Lane Morgan Caviness

Age :48

Gender :Male

Race : White

Date : 08/10/2022

Location : 94825 Overseas Hwy

City : Key Largo

County : Monroe

State : Florida

Agency : Monroe County Sheriff's Office

Officer(s) : Not Disclosed

Cause of Death : Suicide

Event : Suicide under police duress

OPEN RECORDS SUMMARY :

On 08/10/2022, at approximately 10:42 a.m., Monroe County Sheriff's Office deputies responded to the Key Largo Ocean Resort near Mile Marker 94.8 after receiving a 911 call reporting that 48-year-old Lane Morgan Caviness was intoxicated, armed, and suicidal. Caviness was a commercial airline pilot participating in the Transportation Security Administration's Federal Flight Deck Officer (FFDO) program, which authorizes selected pilots to carry firearms to defend aircraft. Initial reports incorrectly described him as an off-duty federal law enforcement officer, a characterization later clarified by the Florida Department of Law Enforcement.
Deputies contacted Caviness, who told them by telephone that he was intoxicated and prepared to confront law enforcement. He first stepped onto a balcony carrying a handgun and, after deputies ordered him to drop it, complied before returning inside. A short time later, he reappeared carrying an AR-15-style rifle and pointed it toward deputies. Four Monroe County Sheriff's deputies fired their service weapons, striking Caviness. Deputies immediately rendered medical aid and applied tourniquets until fire rescue personnel arrived, but Caviness was pronounced dead at the scene.
Monroe County Sheriff Rick Ramsay publicly characterized the encounter as an apparent "suicide by cop" and requested that the Florida Department of Law Enforcement conduct an independent investigation into the shooting.

Jacob Michael James

Age :21

Gender :Male

Race : Native American

Date : 08/09/2022

Location : Burger King parking lot, West 12th Street and South Williams Avenue

City : Sioux Falls 57104

County : Minnehaha

State : South Dakota

Agency : Sioux Falls Police Department; Minnehaha County Sheriff's Office

Officer(s) : Not Disclosed

Cause of Death : Gunshot

Event : Killed by police during traffic stop

OPEN RECORDS SUMMARY :

On 08/09/2022, members of the Sioux Falls Area Drug Task Force initiated a traffic stop on a Pontiac Bonneville near the Burger King at West 12th Street and South Williams Avenue after investigators reported observing suspected drug activity involving the vehicle. As the vehicle stopped, several occupants exited and fled on foot. Jacob Michael James, 21, ran from the vehicle as officers pursued him.
According to the South Dakota Division of Criminal Investigation, James produced a handgun while running and fired multiple shots during the encounter. A Sioux Falls Police Department detective and a Minnehaha County Sheriff's Office investigator returned fire. James was struck four times and died from his injuries. Investigators determined the two officers fired a total of 21 rounds, with the detective firing seven shots and the investigator firing fourteen.
On 08/26/2022, South Dakota Attorney General Mark Vargo concluded the officers' use of deadly force was legally justified and declined to file criminal charges. The Attorney General's report concluded the officers reasonably believed James presented an imminent threat after firing toward law enforcement during the foot pursuit.
James' family, members of the Lakota community, and supporters disputed the official account of the shooting. They organized multiple marches and rallies in Sioux Falls under the banner "Justice for Jacob James," challenged the state's interpretation of dashboard video evidence, and demanded release of additional records and identification of the officers involved. The officers' names remained withheld in the publicly released investigative materials.
Jacob James' obituary described him as an enrolled member of the Oglala Sioux Tribe who loved music, basketball, drawing, and spending time with family. He was survived by his parents, siblings, grandparents, nieces, nephews, and extended family.

