and prolonged mental-health barricade. Police reported that Andrade had locked himself in a bathroom, cut his own neck and asked officers to kill him. After approximately two hours of negotiation, police alleged that Andrade emerged and raised a rifle toward officers. Ortiz and Corrales then fired, killing Andrade.

On 08/21/2022, Spokane County Sheriff’s Office Sergeant Darin Staley shot 36-year-old Terry Allen Starkweather Jr. during an attempted arrest at Lyons Self Storage, 1707 E. Lyons Avenue in Spokane. Starkweather remained hospitalized for three days and died from his wounds on 08/24/2022.

The encounter began after Staley received information at approximately 6:25 a.m. that Starkweather was at the storage facility. Starkweather was wanted on outstanding felony warrants for unlawful possession of a firearm, second-degree assault and possession of a stolen motor vehicle. Staley located Starkweather at a storage unit and called Deputy Roman Maier to assist with the arrest.

Staley and Maier developed a plan to approach Starkweather from opposite directions in the narrow lane between two rows of storage units. Maier drove a marked sheriff’s patrol vehicle into the lane from the south and positioned it in front of Starkweather’s pickup truck, while Staley approached on foot from the north.
.
According to the police investigation, Maier told Starkweather that he was under arrest and ordered him to put his hands in the air. Police reported that Starkweather instead moved from the bed of his pickup into the driver’s seat.
.
Police alleged that Starkweather rapidly reversed the pickup toward Staley, who was standing in the confined space between the storage buildings. Staley fired his service weapon as the truck moved backward. The pickup then struck one of the storage buildings.
.
Police alleged that Starkweather subsequently shifted the pickup into drive and moved forward toward Staley, who remained confined by the parallel storage buildings. Staley fired again. The pickup then stopped. Staley was the only officer identified as firing during the encounter.

Deputies provided emergency medical treatment until medical personnel arrived. Starkweather was transported to a hospital in critical condition. He died from the gunshot wounds on 08/24/2022.

The Spokane Independent Investigative Response Team investigated the shooting, with the Spokane Police Department serving as the managing agency because Spokane County Sheriff’s Office personnel were involved. Investigators obtained body-worn-camera footage, surveillance-camera recordings, forensic evidence and statements from Staley and Maier. The completed investigation was forwarded to the Spokane County Prosecutor’s Office in January 2023.

On 03/31/2023, the Spokane County Prosecutor’s Office announced that Staley would not face criminal charges. Prosecutors accepted Staley’s account that he believed the pickup was about to strike him and concluded that his use of deadly force constituted lawful self-defense.

Darin Staley had served with the Spokane County Sheriff’s Office since December 1994 and had approximately 27 years with the department when he shot Starkweather. He had worked as a patrol deputy and field-training officer before being promoted to detective/corporal in 2005. Staley was placed on administrative leave following the shooting. By the time prosecutors completed their review in March 2023, he was described as a 28-year veteran of the sheriff’s office. Prosecutors declined to charge him in Starkweather’s death.

On 08/21/2022, 6-year-old Iyla Johnson was shot and killed by her father, 40-year-old Eric Johnson, during a police and SWAT standoff at the Lake of the Woods Apartments at 1894 Lakenoll Drive in Mount Healthy. The encounter began during a custody dispute when Eric Johnson would not return Iyla to her mother.

Mount Healthy police were called at approximately 11:40 a.m. Two officers entered the apartment attempting to recover Iyla and return her to her mother. As officers entered, they heard a gunshot coming from a second-floor bedroom. Officers went upstairs but found the bedroom door barricaded from inside.

As officers attempted to force entry into the bedroom, additional gunshots were heard. Officers withdrew and requested the Hamilton County SWAT team. Police secured the apartment while SWAT responded and attempted repeatedly to communicate with Eric Johnson.

After attempts to establish communication failed, SWAT entered the barricaded bedroom and found Iyla and her father dead from gunshot wounds. Mount Healthy Police Chief Vincent Demasi said the preliminary investigation indicated that Johnson shot his daughter and then himself. Police believed the first gunshot officers heard upon entering the apartment was the shot that killed Iyla.

Iyla was preparing to enter first grade at St. Vivian School. Her family described her as a highly energetic and affectionate child who loved art, dancing, swimming and martial arts and was known within the family as a “social butterfly.”

