On 04/28/2024, Master-at-Arms Third Class Lyndon Joel Cosgriff-Flax, age 22, died during a training and area familiarization exercise on the York River while assigned to the Security Department at Naval Weapons Station Yorktown. Reports indicate he fell overboard during an exercise involving a harbor security boat and died of injuries sustained. He enlisted in the U.S. Navy in August 2021 and was stationed at Yorktown since February 2022. The incident led to charges against another sailor for dereliction of duty in relation to the event.

Officer Russell Croxton of the Dubach PD pulled over a vehicle and parked his cruiser on US-167. As he got out of the vehicle, he was hit and killed by a tractor trailer on the narrow highway. Croxton’s father had been a local police officer who had died under similar circumstances.

On 04/26/2024, retired Port Authority of New York and New Jersey Police Captain John M. Collins died in The Villages, Florida after a 37-month battle with cancer attributed to his rescue and recovery work at the World Trade Center following the September 11, 2001 terrorist attacks. Collins began his law enforcement career entering the Port Authority Police Academy in October 1984, served 32 years and retired as Captain in January 2008. He later served as Police Chief at The College of New Jersey for nine years.

Washington State Police Officer Elias Huizar shot and killed his ex-wife and a teenage girl in front of their 9-year-old. The teenage girl was Angelica Santos, 17, who was his girlfriend. He had worked as a SRO officer at a school when he impregnated her, then aged 15. She bore his son who was 1 year old whom Huizar abducted after murdering his teenage girlfriend.

Even when it became known that he had raped at least 2 underage girls and one of them bore his child, he was merely fired from Yakima PD and rehired by the school where he remained, this time as substitute teacher, while not being incarcerated while awaiting his trial and living with the girl who was now 17 and their 1-year-old son.

He had been due in court for child rape for having impregnated the schoolgirl and also for raping her underage friend, when he went on a homicide spree, killing his ex-wife in front of their 9-year-old son and killing his 17-year-old live-in girlfriend. These deaths are not listed individually on this database because they occurred before police became involved in the fatal chase ending in the suicide of Huizar.

After killing his ex-wife and teenage girlfriend, Huizar fled, was chased by police and caught on the I-5 near Eugene, Oregon where he fled and ran into a commercial vehicle. When police approached the vehicle, he committed suicide under police duress. He had the 1-year-old boy he had stolen from his ex-wife with him when he died.

On 04/20/2024, Oakland Police Officer Jordan Wingate died after a prolonged battle with injuries he sustained on 08/13/2018 while responding to a suspicious person call near the Port of Oakland. Wingate was involved in a vehicle collision when his patrol SUV struck another vehicle and a parked semi truck. He was hospitalized in critical condition, underwent multiple surgeries, and was medically retired in March 2020. Wingate was born on 02/19/1995 in Walnut Creek, California, began his career as a cadet in 2013, graduated from the 176th Basic Academy in May 2017, and was named “Rookie of the Year” in 2018. He died surrounded by family after nearly six years of medical care following the crash.

Deputy Sheriff Alfredo "Freddy" Flores passed away on April 20, 2024, after succumbing to third-degree burns from a fire at a mobile shooting range in Castaic. The fire occurred on October 10, 2023, when an equipment malfunction ignited flames in the 53-foot trailer, which was located near the Pitchess Detention Center. Flores and another deputy were severely burned before being rescued. Flores, a 22-year veteran of the department, had been undergoing re-certification training at the time.

On April 17, 2024, Investigator Brian Herbert, a 56-year-old veteran officer with the Osage Nation Police Department, died after collapsing during an annual firearms qualification at a shooting range near Hominy, Oklahoma. After completing his pistol qualification, Investigator Herbert began reloading his magazines for the rifle assessment when he experienced discomfort, including heartburn and overheating. Seeking relief, he moved to a shaded area, but his condition worsened, leading to chest pain and loss of consciousness. Colleagues initiated CPR, and he was transported to Cleveland Area Hospital, where he was pronounced dead.

On 04/16/2024, Hendricks County Sheriff’s Deputy Fred Anthony Fislar died after he came into contact with live power lines while responding to a single-vehicle crash on State Road 267 and County Road 300 North near Plainfield, Indiana. Fislar was transported to a hospital where he later died. He served with the Hendricks County Sheriff’s Office since December 2021 and was a former United States Marine. Obituary records identify his date of birth as 02/09/1996 and list his family survivors.

On April 14, 2024, Syracuse Police Officer Michael Jensen, 29, was killed in the line of duty during an ambush on Darien Drive in Salina, New York. Jensen was among the officers responding to a residence after a vehicle fled a traffic stop and was traced to the location. As they approached, Christopher R. Murphy opened fire with an AR-15-style rifle. Jensen was struck and later pronounced dead at Upstate University Hospital. The suspect was also killed by returning gunfire.

On April 14, 2024, Lieutenant Michael Hoosock, 37, of the Onondaga County Sheriff's Office, was killed during an ambush in Salina, New York. Hoosock and other law enforcement officers were attempting to locate a suspect who had fled from a traffic stop. When they approached the suspect’s residence on Darien Drive, 33-year-old Christopher R. Murphy opened fire with a rifle. Hoosock was struck by gunfire and later died at Upstate University Hospital. The suspect was killed at the scene.

Officer Brent Hall died while working on a moonlighting job and running after juveniles entering an establishment. Officer Hall had been charged and tried for rape and sodomy for a 2020 rape of an intoxicated woman he met on duty when he returned to her house after the police call and raped her. The case resulted in a mistrial and he left Bullitt County Sheriff’s Office, his employer of the rape incident, and was employed until his death as a police officer for the Pioneer Village PD.

On 04/09/2024, Special Agent Derek Sean Baer, age 49, with the United States Postal Service Office of Inspector General was killed in a head-on vehicle collision at the intersection of West Virginia Route 9 and Oak Lee Drive in Ranson, West Virginia when another vehicle crossed the median and struck his department vehicle. Baer had served with the USPS OIG for over nine years and previously served as a federal law enforcement officer with other agencies. Rodney Osborne, a 44-year-old corrections officer with the Ohio Department of Rehabilitation and Correction (ODRC), was fatally shot during a training exercise at the Corrections Training Academy in Orient, Ohio. The incident occurred during a "dry" tactical exercise, which was not supposed to involve live ammunition.

Investigators determined that David Pearson, a former ODRC operations commander, fired a gun that fatally struck Lt. Osborne. Pearson was charged with negligent homicide and has pleaded guilty on 12/09/2024 to a misdemeanor charge. Sentencing is pending.