OPEN RECORDS SUMMARY :

On 08/07/2022, at approximately 9:45 p.m., Kansas City police located a sport utility vehicle that police reported had been stolen parked at a Phillips 66 gas station at East 55th Street and Prospect Avenue. Officers positioned their vehicles behind the SUV and waited while 31-year-old Zachary James Garrard was inside the convenience store. Officer(s) : Sgt. Jeffrey McMackins, Deputy Leland Wilkinson, Deputy Patrick Mabusth, Deputy Mark Voss

Cause of Death : Gunshot

Event : Killed by police during mental health welfare check

OPEN RECORDS SUMMARY :

On 08/07/2022, shortly before 1:00 a.m., Wright County Sheriff's Office deputies responded to a residence in the 12300 block of 72nd Court NE after receiving a report that 21-year-old Jordyn John Hansen was experiencing a mental health crisis and had threatened to harm himself and family members. Deputies spoke with Hansen, who initially agreed to go to a hospital for evaluation while they waited for an ambulance.
Before the ambulance arrived, Hansen ran into the kitchen, obtained a knife, left the residence, and ran into the neighborhood. Deputies established a perimeter and requested assistance, including a Minnesota State Patrol helicopter. Four deputies located Hansen in a nearby backyard. Deputies Patrick Mabusth and Mark Voss each deployed a Taser, but Hansen continued moving. According to the Minnesota Bureau of Criminal Apprehension, squad camera video showed Hansen running toward Deputy Leland Wilkinson while holding the knife. Deputy Wilkinson fired his handgun while falling backward, and Sgt. Jeffrey McMackins also fired his handgun. Hansen was struck and transported to North Memorial Health Hospital, where he was pronounced dead.
The Minnesota Bureau of Criminal Apprehension investigated the shooting. Wright County deputies did not wear body cameras, but squad-car cameras recorded portions of the incident, although not the entire shooting.
Hansen's family disputed the deputies' handling of the mental health crisis, stating they believed deputies had opportunities to safely restrain him inside the home before he obtained the knife. His aunt, Sara Wroblewski, publicly criticized the response and said the family believed earlier intervention could have prevented the fatal encounter.
In December 2022, Wright County Attorney Brian Lutes concluded that Sgt. McMackins and Deputy Wilkinson were legally justified in using deadly force after reviewing the BCA investigation, squad video, photographs, interviews, and other evidence.

Deputies entered a workshop on the property and encountered 56-year-old Darrin Gray holding a handgun in one hand and a flashlight in the other. Deputies gave repeated commands for Gray to put the handgun down. According to the sheriff, Gray instead pointed the handgun toward the deputies. A supervisor and a deputy fired their service weapons, striking Gray. He was transported to an area hospital, where he died from his injuries.
Deputies then located Gray's brother, Cecil Gray, inside a nearby residence suffering from a gunshot wound to the leg. Investigators reported that Darrin Gray had shot him before deputies arrived. The Alabama Law Enforcement Agency conducted the investigation into the shooting.

Jonathan Calderon

Age :28

Gender :Male

Race : Latino/a/e

Date : 07/31/2022

Location : Highway 69/Meridian Road near Kuna Road

City : Kuna

County : Ada

State : Idaho

Agency : Meridian Police Department; Ada County Sheriff’s Office

Officer(s) : Deputy Bilton; Deputy Nydeggar

Cause of Death : Vehicle

Event : Bystander killed by police chase crash

OPEN RECORDS SUMMARY :