On 08/24/2022, a Waukegan Police Department officer shot and killed 26-year-old Jesus Angel Manjarrez outside his home in the 1000 block of Glen Court after police received multiple 911 calls concerning a neighborhood disturbance. At approximately 12:45 p.m., the first Waukegan officer arrived and found the wooden fence burning. Body-worn camera footage shows the officer retrieving a fire extinguisher from his patrol vehicle and approaching the fire. Manjarrez then walked down the driveway toward the officer.

The body-camera footage shows Manjarrez approaching while holding a knife. The officer dropped the fire extinguisher, backed away and repeatedly ordered Manjarrez to drop the knife. Manjarrez continued toward the officer and raised his knife-bearing hand above his head. The officer then fired five rounds at close range, striking Manjarrez.

Police reported that Manjarrez also possessed a hammer and had a gasoline container taped to his chest. An Illinois State Police witness interview subsequently documented a witness reporting that Manjarrez approached the officer carrying what the witness described as a sledgehammer and that the witness saw a knife in Manjarrez's waistband.

On 08/24/2022, members of the U.S. Marshals-led Mountain State Fugitive Task Force shot and killed 37-year-old Jason Arnie Owens outside Amos Carvelli Funeral Home in Nutter Fort immediately after Owens served as a pallbearer at his father Junior Arnie Owens’s funeral. Owens had originally been imprisoned following a 2018 incident in which he fled from a Harrison County sheriff’s deputy and then fought with the deputy during his arrest. He received a sentence of three to 13 years and was released on parole in April 2021. The warrant officers were executing at the funeral, however, was based on the subsequent parole violation.

According to the later 98-page West Virginia State Police investigative report, law enforcement knew Owens would be attending his father’s funeral and believed he might be armed. Officers decided not to approach him while he was near his vehicle. Instead, they watched Owens participate in the funeral and waited until he had helped carry his father’s casket outside and load it into the hearse before moving in to arrest him.

Witnesses described officers arriving rapidly in unmarked vehicles and wearing plain clothes. Owens had just finished carrying his father’s casket and was embracing a relative when officers approached. Family friend Cassandra Whitecotton said an officer jumped from an SUV wearing shorts and a T-shirt and that officers did not announce themselves before yelling Owens’s name. Owens’s aunt Evelyn O’Dell similarly said she was hugging Owens when someone yelled his name and gunfire immediately followed.

The initial U.S. Marshals Service account alleged that Owens produced a firearm during the attempted arrest, causing officers to shoot him. Family members and several witnesses immediately challenged that account, saying they did not see Owens draw a weapon and questioning whether he was given any meaningful opportunity to surrender before officers opened fire.

The later West Virginia State Police investigation provided a more detailed account. According to the police report, Owens resisted as two officers attempted to take him into custody and reached toward a handgun carried in a brown holster on his waist. Police alleged that officers ordered him to stop reaching for the weapon and fired when he continued. At least 10 spent cartridge casings were documented, and Owens sustained multiple gunshot wounds to his chest and side. Investigators reported finding a black handgun near his body.

Witness evidence was not entirely consistent with the officers’ description. One witness told investigators that Owens never attempted to flee, fight or make sudden movements and said the handgun remained visible in its holster and Owens never grabbed it. Owens’s son Justin similarly told investigators that his father had a weapon in a holster but had put his hands behind his back and was surrendering. Other witnesses supported the officers’ account that Owens was reaching toward the weapon and that officers commanded him to stop.

Investigators also searched Owens’s vehicle and reported recovering a .22 Magnum revolver, a .22 Magnum rifle, ammunition and loaded magazines. Those weapons were inside the vehicle and were separate from the handgun investigators reported finding beside Owens after the shooting.

The shooting immediately generated protests and demands for answers from Owens’s family and other funeral attendees. Witnesses disputed the Marshals Service’s statement that officers immediately rendered first aid. Family friend Denzil Pratt said Owens was still breathing after he fell but officers did not initially provide assistance. Other mourners described being threatened with gunfire when they attempted to approach Owens after he was shot.

West Virginia State Police conducted the shooting investigation and interviewed at least 20 witnesses. Investigators ultimately concluded that the officers acted appropriately. The case was forwarded to Grant County Prosecutor John Ours, who served as special prosecutor. In April 2023, Ours determined that the shooting was legally justified, declined to bring criminal charges against the officers and did not present the case to a grand jury.

At approximately 11:37 p.m., the officer was patrolling the park when he saw the vehicle leaving and ran its license plate. The check showed that the vehicle's registered owner had a canceled driver's license.

The officer attempted to stop the vehicle at 2nd Avenue SW and Fern Street South. As the officer approached, the driver drove away. At approximately 11:38 p.m., the officer began pursuing as the vehicle rapidly accelerated south on Fern Street.