.
REASON FOR POLICE CHASE:
Attempted traffic stop followed by flight from Meridian Police Department officers. Meridian police initially pursued Ruben Garcia south on Meridian Road but lost sight of his vehicle and terminated their pursuit. Shortly afterward, Ada County Sheriff’s Deputy Bilton saw Calderon continuing south on Meridian Road without headlights and attempted another traffic stop. Calderon continued driving away and Bilton pursued.
.
EVENT NARRATIVE:
On 07/31/2022, Garcia was killed when the Nissan he was driving crossed into opposing traffic and crashed head-on into an uninvolved vehicle during an Ada County Sheriff’s Office pursuit on Highway 69/Meridian Road in Kuna. The other driver, bystander Jonathan Calderon, was also killed.
.
The encounter initially involved the Meridian Police Department. Meridian Officer Herscowitz pursued Calderon south on Meridian Road, but eventually lost sight of the Nissan as it traveled south over the Interstate 84 overpass. Herscowitz terminated the Meridian police pursuit.
.
Shortly after Meridian police terminated their pursuit, Ada County Sheriff’s Deputy Bilton was positioned along Highway 69/Meridian Road farther south. Bilton observed the fleeing vehicle traveling south without its headlights illuminated. Bilton drove behind the Nissan, activated his emergency lights and siren and attempted to stop it. The vehicle continued driving away and Bilton pursued.
.
As Calderon approached the intersection of Deer Flat Road and Highway 69, Ada County Sheriff’s Deputy Nydeggar deployed spike strips in an attempt to stop the Nissan. The fleeing driver drove around the spike strips and continued south. The spike strips therefore did not disable the Nissan.
.
Moments later, the Nissan moved into the northbound lanes of Highway 69 and struck Calderon oncoming vehicle head-on. Both drivers were killed.
.
No PIT/TVI maneuver or intentional police-vehicle contact immediately preceding the fatal collision is documented in the Critical Incident Task Force reconstruction. Police did attempt to stop Garcia with spike strips, but he maneuvered around them before the head-on crash.
.
The later Boise Police Department Critical Incident Task Force investigative file provides substantially more detail than the initial news coverage. It identifies Meridian Officer Herscowitz as the officer involved in the earlier pursuit, Ada County Sheriff’s Deputy Bilton as the deputy who initiated the second pursuit, and Deputy Nydeggar as the deputy who attempted to deploy spike strips.
.
The public investigative material does not provide enough reliable biographical information about Calderon’s family, occupation or everyday life to add a personal history without speculation.
.
OFFICER BACKGROUND:
Ada County Sheriff’s Deputy Bilton initiated the second pursuit after observing Calderon traveling south on Meridian Road without headlights. Deputy Nydeggar attempted to stop Calderon by deploying spike strips near Deer Flat Road and Highway 69. Meridian Police Officer Herscowitz had participated in the earlier pursuit but terminated it after losing sight of Calderon. The fatal-crash investigation was handled through the Critical Incident Task Force, with Boise Police Officer Chuck Roath assigned as lead detective and Sgt. Justin Kendall supervising.

Ruben Garcia

Age :34

Gender :Male

Race : Latino/a/e

Date : 07/31/2022

Location : Highway 69/Meridian Road and East Avalon Street

City : Kuna

County : Ada

State : Idaho

Agency : Meridian Police Department; Ada County Sheriff’s Office/Kuna Police

Officer(s) : Andrew Herscowitz; Kyle Bilton; Deputy Nydeggar

Cause of Death : Vehicle

Event : Driver killed by police chase crash

OPEN RECORDS SUMMARY :