According to the Cambridge Police Department, the officer lost sight of the vehicle as it rounded the corner near Cambridge Medical Center. Police reported that the officer terminated the pursuit at approximately 11:39 p.m. and radioed dispatch that the pursuit had been discontinued. The active pursuit therefore lasted approximately one minute.

A short time later, while driving south on Dellwood Street South, the officer encountered the vehicle after it had crashed into a tree at Dellwood Street South and 11th Avenue SW. The vehicle had extensive front-end damage and was visibly on fire. Both the driver and Dahlberg remained trapped inside and required extrication.

Cambridge Fire Department personnel responded and extricated both people from the burning vehicle. Dahlberg survived the initial crash but died from her injuries three days later.

Crenshaw’s mother, Wakita Doriety, said the vehicle had been loaned to her son and that it was reported stolen after he failed to return it when expected. The woman who reported the vehicle stolen maintained that Crenshaw did not have permission to take it.

Officers restrained Dorado in a prone, face-down position and handcuffed him. Atlanta Police Department policy called for a restrained person to be rolled over rather than left prone after handcuffing.

Dorado became unresponsive. Officers removed the handcuffs, turned him onto his back, administered Narcan and performed chest compressions. He was transported to Atlanta Medical Center North, where he died.

Police alleged that French became combative when Thompson attempted the pat-down, struck Thompson, pushed him to the ground and ran away. Thompson pursued French on foot and attempted to use a Taser. Police said the Taser was ineffective and that another physical struggle followed.

Police further alleged that during the second struggle French took Thompson's Taser and used it against him. Thompson then drew his firearm and shot French multiple times. French was transported to a hospital, where he died.

French's family has sharply disputed the police narrative and demanded release of the evidence surrounding the killing. Community members held demonstrations and repeatedly called for transparency, including release of Thompson's body-worn-camera footage. One month after the shooting, residents were still publicly demanding answers about why a traffic stop over a minor alleged violation ended with French dead. The body-camera recording was not publicly released during the initial investigation.

The Baldwin County Major Crimes Unit investigated the shooting at the request of the Bay Minette Police Department. Investigators collected police reports, witness statements, dispatch records, physical evidence, photographs and video evidence and submitted the investigation to the Alabama Attorney General's Office for review.

In 2023, the Alabama Attorney General's Office concluded that Thompson would not face criminal charges. The state accepted the account that French had taken the Taser and used it against Thompson and concluded that Thompson's use of deadly force was legally justified.

On 07/31/2024, French's father, Otis French Sr., acting as representative of his son's estate, filed a federal civil-rights lawsuit against Thompson and the City of Bay Minette. The complaint alleges that Thompson unlawfully stopped French, conducted an unlawful pat-down and search, used excessive force and ultimately shot and killed him. The lawsuit disputes the government's characterization of the encounter and alleges constitutional violations arising from both the stop and the use of force.

Brandon Thompson was subsequently publicly identified as the Bay Minette Police Department officer who shot French. On 08/20/2022, Tucson Police Department Officers Angela Gist and Alexander Martin shot and killed 25-year-old Adum Mahamat outside Diamondz Hookah Lounge in the 3700 block of South 16th Avenue near West Ajo Way. Police had been dispatched to the business at approximately 3:30 a.m. after a 911 caller reported a fight involving a person displaying a firearm.

When officers arrived, the people involved in the original reported fight had already left. While officers spoke with employees, they were told that a second fight was occurring inside the lounge. Officers approached the building as multiple patrons began running from the lounge into the parking lot.

Body-worn-camera and surveillance footage subsequently released by Tucson police showed officers encountering Mahamat in the parking lot holding a handgun. Police repeatedly ordered Mahamat to drop the gun. The video shows Mahamat continuing to hold the weapon while moving through the parking lot as officers followed him with their firearms drawn.

Police alleged that Mahamat did not comply with repeated commands to drop the handgun. Officers Angela Gist and Alexander Martin then fired their weapons, striking Mahamat. Police subsequently secured the handgun near where Mahamat fell.

Approximately three minutes elapsed before officers began providing medical aid because police said they first waited for additional officers and secured the scene. Tucson Fire Department personnel transported Mahamat to a hospital, where he was pronounced dead.

The Pima Regional Critical Incident Team investigated the shooting. Tucson police later released body-worn-camera footage, surveillance video and other materials documenting the encounter. The department's Critical Incident Review archive identifies the 08/20/2022 incident as a fatal police shooting.