.
REASON FOR POLICE CHASE:
Erratic driving and driving without headlights. Shortly before midnight, Meridian Police Officer Andrew Herscowitz attempted to stop Ruben Garcia after observing his Nissan Altima being driven erratically without headlights near Meridian Road and East Central Drive. Garcia continued driving away at high speed, and Meridian police pursued him south toward Kuna.
.
EVENT NARRATIVE:
On 07/31/2022, 34-year-old Ruben Garcia of Rigby was killed when his Nissan Altima crossed into opposing traffic and struck a pickup truck head-on during a police pursuit on Highway 69 in Kuna. The uninvolved pickup driver, 28-year-old Jonathan R. Calderon of Boise, was also killed.
.
The encounter began when Meridian Police Officer Andrew Herscowitz attempted to stop Garcia near Meridian Road and East Central Drive after observing the Nissan being driven erratically without its headlights illuminated. According to police, Garcia continued away at high speed, drove the wrong way on Main Street and then traveled south on Meridian Road toward Kuna.
.
Meridian police pursued Garcia south but discontinued their pursuit after he crossed Lake Hazel Road into Kuna's jurisdiction. Meridian police notified Kuna Police that Garcia was continuing south toward them.
.
Ada County Sheriff’s Deputy Kyle Bilton, assigned to Kuna Police, was positioned near Columbia Road when Garcia's Nissan passed him traveling south at high speed without headlights. Bilton activated his emergency equipment, attempted to stop Garcia and pursued him south on Highway 69. The subsequent Critical Incident Task Force investigation identified Bilton by full name.
.
Another Ada County deputy, identified in the investigative file as Deputy Nydeggar, positioned ahead of the pursuit near Deer Flat Road and deployed spike strips across Meridian Road/Highway 69. Garcia maneuvered around the spike strips and continued south. The strips did not disable the Nissan.
.
The official crash reconstruction determined that Garcia was traveling in excess of 100 mph as he continued south on Highway 69 without headlights. As he entered the large curve where Highway 69 becomes East Avalon Street, the Nissan entered a critical-speed yaw. Investigators calculated its speed at approximately 89–91 mph during the yaw.
.
Garcia's Nissan crossed into the northbound lane occupied by Calderon's Chevrolet Silverado. Investigators determined that Calderon was legally traveling in his own lane when Garcia's Nissan struck the pickup head-on. The Nissan's calculated impact speed was approximately 84–92 mph.
.
Garcia died from his injuries at the crash scene. Calderon was critically injured, transported to a hospital and subsequently pronounced dead. Calderon had no involvement in the attempted traffic stop or police pursuit.
.
Police did not use a PIT/TVI maneuver or intentionally strike Garcia's Nissan. Officers did deploy spike strips, but Garcia drove around them. The first Meridian police pursuit had been discontinued before Bilton encountered Garcia and initiated the second pursuit in Kuna.
.
Investigators recovered seven empty 24-ounce Budweiser cans from the front passenger floorboard of Garcia's Nissan. Postmortem toxicology measured Garcia's blood-alcohol concentration at .214. This evidence was developed after the crash and was not the original reason Officer Herscowitz attempted the traffic stop; the initiating reason was Garcia's alleged erratic driving without headlights.
.
Garcia was born on 02/20/1988 in Van Nuys, California, to Max and Elena Castaneda Garcia and grew up in Rigby, Idaho. He attended Jefferson High School and worked in the oil fields and masonry trades and for Amalgamated Sugar Company. He was married to Sara Garcia and was the father of a daughter, Leticia. His obituary described fishing, camping, hunting, swimming and music among his interests and said spending time with his family, particularly his daughter, was especially important to him.
.
The Ada County Critical Incident Task Force investigated the fatal pursuit and collision, with the Boise Police Department serving as the lead investigative agency. The investigation included officer interviews, vehicle evidence, roadway evidence, airbag-control-module data, toxicology and crash reconstruction.
.
OFFICER BACKGROUND:
Meridian Police Officer Andrew Herscowitz attempted the initial traffic stop and participated in the first pursuit. Ada County Sheriff’s Deputy Kyle Bilton initiated the second pursuit after Garcia entered the Kuna area. Deputy Nydeggar deployed the spike strips Garcia drove around.
.
Public records also show that Herscowitz has subsequently been named as a defendant in federal civil-rights litigation concerning unrelated Meridian police encounters. In Wilkins v. City of Meridian, a plaintiff alleged unlawful detention, arrest and search arising from a September 2022 encounter involving Herscowitz and other Meridian officers. Those allegations are civil claims and are not findings that Herscowitz committed misconduct. A related federal action was filed again in March 2026 and remains litigation rather than an adjudicated finding of officer wrongdoing.
.
DISPOSITION:
The fatal pursuit and collision were investigated by the Ada County Critical Incident Task Force with Boise Police as the lead agency. No criminal charges against the involved law-enforcement officers resulted from Garcia's death.
